"ABSTRAK
Praktik Kerja Profesi (PKP) di rumah sakit dilaksanakan di Rumah Sakit Anak
dan Bunda Harapan Kita. Kegiatan ini berlangsung selama 6 minggu dari tanggal
7 September sampai dengan 16 Oktober 2015. PKP di rumah sakit bertujuan agar
mahasiswa apoteker mengerti peranan apoteker dalam pengelolaan sediaan
farmasi, alat kesehatan, dan bahan medis habis pakai, dan pelayanan farmasi
klinik, serta bekerjasama dengan profesi kesehatan lain. Berdasarkan kegiatan
PKP yang dilakukan, dapat disimpulkan bahwa Apoteker di Rumah Sakit Anak
dan Bunda Harapan Kita memiliki peran, tugas, dan tanggung jawab dalam
pengelolaan sediaan farmasi, alat kesehatan, dan bahan medis habis pakai serta
pelayanan farmasi klinik. Mahasiswa apoteker telah berhasil memperoleh
wawasan mengenai kegiatan rutin yang dilakukan di rumah sakit. Kegiatan
pengelolaan meliputi pemilihan, perencanaan kebutuhan, pengadaan, penerimaan,
penyimpanan, pendistribusian, pemusnahan, pengendalian dan administrasi, serta
kegiatan farmasi klinis meliputi pengkajian dan pelayanan resep, pelayanan
informasi obat, visite, dan dispensing sediaan steril (penanganan obat sitostatik).
Kerjasama dengan tenaga kesehatan lain bertujuan untuk menjamin keselamatan
pasien demi terciptanya pelayanan kefarmasian yang bermutu.

Pengaruh komunikasi apoteker - dokter terhadap peresepan antibiotik pada anak di RS Proklamasi- Karawang = Influence of pharmacist doctor communication on antibiotic prescription's pediatric in Proklamasi Hospital Karawang
"ABSTRAK
Masalah terkait obat merupakan suatu kegiatan farmasi klinik yang memiliki peran penting menunjang keberhasilan terapi pasien. Tujuan penelitian adalah mendeskripsikan permasalahan terkait obat dan menilai pengaruh komunikasi apoteker kepada dokter terhadap penurunan masalah terkait obat pada resep anak yang mengandung antibiotik di RS Proklamasi Karawang. Rancangan penelitian menggunakan pre-experimental design. Intervensi berupa informasi masalah terkait obat oleh apoteker kepada dokter penulis resep pasien anak dengan antibiotik. Sampel penelitian berupa 338 lembar resep pasien anak rawat jalan yang menggunakan antibiotik dan diambil secara prospesktif sejak awal bulan Agustus sampai akhir September 2016. Analisis data berupa distribusi frekuensi dan analisis bivariat uji Wilcoxon. Hasil penelitian menunjukkan bahwa Masalah terkait obat yang tercatat sebesar 62,22% dari 338 lembar resep. Permasalahan terbesar terjadi disebabkan oleh efektifitas terapi (P1) 79,3% dan masalah biaya (20,7%). Jenis masalah terkait obat yang terjadi karena frekuensi pemberian obat berlebih (80,1%), pemilihan obat kurang tepatdosis obat berlebih 17,2%, interaksi obat 9,6%, pemilihan obat kurang tepat (12,2%%), kurang dosis (4,1%), dan dosis obat berlebih (0,5%). Pada kelompok biaya terapi faktor penyebab adalah tidak ada indikasi (84,1%) dan pemilihan obat yang tidak tepat (15,9%). Pengaruh komunikasi apoteker kepada dokter terhadap penurunan masalah terkait obat pada resep anak rawat jalan secara statistik bermakna (p < 0,05). Komunikasi apoteker terhadap dokter dapat menurunkan masalah terkait obat sebesar 22,9%.
ABSTRACT
Drug related problems, a clinical pharmacy activities that have an important role to the success of patient therapy. The purpose of research is to describe drug-related issues and assess the effect of communication pharmacists to physicians to the decrease of prescription drug related problems in children containing antibiotics in hospitals Proklamasi Karawang. The study design using pre-experimental design. Intervention in the form of information-related issues of drugs by pharmacists to the prescribers of pediatric patients with antibiotics. The research sample in the form of 338 sheets of prescription pediatric outpatient use of antibiotics and taken prospesktif since the beginning of August until late September 2016. Analysis of the data in the form of a frequency distribution and bivariate analysis Wilcoxon test. The results showed that the drug-related problem that was recorded at 62.22% of 338 sheets of recipes. The biggest problems occur due to the effectiveness of therapy (P1) 79.3% and cost issues (20.7%). Type of drug-related problems that occur because of excessive frequency of drug administration (80.1%), less drug selection drug tepatdosis excess of 17.2%, 9.6% drug interactions, drug selection is less precise (12.2 %%), less dose (4.1%), and excessive drug dose (0.5%). In group therapy costs causative factor is no indication (84.1%) and improper drug selection (15.9%). Influence of communication pharmacists to physicians to the reduction of drug-related problems in children's outpatient prescription statistically significant (p <0.05). Communication pharmacists to physicians can reduce drug related problems, by 22.9%"

