"[ABSTRAK
Latar belakang: Pasien HIV anak berisiko tinggi mengalami gangguan
neurokognitif akibat keterlibatan sistem saraf pusat (SSP). Prevalens gangguan
kognitif tersebut berkisar antara 8%-62%. Pemberian ARV menurunkan viral
load di SSP sehingga mencegah penurunan fungsi kognitif. Tujuan penelitian ini
untuk memberikan gambaran fungsi kognitif pasien HIV anak dalam terapi ARV.
Metode: Studi potong lintang dilakukan terhadap pasien HIV anak berusia 5-15
tahun. Penilaian kognitif dilakukan dengan instrumen Wechsler intelligence scale
for children IV (WISC IV). Pemeriksaan elektroensefalografi bertujuan untuk
membuktikan kerusakan akibat keterlibatan SSP pada infeksi HIV.
Hasil: Sembilan puluh pasien HIV anak median usia 9 tahun telah memperoleh
ARV dengan median 69 bulan. Hasil rerata verbal, performance, dan full-scale IQ
(FSIQ) berturut-turut adalah 88,66 (SB 15,69), 85,30 (SB 15,35), dan 85,73 (SB
15,61). Enam puluh tujuh (74,4%) subjek memiliki verbal IQ normal, 56 (62,2%)
performance scale normal, dan 58 (64,4%) FSIQ normal. Hasil EEG abnormal
didapatkan pada 22 subjek (22,4%) dan tidak memiliki hubungan dengan stadium
klinis, usia dan lama pemberian ARV, serta viral load. Stadium HIV
menunjukkan hubungan bermakna dengan komponen verbal scale IQ dan FSIQ
(p=0,042 dan p=0,044). Hasil IQ tidak memiliki hubungan dengan usia pemberian
ARV, lama pemberian ARV, dan viral load.
Simpulan: Pasien HIV anak dalam terapi ARV memiliki rerata IQ abnormal pada
verbal, performance, dan FSIQ. Berdasarkan kategori hasil IQ lebih dari 50%
subjek memiliki IQ normal pada ketiga skala WISC. Studi kohort diperlukan
untuk menilai apakah pemberian ARV lebih dini dan faktor yang memengaruhi

respectively. Sixty seven subjects (74,4%) had normal verbal IQ, 56 subjects
(62,2%) had normal performance IQ, and 58 subjects (64,4%) had normal FSIQ.
Twenty two children (22,4%) showed EEG abnormality which was not correlated
to clinical stage, onset and duration of ARV, and viral load. Clinical stage of HIV
showed significant association with verbal and FSIQ (p 0,042 and p 0,044). IQ
results did not have association with onset and duration of ARV and viral load.
Conclusion: HIV-infected children on ARV have abnormal mean IQ in verbal,
performance and FSIQ. Based on categorical IQ, most subjects have normal
verbal, performance, and FSIQ. Cohort study is needed to address whether early

"Kemampuan kognitif anak sebagai indikator kualitas pendidikan merupakan variabel penting dalam pembangunan Indonesia. Beberapa studi telah menganalisis faktor apa saja yang mempengaruhi kognitif anak. Namun belum banyak studi yang membahas bargaining power ibu sebagai determinan kemampuan kognitif anak di Indonesia. Dengan menggunakan data IFLS 4 dan 5 serta analisis Ordinary Least Square (OLS), studi ini membahas pengaruh karakteristik ibu khususnya bargaining power berupa pengambilan keputusan dalam pendidikan yang berpengaruh terhadap alokasi sumber daya pengeluaran dan waktu di rumah tangga dan kemampuan kognitif anak di Indonesia. Hasil studi menunjukan bahwa bargaining power ibu pada anak berumur 0-7 tahun dan 7-14 tahun tidak signifikan mempengaruhi kemampuan kognitif anak berumur 7-14 tahun. Namun setelah dilakukan interaksi antara bargaining power ibu dan pendidikan ibu, hasil menunjukan terdapat pengaruh yang signifikan positif terhadap kemampuan kognitif anak.

Children's cognitive ability as an indicator of education quality is essential in Indonesia's development. Several studies have analyzed what factors affect children's cognitive. However, not many studies discuss the bargaining power of mothers as a determinant of children's cognitive abilities in Indonesia. This study uses IFLS 4 and 5 data and Ordinary Least Square (OLS) analysis. Discusses the influence of maternal characteristics, especially bargaining power in the form of decision-making in children's education which affects the allocation of spending and time resources in the household and children's cognitive abilities in Indonesia. The study's results showed that the bargaining power of mothers in children aged 0-7 years and 7-14 years did not significantly affect the cognitive abilities of children aged 7-14 years. However, after the interaction between the mother's bargaining power and education, the results showed a significant positive effect on children's cognitive abilities."

