"ABSTRAK
Pembangunan yang berkesinambungan merupakan pembangunan yang menyeluruh, terarah, dan terpadu di mana salah satunya adalah pembangunan kesehatan guna mencapai tujuan nasional. Adanya sistem otonomi menjadikan suatu daerah diberikan wewenang untuk mengurus sendiri urusan pemerintahannya. Salah satu wujud dari sistem otonomi tersebut adalah adanya Suku Dinas Kesehatan yang menangani pembangunan kesehatan di DKI Jakarta. Tujuan dari praktik kerja profesi ini adalah untuk memahami peran, tugas, dan tanggung jawab apoteker sehingga memiliki pengetahuan, kemampuan, keterampilan, dan pengalaman praktis di Suku Dinas Kesehatan Kota. Tugas khusus dibuat dengan judul Absensi Pelaporan Narkotika dan Psikotropika oleh Apotek di Jakarta Utara Melalui Aplikasi SIPNAP. Tujuannya adalah untuk mengetahui permasalahan absennya apotek dalam pelaporan serta mengetahui peran apoteker dalam pelaporan narkotika dan psikotropika.
ABSTRACT
Sustainable development is a comprehensive development, directed, and integrated where one of them is the development of health to achieve national goals. The existence of an autonomous system makes an area given the authority to take care of its own government affairs. One manifestation of the autonomy system is the existence of the Health Service Tribe that handles health development in DKI Jakarta. The purpose of this professional work practice is to understand the roles, duties, and responsibilities of the pharmacist so as to have knowledge, abilities, skills and practical experience in the City Health Office. Special assignment title is Attendance of Narcotics Reporting and Psychotropic Reporting by Pharmacy in North Jakarta through SIPNAP Application. The goal is to find out the absence of pharmacies in reporting and to know the role of pharmacists in narcotics and psychotropic reporting."

Praktik kerja profesi di Apotek Kimia Farma No. 95 Tangerang Selatan Periode Bulan Juli Tahun 2017 bertujuan untuk memahami tugas dan tanggung jawab apoteker dalam pengelolaan apotek, serta melakukan praktik pelayanan kefarmasian sesuai dengan ketentuan perundang undangan dan etika yang berlaku memiliki wawasan, pengetahuan, keterampilan dan pengalaman praktis untuk melakukan praktek kefarmasian di apotek, memiliki gambaran nyata tentang permasalahan praktek kefarmasian serta mempelajari strategi dan kegiatan kegiatan yang dapat dilakukan dalam rangka pengembangan praktek kefarmasian. Praktik kerja profesi di Apotek Kimia Farma dilakukan selama empat minggu dengan tugas khusus yaitu Analisis Kajian Resep Pasien Autisme di Apotek Kimia Farma No. 95 Tangerang Selatan. Tujuan dari tugas khusus ini agarmemahami tugas dan tanggung jawab Apoteker dalam melakukan analisis pengkajian resep pada penderita autisme memiliki wawasan, pengetahuan, keterampilan dan pengalaman praktis dalam melakukan pengkajian resep pada penderita autism dan memiliki gambaran nyata tentang permasalahan pengkajian resep di apotek serta mempelajari strategi dan kegiatan kegiatan yang dapat dilakukan dalam melakukan analisis pengkajian resep.

Tujuan dari praktik kerja profesi apoteker di Direktorat Jenderal Kefarmasian dan Alat Kesehatan Kementerian Kesehatan RI Periode Bulan Oktober Tahun 2017 adalah untuk memahami peranan, tugas, dan tanggung jawab serta menambah wawasan, keterampilan dan pengalaman praktis sebagai apoteker di pemerintahan. Selain itu, kegiatan ini juga bertujuan untuk memberikan gambaran nyata tentang permasalahan kefarmasian di pemerintahan. Tugas khusus yang dilakukan berjudul Evaluasi Penggunaan Obat di Tujuh RSUD Wilayah Indonesia Tengah Tahun 2014-2017. Tugas ini bertujuan untuk mengetahui persentase kesesuaian penggunaan obat dengan Formularium Nasional di tujuh RSUD serta mengetahui kelas terapi dari obat-obatan yang tidak sesuai Fornas yang digunakan pada RSUD tersebut. Dari kegiatan praktik ini, penulis telah mengetahui peran, tugas, tanggung jawab, wawasan dan pengalaman praktis serta solusi dari permasalahan di Ditjen Kefarmasian dan Alat Kesehatan.

