Abstrak :
Hipertensi atau tekanan darah tinggi merupakan salah satu penyakit sistem kardiovaskular yang memiliki tingkat prevalensi tinggi di Indonesia. Untuk itu dibutuhkan terapi yang efektif dalam pengobatan penyakit tersebut. Pada kebanyakan kasus, dokter banyak memberikan terapi polifarmasi terutama pada pasien yang lebih tua dengan komorbiditas. Hal ini menyebabkan kemungkinan dapat terjadinya permasalahan terkait pengobatan diantaranya: obat yang tidak tepat, dosis obat yang berlebih atau kurang, cara pakai yang tidak tepat, inkompatibilitas obat, alergi obat, duplikasi obat, interaksi obat, dan sebagainya. Oleh karena itu, untuk mencegah terjadinya permasalahan pengobatan tersebut, penting bagi seorang apoteker untuk melakukan pengkajian resep. Penelitian ini bertujuan untuk memberikan gambaran pengkajian resep pada pasien dengan hipertensi yang menebus obat di Apotek Kimia Farma 202 Kejayaan. Penyusunan laporan ini dilakukan melalui hasil pencarian studi literatur dan juga analisis pengkajian resep yang dilakukan pada resep dari pasien yang menerima resep obat hipertensi di Apotek Kimia Farma 202 Kejayaan. Hasil pengkajian resep pada pasien dengan hipertensi menunjukkan terdapatnya informasi yang belum dicantumkan pada salinan resep. Selain itu, terdapat interaksi terhadap obat-obatan yang diresepkan oleh dokter tersebut, sehingga untuk mencegah terjadinya interaksi obat dapat dilakukan dengan memisahkan waktu konsumsi antara obat yang berinteraksi tersebut. Hasil laporan pengkajian resep ini dapat dijadikan sebagai sumber informasi untuk apoteker dalam melakukan proses pengkajian resep. ......Hypertension or high blood pressure is a disease of the cardiovascular system that has a high prevalence rate in Indonesia. Therefore, to prevent these medication problems from occurring, it is important for a pharmacist to review the prescription. This study aims to provide an overview of prescription assessments in patients with hypertension who redeem medication at Kimia Farma 202 Kejayaan Pharmacy. The preparation of this report was carried out through the results of a literature study search and also analysis of prescription studies carried out on prescriptions from patients who received prescriptions for hypertension medication at Kimia Farma 202 Kejayaan Pharmacy. The results of reviewing prescriptions for patients with hypertension showed that there was information that had not been included in the prescription copy. Apart from that, there are interactions with the drugs prescribed by the doctor, so to prevent drug interactions you can do this by separating the consumption time between the interacting drugs. The results of this prescription review report can be used as a source of information for pharmacists in carrying out the prescription review process.