"Latar belakang: Epilepsi merupakan salah satu penyakit kronik dan memiliki risiko
tinggi untuk mengalami gangguan kognitif yang dapat mempengaruhi kualitas hidup.
Pemeriksaan Intelligence quotient (IQ) memerlukan waktu pemeriksaan yang lama dan
biaya yang mahal, diperlukan alat skrining untuk mendeteksi gangguan kognitif pada
pasien epilepsi anak yaitu School Years Screening Test For Evaluation Of Mental
Status-Revised (SYSTEMS-R)
Tujuan: Mengetahui seberapa besar nilai diagnostik dari School Years Screening Test
For Evaluation Of Mental Status-Revised (SYSTEMS-R) dalam mendeteksi gangguan
kognitif pada anak epilepsi usia 6-15 tahun.
Metode: Penelitian potong lintang dilakukan di Rumah Sakit Cipto Mangunkusumo
Jakarta terhadap subjek berusia 6-15 tahun dengan epilepsi. Pada sampel dilakukan
anamnesis dan pemeriksaan fisik, dilanjutkan pemeriksaan fungsi kognitif dengan
School Years Screening Test For Evaluation Of Mental Status-Revised (SYSTEMS-R)
dan kemudian dilakukan pemeriksaan baku emas IQ oleh psikolog.
Hasil: Prevalensi gangguan kognitif pada pasien epilepsi usia 6-15 tahun sebesar
86,3%. School Years Screening Test For Evaluation Of Mental Status-Revised
(SYSTEMS-R) memiliki sensitivitas 84%, spesifisitas 91%, nilai prediksi positif 98%,
nilai prediksi negatif 47%, rasio kemungkinan positif 10,11, rasio kemungkinan negatif
0,17 dan akurasi 85%.
Simpulan: School Years Screening Test For Evaluation Of Mental Status-Revised
(SYSTEMS-R) memiliki nilai diagnostik yang baik dan dapat menjadi pilihan dalam
deteksi dini gangguan kognitif pada pasien epilepsi anak.

Background: Epilepsy is a chronic disease and children with epilepsy are at high risk
of cognitive disorders which can affect the quality of life. Intelligence Quotient (IQ)
examination requires a long examination time and expensive costs, a screening tool for
cognitive clearance is needed in pediatric epilepsy patients, which is School Years
Screening Test For Evaluation Of Mental Status-Revised (SYSTEMS-R)
Objective: To know the diagnostic value of School Years Screening Test For
Evaluation Of Mental Status-Revised (SYSTEMS-R) detecting cognitive impairment in
children aged 6-15 years with epilepsy.
Methods: This is a cross sectional study done in Cipto Mangunkusumo Hospital Jakarta
was conducted on subjects aged 6-15 years with epilepsy. We evaluated history of
illness, physical examination, and cognitive function using School Years Screening Test
For Evaluation Of Mental Status-Revised (SYSTEMS-R) and then a standard gold IQ
examination was carried out by a psychologist.
Results: The Prevalence of cognitive impairment in 6-15 years epilepsy patients is
86,3%. School Years Screening Test For Evaluation Of Mental Status-Revised
(SYSTEMS-R) has a sensitivity of 84%, specificity 91%, positive predictive value 98%,
negative predictive value 47%, positive likelihood ratio 10,11, negative likelihood 0,17
and accuracy 85%.
Conclusion: School Years Screening Test For Evaluation Of Mental Status-Revised
(SYSTEMS-R) has good diagnostic value and it can be an option in early detection of
cognitive impairment in paediatric epilepsy patients

"Prevalensi paparan asap rokok yang tinggi di rumah di Indonesia semakin mengancam anak-anak sebagai pihak mayoritas yang secara sukarela terpapar. Studi ini menyelidiki hubungan antara paparan asap rokok pada masa prenatal dan usia dini dan hasil kognitif anak-anak berusia 7-12 tahun, menggunakan data dari Survei Aspek Kehidupan Rumah Tangga Indonesia (SAKERTI) 2014. Dengan model regresi probit, hasilnya menunjukkan bahwa paparan asap rokok sangat penting dalam mempengaruhi hasil kognitif anak bahkan setelah dikendalikan oleh karakteristik keturunan, karakteristik anak-anak, dan karakteristik sosial ekonomi ibu. Status merokok ibu memiliki dampak negatif yang signifikan terhadap hasil kognitif anak.

The high prevalence of secondhand smoke at home in Indonesia is increasingly threatening children as the majority party who voluntarily exposed. The study investigated the relation between exposure to secondhand smoke on prenatal and early childhood development and cognitive outcome of children aged 7-12 years, using data from Indonesia Family Life Survey (IFLS) 2014. With probit regression model, the result indicates that exposure to secondhand smoke is significant in affecting children cognitive outcome even after controlled by heredity characteristic, children characteristics, and socioeconomic characteristics. Maternal smoking status has negatively significant impact on children cognitive outcome."
