Pelayanan kefarmasian di Apotek harus mampu menjamin ketersediaan obat yang aman, bermutu dan berkhasiat sesuai dengan Undang-undangNomor 36 Tahun 2009 tentang kesehatan. Salah satu standar pelayanan kefarmasian di Apotek adalah pelayanan farmasiklinik. Pelayanan farmasi klinik merupakan pelayanan secara langsung yang diberikan oleh Apoteker kepada pasiendalam rangka untuk meningkatkan outcome terapi serta meminimalkan efek samping obat. Salah satu pelayananfarmasi klinik yang dilakukan oleh Apoteker adalah pelayanan dan pengkajian resep.

Pharmaceutical service in pharmacies must be able to guarantee the availability of safe, quality and efficicaous medicines in accordance with Law Number 36 of 2009 concerning health. One of the standards for pharmaceutical services in pharmacies is clinical pharmacy services. Clinical pharmacy services are direct services provided by pharmacists to patients in order to improve therapeutic results and minimize drug side effects. One of the clinical pharmacy services carried out by pharmacists is prescription service and review."

"Praktik kerja profesi apoteker di Apotek Kimia Farma 202 Kejayaan bertujuan mengkaji resep obat dari salah satu resep pasien diabetes melitus di Apotek Kimia Farma Kejayaan bulan Mei 2023 dari segi administrasi, farmasetik dan klinis. Serta memahami permasalahan yang ditemukan dari hasil pengkajian resep di Apotek Kimia Farma Kejayaan bulan Mei 2023. Metode pelaksanaan yang dilakukan dalam laporan ini adalah studi literatur. Resep yang diperoleh adalah dari resep bulan Mei tahun 2023 di Apotek Kimia Farma 202 Kejayaan. Resep dipilih karena kompleksitas yang ada di dalam resep dan dapat dijadikan bahan ajar tentang pengkajian resep. Pengkajian salah satu resep di Apotek Kimia Farma 202 Kejayaan dilihat dari aspek administratif, yaitu melihat kelengkapan data pasien, dokter, serta obat yang diberikan pada resep. Aspek farmasetik dilihat dengan menyesuaikan nama obat serta kekuatan obat dengan literature. Aspek klinis dilakukan untuk melihat indikasi, dosis, efek samping, kontra indikasi, dan potensi interaksi obat yang ada pada resep. Permasalahan yang sering ditemukan pada penulisan resep yaitu terkait aspek administrasi masih ditemukan beberapa poin yang jarang dicantumkan dalam penulisan resep. Ada pula permasalahan terkait potensi interaksi obat yang harus diwaspadai yang dapat terjadi kepada pasien.

The professional pharmacist work practice at Apotek Kimia Farma 202 Kejayaan aims to review a prescription from a diabetes mellitus patient at Apotek Kimia Farma Kejayaan in May 2023 in terms of administrative, pharmaceutical, and clinical aspects. Additionally, it seeks to understand the issues found from the prescription review at Apotek Kimia Farma Kejayaan in May 2023. The implementation method used in this report is literature study. The prescription reviewed is from May 2023 at Apotek Kimia Farma 202 Kejayaan. This prescription was chosen due to its complexity and can be used as a teaching material for prescription review. The review of the prescription at Apotek Kimia Farma Kejayaan is conducted from three aspects: Administrative aspect involves checking the completeness of the patient data, doctor information, and the medications prescribed. Pharmaceutical aspect involves verifying the drug names and their strengths against the literature. Clinical aspect involves assessing the indications, dosage, side effects, contraindications, and potential drug interactions in the prescription. Common issues found in prescription writing include some administrative details that are often omitted. Additionally, there are potential drug interaction problems that need to be closely monitored to prevent adverse effects on the patient."

