Abstrak :
Praktek Spesialis Keperawatan Medikal Bedah merupakan proses penting dari pendidikan profesi dalam rangka mengaplikasikan ilmu dan peran perawat spesialis dalam memberikan asuhan keperawatan yang profesional kepada pasien. Dalam menjalankan praktek Praktek Spesialis Keperawatan Medikal Bedah, residen juga menerapkan  Evidence Based Nursing. Evidence Based Nursing yang dilakukan adalah dengan menerapkan intervensi Jacobson Progressive Muscle Relaxation, serta  menjalankan peran sebagai inovator di lahan praktek. Residen telah melakukan asuhan keperawatan pada pasien dengan HIV/AIDS, dengan 1 kasus kelolaan utama yang menggunakan pendekatan Teori Roy dan 30 kasus resume kelolaan menggunakan pendekatan  Teori Bandura untuk ruang rawat jalan pada kasus HIV. Dari hasil kelolaan kasus utama, pasien HIV yang dirawat di ruang rawat memiliki diagnosa keperawatan utama gangguan perfusi jaringan serebral sehingga intervensi utama keperawatan fokus pada manajemen tekanan intrakranial. Dari kasus resume rawat jalan, masalah keperawatan yang paling banyak ditemukan adalah ketidakpatuhan akan pengobatan, sehingga Intervensi keperawatan yang dilakukan adalah dengan melakukan proyek inovasi Counseling Minum Obat ARV yang berisikan konseling secara terstruktur pengobatan ART dalam upaya meningkatkan self confidence perawat serta meningkatkan kepatuhan pasien.

Practice of Medical Surgical Nursing Specialist is an important process of professional education in order to apply the knowledge and role of specialist nurses in providing professional nursing care to patients. In carrying out the practice of Medical Surgical Nursing Specialist, the resident also applies Evidence Based Nursing. Evidence Based Nursing is carried out by applying the Jacobson Progressive Muscle Relaxation intervention, and playing the role of an innovator in the practical field. Resident has provided nursing care to patients with HIV / AIDS, with 1 main case managed using the Roy Theory approach and 30 cases managed resumes using the Bandura Theory approach for outpatient rooms in HIV cases. From the results of the main case management, HIV patients who were treated in the ward had a major nursing diagnosis of cerebral tissue perfusion disorders so that the main nursing interventions focused on the management of intracranial pressure. From outpatient resume cases, the most common nursing problem found was non-adherence to treatment, so that the nursing intervention was carried out by conducting the innovative ARV Medication Counseling project which contained structured counseling on ART treatment in an effort to increase nurses' self-confidence and increase patient compliance.