Praktik Kerja Profesi Apoteker di Apotek Kimia Farma No. 352 dilakukan selama satu bulan yaitu bulan Februari 2017. Praktik Kerja Profesi Apoteker ini bertujuan agar calon apoteker mampu memahami tugas dan tanggung jawab apoteker dalam pengelolaan apotek sesuai dengan standar pelayanan kefarmasian; memiliki wawasan, pengetahuan, keterampilan, dan pengalaman praktis untuk melakukan praktik kefarmasian di Apotek; dan memiliki gambaran nyata tentang permasalahan praktik kefarmasian serta mempelajari strategi dan kegiatan yang dapat dilakukan dalam rangka pengembangan praktik kefarmasian. Sedangkan tugas khusus yang dilakukan selama Praktik Kerja Profesi Apoteker berjudul ldquo;Pemberian Informasi Obat pada Pasien yang Terdiagnosis Aterosklerosis Melalui Pelayanan Home Pharmacy Care rdquo; yang bertujuan untuk melakukan monitoring terhadap terapi pasien lansia dengan penyakit aterosklerosis melalui pelayanan Home Pharmacy Care untuk mencapai keberhasilan terapi yang diterima pasien.

Internship at Kimia Farma No. 352 Pharmacy is conducted for one month in February 2017. The Internship aimed the pharmacist able to understand the duties and responsibilities of the pharmacist in pharmacy management in accordance with the standard of pharmaceutical service had the insight, knowledge, skills, and practical experience to practice pharmaceuticals in pharmacies and had an overview of pharmaceutical practice issues and learn strategies and activities that can be done in order to develop pharmaceutical practices. While the special tasks in the internship titled Giving of Drug Information In Patients with a Diagnosis of Atherosclerosis Through Home Pharmacy Care which aimed to monitor the treatment of elderly patients with atherosclerotic disease through Home Pharmacy Care to achieve goal therapy that received the patient."

Praktik Kerja Profesi Apoteker di Puskesmas Kecamatan Tamansari Jakarta Barat Periode Bulan Januari Tahun 2017 dilakukan selama tiga minggu. Praktik Kerja Profesi Apoteker ini bertujuan agar calon apoteker mampu memahami peranan, tugas dan tanggung jawab apoteker dalam praktik pelayanan kefarmasian di Puskesmas sesuai dengan ketentuan perundangan dan etika farmasi yang berlaku, dan dalam bidang kesehatan masyarakat; memahami dan memiliki pengetahuan, keterampilan, sikap perilaku serta wawasan dan pengalaman nyata untuk melakukan praktik profesi dan pekerjaan kefarmasian di Puskesmas; melihat dan mempelajari strategi dan pengembangan praktik profesi Apoteker di Puskesmas; memiliki gambaran nyata tentang permasalahan problem-solving praktik dan pekerjaan kefarmasian di Puskesmas; dan mampu berkomunikasi dan berinteraksi dengan tenaga kesehatan lain yang bertugas di Puskesmas. Sedangkan tugas khusus yang dilakukan selama Praktik Kerja Profesi Apoteker berjudul ldquo;Analisis Ketepatan Dosis Amoksisilin pada Pasien Balita yang Terdiagnosa Infeksi Saluran Pernapasan Akut ISPA Atas di Puskesmas Kecamatan Tamansari rdquo; bertujuan untuk mengetahui ketepatan dosis amoksisilin pada pasien poli MTBS yang terdiagnosa ISPA atas dan diberi antibiotik di Puskesmas Kecematan Tamansari Jakarta Barat.
ABSTRACT
Internship at Tamansari Community Health Center West Jakarta Month Period January 2017 carried out for three weeks. The Internship aimed to enable the pharmacist to understand the role, duties and responsibilities in the practice of pharmaceutical services at the Community Health Center in accordance with applicable laws and pharmaceutical ethics, and in the field of public health had the insight, skills, attitude and real experience to practice profession and pharmaceutical work in Tamansari Community Health Center view and learn strategies and development of profession practice of Apothecary in Tamansari Community Health Center had an overview of the issue problem solving and employment practices in the health center pharmacy and able to communicated and interact with other health personnel who served at the Tamansari Community Health Center. While the special tasks in the internship titled Analysis of Accuracy Amoxicillin Dosage in Patients Under Fives Age Infected with Acute Upper Respiratory Infection ISPA Above at Tamansari Community Health Center aimed to determine the accuracy of doses of amoxicillin in MTBS poly in patients who diagnosed with upper respiratory tract and given antibiotics at the Tamansari Tamansari Community Health Center."