"Resep merupakan salah satu bagian dari pelayanan kefarmasian. Resep yang baik harus memiliki cukup informasi untuk memungkinkan ahli farmasi mengerti obat apa yang harus diberikan kepada pasien. Penulisan resep yang tidak jelas dapat memicu kesalahan interpretasi yang dapat berefek pada terjadinya kesalahan medikasi (medication error) sehingga berdampak fatal bagi pasien. Pengkajian resep dilakukan untuk meminimalisir kemungkinan timbulnya permasalahan terkait obat. Pengerjaan tugas khusus ini menggunakan metode kualitatif melalui studi literatur sebagai acuan dan metode observasi dengan melakukan pengkajian atau skrining terhadap resep yang ada di Apotek Kimia Farma Salemba. Resep yang diambil bersifat acak yaitu resep pembelian tunai, resep asuransi kesehatan seperti BPJS, Mandiri Inhealth, Pertalife dan resep Klinik Tunggal. Aspek yang dikaji berupa aspek administrasi, farmasetik dan klinik. Kesalahan peresepan terbanyak pada aspek administrasi yaitu tidak dicantumkan keterangan berat badan pasien. Aspek farmasetik yaitu keterangan aturan pakai dan cara penggunaan obat yang belum terdapat pada semua item obat. Sedangkan pada aspek klinis, kesalahan peresepan terbanyak yaitu tidak adanya keterangan lama pemakaian obat.

Prescription is one part of pharmaceutical services. A good prescription should have enough information to enable the pharmacist to understand appropriate medication given to the patient. Unclear prescription can lead to misinterpretation, which result in medication errors that are fatal to the patient. Prescription-screening is done to minimize the possibility of drug-related problems. This assignment uses a qualitative method through literature study as a reference and observation method by assessing or screening existing prescriptions at Kimia Farma Salemba Pharmacy. Prescriptions were taken randomly, cash purchase prescriptions, health insurance prescriptions such as BPJS, Mandiri Inhealth, Pertalife and Tunggal Clinic prescriptions. The aspects studied were administrative, pharmaceutical and clinical aspects. The most common prescribing error in the administrative aspect is that the patient's weight information is not included. The pharmaceutical aspect is the use of medication that are not yet contained in all drug items. While in the clinical aspect, the most prescribing error was the absence of information on the duration of drug use."

"Penyakit kardiovaskular adalah penyebab utama kematian secara global, merenggut sekitar 17,9 juta jiwa setiap tahunnya. Penyakit kardiovaskular adalah sekelompok penyakit pada jantung dan pembuluh darah, termasuk penyakit jantung koroner, penyakit serebrovaskular, penyakit jantung rematik, dan lain-lain. Berdasarkan Permenkes No. 73 Tahun 2016 tentang Standar Pelayanan Kefarmasian di Apotek, salah satu pelayanan farmasi klinik yang dilakukan adalah pengkajian resep. Tujuan pengkajian resep adalah untuk mencegah kesalahan pengobatan dan menganalisis masalah terkait obat. Kegiatan pengkajian resep meliputi administrasi, kesesuaian farmasetik dan pertimbangan klinis. Berdasarkan latar belakang tersebut, dilakukan identifikasi salah satu resep terapi kardiovaskular yang masuk di Apotek Roxy Ciledug pada bulan Maret 2023. Metode yang dilakukan dengan mengobservasi resep di bulan Maret 2023, kemudian dipilih satu sampel resep yang akan ditelaah lebih lanjut. Lalu, dilakukan analisa resep berdasarkan telaah aspek administratif, farmasetik, dan klinis. Berdasarkan analisa sampel resep yang masuk di Roxy Ciledug pada bulan Maret dapat disimpulkan bahwa kajian administratif dan farmasetika resep belum lengkap. Berdasarkan kajian klinis semua obat tepat indikasi dan dosis, dan aturan pakai. Penggunaan obat herbesser dan atorvastatin perlu diperhatikan karena adanya interaksi yang dapat meningkatkan risiko rabdomiolisis, sehingga penggunaan keduanya harus diberi jeda dan dipantau efek samping yang mungkin timbul. Penggunaan bersamaan Clopidogrel dan Esomeprazole dapat menyebabkan penurunan konsentrasi plasma dari metabolit aktif clopidogrel dan penurunan aktivitas antiplatelet. Interaksi keduanya dapat dicegah dengan menjeda waktu pemberian.

