Abstrak :
Tujuan : Untuk mengetahui status besi anak usia sekolah dan faktor yang berhubungan serta hubungan status besi dengan daya konsentrasi belajar. Tempat : Sekolah Dasar Pekayon 18 dan Jasmin, Kecamatan Cibubur, Jakarta Timur. Bahan dan cara : Penelitian desain cross sectional, pada 92 orang subyek yang dipilih secara purposive. Data yang dikumpulkan meliputi data umum subyek dan responden/ibu subyek, status gizi, asupan makan, serta pemeriksaan laboratorium darah dan tinja. Setelah status besi ditetapkan berdasarkan kadar hemoglobin dan feritin plasma, subyek dibagi secara acak sederhana (sub sampel) menjadi status besi normal dan defisiensi besi kemudian terhadap ke dua kelompok dilakukan pemeriksaan daya konsentrasi belajar. Uji statistik yang digunakan untuk mengetahui hubungan dua variabel adalah uji Kolmogorov-Smirnov dengan dua variabel dan uji eksak Fisher. Hasil : Prevalensi anemia ditemukan sebesar 14,1% dan defisiensi besi 14,2%, sebanyak 1,1% di antaranya menderita anemia defisiensi besi. Dari penilaian food frequency amount didapatkan 85,9% subyek mempunyai asupan besi yang tergolong kurang dan 79,3% subyek asupan energinya termasuk kriteria kurang. Data pola makan menunjukkan 50% subyek termasuk dalam kriteria kurang. Penilaian terhadap status gizi mendapatkan 7,5% subyek mempunyai bentuk tubuh pendek dan 2,2% subyek tergolong kurus. Tidak didapatkan hubungan yang bermakna (p > 0,05) antara daya konsentrasi belajar dengan status besi pada subyek penelitian dan ditemukan pula hubungan yang tidak bermakna (p>0,05) antara sebagian besar variabel yang diteliti. Simpulan: Pada penelitian ini didapatkan prevalensi anemia 14,1% dan defisiensi besi 14,2%, sejumlah 1,1% subyek di antaranya menderita anemia defisiensi besi. Tidak ditemukan hubungan bermakna (p > 0,05) antara status besi dengan daya konsentrasi belajar.
Iron Status of Children of Two Elementary Schools and Its Related Factors in Cibubur District, East Jakarta: Relationship between Iron Status and Capacity of Learning ConcentrationObjective: To know the iron status of school-age children and its related factors and the relationship between iron status and capacity of learning concentration. Location: Pekayon 18 and Jasmin Elementary Schools, Cibubur District, East Jakarta. Material and Method: A cross-sectional study has been carried out on 92 subjects? selected using purposive sampling method. Data collected consist of socio-economic status, nutritional status, dietary intake, and laboratory examinations for hemoglobin, plasma ferritin and stool egg count. Iron status was determined by hemoglobin concentration and plasma ferritin level. Subjects were divided into normal and iron deficiency based on these laboratory findings, and capacity of learning concentration was examined on both groups. Statistical analysis was performed by Kosmogorov-Smirnov and Fisher exact test for the relationship between variables. Results: Anemia was found in 14.1% subjects, and 14.2% of subjects had iron deficiency in which 1.1% of them had iron deficiency anemia. Inadequate iron and energy intake was found in 85.9 and 79.3% of subjects respectively, and 50% of subjects had poor eating pattern. Nutritional status assessment showed that 7.6% of subjects had short stature (stunted) and 2.2% were wasted. There was no significant relationship (p>0.05) between iron status and the capacity of learning concentration and between most variables. Conclusion: The prevalence of anemia in this study was 14.1%, and iron deficiency was found in 14.2% of subjects, in which 1.1% of them had iron deficiency anemia. There was no significant relationship (p>0.05) between iron status and the capacity of learning concentration.

ABSTRACT
The study was conducted to investigate whether the intestinal helminthiasis influence the acute phase response (APR), nutritional status and iron status, and the impact of anthelminthic treatment on the APR and iron status among school children 8 - 11 years old in SD 01 and 02 Papanggo, Tanjung Priok, North Jakarta. The prevalence of helminthiasis among these children was regarding to Ascaris lumbricoides 81.6 %, T richuris Trichiura 88.3 % and mixed infection A.lumbricoides and T : trichiura 70 %. Of 120 children enrolled in this study, 59 children received a single 400 mg dose Albendazole, 61 children received placebo.

