Abstrak :
Seiring dengan target pemerintah dalam peningkatan produksi minyak dan gas di lepas pantai, maka penggunaan bahan kimia dalam kegiatan produksi minyak dan gas semakin meningkat, hal ini memunculkan kekhawatiran akan potensi permasalahan kesehatan pekerja, oleh karenanya perlu dilakukan kajian risiko kesehatan. Penelitian ini bertujuan untuk menganalisis tingkat risiko (risk rating/RR) kesehatan terkait pajanan dari kesebelas bahan kimia utama yang digunakan pekerja, pada proses produksi minyak dan gas di kapal FPSO XYZ tahun 2022. Metode Chemical Risk Assessment (CRA) yang digunakan adalah Stoffenmanager® 8 version 5.0 yang merupakan tools untuk menilai risiko kesehatan jalur pajanan inhalasi dan dermal dari penanganan enam bahan kimia oleh production technician di area kerja topside deck dan lima bahan kimia oleh utility operator di area kerja machinery deck. Hasil CRA menunjukkan bahwa tingkat risiko (RR) jalur inhalasi dimana satu bahan kimia kategori risiko tinggi (1,highest) delapan bahan kimia kategori risiko sedang (2, medium), dan dua bahan kimia risiko rendah (3, lowest). Sedangkan berdasarkan risk characterization ratio (RCR) pajanan inhalasi, ada dua bahan kimia yang diketahui nilai RCR task ≥ 1, yang berarti perkiraan konsentrasi emisi yang dihasilkan saat beraktivitas (task concentration estimation/TCE) terhadap potensi bahaya terhirup oleh production technician dan utility operator saat beraktivitas pada jarak yang dekat dengan sumber emisi, dikategorikan berbahaya atau risiko tidak dapat di tolerir (Unacceptable risk). Untuk tingkat risiko dermal efek lokal (skin local), sembilan bahan kimia masuk kategori risiko tinggi dan dua bahan kimia masuk kategori risiko sedang. Sedangkan tingkat risiko dermal efek sistemik (skin uptake), empat bahan kimia kategori risiko sedang, dan tujuh bahan kimia kategori risiko rendah. Hasil risk rating (RR) menentukan pula prioritas tindakan (Action Priority/AP) pengendalian risiko kesehatan. Rekomendasi pengendalian adalah menurunkan tingkat bahaya (HR) dengan melakukan penggantian bahan kimia (subtitusi) dengan bahan kimia yang lebih rendah tingkat bahayanya bagi kesehatan, dan untuk pajanan dermal (ER), otomatisasi proses penanganan, modifikasi teknik pekerjaan dengan membuat sistem penambahan bahan kimia secara gravitasi, menurunkan jumlah dosis pemakaian namun tetap efektif efisien (workplace-related modifiers), mengurangi waktu dan frekuensi penggunaan bahan kimia tersebut (activity time), penambahan ventilasi lokal (LEV) selain ventilasi mekanik, serta menggunakan baju khusus tahan kimia beserta sarung tangannya atau Chemsuit (control measures modifiers). ......Along with the government's target to increase offshore oil and gas production,  the use of chemicals in oil and gas production activities tends to increase, this raises concerns about potential health problems for workers, therefore it is necessary to conduct a chemical health risk assessment. This study aims to analyze the health risk rating (RR) related to exposure to the eleven main chemicals used by workers in the oil and gas production process on the FPSO XYZ ship in 2022. The Chemical Risk Assessment (CRA) method that is used is Stoffenmanager® 8 version 5.0 which is a tool to assess the health risks of inhalation and dermal exposure lines from the handling of six chemicals by production technicians on the topside deck work area and five chemicals by utility operators on the machinery deck work area. The results of the CRA show that the risk level (RR) for the inhalation route is one chemical in the high-risk category (1,highest), eight chemicals in the medium risk category (2, medium), and two chemicals in the low-risk category (3, lowest). Meanwhile, based on the risk characterization ratio (RCR) of inhalation exposure, there are two chemicals whose RCR task value is ≥ 1, which means the estimated concentration of emissions produced during the activities (task concentration estimation/TCE) against the potential inhalation hazard by production technicians and utility operators when activities at a close distance to the emission source, are categorized as a dangerous or unacceptable risk. For the level of dermal risk of local effects (skin local), nine chemicals are in the high-risk category and two chemicals are in the medium risk category. While the level of risk of dermal systemic effects (skin uptake), four chemicals were in the moderate risk category, and seven chemicals were in the low-risk category. The results of the risk rating (RR) also determine the priority of action (Action Priority/AP) for controlling health risks. Control recommendations are to reduce the level of hazard (HR) by replacing chemicals (substitutions) with lower chemicals levels of danger to health, and for dermal exposure (ER), automation of handling processes, modification of work techniques by making chemical addition systems automatically. gravity, reducing the number of doses used but still being effective and efficient (workplace-related modifiers), reducing the time and frequency of using these chemicals (activity time), adding local ventilation (LEV) in addition to mechanical ventilation, and using special chemical resistant clothing and gloves or Chemical suit (control measures modifiers).

