Kegiatan praktek kerja profesi di puskesmas dilakukan agar calon apoteker dapat mampu memahami peran, tugas, dan tanggung jawab apoteker di puskesmas sesuai dengan standar pelayanan kefarmasian di puskesmas sebagai lini pertama dari tempat pelayanan kesehatan di masyarakat. Calon apoteker dapat belajar secara langsung mengenai pengelolaan sediaan farmasi maupun pelayanan farmasi klinis yang berlangsung di puskesmas. Analisa risk assessment dilakukan sebagai tugas khusus untuk dapat menganalisa kejadian atau hal-hal berbahaya apa saja yang dapat atau telah terjadi di dalam pelayanan kefarmasian sehingga dapat dicarikan solusi baik secara preventif maupun koreksi agar dapat meningkatkan kualitas pelayanan terhadap pasien.

ABSTRACT
Professional work practices at puskesmas is conducted to enable the candidate of apothecary program to be able to understand the role, duty, and responsibility of the pharmacist in puskesmas so that accordance with the standard of pharmaceutical services as the first line of health service in the community. Pharmacist candidates can learn directly about the management of pharmaceutical preparations and clinical pharmacy services that take place at the puskesmas. Risk assessment is conducted as a special task to analyze the event or any dangerous things that can or may happen in the pharmaceutical service so that the solution can be found either preventively or correction in order to improve the service quality for the patient.

Pusat kesehatan masyarakat (Puskesmas) merupakan fasilitas pelayanan kesehatan yang menyelenggarakan Upaya Kesehatan Masyarakat (UKM) dan Upaya Kesehatan Perseorangan (UKP) tingkat pertama. Salah satu kegiatan dalam standar pelayanan kefarmasian Puskesmas adalah pengelolaan sediaan farmasi dan Bahan Medis Habis Pakai (BMHP). Tujuan dari kegiatan tersebut ialah tercapainya kelangsungan ketersediaan dan keterjangkauan sediaan farmasi dan BMHP yang efisien, efektif, dan rasional. Dalam menilai efektivitas perencanaan, dapat dilakukan analisa terhadap perencanaan obat. Persentase perencanaan kebutuhan obat idealnya sebesar 100% dari kebutuhan. Dalam rangka menilai efektivitas perencanaan obat pada tahun 2023, dilakukan evaluasi ketepatan perencanaan obat pada tahun 2023 berdasarkan data pemakaian obat pada bulan Januari hingga September 2022 di Puskesmas Kecamatan Cakung. Hasil menunjukkan nilai ketepatan perencanaan yang tepat 100% sebanyak satu jenis obat, yaitu Nistatin 100.000 IU/mL. ......A community health center (Puskesmas) is a health service facility that organizes first-level Public Health Efforts (UKM) and Individual Health Efforts (UKP. One of the activities in the pharmaceutical service standards of Puskesmas is the management of pharmaceutical preparations and Medical Consumables (BMHP). The purpose of this activity is to achieve the continuity of availability and affordability of efficient, effective, and rational pharmaceutical and BMHP preparations. In assessing the effectiveness of planning, analysis of drug planning can be carried out. The percentage of planning drug needs should ideally be 100% of the needs. In order to assess the effectiveness of drug planning in 2023, an evaluation of the accuracy of drug planning in 2023 was carried out based on drug usage data from January to September 2022 at the Puskesmas Kecamatan Cakung. The results showed the value of 100% precise planning accuracy as much as one type of drug, namely Nystatin 100,000 IU / mL.

Apoteker dalam penyelenggaraan pelayanan kefarmasian di apotek wajib mengikuti standar pelayanan kefarmasian yang tertera dalam Peraturan Menteri Kesehatan Nomor 73 Tahun 2016, yaitu salah satunya standar pengelolaan sediaan farmasi, alat kesehatan, dan bahan medis habis pakai (BMHP). Hal tersebut dilakukan agar menjamin ketersediaan sediaan farmasi, alat kesehatan, dan BMHP yang aman, bermutu, bermanfaat, dan terjangkau sehingga masyarakat terlindungi dari bahaya yang disebabkan oleh penggunaan sediaan farmasi, alat kesehatan, dan BMHP yang tidak memenuhi persyaratan mutu, keamanan, dan/atau kemanfaatan. Oleh karena itu, calon Apoteker harus memahami dan memiliki keterampilan pengelolaan perbekalan farmasi di apotek melalui analisis pengelolaan tersebut dalam pelaksanaan Praktik Kerja Profesi Apoteker (PKPA) di Apotek Roxy Galaxy. Pelaksanaan analisis dilakukan dengan metode bimbingan dalam beberapa tahap, yaitu orientasi, observasi, diskusi, simulasi, kerja mandiri, dan hasil pengamatan dibandingkan dengan Peraturan Menteri Kesehatan Nomor 73 Tahun 2016. Hasil yang diperoleh adalah pengelolaan sediaan farmasi, alat kesehatan, dan BMHP di Apotek Roxy Galaxy mencakup perencanaan, pengadaan, penerimaan, penyimpanan, pemusnahan, pengendalian, dan pencatatan dan pelaporan. Kegiatan pengelolaan tersebut menggunakan sistem informasi apotek yang terintegrasi secara terpusat dan telah banyak mengikuti ketentuan dalam peraturan perundang-undangan. ......Pharmacist in providing pharmaceutical care at pharmacy is required to comply with standard of pharmaceutical care that stated in the Minister of Health Regulation Number 73 year of 2016, one of which is standard of management for pharmaceutical preparations, medical devices, and consumable medical materials. Therefore, the undergraduate pharmacist should have understand and have skills to manage pharmaceutical supplies at the pharmacy in the way of analyzing the management activity at pharmacy by Internship of Pharmacist Study Program at Apotek Roxy Galaxy. The analysis was carried out with the mentoring method by preceptor at Apotek Roxy Galaxy in several stages, namely orientation, observation, discussion, simulation, independent work. After that, result is compared to the Minister of Health Regulation Number 73 year of 2016. The results obtained is the management activities of pharmaceutical preparations, medical devices, and consumable medical materials at Apotek Roxy Galaxy are planning, procuring, receiving, storing, destroying, controlling, and recording and reporting. These management activities use a pharmacy information system that is centrally integrated and has followed the provisions of the laws and regulations