Latarbelakang: Praktik Spesialis Keperawatan Medikal Bedah Kekhususan Endokrin di RSUPN dr. Cipto Mangunkusumo Jakarta merupakan kegiatan pembelajaran dalam mengaplikasikan peran dan fungsi Ners Spesialis. Belum ada Karya Ilmiah Akhir Spesialis (KIAS) yang menelaah pasien diabetes melitus dengan komplikasi selulitis dan Chronic Kidney Disease (CKD) dengan penerapan teori adaptasi Roy. Tujuan: KIAS ini bertujuan untuk menganalisis asuhan keperawatan pada pasien diabetes melitus yang mengalami selulitis dan CKD maupun resume 30 kasus. Metode: KIAS ini terdiri dari analisis kasus utama, resume 30 kasus, penerapan evidence based nursing dan proyek inovasi. Teori model keperawatan yang dipakai adalah teori adaptasi Roy. Hasil: Ners spesialis berperan tidak hanya dalam memberikan asuhan keperawatan langsung tetapi menerapkan intervensi berbasis bukti ilmiah pada area endokrin yaitu memberikan aplikasi pelembab mengandung urea, gliserin dan petrolatum untuk mengatasi xerosis dan fisura tumit kaki pasien diabetes melitus tipe 2. Selain itu, Ners spesialis juga melakukan inovasi melalui analisis situasi dan kajian literatur untuk mencegah kejadian hipoglikemia berat pada pasien diabetes melitus tipe 2 melalui penerapan Hy-Newss bundle. Kesimpulan: Teori model adaptasi Roy sesuai untuk diaplikasikan pada pasien dengan gangguan endokrin. Pemberian pelembab mengandung urea, gliserin dan petrolatum efektif untuk menurunkan skor derajat xerosis pada pasien diabetes melitus tipe 2. Hy- Newss dapat diterapkan sebagai alat skrining risiko terjadi hipoglikemia berat pada pasien diabetes melitus tipe 2. ......Background: Medical surgical nursing specialist practice in endocrine specific at RSUPN dr. There is no KIAS that examines diabetes mellitus patients with complications of cellulitis and Chronic Kidney Disease (CKD). Aim: This KIAS aims to analyze nursing care in patients with diabetes mellitus who have cellulitis and CKD or resume 30 cases. Method: This KIAS consists of main case analysis, 30 case resumes, application of evidence based nursing and innovation projects. The nursing model theory used is Roy’s adaptation theory. Results: Nurse specialist play a role not only in providing direct care but applying scientific evidence-based practice in the endocrine area, which is providing moisturizing applications containing urea, glycerin and petrolatum to treat xerosis and heel fissures in patient with type 2 diabetes mellitus. In addition, nurse specialist innovate through situation analysis and literature review to prevent the incidence of severe hypoglycemia in patients with type 2 diabetes mellitus through the application of Hy-Newss bundle. Conclusion: Roy’s adaptation model theory is suitable for application in patients with endocrine disorders. Moisturizers containing urea, glycerin and petrolatum are effective in reducing the degree of xerosis score in patient with type 2 diabetes mellitus. Hy-Newss bundle can be applied as a screening tool for the risk of severe hypoglycemia in patients with type 2 diabetes mellitus.

Praktek Spesialis Keperawatan merupakan sebuah proses penting dari Pendidikan profesi dalam rangka mengaplikasikan peran perawat spesialis yang berdampak pada upaya peningkatan kualitas layanan keperawatan. Praktik ini dilakukan dan dikembangkan berdasarkan ilmu pengetahuan dan tekhnologi melalui berbagai penelitian. Tiga kompetensi yang harus dicapai dalam praktik residensi ini adalah memberikan asuhan keperawatan menggunakan pendekatan teori keperawatan, menerapkan implementasi keperawatan yang berbasis bukti ilmiah (evidence based practice) dan melakukan proyek inovasi keperawatan yang berguna bagi pelayanan keperawatan. Sebagai pemberi asuhan keperawatan, mahasiswa residensi telah memberikan asuhan keperawatan menggunakan pendekatan Teori Adaptasi Roy pada 1 kasus kelolaan utama dan 30 kasus resume pasien HIV/AIDS dengan berbagai infeksi oportunistik. Sebagai peneliti, mahasiswa residensi telah mengaplikasikan penerapan tindakan keperawatan yang berbasis bukti ilmiah “Pemberian buah pisang gepok hijau (green banana) pada motilitas usus pasien dengan Diare di RSUP Fatmawati”. Sedangkan sebagai peran inovator, telah dilakukan proyek “VOSIL SPARTAN” berupa pemberian edukasi pencegahan infeksi oportunistik pada ODHIV dengan hasil peningkatan pengetahuan dan sikap ODHIV guna pencegahan munculnya infeksi oportunistik. ......Nursing Specialist Practice is an important process of professional education in order to apply the role of specialist nurses which has an impact on efforts to improve the quality of nursing services. This practice is carried out and developed based on science and technology through various studies. Three competencies that must be achieved in this residency practice are providing nursing care using a nursing theory approach, implementing evidence-based nursing practice and conducting nursing innovation projects that are useful for nursing services. As a nursing care provider, residency students have provided nursing care using the Roy Adaptation Theory approach in 1 main management case and 30 resume cases of HIV/ AIDS patients with various opportunistic infections. As a researcher, residency students have applied the application of scientific evidence-based nursing actions "Giving green banana to the intestinal motility of patients with diarrhea at Fatmawati General Hospital". While as an innovator role, the "VOSIL SPARTAN" project has been carried out in the form of providing education on the prevention of opportunistic infections in ODHIV with the result of increasing the knowledge and attitude of ODHIV to prevent the emergence of opportunistic infections.