The design of this study was a cross sectional association study combined with a randomized, doubly-masked, community intervention trial. At the beginning of the study, anthropometric measurements were taken. In addition, stool samples, plasma iron, hemoglobin, C-reactive protein (CRP), White Blood Cell (WBC), Erythrocyte Sedimentation Rate (ESR), Interleukin-1 (IL-I), Interleuldn-6 (IL-6) and Tumor Necrosis Factor (TNF) concentrations were determined prior to the interventions and 10 days after.

The children with Z-score of WFA, WFH and FIFA less than -2 were 24,2 %, 6,7 % and 19.2 %, respectively. Of 30 % of the subjects were anemic (Hb <12 g /dl) and 21.6 % had plasma iron levels below normal (male < 59 µg/dl, female < 37µg/dl).

CRP,IL-1,IL-6 and TNF showed normal values in both groups before and 10 days after treatment. ESR was significantly increased in both groups 10 days after treatment. Within group increases in WBC count was significant only in the treatment group.

Plasma iron concentration was significantly increased in the treatment group (P = < 0.05) whereas it was significantly decreased in the placebo group (P = < 0.05). Increases in hemoglobin level in the treatment group and the decrease in the control group were no statistically significant.

This study concluded that the APR were normal during the intestinal helminthiasis and the intensity of infection were not proportional with the APR level, The helminth treatment with single 400 mg dose of Albendazole has not only a significant effect on decreasing worm burden but also a rise in plasma iron.

Defisiensi besi adalah defisiensi mikronutrien yang paling sering ditemui. Jika tidak diobati, dapat menyebabkan anemia defisiensi besi dan gangguan kognitif, terutama pada anak usia sekolah, yang ireversibel. Diagnosis defisiensi besi rumit, tidak praktis, dan mahal. Organisasi AAP merekomendasikan RET-He sebagai pemeriksaan laboratorium untuk skrining defisiensi besi. Tujuan penelitian adalah untuk mencari nilai batasan RET-He untuk skrining status besi pada anak usia 6 – 18 tahun. Studi ini merupakan studi potong lintang terhadap 207 anak sehat usia 6 - 18 tahun di Indonesia. Penelitian ini mencari nilai batasan RET-He untuk skrining status besi, kemudian dibandingkan dengan hemoglobin, mean corpuscular volume, feritin, dan saturasi transferin. Kurva ROC dikerjakan untuk menentukan nilai batasan RET-He untuk skrining status besi dengan menggunakan IBM SPSS versi 22. Pemeriksaan RET-He mendapatkan nilai batasan ≤ 30,3 pg (sensitivitas 100%, spesifisitas 19,7%, NDN 100%, NDP 5,4%) untuk skrining deplesi besi; nilai batasan RET-He ≤ 28,9 pg (sensitivitas 78,9%, spesifisitas 56,2%, NDN 92,2%, dan NDP 28,9%) untuk defisiensi besi; dan nilai batasan RET-He ≤ 27 pg (sensitivitas 75%, spesifisitas 80%, NDN 98,1%, dan NDP 18,7%) untuk anemia defisiensi besi. Peneliti menarik kesimpulan bahwa RET-He dapat digunakan sebagai parameter skrining defisiensi besi dengan nilai batasan ≤ 28,9 pg. Skrining untuk anemia defisiensi besi dapat menggunakan RET-He dengan nilai batasan ≤ 27 pg, namun harus dilakukan dengan parameter lain, seperti Hb. Pemeriksaan RET-He dengan nilai batasan ≤ 30,3 pg tidak dapat digunakan untuk skrining deplesi besi. ......Iron deficiency (ID) is the most common micronutrient deficiency in the world. Left untreated, ID will lead to iron deficiency anemia (IDA) and other irreversible consequences. Screening iron deficiency is complex, impractical, and expensive. The AAP recommended RET-He as an alternative laboratory examination to screen ID. The objective is to find RET-He cut-off value to screen for iron status in healthy children, aged 6 – 18 years old. This study is a cross-sectional study of 207 children aged 6 – 18 years old in Indonesia. RET-He was compared with hemoglobin, mean corpuscular volume, ferritin to assess iron status in children. Receiver operating curve was performed to determine the optimal cut-off value for RET-He using IBM SPSS 22. Reticulocyte hemoglobin equivalent with cut-off value ≤ 30.3 pg was established to screen iron depletion (100% sensitivity, 19.7% specificity, 100% NPV, 5.4% PPV); meanwhile RET-He ≤ 28.9 pg to screen iron deficiency (78.9% sensitivity, 56.2% specificity, 92.2% NPV, 28.9% PPV); and RET-He ≤ 27 pg to screen IDA (75% sensitivity, 80% specificity, 98.1% NPV, 18.7% PPV). The researcher concluded that RET-He can be used as an iron deficiency screening parameter with a cut-off value ≤ 28.9 pg. Screening for IDA with RET-He ≤ 27 pg need to be done with other parameters, such as Hb. RET-He ≤ 30.3 pg cannot be used for iron depletion.

