ABSTRAK
Praktik Kerja Profesi (PKP) di rumah sakit dilaksanakan di Rumah Sakit Anak dan Bunda Harapan Kita. Kegiatan ini berlangsung selama 6 minggu dari tanggal 7 September sampai dengan 16 Oktober 2015. PKP di rumah sakit bertujuan agar mahasiswa apoteker mengerti peranan apoteker dalam pengelolaan sediaan farmasi, alat kesehatan, dan bahan medis habis pakai, dan pelayanan farmasi klinik, serta bekerjasama dengan profesi kesehatan lain. Berdasarkan kegiatan PKP yang dilakukan, dapat disimpulkan bahwa Apoteker di Rumah Sakit Anak dan Bunda Harapan Kita memiliki peran, tugas, dan tanggung jawab dalam pengelolaan sediaan farmasi, alat kesehatan, dan bahan medis habis pakai serta pelayanan farmasi klinik. Mahasiswa apoteker telah berhasil memperoleh wawasan mengenai kegiatan rutin yang dilakukan di rumah sakit. Kegiatan pengelolaan meliputi pemilihan, perencanaan kebutuhan, pengadaan, penerimaan, penyimpanan, pendistribusian, pemusnahan, pengendalian dan administrasi, serta kegiatan farmasi klinis meliputi pengkajian dan pelayanan resep, pelayanan informasi obat, visite, dan dispensing sediaan steril (penanganan obat sitostatik). Kerjasama dengan tenaga kesehatan lain bertujuan untuk menjamin keselamatan pasien demi terciptanya pelayanan kefarmasian yang bermutu. Profession Internship at pharmacy was intended to make apothecary student understand the role of pharmacists, have insight into the implementation of pharmaceutical practice, learning strategies and the development of professional practice of pharmacists, have real description of the issues of pharmaceutical practice in health centers, as well as to communicate and interact with other health professionals. Based on the activities, it can be concluded that the pharmacist at Women's and Children's Hospital Harapan Kita have a role, duties and responsibilities in the management of pharmaceutical, medical devices, and medical materials and consumables pharmaceutical care clinic. Apothecary student have obtained insight into routine activities in pharmacy. Pharmaceutical technical activities include selecting, requirement planning, procurement, reception, storage, extermination, control and administration, and activities include assessment of clinical pharmacy services and prescription drug information service, visite, and dispensing sterile preparations (handling cytostatic drugs). Pengaruh komunikasi apoteker - dokter terhadap peresepan antibiotik pada anak di RS Proklamasi- Karawang = Influence of pharmacist doctor communication on antibiotic prescription's pediatric in Proklamasi Hospital Karawang

ABSTRAK
Masalah terkait obat merupakan suatu kegiatan farmasi klinik yang memiliki peran penting menunjang keberhasilan terapi pasien. Tujuan penelitian adalah mendeskripsikan permasalahan terkait obat dan menilai pengaruh komunikasi apoteker kepada dokter terhadap penurunan masalah terkait obat pada resep anak yang mengandung antibiotik di RS Proklamasi Karawang. Rancangan penelitian menggunakan pre-experimental design. Intervensi berupa informasi masalah terkait obat oleh apoteker kepada dokter penulis resep pasien anak dengan antibiotik. Sampel penelitian berupa 338 lembar resep pasien anak rawat jalan yang menggunakan antibiotik dan diambil secara prospesktif sejak awal bulan Agustus sampai akhir September 2016. Analisis data berupa distribusi frekuensi dan analisis bivariat uji Wilcoxon. Hasil penelitian menunjukkan bahwa Masalah terkait obat yang tercatat sebesar 62,22% dari 338 lembar resep. Permasalahan terbesar terjadi disebabkan oleh efektifitas terapi (P1) 79,3% dan masalah biaya (20,7%). Jenis masalah terkait obat yang terjadi karena frekuensi pemberian obat berlebih (80,1%), pemilihan obat kurang tepatdosis obat berlebih 17,2%, interaksi obat 9,6%, pemilihan obat kurang tepat (12,2%%), kurang dosis (4,1%), dan dosis obat berlebih (0,5%). Pada kelompok biaya terapi faktor penyebab adalah tidak ada indikasi (84,1%) dan pemilihan obat yang tidak tepat (15,9%). Pengaruh komunikasi apoteker kepada dokter terhadap penurunan masalah terkait obat pada resep anak rawat jalan secara statistik bermakna (p < 0,05). Komunikasi apoteker terhadap dokter dapat menurunkan masalah terkait obat sebesar 22,9%.