Dalam praktik kerja profesi diberikan pengetahuan, ketrampilan, sikap perilaku, serta wawasan dan pengalaman nyata untuk melakukan praktik profesi dan pekerjaan kefarmasian di puskesmas. Hal-hal lain yang dipelajari selama praktik kerja profesi di puskesmas antara lain mengenai strategi dan pengembangan praktik profesi apoteker; gambaran nyata tentang permasalahan praktik dan pekerjaan kefarmasian; berkomunikasi dan berinteraksi dengan tenaga kesehatan lain yang bertugas. Tugas khusus yang diberikan berjudul ldquo;Analisis Kelengkapan Administratif dan Kesesuaian Dosis Amoxicillin pada Resep Racikan di Puskesmas Kecamatan Kalideres rdquo;. Tugas khusus dilakukan untuk mengetahui persentase kelengkapan administratif resep racikan di Puskesmas Kecamatan Kalideres berdasarkan peraturan yang berlaku dan kesesuaian dosis amoxicillin pada resep racikan yang diberikan untuk anak-anak.
ABSTRACT
Internship at Puskesmas was conducted to enable pharmacist students to recognize job description, positions, and responsibilities of pharmacists in pharmacy services. During internship, they were given knowledge, skills, and practical experiences on pharmacy practices at Puskesmas. The other things learned in internship were strategies and development of pharmacy practices illustrations of pharmacy practices obstacles and how to communicate with other health practitioners. A specific assignment was given, titled ldquo Analysis of Standards Administrative Prescription and Conformity of Amoxicillin Doses in Puskesmas Kecamatan Kalideres rdquo . This assignment aimed to know percentage of standards administrative prescriptions at Puskesmas Kecamatan Kalideres based on regulation and adjusting amoxicillin doses for children."

Praktik kerja profesi di apotek dilakukan agar calon apoteker mampu memahami tugas dan tanggung jawab apoteker dalam pengelolaan apotek, serta melakukan praktik pelayanan kefarmasian sesuai dengan ketentuan perundang-undangan yang berlaku. Apoteker di apotek harus memiliki wawasan, pengetahuan, keterampilan, dan pengalaman praktis untuk melakukan praktik kefarmasian di apotek serta gambaran nyata tentang permasalahan praktik kefarmasian dan mempelajari strategi juga kegiatan-kegiatan yang dapat dilakukan dalam rangka pengembangan praktik kefarmasian di apotek. Tugas khusus yang diberikan berjudul ldquo;Pengkajian Resep Terapi Kolesterol di Apotek Kimia Farma No.143 rdquo;. Tugas khusus dilakukan untuk mengkaji kelengkapan persyaratan resep terapi kolesterol di Apotek Kimia Farma No. 143.
ABSTRACT
Internship at apotek was conducted to enable pharmacist students to recognize job description and responsibilities of pharmacist in apotek management, and to practice pharmacy services based on regulation. Pharmacists should have knowledge, skills, practical experiences to practice pharmacy services, as well as a clear picture of pharmaceutical practice issues, and to learn strategies as well as activities that can be undertaken in order to develop pharmaceutical practices in pharmacies. A specific assignment was given titled ldquo Assessment of Cholesterol Therapy Prescription at Apotek Kimia Farma No. 143 rdquo . This specific assignment was done to examined the completeness of the cholesterol therapy prescription requirements at Apotek Kimia Farma No.143"