Latar belakang: Perkembangan anak yang optimal diperlukan untuk mendukung proses belajar di kemudian hari. Zat besi merupakan salah satu nutrisi yang dibutuhkan dalam perkembangan otak untuk mendukung perkembangan anak, yang masih sangat diperlukan hingga usia 3 tahun. Penelitian ini bertujuan mengetahui hubungan antara status zat besi dengan status perkembangan anak usia 24-36 bulan. Metode: Penelitian dengan desain potong lintang eksplorasi dilakukan di Kampung Melayu, Jakarta pada bulan September sampai Oktober 2020. Subjek yang memenuhi kriteria penelitian didapatkan dengan teknik total population sampling. Data didapatkan dari wawancara karakteristik dan asupan zat besi menggunakan semi quantitative-food frequency questionnaire (SQ-FFQ), pemeriksaan antropometri, status perkembangan berdasarkan Ages and Stages Questionnaire-3 (ASQ-3), dan status zat besi dari pemeriksaan feritin, hemoglobin, dan high sensitivity C-reactive protein(hs-CRP). Analisis data menggunakan uji Chi square/Fisher, Mann-Whitney, dan regresi logistik. Hasil: Dari 80 subjek yang berhasil diperoleh, terdapat status gangguan perkembangan pada 17,5% subjek dan status zat besi kurang pada 41,3% subjek. Tidak terdapat hubungan bermakna antara status zat besi dengan status perkembangan. Dari analisis multivariat regresi logistik didapatkan status zat besi (p = 0,019) dan status gizi (p = 0,018) berkontribusi terhadap gangguan perkembangan, yaitu masing-masing sebesar 7,5 (95% CI 1,397-40,635) dan 11,45 (95% CI 1,518-86,371). Kesimpulan: Status zat besi berkontribusi dalam perkembangan anak usia 24–36 bulan, sehingga dibutuhkan upaya untuk menjaga status zat besi selain juga status gizi anak. ......Background: Optimal child development is needed to support the learning process at a later date. Iron is one of the nutrients needed in brain development to support child development, which is still very needed until the age of 3 years. This study aims to determine the association between iron status and developmental status in children aged 24-36 months. Methods: An explorative cross-sectional study was conducted in Kampung Melayu, Jakarta, from September to October 2020. Subjects who met the research criteria were obtained using the total population sampling method. Data were obtained from interviews on characteristics and iron intake using semi quantitative-food frequency questionnaire (SQ-FFQ), anthropometric examinations, developmental status based on Ages and Stages Questionnaire-3 (ASQ-3), and iron status from ferritin, hemoglobin, and high sensitivity C-reactive protein (hs-CRP) tests. Data analysis used Chi square/Fisher, Mann-Whitney test, and logistic regression. Results: Of the 80 subjects that were obtained, there was developmental disorder in 17.5% of subjects and deficient iron status in 41.3% of subjects. There was no significant relationship between iron status and developmental status. From the multivariate logistic regression analysis, it was found that iron status (p = 0.019) and nutritional status (p = 0.018) contributed to developmental disorder, namely 7.5 (95% CI 1.397-40.635) and 11.45 (95% CI 1.518-86.371), respectively. Conclusion: Iron status contributed to the development of children aged 24–36 months, so efforts are needed to maintain iron status as well as children’s nutritional status.

