Abstrak :
Cara Distribusi Obat yang Baik (CDOB) mengatur mengenai ketentuan khusus untuk Narkotika, Psikotropika dan Prekursor Farmasi supaya memenuhi ketentuan perundang-undangan. Ketentuan tersebut bertujuan untuk mencegah kejadian penyalahgunaan, penyimpangan dan kehilangan Narkotika, Psikotropika dan Prekursor Farmasi dari jalur distribusi resmi. Pedagang Besar Farmasi (PBF) berperan untuk memastikan kualifikasi pemasok yaitu memiliki izin produksi narkotika dari Menteri Kesehatan. Apoteker berperan sebagai penganggung jawab fasilitas distribusi Narkotika, Psikotropika dan Prekursor Farmasi (NPP) seperti melakukan pembaharuan spesimen tanda tangan dan stempel untuk memastikan keabsahan tersebut. KFTD Tangerang merupakan salah satu Perusahaan Besar Farmasi yang telah memiliki izin penyaluran narkotika ke fasilitas lain seperti instalasi farmasi pemerintah, apotek, klinik dan rumah sakit sesuai ketentuan perundang-undangan dan CDOB. Pada tugas khusus ini akan dilakukan analisis kewajaran pemesanan Narkotika, Psikotropika, dan Prekursor Farmasi pada Apotek V yang merupakan salah satu dari sepuluh outlet dengan pemesanan Narkotika, Psikotropika, dan Prekursor Farmasi terbanyak selama periode Mei-Juli 2022. Berdasarkan hasil analisis kewajaran pesanan Narkotika, Psikotropika, dan Prekursor Farmasi di salah satu outlet KFTD Cabang Tangerang menunjukkan bahwa Apotek V melakukan pesanan Narkotika, Psikotropika, dan Prekursor Farmasi dalam batas yang wajar dengan didukung oleh lokasi geografis, kerjasama praktek dokter, jam operasi dan kerjasama BPJS. ......Good Drug Distribution Method regulates special provisions for Narcotics, Psychotropics and Pharmacy Precursors so that they comply with statutory provisions. The provision aims to prevent abuse, deviation and loss of Narcotics, Psychotropics and Pharmacy Precursors from official distribution channels. Pharmaceutical Wholesalers play a role in ensuring supplier qualifications, namely having a narcotics production permit from the Minister of Health. Apothecary has the role of being in charge of the Narcotics, Psychotropics and Pharmacy Precursors distribution facilities such as renewing specimen signatures and stamps to ensure the validity. KFTD Tangerang is one of the large pharmaceutical companies that already has a license to distribute narcotics to other facilities such as government pharmaceutical installations, pharmacies, clinics and hospitals in accordance with statutory provisions and the Good Drug Distribution. This assignment will carry out a fairness analysis of Narcotics, Psychotropics and Pharmacy Precursors orders at pharmacy V which is one of ten outlets with the most orders of Narcotics, Psychotropics and Pharmacy Precursors during the period May-July 2022. Based on the results of the fairness analysis of Narcotics orders, Psychotropics, and Pharmacy Precursors at one of the KFTD outlets of the Tangerang Branch shows that pharmacy outlet V orders Narcotics, Psychotropics, and Pharmacy Precursors within reasonable limits supported by geographic location, doctor practice cooperation, operating hours and BPJS cooperation.

Proses operasional di PBF merujuk kepada Cara Distribusi Obat yang Baik (CDOB) untuk obat, dan Cara Distribusi Alat Kesehatan yang Baik (CDAKB) untuk alat-alat kesehatan. Jenis produk obat yang disalurkan oleh PBF antara lain obat bebas, obat bebas terbatas, obat keras, narkotika, psikotropika, prekursor farmasi, obat-obat tertentu (OOT), dan produk rantai dingin. Diantara produk tersebut, prekursor farmasi dan obat-obat tertentu merupakan produk obat yang memiliki ketentuan khusus dalam hal penyalurannya, dimana ketentuan tersebut hampir mirip dengan ketentuan penyaluran produk narkotika dan psikotropika. Alur operasional produk prekursor dan OOT meliputi pengadaan, penerimaan, penyimpanan, penyaluran, dan pelaporan. Pengadaan prekursor dan obat-obat tertentu dilakukan dengan pemesanan ke gudang induk yang harus disertai surat pesanan dengan format khusus. Selanjutnya, prekursor farmasi dan obat-obat tertentu diterima dan dilakukan pengecekan fisik dan kesesuaian dengan surat pesanan. Produk obat yang sudah lolos pengecekan disimpan diruangan khusus yang memiliki sistem pintu ganda, dilengkapi tempat khusus untuk karantina obat, termometer suhu ruang, dan juga terdapat kartu stok untuk mendokumentasikan setiap pengeluaran dan penerimaan obat. Penyaluran prekursor farmasi dan obat-obat tertentu hanya dapat dilakukan berdasarkan surat pesanan dari apoteker penanggung jawab instansi pelanggan (RS, apotek, klinik), atau dari Tenaga Teknis Kefarmasian (bagi toko obat). Apabila terjadi kehilangan selama proses pengiriman, APJ wajib melapor ke BPOM dengan melampirkan berita acara lengkap dengan surat kehilangan dari pihak kepolisian. Pelaporan prekursor farmasi dan obat-obat tertentu dilakukan setiap bulan ke kemenkes dan BPOM melalui melalui situs e-report PBF dan e-was BPOM.  ......The operational process at PBF refers to Good Medicine Distribution Methods (CDOB) for medicines, and Good Medical Device Distribution Methods (CDAKB) for medical devices. The types of medicinal products distributed by PBF include over-the-counter drugs, limited over-the-counter drugs, hard drugs, narcotics, psychotropics, precursors, certain drugs (OOT), and cold chain products. Among these products, pharmaceutical precursors and certain drugs are medicinal products that have special provisions regarding their distribution, which are almost similar to that of narcotic and psychotropic products. The operational flow of precursor and OOT products includes procurement, receiving, storage, distribution and reporting. Procurement of precursors and certain medicines is carried out by ordering from the main warehouse which must be accompanied by an order letter in a special format. Next, pharmaceutical precursors and certain drugs are received and carried out physical checks and compliance with the order letter. Medicinal products that have passed inspection are stored in a special room that has a double door system, equipped with a special place for drug quarantine, a room thermometer, and also stock card to document every drug dispensed and received. Distribution of pharmaceutical precursors and certain medicines can only be carried out based on an order letter from the pharmacist in charge of the customer agency (hospital, pharmacy, clinic), or from Pharmaceutical Technical Personnel (for drug stores). If a loss occurs during the delivery process, the pharmacist in charge is required to report to BPOM by attaching an official report complete with a letter of loss from the police. Report of pharmaceutical precursors and certain drugs is carried out every month to the Ministry of Health and BPOM via the PBF e-report site and BPOM e-was.

Pedagang Besar Farmasi (PBF) berperan sebagai distributor serta penghubung antara pemasok bahan baku ke industri farmasi, maupun industri farmasi ke fasilitas kesehatan. Dalam menjalankan tugasnya sebagai distributor obat dan/atau bahan obat, diperlukan standar untuk mengatur cara distribusi yang baik untuk memastikan terjaganya mutu selama proses distribusi. Salah satu hal penting dalam proses distribusi oleh PBF adalah penyimpanan yang memiliki ketentuan berbeda tergantung kategori atau jenis produknya. Pada penelitian ini, dilakukan evaluasi kesesuaian penyimpanan obat reguler, cold-chain product, psikotropika, dan prekursor farmasi di salah satu PBF terbesar di Indonesia, yaitu PT Anugerah Pharmindo Lestari (PT APL) – Jakarta Distribution Center (JDC) terhadap CDOB 2020. Pengumpulan data dilakukan melalui pengamatan di lapangan (observasi) dan melalui kegiatan tanya-jawab dengan narasumber (wawancara). Berdasarkan hasil evaluasi kesesuaian menggunakan metode evaluasi deskriptif dan analisis campuran dengan skala Guttman, PT APL-JDC telah memenuhi 90,91% (10 dari total 11 poin) ketentuan penyimpanan obat reguler menurut CDOB 2020, 83,33% (5 dari total 6 poin) ketentuan penyimpanan cold-chain product menurut CDOB 2020, serta 100% (3 dari total 3 poin) ketentuan penyimpanan psikotropika dan prekursor farmasi menurut CDOB 2020. ......Pharmaceutical Wholesalers (PBF) act as distributors and intermediaries between suppliers of raw materials to the pharmaceutical industry, as well as the pharmaceutical industry to healthcare facilities. In carrying out its role as a distributor of drugs and pharmaceutical materials, PBF requires a standard to regulate suitable distribution methods to ensure quality is maintained during the distribution process. One of the crucial things in the distribution process by PBF is storage which has different provisions depending on the category or type of product. This study evaluated the suitability of regular drug storage, cold-chain products, psychotropics, and pharmaceutical precursors at one of the largest PBFs in Indonesia, namely PT Anugerah Pharmindo Lestari (PT APL) – Jakarta Distribution Center (JDC) for CDOB 2020. Data was collected through field observations and question-and-answer activities with informants (interviews). Based on the results of the conformity evaluation using the descriptive evaluation method and mixed analysis with the Guttman scale, PT APL-JDC has complied with 90.91% (10 out of a total of 11 points) of the regular drug storage requirements according to CDOB 2020, 83.33% (5 out of a total of 6 points) provisions for storage of cold-chain products according to CDOB 2020, and 100% (3 out of a total of 3 points) provisions for storage of psychotropics and pharmaceutical precursors according to CDOB 2020.
