Abstrak :
Praktek Kerja Profesi PKP di Puskesmas Kecamatan Tambora dilaksanakan pada bulan Oktober 2016. Kegiatan PKP ini bertujuan agar mahasiswa memahami peranan, tugas dan tanggung jawab Apoteker, mendapatkan ketrampilan dan pengalaman mengenai kegiatan manajerial dan pelayanan farmasi klinis, melihat dan mempelajari strategi dan pengembangan profesi Apoteker, memperoleh gambaran dalam menghadapi permasalahan pada praktik kefarmasian, serta mampu berinteraksi dengan tenaga kesehatan lain di puskesmas. Tugas khusus yang diberikan berjudul Evaluasi Kesesuaian Penggunaan Obat dengan Formularium Nasional di Puskesmas Kecamatan Tambora Bulan Juli ndash; Desember 2015 Menggunakan Sistem Klasifikasi ATC/DDD. Tugas khusus ini bertujuan untuk mengevaluasi kesesuaian penggunaan obat bulan Juli hingga Desember 2015 yang memiliki ATC/DDD di Puskesmas Kecamatan Tambora dengan Formularium Nasional yang dibandingkan dengan kesesuaian target yang ditetapkan oleh Direktorat Bina Pelayanan Kefarmasian pada tahun 2015. ......Profession Internship at Puskesmas Kecamatan Tambora was held on Oktober, 2016. This internship aims so that student can understand the role, duties and responsibilities of the Pharmacist, gaining the skills and experiences on managerial activities and clinical pharmacy services, looking and learning the strategies and the development of the Pharmacist profession, obtaining an overview in the face of problems on pharmaceutical practices, as well as being able to interact with other health care personnel at Puskesmas. The specific assignment was given by title Evaluation of The Suitability of The Drugs Used at Puskesmas Kecamatan Tambora with National Formularium from July to December 2015 Using ATC DDD Classification System. The aim of this specific assignment was to evaluate the suitability of drugs used in July through December 2015 which has ATC DDD code at Puskesmas Kecamatan Tambora wih national formularium compared with target set by the Directorate of Community Development Pharmaceutical Services by 2015.

Puskesmas sebagai fasilitas kesehatan tingkat pertama, bertanggung jawab dalam kegiatan pelayanan kesehatan secara menyeluruh, terpadu, dan berkesinambungan. Dalam rangka melaksanakan program JKN dan memenuhi himbauan dari Kepala Dinas Kesehatan DKI Jakarta terkait akreditasi, Puskesmas Kecamatan Ciracas telah menyusun pedoman berupa Formularium Puskesmas. Pemilihan obat untuk Formularium Puskesmas mengacu pada daftar Obat Esensial Nasional (DOEN) dan Formularium Nasional (Fornas). Formularium tersebut digunakan sebagai acuan oleh Puskesmas Kecamatan Ciracas dalam menjamin ketersediaan obat yang berkhasiat, bermutu, aman, dan terjangkau. Tujuan dari laporan PKPA ini adalah untuk memahami alur penyusunan formularium puskesmas serta mengevaluasi kesesuaian daftar obat di formularium puskesmas dengan formularium nasional. Laporan ini didasarkan dari penelusuran literatur dan observasi data formularium puskesmas serta wawancara dengan apoteker yang terlibat dalam penyusunan formularium puskesmas. Berdasarkan analisis, alur penyusunan formularium telah sesuai dengan prosedur yang berlaku dan dilakukan evaluasi satu tahun sekali. Daftar obat di formularium puskesmas secara keseluruhan telah sesuai dengan formularium nasional yang berlaku. Namun, terdapat beberapa obat kategori fasilitas kesehatan tingkat 2 dan/atau 3 yang tetap dimasukkan ke Formularium Puskesmas Kecamatan Ciracas Tahun 2023 didasarkan atas hasil rapat dan disertai kajian/justifikasi dari dokter yang mengusulkan. ......Puskesmas, as the first-level health facility, is responsible for comprehensive, integrated, and sustainable health service activities. In order to implement the JKN program and comply with the appeal from the Head of the DKI Jakarta Health Office regarding accreditation, the ciracas subdistrict health center has developed guidelines in the form of a Puskesmas Formulary. The selection of drugs for the Puskesmas Formulary refers to the National Essential Medicines List and the National Formulary. The formulary is used as a reference by the ciracas subdistrict health center to ensure the availability of efficacious, quality, safe, and affordable medicines. The purpose of this PKPA report is to understand the flow of the formulation of the puskesmas formulary and to evaluate the conformity of the drug lists in the puskesmas formulary with the national formulary. This report is based on a literature search and observation of puskesmas formulary data, as well as interviews with pharmacists who were involved in preparing the puskesmas formulary. Based on the analysis, the flow of the formulation of the formulary is in accordance with the applicable procedures and is evaluated once a year. The list of medicines in the Pukkesmas formulary as a whole is in accordance with the applicable national formulary. However, there are several level 2 and/or 3 health facility category drugs that are still included in the ciracas subdistrict health center formulary based on the results of the meeting and accompanied by a review or justification from the doctor who proposed it.