Kegiatan praktek kerja profesi di rumah sakit dilakukan agar calon apoteker dapat mampu memahami peran, tugas, dan tanggung jawab apoteker di rumah sakit sesuai dengan standar pelayanan kefarmasian di rumah sakit sebagai tempat pelayanan kesehatan di masyarakat. Calon apoteker dapat belajar secara langsung mengenai pengelolaan sediaan farmasi maupun pelayanan farmasi klinis yang berlangsung di rumah sakit. Analisa drug related problem dilakukan sebagai tugas khusus untuk dapat menganalisa terapi yang diberikan kepada bayi prematur di ruangan NICU agar dapat menjadi masukkan sehingga tujuan terapi dapat tercapai.

Professional work practices in hospital is conducted so that the candidate of apothecary can understand the role, duties, and responsibilities of pharmacist in hospital, in accordance with the standards of pharmaceutical services in hospital as the health care center in the community. Pharmacist can learn directly about the management of pharmaceutical preparations and clinical pharmacy services that take place in the hospital. Drug related problem analysis is performed as a special task to analyze the therapy that being given to premature infants in the NICU room so that therapeutic objectives can be achieved.

Pelayanan Kefarmasian di Rumah Sakit adalah salah satu bagian penting dalam sistem pelayanan kesehatan Rumah Sakit yang berfokus pada pengelolaan Sediaan Farmasi, Alat Kesehatan, dan Bahan Medis Habis Pakai serta pelayanan farmasi klinik. Rumah Sakit Universitas Indonesia (RS UI) menyediakan berbagai fasilitas dan pelayanan kesehatan, salah satunya yaitu Unit Central Operation Theater (COT) yang berfokus pada penyediaan layanan prosedur tindakan operasi atau pembedahan, salah satunya yaitu tindakan CAG/PCI. Tindakan CAG/PCI adalah rangkaian tindakan terhadap jantung. Depo Instalasi Farmasi OK (Operatio Kamer) berperan dalam mendukung kegiatan unit COT dengan menyediakan berbagai paket kebutuhan tindakan operasi. Penelitian ini bertujuan untuk mengevaluasi penggunaan standar paket tindakan CAG/PCI yang telah disiapkan untuk mengoptimalkan efisiensi dan efektivitas pelayanan dan pengelolaan sediaan farmasi di unit farmasi OK RS UI. Pengambilan data dilakukan secara retrospektif menggunakan data periode Juni – Agustus 2023. Data kemudian diolah menggunakan Microsoft Excel dan dinilai kesesuaiannya per pasien, per item, dan per bulan. Hasil analisis menunjukkan bahwa penggunaan item tindakan CAG/PCI pada 77 tindakan pasien tidak sesuai dengan paket standar. Penggunaan 14% item paket standar telah sesuai dengan jumlah pada paket standar, sedangkan 51% item melebihi paket standar dan 35% lainnya di bawah paket standar. Selama bulan Juni – Agustus 2023, penggunaan item untuk tindakan CAG/PCI melebihi dari paket standar. ...... Pharmaceutical services in hospitals are an important part of the hospital health service system which focuses on the management of pharmaceutical supplies, medical devices and consumable medical materials as well as clinical pharmacy services. The University of Indonesia Hospital (Rumah Sakit Universitas Indonesia) provides various health facilities and services, one of which is the Central Operation Theater (COT) Unit which focuses on providing surgical procedures or surgical procedures, one of which is CAG/PCI procedures. The OK (Operatio Kamer) Pharmacy Unit plays a role in supporting the activities of the COT unit by providing various packages of operational needs. This study aims to evaluate the use of the standard CAG/PCI action package that has been prepared to optimize the efficiency and effectiveness of service and management of pharmaceutical preparations in the OK UI Hospital pharmacy unit. Data collection was carried out retrospectively using data for the period June – August 2023. The data was then processed using Microsoft Excel and assessed for suitability per patient, per item and per month. The results of the analysis showed that the use of CAG/PCI action items in 77 patient procedures was not in accordance with the standard package. The use of 14% of standard package items is in accordance with the amount in the standard package, while 51% of items exceed the standard package and the other 35% are below the standard package. During June – August 2023, item usage for CAG/PCI actions exceeds that of the standard package.

Analisa resep dilakukan sebagai tugas khusus untuk dapat mempelajari cara pengkajian resep untuk resep demam tifoid.