Insektisida merupakan salah satu jenis pestisida yang banyak digunakan di masyarakat guna melindungi kesehatan masyarakat. Namun perlu diingat bahwa daya racun dari insektisida juga dapat mengancam kesehatan teknisi pengendali hama. Pada saat proses pembuatan, pencampuran, dan aplikasi insektisida, pekerja terpajan bahaya kimia dan berpotensi menimbulkan risiko terhadap kesehatan. Dalam penelitian ini, risiko pajanan dermal insektisida terhadap teknisi pengendali hama yang dikaji dengan menggunakan metode semi kuantitatif DREAM (Dermal Risk Exposure Method). Unit analisis yang diamati adalah pekerjaan hot fogging, cold fogging, spraying dan misting dengan risiko pajanan dermal dari insektisida dengan empat aktifitas yang diamati yaitu menuang insektisida dari botol konsentrat, mencampur insektisida, menuang campuran insektisida dan pelaksanaan. Besarnya nilai total pajanan dermal dipengaruhi oleh nilai emisi, desposisi dan transfer, serta dipengaruhi oleh jenis bahan kimia, proteksi perlindungan tangan dan lama pajanan. Hasil penelitian didapatkan nilai total pajanan dermal pada pekerjaan hot fogging tugas menuang insektisida dari botol konsentrat 1,19 (sangat rendah), mencampur insektisida 1,19 (sangat rendah), menuang campuran insektisida 2,94 (sangat rendah), pelaksanaan hot fogging 78,73 (medium). Nilai total pajanan dermal pada pekerjaan cold fogging tugas menuang insektisida dari botol konsentrat 1,19 (sangat rendah), mencampur insektisida 1,19 (sangat rendah), menuang campuran insektisida 2,94 (sangat rendah), pelaksanaan cold fogging 34,99 (medium). Nilai total pajanan dermal pada pekerjaan spraying tugas menuang insektisida dari botol konsentrat 1,19 (sangat rendah), mencampur insektisida 1,19 (sangat rendah), menuang campuran insektisida 2,94 (sangat rendah), pelaksanaan spraying 3,30 (sangat rendah). nilai total pajanan dermal pada pekerjaan misting tugas menuang insektisida dari botol konsentrat 11,91 (rendah), mencampur insektisida 1,19 (sangat rendah), menuang campuran insektisida 1,63 (sangat rendah), pelaksanaan misting 2,18 (sangat rendah. Kesimpulan bahwa nilai pajanan dermal tertinggi terdapat pada aktifitas pelaksanaan fogging dengan nilai total pajanan dermal sebesar 78,73 (medium).
Insecticides are one type of pesticide that is widely used in the community to protect public health. However, toxicity of pesticide may expose to pest control officer as well. During the process of manufacturing, mixing and applying insecticides, workers are exposed to chemical hazards from thr active ingredient of pesticide and risk to their health. In this study, insecticide dermal exposure to the pest controll technicians during was assessed using the semi-quantitative method, called DREAM (Dermal Risk Exposure Method). The unit of analysis observed were hot fogging job, cold fogging job, spraying job and misting job of pouring the insecticides from a concentrated bottle into a measuring cup, mixing insecticides, pouring the insecticide mixture into the fogging machine, and implementing. The amount of total dermal exposure is influenced by the value of emissions, desposition and transfer, and is influenced by the type of chemical, protection of the hand and the length of exposure. The results showed value of total actual dermal exposure at task level of hot fogging job task pouring insecticides from bottle concentrates 1.19 (very low), mixing insecticides 1.19 (very low), pouring insecticide mixture 2.94 (very low), implementing hot fogging 78,73 (moderate). The value total actual dermal exposure at task level of cold fogging job task pouring insecticides from bottle concentrates 1.19 (very low), mix insecticides 1.19 (very low), pour insecticide mixture 2.94 (very low), implementing cold fogging 34.99 (moderate). The value of total actual dermal exposure at task level of spraying work task pouring insecticides from bottle concentrate 1.19 (very low), mixing insecticides 1.19 (very low), pouring insecticide mixture 2.94 (very low), implementing spraying 3.30 (very low). the value of total actual dermal exposure at task level of misting job task pouring insecticides from bottle concentrate 11.91 (low), mix insecticides 1.19 (very low), pour insecticide mixture 1.63 (very low), implementing misting 2.18 (very low). Conclusion the high value of dermal exposure is implementing hot fogging 78.73 (Moderate).