Cardiovascular disease is a group of diseases of the heart and blood vessels, including coronary heart disease, cerebrovascular disease, rheumatic heart disease, and others. Based on Permenkes No. 73 of 2016 concerning Pharmaceutical Service Standards in Pharmacies, one of the clinical pharmacy services carried out is prescription review. The purpose of prescription reviews is to prevent medication errors and analyze drug-related problems. Prescription review activities include administration, pharmaceutical suitability and clinical considerations. Based on this background, identification of one of the cardiovascular therapy prescriptions entered at the Roxy Ciledug Pharmacy was carried out in March 2023. The method was carried out by observing prescriptions in March 2023, then selecting a sample recipe to be studied further. Then, prescription analysis was carried out based on administrative, pharmaceutical, and clinical aspects. Based on an analysis of prescription samples that entered Roxy Ciledug in March, it can be concluded that the administrative and pharmaceutical prescription studies were incomplete. Based on clinical aspects, all drugs have the right indications and dosages, and the rules for use. It is necessary to pay attention to the use of the drugs Herbesser and atorvastatin because there are interactions that can increase the risk of rhabdomyolysis, so the use of both of them must be paused and monitored for side effects that may arise. The interaction between clopidogrel and esomeprazole can lead to a decrease in the plasma concentration of the active metabolite of clopidogrel and a decrease in its antiplatelet activity. The interaction between the two can be prevented by delaying the time of administration."

"Asma merupakan penyakit gangguan nafas dengan intensitas dan durasi yang bervariasi. Penyakit ini menjadi penyakit yang diderita oleh 334 juta jiwa pada 2017. Pengobatan asma perlu dilakukan dengan tepat melalui pengkajian resep yang sesuai dengan standar peraturan yang berlaku. Laporan ini bertujuan agar calon apoteker dapat memahami pelaksanaan pengkajian resep asma yang dilaksanakan di Apotek Roxy Pondok Labu. Pada laporan ini terdapat dua resep yang dikaji dengan masing-masing resep terdiri lebih dari satu obat. Resep pertama tertuliskan obat berupa Bricasma dan Lansoprazole. Hasil kajian resep pertama ini menunjukkan aspek administratif resep belum lengkap, aspek farmasetis yang sesuai, dan pada aspek klinis perlu komunikasi lebih lanjut kepada dokter terkait dosis yang diberikan. Resep kedua terdiri dari Ventolin dan Pulmicort yang diracik menjadi nebulizer dengan larutan tambahan berupa NaCl. Kajian yang dilakukan pada resep ini juga menunjukkan aspek administrative yang belum lengkap, aspek farmasetis sudah sesuai, dan pada aspek klinis dibutuhkan konfirmasi kepada dokter yang menulis resep. Kedua resep ini membutuhkan konfirmasi kepada dokter terkait aspek administratif dan aspek klinis. Selain itu, untuk menunjang keberhasilan terapi, seorang apoteker juga harus membekali pasien dengan pengetahuan terkait cara penggunaan obat. Oleh karena itu pengetahuan yang baik akan ilmu kefarmasian dan keterampilan dalam berkomunikasi dengan pasien diperlukan oleh seorang apoteker untuk dapat mengkaji resep dari dokter dan mengkomunikasikan penggunaannya kepada pasien untuk keberhasilan terapi.

Asthma is a respiratory disorder with varying intensity and duration. This disease affected 334 million people in 2017. Asthma treatment needs to be carried out properly through a review of prescriptions in accordance with applicable regulatory standards. This report aims to enable prospective pharmacists to understand the implementation of the asthma prescription assessment carried out at Apotek Roxy Pondok Labu. In this report, there are two recipes that were reviewed with each recipe consisting of more than one drug. The first prescription listed drugs in the form of Bricasma and Lansoprazole. The results of this first prescription review showed that the administrative aspect of the prescription is incomplete, the pharmaceutical aspect was compatible, and on the clinical aspect, further communication doctor was needed regarding the dose given. The second recipe consisted of Ventolin and Pulmicort which were mixed into a nebulizer with additional NaCl solution. The study conducted on this prescription also showed that the administrative aspects were incomplete, the pharmaceutical aspects were compatible, and the clinical aspects required confirmation to the doctor regarding the dose used. Both of these prescriptions required confirmation from the doctor regarding administrative and clinical aspects. In addition, to support the success of therapy, a pharmacist must also provide patients with knowledge related to how to use drugs. Therefore, a good knowledge of pharmacy science and skills in communicating with patients are needed by a pharmacist to be able to review prescriptions from doctors and communicate how to use the drug to patients for success therapy."
