Praktik residensi keperawatan medikal bedah kekhususan sistim perkemihan merupakan proses yang harus ditempuh untuk menjadi ners spesialis. Praktikan berperan sebagai care provider dalam memberikan asuhan keperawatan profesional dengan pendekatan model adaptasi Roy pada kasus utama gagal ginjal kronik stage V on hemodialisis dengan komplikasi subarachnoid hemoragic dan tiga puluh kasus perkemihan. Praktikan berperan sebagai peneliti dalam menerapkan tindakan keperawatan berbasis bukti ilmiah dengan topik holi Qur rsquo;an untuk menurunkan skore depresi dengan tahap awal skrining depresi. Peran sebagai inovator adalah dengan menerapkan model edukasi self instructional module SIM untuk meningkatkan pengetahuan dan kepatuhan pasien hemodialisis. Hasil praktik residensi adalah teori model adaptasi Roy efektif digunakan dalam memberikan asuhan keperawatan pada sistim perkemihan, holi Qur rsquo;an efektif menurunkan skore depresi dengan skrining awal menggunakan Beck Depression inventory II dan edukasi model SIM efektif meningkatkan pengetahuan dan kepatuhan pasien hemodialisis. Surgical-medical nursing residency practice on the specificity of urinary system is a process which must be taken to become a nursing specialist. During the residency practice, the student played several roles. As a care provider, the student delivered professional nursing actions with Roy adaptation model theory on the main case of chronic renal failure stage V on haemodialysis with subarachnoid haemorrhagic complication and thirty urinary cases. As a researcher, the student applied scientific evidence-based nursing actions with holy Qur rsquo;an to decrease depression score. Depression screening was conducted as the initial stage. And as an innovator, the student applied self-instructional module SIM to improve haemodialysis patients rsquo; knowledge and obedience. The results of the residency practice were Roy Adaption model was effective to deliver nursing care on urinary system, holy Qur rsquo;an was successful to decrease depression score with Beck Depression inventory II used in the initial screening , and SIM was also effective to improve haemodialysis patients rsquo; knowledge and obedience.

Banyaknya permasalahan yang ditimbulkan akibat gangguan muskuloskeletal maka diperlukan peran Ners spesialis keperawatan muskuloskeletal untuk memberikan asuhan keperawatan profesional dan advance berdasarkan kebutuhan pasien. Tujuan penulisan Karya Ilmiah Akhir ini adalah menganalisis kegiatan praktik Ners Spesialis Keperawatan Medikal Bedah berdasarkan standar kompetensi spesialis keperawatan medikal bedah kekhususan sistem muskuloskeletal. Ners Spesialis dalam melakukan asuhan keperawatan pada pasien dengan gangguan muskuloskeletal menggunakan pendekatan teori Model Adaptasi Roy baik pada kasus kelolaan utama maupun resume. Penerapan Evidence Based Nursing (EBN) melalui intervensi latihan isometrik pada pasien pasca operasi fraktur ektremitas bawah, efektif menurunkan nyeri dan meningkatkan kekuatan otot. Proyek inovasi berupa pengembangan Enhanced Recovery after Surgery (ERAS) for orthopaedic untuk mempercepat proses pemulihan pasien operasi hip. Pemberian asuhan keperawatan pada pasien dengan gangguan muskuloskeletal menggunakan pendekatan teori Model Adaptasi Roy dapat membantu pasien untuk beradaptasi terhadap penyakitnya. Intervensi latihan isometrik jika dikombinasikan dengan terapi non farmakologis yang lain seperti nafas dalam berpotensi bisa memberikan efek yang lebih baik terhadap penurunan nyeri.

ABSTRACT
The number of problems caused by musculoskeletal disorders requires the role of nurse specialist musculoskeletal nursing to provide professional and advanced nursing care based on patient needs. The purpose of writing this final scientific paper is to analyze the practical activities of nurse specialist medical surgical nursing based on the competency standards of medical surgical nursing specialists, specifically the musculoskeletal system. Nurse specialists in conducting nursing care in patients with musculoskeletal disorders use the Roy Adaptation Model theory approach both in the case of main and resume managed. The application of Evidence Based Nursing (EBN) through isometric exercise intervention in patients with lower extremity fracture surgery, effectively reduces pain and increases muscle strength. The innovation project was in the form of developing Enhanced Recovery after Surgery (ERAS) for orthopedics to accelerate the recovery process for hip surgery patients. Giving nursing care to patients with musculoskeletal disorders using the Roy Adaptation Model theory approach can help patients adapt to the disease. An isometric exercise intervention if combined with other non-pharmacological therapies such as deep breathing has the potential to have a better effect on reducing pain.