Penggunaan berbagai pelarut organik volatil di labotatorium pengujian menimbulkan risiko terhadap dampak kesehatan baik dalam jangka pendek maupun jangka panjang. Oleh karena itu perlu dilakukan kajian risiko kesehatan. Chemical Health Risk assessment (CHRA) atau kajian risiko kesehatan yang dikembangkan oleh Department of Occupational Safety and Health (DOSH), Ministry of Human Resources, Malaysia (2018) digunakan dalam studi ini untuk menilai risiko kesehatan akibat pajanan inhalasi dan dermal dari 3 (tiga) pelarut organik volatil yaitu chloroform, dichlorometane, dan tetrachloroethylee. Penelitian dilakukan terhadap 3 (tiga) karyawan laboratorium PT X yang bekerja di 3 (tiga) lokasi ruangan yang berbeda. Penilaian tingkat risiko atau risk rating (RR) pajanan bahan kimia melalui inhalasi dilakukan secara kualitatif dan kuantitaif, sedangkan pajanan melalui dermal dinilai secara kualitatif saja. Diperoleh bahwa hasil penilain tingkat risiko pajanan bahan kimiakimia melalui inhalasi secara kualitatif adalah chloroform (RR=16) dengan tingkat risiko tinggi, dichlorometane (RR=15) dengan tingkat risiko menengah, dan tetrachloroethylene (RR=12) dengan tingkat risiko menengah Hasil penilaian tingkat risiko pajanan bahan kimia melalui inhalasi secara kuantitaif adalah chloroform (TWA pengukuran = 18,460 ppm) dengan tingkat risiko tinggi (RR=20), dichlorometane (TWA pengukuran = 0,362 ppm) dengan tingkat risiko rendah (RR=3), dan tetrachloroethylene (TWA pengukuran = 0,560) dengan tingkat risiko rendah (RR=3). Hasil penilaian tingkat risiko pajanan bahan kimia melalui dermal secara kualitatif dengan luas area kontak kecil dan durasi panjang adalah chloroform (M2) dengan tingkat risiko menengah, dichlorometane (M2) dengan tingkat risiko menengah dan tetrachloroethylene (M2) dengan tingkat risiko menengah. Pengendalian untuk menurunkan risiko pajanan chloroform melalui inhalasi (AP-3) direkomendasikan dalam penelitian ini.
The use of various organic solvents in the laboratory test the risks to health risks in both the short and long term. Therefore a health risk assessment is needed. The Chemical Health Risk Assessment (CHRA) or health risk assessment developed by the Department of Occupational Safety and Health (DOSH), Ministry of Human Resources, Malaysia (2018) was used in this study to estimate health risks due to inhalation and dermal exposure of 3 (three ) volatile organic solvents namely chloroform, dichloromethane and tetrachloroethylee. The study was conducted on 3 (three) PT X laboratory employees who worked in 3 (three) different room locations. Assessment of the level of risk or risk rating (RR) exposure chemicals through inhalation is carried out qualitatively and quantitatively, while exposure through dermal through qualitative publications. Obtained from the results of the qualitative assessment of the risk of exposure to chemicals through inhalation were chloroform (RR = 16) with high risk levels, dichloromethane (RR = 15) with the level of moderate risk, and tetrachlorethylene (RR = 12) with a high risk of exposure risk chemicals through determination of chloroform levels (TWA = 18,460 ppm) with high risk levels (RR = 20), dichloromethane (measurement TWA = 0.362 ppm) with low risk levels (RR = 3), and tetrachlorethylene (TWA. = 0.560) with levels low risk (RR = 3). The qualitative progress of the level of exposure to chemicals through the skin with a large area of small contact and long duration is chloroform (M2) with an inflation rate, dichloromethane (M2) with a high risk level and tetrachlorethylene (M2) with a medium risk level. Control to reduce the risk of exposure to chloroform through inhalation (AP-3) was approved in this study.