Defisiensi besi menganggu proses eritropoiesis sehingga dapat berlanjut menjadi anemia defisiensi besi. Defisiensi besi dan anemia didefinisikan berdasarkan indikator status besi, berupa parameter hematologi dan biomarka darah, yaitu hemoglobin, hematokrit, ferritin, MCV, MCH, MCHC, dan retikulosit. Salah satu faktor penyebab terjadinya defisiensi besi pada ibu hamil adalah kurangnya asupan zat besi. Penelitian ini bertujuan untuk mengetahui hubungan antara asupan zat besi dan status besi ibu hamil trimester 1 yang diukur melalui kadar hemoglobin, hematokrit, ferritin, MCV, MCH, MCHC, dan retikulosit. Rancangan penelitian adalah potong-lintang pada trimester 1 kehamilan. Asupan zat besi diukur menggunakan metode food frequency questionnaire dan 24 hour recall. Uji korelasi Spearman digunakan untuk mengetahui hubungan kedua variabel. Terdapat 120 sampel ibu hamil, 53,3 berpendidikan tinggi, 58,3 bekerja, dan median usia 28 tahun. Nilai median asupan zat besi pada seluruh sampel adalah 10,64 mg. Sebanyak 86,67 sampel tidak memenuhi kecukupan asupan zat besi pada ibu hamil trimester 1 berdasarkan AKG 26 mg/hari. Sebanyak 8,33 sampel mengalami anemia Hb0,05 . Disimpulkan bahwa tidak terdapat hubungan antara asupan zat besi dan status besi ibu hamil pada trimester 1.
ABSTRACT
Iron deficiency disrupts erythropoiesis process that leads to iron deficiency anemia. Iron deficiency and anemia are defined by iron status indicator, in the form of hematological parameters and blood biomarkers, such as hemoglobin, hematocrite, ferritin, MCV, MCH, MCHC, and reticulocyte count. One of the factors causing iron deficiency in pregnant women is inadequate iron intake. This research aims to assess the relationship between iron dietary intake and iron status of pregnant women in 1st trimester. Iron status is measured by the value of hemoglobin, hematocrite, ferritin, MCV, MCH, MCHC, and reticulocyte. This research implemented a cross sectional design during the 1st trimester of pregnancy. Iron dietary intake was assessed by food frequency questionnaire and 24 hour recall. Spearman correlation analysis was used to identify the relationship between the two variables. There were 120 samples of pregnant women, 53.3 were high educated, 58.3 were employed, with the age median of 28 years old. The median of iron dietary intake is 10.64 mg, with 86.67 of samples did not meet the Recommended Dietary Allowance of 26 mg. There were 8,33 of pregnant women with anemia Hb0.05 . It was concluded that iron dietary intake is not related to iron status of pregnant women in 1st trimester.

Latar belakang. Defisiensi besi adalah salah satu gangguan gizi yang paling umum di seluruh dunia dan ini bisa terjadi pada para donor darah laki-laki yang rutin. Seorang donor tetap diharapkan dapat menyumbangkan darahnya secara teratur dalam jangka waktu yang tertentu. Pada donor darah yang seringkali diambil, dikhawatirkan pada suatu waktu dapat terjadi defisiensi besi, tanpa anemia. Dengan demikian menjadi perhatian utama para donor tersebut untuk dilakukan skrining defisiensi besi yang bertujuan bagi para donor darah ini agar tetap sehat dan terus mendonorkan darahnya. Metodologi. Penelitian ini menggunakan desain potong lintang pada para donor darah laki-laki yang menyumbangkan darahnya pertama, kelima dan kesepuluh kali. Masing-masing donasi terdiri dari 25 orang yang diambil sampel darahnya untuk dilakukan pemeriksaan hematologi darah lengkap dan pemeriksaan serum iron, TIBC, saturasi transferin dan feritin serum. Hasil. Didapatkan hasil pada donasi pertama, rerata kadar feritin adalah 91,78; pada donasi kelima terjadi peningkatan kadar feritin yaitu sebesar 111,49 dan menurun lagi pada kelompok pendonor donasi kesepuluh yakni 65,28. Hasil uji kruskal wallis menunjukkan ada perbedaan rerata yang bermakna antara kadar feritin pada donasi pertama, kelima dan kesepuluh kali (nilai p = 0,044). Simpulan. Terdapat penurunan cadangan besi tubuh (feritin serum) pada donasi pertama dan kesepuluh. Semakin sering kita menyumbangkan darah dapat terjadi defisiensi besi tahap pertama yang kita sebut juga iron depletion. Karena itu perlu diperhatikan pola makan atau status gizi dan juga suplemen yang diberikan sesudah donor.
ABSTRACT
Background : Iron deficiency is one of the most common nutritional disorder in the world and this can occur in the routine male blood donors. A blood donor is expected to donate blood regularly in a certain period of time. In routine blood donors, it is feared that they could have iron deficiency without anemia. Thus the need for screening these donors the iron status of these donors, becomes major concern to keep these blood donors healthy and can donate their blood intensly continue to donate blood. Methodology : This study used a cross-sectional design on the first, fifth and tenth times male blood donors. Each donation consists of 25 people who were test for serum iron, total iron binding capacity ( TIBC), transferrin saturation and serum ferritin. Results : it is increasing in the first donation, the mean ferritin levels were 91,78, the fifth donation ferritin levels increase in the amount of 111,49 and declined again in the tenth donation donor group 65,28. Results of Kruskal Wallis test showed significant difference between the mean ferritin levels at the first donation, the fifth and the tenth time (p = 0,044). Conclusion : There is a significant of serum ferritin in the first and tenth routine male male blood donors. Therefore need to be considered diet or nutritional status and iron supplements were given after the donor.
