"ABSTRAKPraktik kerja profesi apoteker di Puskesmas Kecamatan Cakung bertujuan untuk Mampu memahami peranan, tugas dan tanggung jawab apoteker dalam praktek pelayanan kefarmasian di Puskesmas sesuai dengan ketentuan perundangan dan etika farmasi yang berlaku, dan dalam bidang kesehatan masyarakat. Fokus pembelajaran tugas dan fungsi apoteker ini terdapat pada bagian pengelolaan sediaan farmasi, alat kesehatan, dan bahan medis habis dan pelayanan farmasi klinik. Salah satu tugas dan fungsinya adalah mengawasi serta menerima laporan Penggunaan Obat Rasional POR yang berada di bawah suku dinas kesehatan Jakarta Timur. Berdasarkan data POR bulan Januari-September 2016 didapatkan hasil persentase penggunaan antibiotik periode Januari-September 2016 pada ISPA non pneumonia di Puskesmas Kecamatan Cakung adalah sebesar 9 hal ini menunjukkan masih berada dibawah batas toleransi 20 . Sedangkan pada kasus diare non spesifik periode Januari-September 2016, Puskesmas Kecamatan Cakung tepat berada pada angka batas toleransi, yaitu 8 . Indikator kerasionalan resep, kasus ISPA non pneumonia dan diare non spesifik tidak perlu diterapi dengan antibiotik. Hal ini menunjukkan bahwa pada penanganan kasus kasus ISPA non pneumonia dan diare non spesifik tidak memenuhi kerasionalan Sedangkan untuk penggunaan injeksi pada myalgia periode Januari-September 2016 adalah sebesar 0 . Hal ini menunjukkan bahwa pada penanganan kasus myalgia telah memenuhi kerasionalan peresepan.

Pelayanan Kefarmasian di Rumah Sakit meliputi Pengelolaan Sediaan Farmasi, Alat Kesehatan, dan Bahan Medis Habis Pakai; serta Pelayanan Farmasi Klinik. Pengelolaan Sediaan Farmasi, Alat Kesehatan, dan Bahan Medis Habis Pakai terdiri dari pemilihan, perencanaan kebutuhan, pengadaan, penerimaan, penyimpanan, pendistribusian, pemusnahan dan penarikan, pengendalian, administrasi. Pelayanan Farmasi Klinik terdiri dari pengkajian dan pelayanan resep, penelusuran riwayat penggunaan obat, rekonsiliasi obat, pelayanan informasi obat, konseling, visite, monitoring efek samping obat, evaluasi penggunaan obat, dispensing sediaan steril, pemantauan kadar obat dalam darah. Pelayanan kefarmasian di Rumah Sakit Umum Pusat Fatmawati sudah berjalan secara optimal, namun pelayanan farmasi klinik berupa pemantauan kadar obat dalam darah PKOD belum dilakukan.

"Praktik Kerja Profesi Apoteker di Apotek Roxy Cabang Biak bertujuan agar calon apoteker dapat meningkatkan pengetahuan dan keterampilan dalam pelayanan kefarmasian salah satunya dengan melaksanakan pengkajian resep di apotek. Resep yang baik harus mengandung cukup informasi administratif, farmasetik dan klinis yang memungkinkan tenaga kefarmasian untuk memahami terapi yang akan diberikan kepada pasien. Apoteker bertanggung jawab dalam melaksanakan kegiatan pengkajian resep agar tercapai pengobatan yang optimal, meminimalisir medication error dan menghindari penyalahgunaan obat sehingga dapat meningkatkan dan menjamin mutu pelayanan obat. The purpose of this special task was to assess administrative, pharmaceutical and clinical aspects of psychotropic prescriptions at Apotek Roxy Biak Branch in May 2021."