Pelayanan kefarmasian di Puskesmas merupakan salah satu hal yang dapat menunjang pelayanan kesehatan yang bermutu karena sifatnya yang langsung bertanggung jawab kepada pasien. Salah satu pelayanan kefarmasian yang penting di puskesmas yaitu pelayanan resep. Peresepan yang baik dapat meningkatkan penggunaan obat secara rasional sehingga pasien menerima obat sesuai dengan indikasi klinis, dalam dosis yang tepat, untuk jangka waktu yang cukup, serta dengan biaya yang rendah. Untuk itu, diperlukan suatu daftar (formularium) dari obat yang harus tersedia dalam fasilitas kesehatan tingkat pertama. Formularium Nasional berfungsi sebagai acuan atau pedoman bagi penyedia layanan kesehatan yang bertujuan untuk menyediakan obat-obatan yang aman, berkhasiat, bermutu, dan terjangkau dalam jenis dan jumlah tertentu. Namun, pengelolaan obat yang tidak efisien dapat memberikan dampak negatif, baik secara medis maupun ekonomi. Oleh karena itu, perlu adanya seleksi obat yang tepat melalui sistem formularium puskesmas untuk meningkatkan mutu terapi obat dan menurunkan kejadian efek samping obat. Analisis formularium nasional dilakukan dengan cara mendata obat-obat untuk dimasukkan ke formularium puskesmas lalu membandingkannya dengan Rencana Kebutuhan Obat (RKO) tahun 2022 dan tahun 2023. Daftar obat yang disusun sebagai formularium puskesmas didasarkan pada formularium nasional tahun 2021. Obat-obatan yang telah diseleksi dari formularium nasional lalu dibandingkan dengan RKO tahun 2022 dan tahun 2023. Dari hasil perbandingan, terdapat obat-obatan yang termasuk ke dalam RKO 2022 namun tidak termasuk dalam RKO 2023. Selain pelayanan resep, apoteker sebagai tenaga kesehatan di puskesmas memiliki tugas untuk meningkatkan kesejahteraan masyarakat, salah satunya dengan memberikan edukasi kesehatan kepada pengunjung puskesmas. Pemberian edukasi menggunakan alat bantu berupa leaflet dapat memudahkan peserta edukasi untuk memahami materi yang disampaikan. Pembuatan leaflet dilakukan dengan metode studi literatur, serta pelaksanaan edukasi kesehatan dilakukan dengan penyuluhan singkat serta penyebaran leaflet pada peserta penyuluhan. Pelaksanaan penyuluhan singkat menggunakan leaflet sebagai alat bantu sangat memudahkan materi sampai kepada peserta penyuluhan dan dipahami dengan baik. ......Pharmaceutical services at Community Health Centers are one of the things that can support quality health services because they are directly responsible to patients. One of the important pharmaceutical services at community health centers is prescription services. Good drug prescribing can increase the rational use of drugs so that patients receive drugs according to clinical indications, in the right dose, for a sufficient period of time, and at a low cost. For this reason, a list (formulary) of drugs that must be available in first-level health facilities is needed. The National Formulary functions as a reference or guideline for health service providers whose aim is to provide safe, efficacious, quality and affordable medicines in certain types and quantities. However, inefficient drug management can have negative impacts, both medically and economically. Therefore, there is a need for appropriate drug selection through the health center formulary system to improve the quality of drug therapy and reduce the incidence of drug side effects. National formulary analysis is carried out by listing the drugs to be included in the health center formulary and then comparing it with the 2022 and 2023 Drug Needs Plans (RKO). The list of drugs compiled as a health center formulary is based on the 2021 national formulary. from the national formulary and then compared with the 2022 and 2023 RKO. From the comparison results, there are medicines that are included in the 2022 RKO but are not included in the 2023 RKO. Apart from prescription services, pharmacists as health workers at community health centers have the task of improving community welfare, one of which is by providing health education to health center visitors. Providing education using tools in the form of leaflets can make it easier for education participants to understand the material presented. Making leaflets was carried out using the literature study method, and the implementation of health education was carried out by providing short counseling and distributing leaflets to counseling participants. Carrying out short counseling using leaflets as a tool really makes it easier for the material to reach the counseling participants and be understood well.
