"Sistem muskuloskeletal memberikan perlindungan terhadap organ vital tubuh gangguan pada sistem tersebut memberikan dampak serius bagi kondisi kesehatan seseorang kualitas hidup keuangan kenyamanan fisik dan kesehatan psikologis Penyakit dan cidera di Indonesia paling banyak merupakan kejadian patah tulang Pada kondisi patah tulang dapat mengakibatkan insufisiensi vaskuler dan kompresi saraf serta sindroma kompartemen Perawat spesialis medikal bedah berperan penting dalam penatalaksanaan asuhan keperawatan melalui pendekatan Model Adaptasi Roy pada kasus multipel fraktur dan sindroma kompartemen penerapan pemberian edukasi singkat dan latihan relaksasi nafas dalam sebagai upaya menurunkan nyeri pada klien post operasi ORIF Open Reduction Intra Fixation serta peran sebagai inovator melalui pengembangan program SBAR Situation Background Assessment Recommendation dalam handover perawat ruangan Hasil analisis praktik menunjukkan bahwa Model Adaptasi Roy sesuai dalam pengelolaan asuhan keperawatan pada multipel fraktur dan sindroma kompartemen Pemberian edukasi singkat dan latihan relaksasi nafas dalam mampu menurunkan tingkat nyeri secara efektif hingga hari ke 7 setelah operasi pemasangan ORIF Pengembangan program SBAR dilakukan melalui pembuatan panduan berdasarkan kasus yang sering terjadi di ruangan bedah orthopedi RSUP Fatmawati sehingga melalui panduan tersebut perawat dapat melakukan timbang terima menggunakan SBAR khususnya pada pasien dengan gangguan muskuloskeletal Kata kunci Model Adaptasi Roy fraktur multipel Edukasi singkat dan nafas dalam SBAR Depok 21 Agustus 2015 Mengetahui Suvervisor Utama Mahasiswa Dr Debie Dahlia S Kp MHSM Annas Sumeru.

"Praktek residensi keperawatan medikal bedah pada sistem muskuloskeletal bertujuan untuk mengaplikasikan peran perawat sebagai pemberi asuhan, pendidik, pengelola, peneliti dan agen pembaharu. Perawat spesialis sebagai pemberi asuhan dengan melakukan pengeloaan pasien yang menjadi kasus utama dan resume. Kasus utama diberikan pada pasien dengan multiple fraktur dengan pendekatan teori model Virginia Henderson. Perawat spesialis sebagai peneliti dengan menerapkan Evidence-Based Nursing Practice dalam melakukan tindakan keperawatan. Penerapan terapi musik menjadi salah satu pilihan tindakan keperawatan yang berbasis bukti untuk mengurangi kecemasan pada pasien rawat inap ortopedi. Sedangkan perawat spesialis sebagai agen pembaharu dilakukan dengan menerapkan program Enhanced Recovery After Surgery pada pasien dengan Total Hip Replacement. Rangkaian kegiatan yang dilakukan dalam prakteik residensi bertujuan untuk meningkatkan kualitas pelayanan asuhan keperawatan yang holistik.

Clinical practice of Medical-Surgery Nursing residency in orthopaedic nursing aims to apply the role of nurses as patient care providers, educators, manager, researchers and reform agents. Nurse specialists as providers of care by conducting patient management which is the main case and resume. The main case was given to patients with multiple fractures with Virginia Hendersons model theory approach. Nursing specialists as researchers by applying Evidence Based Nursing Practice in caryying out nursing actions. The application of the music therapy is one of the choices of evidence based nursing actions to reduce anxiety in erthopaedic inpatients. Nurse specialists as reform agents are carried out by implementing enhanced recovery after surgery programs in patients with Total Hip Replacement. The series of activities carried our in the residency practice aims to improve the quality of holistic nursing care services."

"Nyeri pasca Open Reduction Internal Fictation (ORIF) atau bedah terbuka selalu menjadi masalah bagi pasien fraktur. Karya Ilmiah Akhir Ners ini menyajikan hasil analisis asuhan keperawatan pada pasien laki-laki yang mengalami fraktur multiple pasca kecelakan lalu lintas yang kemudian dilakukan ORIF fasialis. Metode yang digunakan adalah studi kasus dan penerapan EBN yaitu intervensi napas dalam untuk mengurangi nyeri akut yang dilakukan selama lima hari perawatan pasca bedah. Skala nyeri pasien pada awal pasca tindakan ORIF berada pada rentang moderat hingga berat. Sedangkan skala nyeri saat hari ke lima pasca bedah dan pemberian intervensi napas dalam, mengalami penurunan di rentang ringan hingga sedang/moderat. Satu hal yang menjadi masalah saat melakukan intervensi napas dalam pada pasien pasca ORIF fasialis adalah kesulitan pasien dalam mengeluarkan napas dengan mencucu. Adapun intervensi lain yang direkomendasikan berdasar pada kajian praktik berbasis bukti meliputi penerapan intervensi metode Guided Imagery pada pasien pasca ORIF untuk menurunkan tingkat nyeri. Penggabungan penerapan teknik relaksasi napas dalam dengan metode Guided Imagery dan terapi farmakologi diharapkan dapat diaplikasikan oleh perawat dalam memberikan asuhan keperawatan, khusus nya pada pasien yang mengalami nyeri akut pasca ORIF

Pain after Open Reduction Internal Fictation (ORIF) or open surgery is always a problem for fracture patients. This Paper presents the results of the analysis of nursing care in a male patient who has multiple fractures after a traffic accident, which is then performed a facial ORIF. The method used was a case study and the application of EBN, namely deep breath intervention to reduce acute pain during five days of postoperative care. The patients pain scale at the start of the post-ORIF treatment ranged from moderate to severe. Meanwhile, the pain scale on the fifth day after surgery and giving deep breath intervention, decreased in the mild to moderate range. One thing that becomes a problem when doing deep breath interventions in patients after facial ORIF is the difficulty of the patient in exhaling with clenching. Other interventions recommended based on evidence-based practice studies include the application of the Guided Imagery method intervention in post-ORIF patients to reduce pain levels. Combining the application of deep breath relaxation techniques with the Guided Imagery method and pharmacological therapy is expected to be applied by nurses in providing nursing care, especially for patients experiencing acute pain after ORIF procedure