"Apotek Kimia Farma Unit 389 merupakan salah satu apotek yang melayani resep pasien BPJS di Depok. Laporan ini disusun untuk mengkaji dan menganalisis resep di Apotek Kimia Farma Unit 389 Depok sehingga dapat memberikan informasi mengenai kelengkapan resep dan kejadian 'drug related problems' (DRPs) pada resep. Sampel yang digunakan berjumlah lima resep pasien BPJS kronis poli penyakit dalam di Apotek Kimia Farma Unit 389 periode April 2023. Hasil pengkajian dan analisis resep menunjukkan bahwa semua resep tidak lengkap ditinjau dari aspek administratif, aspek farmasetik, dan aspek klinis berdasarkan Peraturan Menteri Kesehatan Nomor 73 Tahun 2016 tentang Standar Pelayanan Kefarmasian di Apotek. Kejadian DRPs terjadi pada semua resep. DRPs yang berkaitan dengan masalah efektivitas terapi terjadi pada resep 1 dan 3, sedangkan yang berkaitan dengan masalah keamanan terapi terjadi pada resep 1, 2, 4, dan 5.
Kimia Farma Pharmacy Unit 389 is one of the pharmacies that serve prescriptions for BPJS patients in Depok. This report was prepared to assess and analyze prescriptions at the Kimia Farma Pharmacy Unit 389 Depok so that it can provide information about the completeness of prescriptions and the incidence of drug related problems (DRPs) in prescriptions. The sample used consisted of five prescriptions for chronic BPJS patients of internal medicine poly at Kimia Farma Pharmacy Unit 389 for the period of April 2023. The results of the assessment and analysis of prescriptions showed that all prescriptions were incomplete in terms of administrative aspects, pharmaceutical aspects, and clinical aspects based on the Regulation of the Minister of Health Number 73 of 2016 concerning Pharmaceutical Service Standards in Pharmacies. DRPs occurred in all prescriptions. DRPs related to therapeutic effectiveness issues occurred in prescriptions 1 and 3, while those related to therapeutic safety issues occurred in prescriptions 1, 2, 4, and 5."