ABSTRACT
Drug related problems, a clinical pharmacy activities that have an important role to the success of patient therapy. The purpose of research is to describe drug-related issues and assess the effect of communication pharmacists to physicians to the decrease of prescription drug related problems in children containing antibiotics in hospitals Proklamasi Karawang. The study design using pre-experimental design. Intervention in the form of information-related issues of drugs by pharmacists to the prescribers of pediatric patients with antibiotics. The research sample in the form of 338 sheets of prescription pediatric outpatient use of antibiotics and taken prospesktif since the beginning of August until late September 2016. Analysis of the data in the form of a frequency distribution and bivariate analysis Wilcoxon test. The results showed that the drug-related problem that was recorded at 62.22% of 338 sheets of recipes. The biggest problems occur due to the effectiveness of therapy (P1) 79.3% and cost issues (20.7%). Type of drug-related problems that occur because of excessive frequency of drug administration (80.1%), less drug selection drug tepatdosis excess of 17.2%, 9.6% drug interactions, drug selection is less precise (12.2 %%), less dose (4.1%), and excessive drug dose (0.5%). In group therapy costs causative factor is no indication (84.1%) and improper drug selection (15.9%). Influence of communication pharmacists to physicians to the reduction of drug-related problems in children's outpatient prescription statistically significant (p <0.05). Penerjemahan Pharmaceutical Care Network Europe (PCNE Versi 9.00 Bahasa Indonesia Sebagai Instrumen untuk Mendeteksi Masalah Terkait Obat = Translation of the Indonesian Version of Pharmaceutical Care Network Europe v9.00 as an Instrument to Detect Drug-Related Problem.
Abstrak :
Masalah terkait obat merupakan permasalahan yang umum ditemukan khususnya pada pelayanan kesehatan tersier yaitu di rumah sakit. Penelitian ini bertujuan untuk melakukan penerjemahan instrumen Pharmaceutical Care Network Europe (PCNE) ke dalam Bahasa Indonesia yang valid dan reliabel. Sebuah draft terjemahan PCNE versi 9.00 Bahasa Indonesia didiskusikan oleh 4 panel ahli dengan menggunakan metode Delphi dan dinilai oleh 46 apoteker rumah sakit di seluruh Indonesia dari segi keterbacaan, kejelasan, dan komprehensivitas instrumen. Uji reliabilitas antar penilai dilakukan dengan melibatkan 2 apoteker rumah sakit untuk menilai 20 kasus yang dimana akan dilihat persentase kecocokan pemberian kode antar 2 penilai. Instrumen dinyatakan valid setelah diperoleh nilai S-CVI masing-masing sebesar 0,9 ; 0,89 ; 0,93 ; 0,97; dan 0,93 untuk masing-masing domain PCNE. Instrumen pula dinyatakan reliabel dengan nilai persen kecocokan antar kedua penilai sebesar 40-90%. Dengan demikian, instrumen PCNE versi 9.00 Bahasa Indonesia memiliki validitas yang tinggi dan reliabilitas yang cukup untuk dapat digunakan pada studi masalah terkait obat (MTO). ......Drug-related problems are a problem that often occurs, especially in tertiary health services such as hospitals. This study aims to translate the Pharmaceutical Care Network Europe (PCNE) instrument into Bahasa Indonesia which is valid and reliable. A draft of the Indonesian version from the PCNE v9.00 was discussed by four experts using the Delphi method and assessed by 46 hospital pharmacists throughout Indonesia in terms of readability, clarity, and comprehensiveness of the instrument. Two hospital pharmacists carried out the inter-rater reliability test to assess 20 cases where the proportion of coding matches between the two raters would be seen. The instrument was declared valid after the S-CVI value was 0.91; 0.89; 0.93; 0.97; and 0.93 for each PCNE domain respectively. The instrument was also declared reliable, after the inter-rater agreement study with the percent of agreement between the two raters was 40-90%. Therefore, the Indonesian version of the PCNE v9.00 instrument has high validity and fair reliability to use in drug-related problems (DRP) study