Dalam produksi minyak dan gas bumi, pengendalian kimiawi dari kontaminasi mikrobiologi bagi integritas jaringan pipa dan vessels salah satu caranya yaitu dengan menggunakan biosida Glutaraldehid. Dari data pencatatan Penyakit Akibat Kerja (PAK) PT. X, pada tahun 2019 telah terjadi insiden akibat kesalahan penanganan bahan kimia dan informasi yang tidak memadai pada Lembar Data Keselamatan (LDK) yang mengakibatkan ketidaksesuaian pemilihan sarung tangan kimia sehingga menyebabkan 7 kasus dermatitis kontak iritan pada pekerja yang melakukan injeksi biosida Glutaraldehid. Tujuan penelitian ini adalah melakukan identifikasi, menilai besarnya risiko kesehatan melalui rute paparan kulit dan potensi dampak terjadinya iritasi pada kulit yang berkaitan dengan faktor-faktor risiko kulit, menentukan tingkat bahaya pada rute paparan, kulit serta mengevaluasi efektifitas pengendalian risiko dan memperoleh rekomendasi mitigasi yang tepat untuk mencegah terjadinya penyakit kulit akibat kerja pada proses injeksi biosida Glutaraldehid di fasilitas produksi hulu migas PT. X. Metode dalam penelitian ini yaitu observasional melalui pendekatan deskriptif yang bersifat semikuantitatif menggunakan metode Dermal Risk Assessment (DREAM) dan survei Nordic Occupational Skin Questionnaires (NOSQ 2002/SHORT) modified pada enam lapangan operasi di PT. X yang memiliki proses injeksi biosida Glutaraldehid. Hasil penelitian didapatkan, tingkat paparan dermal pada task level site B keseluruhan SkinW-Atask yaitu 118.97 tingkat risiko paparan tinggi; site S memiliki tingkat risiko paparan ekstrim tinggi yaitu 5809.38; site C memiliki tingkat risiko paparan ekstrim tinggi yaitu 11864.48, site CU tingkat risiko paparan ekstrim tinggi yaitu 11607.97 dan site SU dengan injeksi manual memiliki tingkat risiko paparan tinggi dengan hasil 492.45, sedangkan hasil open dan closed drain yaitu tingkat risiko sangat rendah. Tingkat paparan dermal pada task level tertimbang waktu (SkinW-Atask.w) pada proses injeksi Glutaraldehid di enam lapangan operasi memiliki tingkat risiko paparan rendah pada site B (18.34), risiko paparan sedang pada site S (76.98) dan site SU dengan proses manual (49.75); risiko paparan tinggi pada site C (175.02) dan site CU (141.20) serta risiko paparan sangat rendah pada site SU proses open drain (1.75) dan closed drain (4.37). Tingkat paparan dermal pada job level (Skinw-Ajob) pada proses injeksi Glutaraldehid di enam lapangan operasi memiliki tingkat risiko paparan sedang, rendah hingga sangat rendah. Perhitungan faktor-faktor dalam DREAM yang dikombinasikan dengan evaluasi faktor pendukung lainnya serta survei NOSQ 2002/SHORT modified dapat menangkap beberapa informasi dan gambaran awal paparan kulit serta adanya potensi terjadinya Penyakit Kulit Akibat Kerja (PKAK) pada proses injeksi Gluataraldehid di fasilitas produksi hulu migas PT. X. ......Microbiological contamination using biocide glutaraldehyde is one of the applications to maintain the integrity of pipelines and vessels in oil and gas production. PT. X’s data on the recording of occupational illness shows an incident that occurred in 2019 due to chemical mishandling. The incident resulted in an inappropriate selection of chemical gloves and caused seven cases of irritant contact dermatitis in workers who injected biocide containing glutaraldehyde. The purpose of this research are to Identify and assess the magnitude of health risks through the route of skin exposure and potential irritant effects on the skin related to dermal risk factors, determining the level of hazards on the skin exposure route, and evaluating the effectiveness of risk control to obtain appropriate mitigation in the biocide injection process at PT. X upstream oil and gas production facilities. The method used in this study is observational through a descriptive semi-quantitative approach using the Dermal Risk Assessment (DREAM) and Nordic Occupational Skin Questionnaires (NOSQ 2002/SHORT) modified in six operating sites at PT. X, which has a Glutaraldehyde biocide injection process. The results showed that the level of dermal exposure at the task level site B, overall SkinW-Atask was 118.97 with a high risk level of exposure; site S has a high level of risk of extreme exposure, which is 5809.38; site C has a high level of risk of extreme exposure, which is 11864.48; site CU has a high level of risk of extreme high exposure, which is 11607.97; and site SU with manual injection has a high level of risk (492.45). Total Actual Time Weighted Dermal Exposure at Task Level (SkinW-Atask.w) during the Glutaraldehyde injection procedure in six operating sites was low at site B (18.34), moderate at site S (76.98), and high at site SU during manual processing (49.75); significant exposure risk at site C (175.02) and site CU (141.20); and extremely low exposure risk at open drain (1.75), and closed drain (4.37) SU sites. Total Actual Time Weighted Dermal Exposure at Job Level (Skinw-Ajob) in six operating sites during the Glutaraldehyde injection process has a moderate, low to extremely low risk of exposure. The calculation of the DREAM factors, in conjunction with the evaluation of other supporting factors and the modified NOSQ 2002/SHORT survey, can provide some information and a preliminary description of dermal exposure and the potential for Occupational Dermatoses (OD) that occur in the Gluataraldehyde injection process at PT. X's upstream oil and gas production facility