"Laporan Praktik Kerja Profesi Apoteker ini membahas terkait kesesuaian resep pasien BPJS Kronis Poli Jantung dan Pembuluh di Apotek Kimia Farma 0389 jika ditinjau dari segi administratif, farmasetik, dan klinis yang dianalisis melalui hasil DRPs (Drug Related Problems) yang terjadi. Penelitian ini dilakukan secara observasional dengan metode deskriptif. Dari hasil penelitian ini disarankan agar kegiatan pemberian informasi obat dan konseling oleh apoteker lebih ditingkatkan terutama pada pasien polifarmasi yang memiliki risiko interaksi antara obat yang digunakan dan perlu adanya peningkatan pengetahuan bagi pihak rumah sakit atau penulis resep melalui sosialisasi peraturan obat yang dipersyaratkan oleh BPJS sehingga pasien bisa mengklaim obat sesuai dengan apa yang doketr resepkan untuk menghindari indikasi yang tidak diobatai karena ketidaktersediaan obat dan fakktor tidak sesuai persyaratan BPJS.
This Pharmacist Professional Work Practices Report discusses the suitability of prescriptions for BPJS Chronic Heart and Vessel Poly patients at Kimia Farma 0389 Pharmacy when viewed from an administrative, pharmaceutical, and clinical perspective which is analyzed through the results of the DRPs (Drug Related Problems) that occur. This research was conducted observationally using descriptive methods. From the results of this research, it is recommended that the activities of providing drug information and counseling by pharmacists be improved, especially for poly pharmacy patients who have a risk of interactions between the drugs used and there needs to be increased knowledge for hospitals or prescribers through socialization of drug regulations required by BPJS so that patients can claim medicine according to what the doctor prescribed to avoid indications that are not treated due to unavailability of medicine and factors that do not meet BPJS requirements."

Pengembangan basis data Drug Related Problems pada pasien dengan hipertensi di RSUD Ulin Banjarmasin
"Drug related problems (DRPs) adalah suatu keadaan yang tidak diinginkan yang terjadi pada pasien yang disebabkan oleh terapi obat dan secara nyata atau potensial mengurangi hasil terapi yang diharapkan.
Sistem informasi drug related problems merupakan sistem informasi untuk mengetahui DRPs yang terjadi pada pasien yang sedang menjalani terapi obat yang diberikan dokter, agar dapat diatasi dan dicegah adanya masalah yang ditimbulkan diluar penyakit yang sedang diterapi. Supaya sistem dapat berjalan dengan baik harus didukung informasi yang lengkap dan terdokumentasi dengan baik.
Sistem informasi drug related problems di RSUD Ulin Banjarmasin belum ada. Selama ini data tentang DRPs tidak terdokumentasi dengan baik dan lengkap. Tidak ada basis data yang mendukung sistem tersebut. Penanganan kejadian DRPs bersifat tindakan langsung berdasarkan respon pasien terhadap obat dan keluhan pasien. Hal itu disebabkan tidak ada SOP yang mengaturnya.
Tujuan pengembangan sistem informasi drug related problems adalah membuat sistem informasi yang dapat mengetahui adanya DRPs yang terjadi pada pasien dengan penambahan formulir khusus DRPs dan pembuatan basis data DRPs.
Metode pengumpulan data yang digunakan adalah wawancara mendalam dan studi dokumentasi. Pengembangan sistem menghasilkan formulir pemantauan obat untuk yang memuat kejadian DRPs dan prototype DRPs dengan basis data. Hasil uji coba prototype menunjukkan sistem berjalan baik berdasarkan input, proses, basis data, kendali dan platform teknologi.
Hasil analisis DRPs dari 109 orang pasien hipertensi di RSUD Ulin Banjarmasin berdasarkan data sekunder tahun 2003 ditemukan pasien yang mengalami DRPs 49 orang (44,95%). Proporsi jenis DRPs yang ditemukan paling banyak adalah interaksi obat 54,2%, efek samping obat 33,9% dan duplikasi obat 11,9%.
Drug related problems (DRPs) is an unwanted condition that occurred in patient caused by drugs therapy, actual and potentially reduced the expected therapy results.
Information system of drug related problems is an information system to find out DRPs which occurred in patient who endure on drug therapy which has been given by doctor, in order to handle and prevent the problems that may be occurred beyond the disease that being treated. The system can be going on all right, if be supported by complete information and well documented.
There is no information system for drug related problems in RSUD Ulin Banjarmasin. Until now, data of DRPs uncompleted and not well documented. No databases support to this system. Incident DRPs handling only by direct action based on patient response to drugs and patient complaints. That case caused no standard operational procedure to arrange.
The purpose of development information system of drug related problems is to build information system which could find DRPs that occur in patient by adding special form for drugs and DRPs monitoring and establishing DRPs database.
Results of the prototype testing showed that the system working well based on input, process, output, control and technology platform.
Result of DRPs from 109 patients with hypertension in RSUD Ulin Banjarmasin based on secondary data year of 2003 found that patient who experience DRPs is 49 patients (45,9%). The high proportion the type of DRPs that has been found are drug interactions (54,2%), adverse drug reactions (33,8%) and drug duplications (11,9%)"

