Abstrak :
Pelayanan kefarmasian merupakan pelayanan terkait sediaan farmasi kepada pasien yang bertujuan untuk meningkatkan kualitas kehidupan pasien. Salah satu pelayanan kefarmasian di apotek adalah pelayanan farmasi klinik pengkajian resep, pengkajian merupakan salah satu aspek penting dalam penilaian kelengkapan dan kesesuaian resep dengan kondisi pasien. Terdapat 3 bagian telaah pengkajian/skrining resep yaitu telaah administrasi, farmasetik dan klinik. Saat ini prevalensi polifarmasi dari tahun ke tahun terus meningkat pada semua kelompok usia, kelompok terbesar terjadi pada kelompok usia 10−19 tahun (9,1%), kelompok usia 60−69 tahun (7,2%), dan usia 70−90 tahun (8,6%). Resep polifarmasi rentan terhadap Medication Error dan Drug Related Problems dikarenakan penggunaan obat yang lebih dari 1. Tujuan dari skrining resep pada pasien polifamasi pada penelitian ini adalah untuk mencegah Medication Error dan Drug Related Problem yang rentan terjadi pada pasien dengan penggunaan obat bersamaan lebih dari 5. Pengkajian Skrining resep menggunakan metode deskriptif retrospektif dari data resep-resep pasien poliarmasi dalam 1 bulan terakhir selama proses praktik kerja berlangsung. Data skrining resep menunjukan ada beberapa ketidaksesuaian pada aspek administrasi seperti cap dokter, tanda tangan dokter dan berat badan pasien maupun aspek klinik seperti adanya interaksi obat. ......Pharmaceutical services are types of clinical services that are related to pharmaceutical dosage and patients to improve quality of life. There are 3 sections of prescription screening, namely administrative, pharmaceutical, and clinical aspects. Currently, the prevalence of polypharmacy continues to increase from year to year in all age groups, the largest group occurred in the age group of 10−19 years (9.1%), age group 60−69 years (7.2%), and age 70−90 years (8.6%). Polypharmacy prescriptions are vulnerable to Medication Errors and Drug Related Problems due to the use of more than 1 drug. The purpose of prescription screening in polypharmacy patients in this study was to prevent Medication Errors and Drug Related Problems which are prone to occur in patients with more than 5 concurrent medicine use. Prescription screening studies used a descriptive method retrospective of polypharmacy patient prescription data in 1 last month during the internship process. Prescription screening data shows that there are some discrepancies in administrative aspects such as doctor's stamp, doctor's signature, and patient's weight as well as clinical aspects such as drug interactions.