Abstrak :

Menopause menyebabkan hipoestrogenisme dan mengakibatkan penuaan kulit. Fitoestrogen dari biji T. foenum-graecum (klabet) diharapkan dapat mengatasi penuaan kulit pascamenopause. Penelitian ini bertujuan untuk mengetahui konsentrasi optimal ekstrak klabet dalam menstimulasi sekresi COL1A1 dan COL3A1, cara kerjanya melalui jalur reseptor estrogen α (REα) atau REβ, dan peran klabet dalam mengurangi kerutan serta meningkatkan ketebalan kulit wajah.

Studi in vitro menggunakan human dermal fibroblast (HDF) tua yang diperoleh dari kultur sel fibroblas kulit perempuan pascamenopause dan HDF muda dari prepusium, dilakukan di Laboratorium Universitas YARSI. Sekresi COL1A1 dan COL3A1 diperiksa dengan ELISA lalu ditambah antagonis REa dan b. Penelitian in vivo merupakan uji klinis acak tersamar ganda berdurasi 12 minggu, di RSCM, Januari–November 2019. Subjek 50 perempuan pascamenopause yang dibagi dua: kelompok perlakuan mendapat krim klabet 5% dan kelompok plasebo mendapat krim dasar. Skor kerutan dahi, crow’s feet, dan nasolabial diukur dengan skoring atlas skin aging Bazin dan tebal dermis dengan high resolution ultrasound (HRU) 18 MHz.

Ekstrak klabet 2 µg/mL merupakan konsentrasi optimal dalam meningkatkan sekresi COL1A1 dan COL3A1 pada HDF tua dan muda dibandingkan tanpa perlakuan dan 5 nM estradiol. Uji inhibisi menunjukkan hambatan REα 50%; REβ dan RE α,β sampai 75%. Penurunan skor kerutan dahi, crow’s feet, dan nasolabial berbeda bermakna pada kedua kelompok minggu ke-4, ke-8, dan ke-12.  Peningkatan selisih skor kerutan dahi, crow’s feet, dan nasolabial tidak bermakna pada kedua kelompok antara baseline, minggu ke-4, ke-8, dan ke-12. Ketebalan dermis meningkat bermakna pada minggu ke-4 dibandingkan baseline. Pada minggu ke-8 dibandingkan minggu ke-4 dan pada minggu ke-12 dibandingkan minggu ke-8, ketebalan dermis menurun bermakna pada kedua kelompok. Peningkatan selisih ketebalan dermis pada minggu ke-4 dan ke-8 tidak bermakna.