"ABSTRAK
Praktek Kerja Profesi Apoteker dilaksanakan di Puskesmas Kecamatan Kebayoran Lama Jl. Ciputat Raya, Kebayoran Lama, Jakarta Selatan. Kegiatan PKPA bertujuan agar mahasiswa dapat melihat langsung kegiatan kefarmasian yang dilakukan, memahami tugas dan fungsi Instalasi Farmasi Puskesmas Kecamatan Kebayoran Lama, memahami peran dan tanggung jawab apoteker di puskesmas serta meningkatkan pengetahuan dan kemampuan dalam bidang manajerial (perencanaan, pengadaan,penerimaan, penyimpanan, distribusi, pemusnahan, pengendalian dan administrasi) dan farmasi klinis (pengkajian dan pelayanan resep, Pemantauan Terapi Obat (PTO), Pemberian Informasi Obat (PIO), konseling. Tugas khusus yang diberikan berjudul analisis pola peresepan dan identifikasi terjadinya drug related problems pada pasien hipertensi periode agustus 2018. Tujuan dari tugas khusus ini adalah untuk mengetahui masalah pola peresepan dan tanggung jawab apoteker pada pasien hipertensi yang dapat mengakibatkan drug related problems.

"Praktik kerja profesi di Apotek Kimia Farma nomor 551 Bosih periode bulan September Tahun 2018 bertujuan untuk mengetahui peran dan tanggung jawab Apoteker dalam melaksanakan pengelolaan dan pelayanan di Apotek sesuai dengan perundang-undangan yang berlaku, mendapatkan wawasan, pengetahuan, keterampilan dan pengalaman praktis untuk melakukan praktik kefarmasian di Apotek, dan agar mendapatkan gambaran nyata tentang permasalahan praktik kefarmasian. Praktik kerja profesi ini dilaksanakan selama satu bulan. Selama praktik kerja profesi diberikan tugas khusus mengenai Skrining Resep dan Analisis Drug Related Problems dari Resep Pasien Hipertensi di Apotek Kimia Farma Nomor 551 Bosih Periode September 2018. Tujuan dari pelaksanaan tugas khusus ini adalah untuk mengevaluasi kelangkapan administratif, farmasetik, dan klinis resep pasien hipertensi dengan drug related problems (DRPs) dan menganalisa permasalahan terkait obat yang ada pada resep yang mengandung obat hipertensi. Dari hasil analisis yang dilakukan dapat diketahui bahwa salah satu resep pasien hipertensi ditemukan adanya DRPs kategori pemilihan obat dan DRPs kategori bentuk sediaan yang dapat menjadi potensi tidak tercapainya pengobatan yang diharapkan.

This professional work practice is carried out for one month. During the work practice the profession was given a special assignment on Skrining Resep dan Analisis Drug Related Problems dari Resep Pasien Hipertensi di Apotek Kimia Farma Nomor 551 Bosih Periode September 2018. The purpose of implementing this particular task is to evaluate the administrative, pharmacetic, and clinical prescriptions of hypertensive patients with drug related problems (DRPs) and analyze problems related to drugs in prescriptions that contain hypertension drugs. From the results of the analysis carried out it can be seen that one of the prescriptions of hypertensive patients found DRPs in the category of drug selection and DRPs in the category of dosage forms that could be potential for not achieving the expected treatment."

