Indonesian Journal of Dentistry 2006; Edisi Khusus KPPIKG XIV: 12-16 A case of chronic bullous type mucocutaneus disease involving oral mucosa was reported from a 56 years old man with never healed oral ulcers and wound on the perianal skin for three years. There were also red and black sports on the limb and back skin and a lesion on nail. Painful oral lesion consisted of mucous erosion, desquamative gingivitis, and sloughing area on palate and tongue. The patient is diabetic. The first perianal skin diagnosis was granulomatous candidiasis with differential diagnosis pemphigus vegetans and acuminatum condiloma. However, the histopathologic examination did not support those diagnosis. After several histopathologic examinations, the latest perianal skin diagnosis was lichen planus with differential diagnosis granulomatous vasculitis, bowenoid papulosis and pyoderma gangrenosum. Other skin diagnosis was erythema multiforme. Oral diagnosis was mucous membrane pemphigoid with differential diagnosis lichen planus, Behcet's syndrome and erythema multiforme oral histopathologic examination showed a sub-epithelial blister, which supported mucous membrane pemphigoid. A lip balm, prednisone 5 mg oral rinse and multivitamins were given but oral improvement started after blood sugar level controlled. Conclusion: lt is not yet known whether skin and oral mucous lesions are from the same disease or not.

Oral lichen planus (OLP) is a rather common oral disease, mainly affecting adults, occurring more often in women than in men. The etiopathogenesis is still unclear. The diagnosis may be cumbersome, even in the presence of a biopsy. In addition, there are several lesions that may resemble lichen planus (lichenoid lesions) both clinically and histopathologically. Treatment of OLP can only be symptomatic and usually consists of topical application of corticosteroids. The disease is characterized by remissions and exacerbations and may persist in some patients lifelong. There is an ongoing debate in the literature as whether OLP is a potentially malignant disease. Because of this uncertainty, annual follow-up is advised.

Studi ini fokus membahas tentang variasi anatomis normal pada mukosa oral. Tujuan dari studi ini dalah untuk menentukan prevalensi dan distribusi lesi pada 312 pasien yang mengunjungi Rumah Sakit Gigi dan Mulut Pendidikan Fakultas Kedokteran Gigi Universitas Indonesia. Studi ini dilakukan dengan survei epidemiologi dan menggunakan pendekatan potong lintang. Hasilnya menunjukkan bahwa terdapat 7 (2,2%) pasien dengan leukoedema, 69 (22,1%) pasien dengan fordyce granules, dan 207 (66,3%) pasien dengan linea alba pada mukosa oral mereka. Semua lesi lebih banyak ditemukan secara bilateral. Leukoedema dan fordyce granules lebih banyak ditemukan pada pria, sedangkan linea alba lebih banyak pada wanita. Leukoedema dan fordyce granules paling banyak ditemukan pada kelompok usia 69-76 tahun, sedangkan linea alba paling banyak ditemukan pada usia 13-20 tahun.
This study is focused on variations of anatomic structures of oral mucosa. The purpose of this study is to determine the prevalence and the distribution of these lesions in 312 patients who visited University of Indonesia Dental Hospital according to the location, age and gender. This study has been done by cross sectional descriptive epidemiological survey. The result showed that there were 7 (2.2%) people who had leukoedema, 69 (22.1%) people who had fordyce granules, and 207 (66.3%) people who had linea alba on their oral mucosa. All lesions were more common in bilateral location. Leukoedema and fordyce granules were more common among males, while linea alba were more common among females. Leukoedema and fordyce granules had the highest prevalence in 69-76 years age-group, while linea alba was highest in 13-20 years age-group.