Praktik residensi keperawatan medikal bedah adalah program pendidikan yang bertujuan untuk menghasilkan perawat ners spesialis dengan kekhususan tertentu. Kegiatan praktik residensi terdiri dari pemberian asuhan keperawatan pada pasien dengan gangguan sistem perkemihan menggunakan pendekatan model adaptasi Roy. Asuhan keperawatan dilakukan kepada pasien dengan kasus gagal ginjal terminal, keganasan dan obstruksi. Penggunaan pendekatan teori Roy bertujuan untuk membantu meningkatkan adaptasi individu terhadap lingkungannya. Penerapan evidence based nursing yang berupa ice therapy bertujuan untuk mengurangi nyeri penusukan AV fistula pada pasien yang menjalani hemodialisis. Tindakan ini dapat digunakan sebagai salah satu intervensi manajemen nyeri non farmakologi karena efektif, sederhana dan tidak memiliki resiko. Pelaksanaan proyek inovasi berupa program ambulasi dini pada pasien transplantasi ginjal bertujuan untuk meningkatkan pengetahuan dan kemampuan pasien untuk melakukan ambulasi dini sehingga mempercepat lama rawat inap di rumah sakit

The residency practice of medical surgical nursing is an educational program that aims to produce nurse specialist with particular specialty. It consist of giving nursing care to patients with urinary system disorder using Roy's model adaptation. Nursing care during residency practice was given to patient with renal failure, malignancy and urinary tract obstruction. The using Roy adaptation model is aimed to improve patient adaptation with the environment. Implementation of evidence based nursing (EBN) using ice therapy is aimed to reduce pain cannulation AV fistula in patients undergoing hemodialysis. Ice therapy is simple, more effective, and without side effect so that it can be applied as non-pharmacological pain management intervention. The implementation of innovation project through early ambulation for kidney transplantation patient to increase knowledge and ability of patients to carry out early ambulation as to accelerate the length of stay hospital.

Praktik klinik residensi keperawatan medikal bedah pada pasien gangguan sistem kardiovaskular bertujuan untuk mengimplementasikan peran perawat spesialis sebagai pemberi asuhan keperawatan, edukator, peneliti, dan inovator. Peran pemberi asuhan keperawatan dilakukan dengan menggunakan pendekatan Teori Model Adaptasi Roy pada pasien ADHF Acute Decompensated Heart Failure dan 30 pasien lainnya dengan berbagai gangguan sistem kardiovaskular. Peran sebagai edukator dilakukan oleh perawat spesialis terhadap pasien dan juga rekan sejawat di tatanan pelayanan rumah sakit. Penerapan EBN Evidence Based Nursing yaitu pemberian Ice Bag di area femoralis untuk nyeri pada pasien yang menjalani PCI Percutaneous Coronary Intervention dilakukan untuk mengaplikasikan peran sebagai peneliti. The clinical practice of medical surgical nursing residency in patients with cardiovascular system disorders aims to implement the role of specialist nurse as care provider, educator, researcher, and innovator. The role of nursing care providers is have been done using the Roy Adaptation Model Theory approach in ADHF Acute Decompensated Heart Failure patients as well as 30 other patients with various other cardiovascular system disorders. The role of educator is performed by specialist nurses on patients as well as colleagues in hospital service arrangements. The implementation of EBN Evidence Based Nursing is the application of Ice Bag in the femoral region for pain in patients undergoing PCI Percutaneous Coronary Intervention performed to apply the role as a researcher. The innovation project is also carried out as a manifestation of the role of innovator through the implementation of innovation projects in the formulation of patient safety culture guidance using HSOPSC Hospital Survey On Patient Safety Culture questionnaire.