Simpulan: Ekstrak klabet 2 µg/mL meningkatkan COL1A1 dan COL3A1 lebih banyak dibandingkan kelompok tanpa perlakuan dan estradiol 5 nM. Klabet bekerja terutama melalui REβ. Penurunan skor kerutan kulit dan peningkatan ketebalan kulit wajah tidak bermakna dibandingkan plasebo. Diduga, krim klabet konsentrasi 5% tidak tepat untuk mengurangi kerutan dan menambah ketebalan kulit. Perlu penelitian lebih lanjut untuk mengetahui konsentrasi optimal klabet sebagai terapi penuaan kulit wajah pascamenopause.

Hypoestrogenism in menopause leads to skin aging, for which phytoestrogen originated from the seeds of T. foenum-graecum (fenugreek) is expected to be a solution. This study aims to show the effect of phytoestrogen from fenugreek extract in stimulating COL1A1 and COL3A1 through its mechanism of action on estrogen receptor (ER) ERa or ERb and its role in diminishing facial wrinkles and increasing dermal thickness.

The in vitro study was carried out in YARSI University Laboratory throughout March 2017–March 2018. This study looks into the effect of fenugreek extract on collagen level secretion in old and young human dermal fibroblast (HDF) compared to control. The in vivo study was a randomized, double-blind, 12-week trial conducted in RSCM from January–November 2019. Fifty postmenopausal women divided into two groups: the intervention group was given 5% fenugreek cream while the placebo group was given base cream.

The in vitro study showed that a concentration of 2 mg/mL was the optimal dose to stimulate COL1A1 and COL3A1 secretion in both old and young HDF compared to control (no treatment) and 5 nM estradiol. The inhibition test demonstrated suppression of ERa by 50%; ERb and ERa,b by up to 75%, indicating that the fenugreek activates both receptors, especially ERb. However, the success of the in vitro study did not translate into the in vivo study. Both the intervention group and the placebo group were able to achieve statistically significant in facial wrinkle scores from all focus areas without any significant disparity between both groups at all timepoints. Dermal thickness of facial skin showed similar results for both groups with significant improvements in the 8thweek compared to baseline and significant decrease by the 12thweek.

Conclusion: Fenugreek extract with a concentration of 2 mg/mL increased COL1A1 and COL3A1 secretion more potently compared to control and estradiol 5 nM. However, the decrease in facial skin wrinkles scores and the increase in dermal thickness were not significant compared to placebo. We suspect that a concentration of 5% was not adequate for the expected antiaging effects. Further studies are necessary to determine a more appropriate fenugreek concentration to permit clinical use as a postmenopausal antiaging therapy.

ABSTRAK
Latar Belakang: Subepithelial connective tissue graft SCTG dan acelullar dermal matrix ADM seringkali digunakan dalam perawatan resesi gingiva. Tujuan Penelitian: Mengevaluasi kondisi klinis jaringan periodontal setelah perawatan resesi gingiva antara menggunakan SCTG dengan ADM. Metode: Data resesi gingiva, tingkat perlekatan klinis gingiva, dan lebar gingiva cekat sebelum perawatan diambil dari rekam medik. Pasien dihubungi untuk pengambilan data setelah perawatan. Hasil: Penggunaan SCTG dan ADM memberikan hasil yang signifikan. Perbandingan antara kedua kelompok tidak menunjukkan perbedaan signifikan. Kesimpulan: Perawatan resesi gingiva dengan SCTG dan ADM memberikan hasil yang serupa. Kata kunci: resesi gingiva, subepithelial connective tissue graft, acellular dermal matrix
ABSTRACT Background Subepithelial connective tissue graft SCTG and acellular dermal matrix ADM are frequently used in treatment of gingival recession.Objectives To evaluate periodontal clinical conditions after treatment of gingival recession using SCTG and ADM.Methods Pre operative data of gingival recession, clinical attachment level, and attached gingiva were retrieved from medical records. Patients were recalled and post operative data were recorded.Results Application of SCTG and ADM yield significant changes. Comparisons between the two groups showed no statistically significant differences.Conclusion Treatment of gingival recession with SCTG and ADM yield similar outcomes.Keywords gingival recession, subepithelial connective tissue graft, acellular dermal matrix