ABSTRAK Praktek kerja profesi apoteker di Puskesmas Kecamatan Gambir Jakarta Pusaat memiliki 3 tujuan yaitu yang pertama untuk memahami peranan, tugas dan tanggung jawab apoteker dalam praktek pelayanan kefarmasian di puskesmas sesuai dengan ketentuan perundangan dan etika farmasi yang berlaku; yang kedua adalah agar memiliki pengetahuan, keterampilan, sikap perilaku (profesionalism) serta wawasan dan pengalaman nyata (reality) untuk melakukan praktik profesi dan pekerjaan kefarmasian di puskesmas; serta yang ketiga adalah untuk melihat dan mempelajari strategi dan pengembangan praktik profesi apoteker di puskesmas; memiliki gambaran nyata tentang permasalahan (problem-solving) praktik dan pekerjaan kefarmasian di puskesmas; serta mampu berkomunikasi dan berinteraksi dengan tenaga kesehatan lain yang bertugas di puskesmas. ABSTRACT Pharmacists professional practice at Puskesmas Kecamatan Gambir Center Jakarta has 3 main puposes, first is to understand the duties and responsibilities of pharmacists in puskesmas pharmaceutical care activities accordance with statutory provisions and ethics. Second is to earn knowledge, skills, professional behaviors and the real experiences to carry out pharmacist practices in the puskesmas. And the third purpose is to learn about the strategy to develop pharmaceutical care activities in puskesmas, be able to interact with other healthcare professional profession, and also have a real picture about problem-solving activities inside puskesmas. Given special assignment entitled "Drug Related Problems analysis and the improvement of treatment plan in the diabetes mellitus patient at Puskesmas Kecamatan Gambir January 2016 Period". The purpose of this special assignment are to identify the type and the number of potential or actual DRP's of diabetes mellitus patient's treatments and also to give a better treatment plan to them.

Tujuan pelaksanaan praktek kerja profesi di Rumah Sakit Kepresidenan RSPAD Gatot Soebroto Ditkesad periode Bulan Juli-Agustus tahun 2016 adalah untuk memahami peran, tugas, dan tanggung jawab Apoteker di Rumah Sakit sesuai dengan perundangan dan etika pelayanan kefarmasian, memiliki wawasan, pengetahuan, dan pengalaman praktis untuk melakukan praktek pelayanan kefarmasian dan memiliki gambaran nyata terhadap permasalahan praktek kefarmasian di rumah sakit. Praktek kerja profesi dilakukan selama dua bulan dengan tugas khusus yaitu identifikasi Drug Related Problem pada Pasien dengan CKD on HD, Hipertensi dan Seizure di unit perawatan umum RSPAD Gatot Soebroto Ditkesad. The aims of internship at Rumah Sakit Kepresidenan RSPAD Gatot Soebroto Ditkesad in July August 2015 are to understand the roles, duties, and responsibilities clinical pharmacist in hospital according to regulations and ethics in pharmaceutical care, have insight, knowledge, skill and practical experience for doing pharmaceutical care, and have an example about pharmaceutical care problem in hospital. Internship was held for two months with a special assignment to identify drug related problem in patient with CKD on HD, hypertension and seizure in general care unit Rumah Sakit Kepresidenan RSPAD Gatot Soebroto Ditkesad. The implimentation of Standarization of Pharmaceutical Care in Hospital according to Regulation of Minister of Health No 58 Year 2014 consist of managerial activities of pharmaceutical products, medical devices and single use medical equipments and clinical pharmacy services. The managerial activities of pharmaceutical products, medical devices and single use medical equipments in hospital is already suitable, and clinical pharmacy services that have not been implemented are Drug Use Evaluation and Drug Levels Monitoring in Blood.
