Abstrak :
Perkembangan di industri semen saat ini sangat terasa dengan munculnya beberapa pabrik semen baru. Proses pembuatan semen pada suhu 1500 - 2000°C, peralatan-peralatan yang besar dan teknologi tinggi menimbulkan potensi yang tinggi terhadap timbulnya bahaya. PT. X salah satu penghasil semen terbesar di Indonesia menjadikan keselamatan adalah yang utama sebagai dasar dalam menjalankan bisnisnya. Tetapi kecelakaan kerja masih saja terjadi meskipun trend yang terjadi dari tahun 2009 - 2014 terlihat menurun. Dari jumlah laporan kecelakaan yang cukup banyak, penulis ingin mengetahui sebaran penyebab dasar yang menyebabkan kecelakaan terjadi. Metode yang digunakan untuk menganalisa data kecelakaanan adalah Tripod Beta. Tripod Beta memiliki beberapa kelebihan dibandingan metode yang digunakan PT. X untuk melakukan investigasi kecelakaan antara lain : adanya gambaran yang berbeda antara target, bahaya, dan kejadian, serta hambatan yang akan dijelaskan dengan penyebab langsung, kondisi awal, dan penyebab dasarnya. Selain itu dengan metode Tripod Beta ini urutan kejadian kecelakaan bisa terlihat dengan jelas.

The development of the cement industry currently is very pronounced with the advent of several new cement plants. Cement manufacturing process at temperature 1500 - 2000°C, heavy equipments, and high technology create a high potential og hazards. PT. X which one of the largest cement producers in Indonesia make ?safety first? as a basis in its business. However accidents still happen despite the trend going from the year 2009 - 2014 looks to decline. From a lot of number of accident, the writer intend to know the distribution of the basic causes that led to the accident occurred. The method used to analyze accident report is Tripod Beta. Tripod Beta has several advantages compared to methods used by PT. X to investigate the accident, among which are: illustrates graphically a target, a hazard, and the event, in addition to the failedor missing barrier caused by the direct cause, initial conditions, and the underlying cause. In addition Tripod Beta can give event sequence from the accident.

Tesis ini membahas tentang evaluasi kegiatan investigasi kecelakaan yang dilakukan di PT. HPU pada tahun 2018. Jenis penelitian termasuk penelitian kualitatif dengan pendekatan phenomology. Tujuan dari penelitian ini dimaksudkan untuk mengetahui hasil implementasi metode investigasi dua level yang digunakan semenjak tahun 2015. Dari hasil penelitian diketahui bahwa secara teoritis, metode investigasi yang diterapkan masih masuk ke dalam metode investigasi yang sesuai berdasarkan karakteristik sistem pada industri tambang. Dalam proses investigasi insiden terlihat bahwa kegiatan investigasi belum berjalan sesuai dengan tujuan implementasi investigasi kecelakaan dua level. Investigator masih belum begitu sempurna dalam memahami penggunaan metode investigasi level dua akibat pengaruh dari metode investigasi level pertama yang sudah sangat dikuasai. Dan pada komponen output, terlihat bahwa metode investigasi level dua mampu secara lebih baik menghasilkan laporan yang dapat menggambarkan faktor penyebab kecelakaan secara lebih lengkap dibandingkan metode level pertama. Dalam pelaksanaan kegiatan investigasi, metode level kedua masih memerlukan adanya panduan yang lebih jelas bagi investigator untuk memudahkan dalam penggunaannya. Perlu juga dilakukan peningkatan pengetahuan bagi investigator untuk lebih memahami model yang melandasi metode investigasi yang digunakan. ...... This thesis discusses the evaluation of accident investigation activities conducted at PT. HPU in 2018. The type of the research is qualitative research with phenomology approach. The purpose of this research is to describe the result of the implementation of two-level investigation method used since 2015 in PT. HPU, with interview and document observation as a research tools. From the results of the research it is known that based on theoretically, the investigation methods applied still suitable with appropriate investigation method characteristics of the mining industry system. In the investigation process, it is showing that the investigation activities have not been run in accordance with the purpose of the implementation of two-level accident investigation. There is lack of understanding about how to apply the second-level investigation methods by investigator due to the influence of the already highly-trained first-level investigation methods in the past. On the output component, it is showing that the second-level investigation methods are able to better generate reports that can be illustrate the accident causal factors more comprehensive than the first level methods. In the investigation process, the second level methods still requires a guidance about how the methods should be implementing in the investigation activities. It is also necessary if an investigator having better understanding about the underlying model as a basic model framework for the investigation methods.
