"Diabetes Melitus (DM) merupakan penyakit tidak menular yang memerlukan pengobatan yang kompleks. Banyaknya risiko komplikasi pada pasien DM menyebabkan peningkatan jumlah obat-obat yang harus dikonsumsi oleh pasien yang mengarah pada polifarmasi. Pemberian obat polifarmasi memiliki risiko yang tinggi terhadap terjadinya Drug Related Problems (DRPs) yang dapat membahayakan pasien. Laporan ini bertujuan untuk mengkaji resep obat pada pasien DM di Apotek Kimia Farma Kejayaan untuk mengetahui ada tidaknya DRPs. Pengumpulan informasi dilakukan dengan studi literatur terhadap setiap obat yang diresepkan. Pada hasil pengkajian resep, diperoleh masih banyaknya aspek yang belum terpenuhi, yaitu aspek administratif, berat badan, usia, nomor telepon dan SIP dokter perlu dicantukan. Pada aspek farmasetik bentuk sediaan dan data stabilitas perlu ditanyakan kembali kepada dokter penulis resep. Sementara pada aspek klinis, perlu dicek kembali dosis obat (resep 1), penambahan aturan pakai dan risiko interaksi obat yang dapat mengarah pada efek samping yang serius terhadap pasien.

Diabetes Mellitus (DM) is a non-communicable disease that requires complex treatment. The many risks of complications in DM patients cause an increase in the number of drugs that must be consumed by patients which leads to polypharmacy. Administering polypharmacy drugs has a high risk of developing Drug Related Problems (DRPs) which can harm the patient. This report aimed to assest drug prescriptions in DM patients at Kimia Farma Kejayaan Pharmacy to determine whether DRPs were present. Information collection was carried out by studying the literature on each drug prescribed. Based on the results of the review of prescriptions, it was found that there were still many aspects that had not been fulfilled, namely administrative aspects, weight, age, telephone number and doctor's SIP that needed to be included. Regarding the pharmaceutical aspects of dosage forms and stability data, it is necessary to ask the prescribing doctor again. While on the clinical aspect, it is necessary to re-check the dosage of the drug (recipe 1), add the rules for use and the risk of drug interactions which can lead to serious side effects in patients."
