Praktek Kerja Profesi di Apotek Kimia Farma No. 394 Depok bertujuan untuk memahami tugas dan tanggung jawab apoteker dalam pengelolaan apotek serta melakukan pelayanan kefarmasian sesuai dengan peraturan dan etika yang berlaku. Tugas khusus yang diberikan berjudul Pelayanan Informasi Obat dan Home Pharmacy Care terhadap pasien diabetes terkomplikasi hipertensi dan hiperurisemia. Tujuan dari penyusunan tugas khusus ini adalah untuk monitoring terapi pasien dengan terlaksananya pendampingan pengobatan pasien oleh apoteker serta tercipta kerjasama antara apoteker, pasien, dan keluarga dalam pengobatan pasien.

ABSTRACT
Pharmacy intership program in Apotek Kimia Farma No. 394 Depok aims to understand the duties and responsibilities of pharmacists in the management of pharmacies as well as to undertake pharmaceutical services in accordance with applicable rules and ethics. The special task is entitled Drug Information Services and Home Pharmacy Care to patients with complicated diabetes with hypertension and hyperuricemia. The purpose of this special task is to monitor patient therapy with the implementation of patient care assistance by pharmacist and create cooperation between pharmacist, patient, and family in patient treatment.