"ABSTRAK
Praktek Kerja Profesi Apoteker di Rumah Sakit Anak dan Bunda Harapan Kita dilakukan selama 6 enam minggu sejak tanggal 1 Maret sampai dengan 11 April 2017. Tujuan kegiatan ini adalah agar mahasiswa memahami peranan, tugas dan tanggung jawab apoteker di Rumah Sakit sesuai dengan ketentuan Peraturan Menteri Kesehatan No. 72 Tahun 2016; memiliki wawasan, pengetahuan, keterampilan, sikap perilaku profesional dan pengalaman nyata untuk melakukan praktek profesi dan pekerjaan kefarmasian di Rumah Sakit; dan memiliki gambaran nyata tentang permasalahan praktik kefarmasian serta mempelajari strategi dan kegiatan-kegiatan yang dapat dilakukan dalam rangka pengembangan praktek kefarmasian di Rumah Sakit. Kegiatan pelayanan farmasi klinis yang sudah dilakukan Rumah Sakit Anak dan Bunda Harapan Kita diantaranya pengkajian dan pelayanan resep, pelayanan informasi obat, rekonsiliasi, visite, dan dispensing sediaan steril. Tugas khusus yang diberikan berjudul Analisis Drug Related Problem pada Pasien Anak Z dengan Diagnosa Sindrom Nefrotik di Ruang Rawat Inap Gambir RSAB Harapan Kita. Pelayanan farmasi klinik belum dilakukan sepenuhnya dikarenakan jumlah sumber daya manusia yang tidak memadai.
ABSTRACT
Internship at Children 39 s and Mother 39 s Hospital Harapan Kita held for six 6 weeks from March 1st to April 11th 2017. The internship was intended to make pharmacist students understand the role, duties and responsibilities of pharmacists in hospitals appropriate with the Regulation of the Minister of Health No. 72 in 2016 have insight, knowledge, skills, professionalism and real experience to do pharmaceutical practice in hospital have a real experience of the problems pharmacy practice and learn the strategies and activities that can be done in order to develop the pharmaceutical practice in hospital. Clinical pharmacy activities that are conducted in Children 39 s and Mother 39 s Hospital Harapan Kita such as assessment and prescription care, drug information care, reconciliation, visite, and dispensing sterile preparations. Special assignment was given that titled Analysis of Drug Related Problems in Pediatric Z Children with Diagnosis of Nephrotic Syndrome in the Gambir Room Children 39 s and Mother 39 s Hospital Harapan Kita. However, the activities carried out clinical pharmacy not yet fully due to the amount of human resources inadequate."

Praktek kerja profesi di Rumah Sakit Pusat Angkatan Darat RSPAD Gatot Soebroto Ditkesad periode bulan Januari ndash; Februari tahun 2017 bertujuan untuk memahami peranan, tugas dan tanggung jawab Apoteker di Rumah Sakit sesuai dengan ketentuan dan etika pelayanan farmasi pada khususnya dan pelayanan kesehatan pada umumnya, memiliki wawasan, pengetahuan, keterampilan dan pengalaman praktis untuk melakukan praktek kefarmasian di Rumah Sakit serta memiliki gambaran nyata tentang permasalahan praktek kefarmasian serta mempelajari strategi dan kegiatan-kegiatan yang dapat dilakukan dalam rangka perkembangan praktek kefarmasian di Rumah Sakit. Praktek kerja profesi dilakukan selama dua bulan dengan tugas khusus yaitu identifikasi drug related problems drps pada pasien pneumonia geriatri komplikasi congestive heart failure fc IV pada OMI, dan hipertensi gr II di Ruang Rawat Inap Lantai 4 Paru Rumah Sakit Pusat Angkatan Darat RSPAD Gatot Soebroto Ditkesad. ABSTRACT
The aims of apothecary profession internship program at Rumah Sakit Pusat Tni Angkatan Darat Gatot Soebroto Ditkesad on January February 2017 are to understand the duties and responsibilities clinical pharmacists in accordance with the provisions and ethics in particular thepharmaceutical and healthcare service in general, have insight, knowledge, skill and experience the practical for doing pharmaceutical care in hospitals and having an example about pharmaceutical care and learning strategy and activities can be implemented in pharmaceutical care to the hospital. The apothecary profession internship program conducted during two months in a special assignment that is given entitled the identification of drug related problems drps in geriatric pneumonia with congestive heart failure fc IV on OMI complication, and hypertension gr II in Inpatient Room on 4th Floor in Rumah Sakit Pusat Angkatan Darat RSPAD Gatot Soebroto Ditkesad.
