"Praktik kerja profesi apoteker di Apotek Kimia Farma 202 Kejayaan bertujuan mengkaji resep obat dari salah satu resep pasien diabetes melitus di Apotek Kimia Farma Kejayaan bulan Mei 2023 dari segi administrasi, farmasetik dan klinis. Serta memahami permasalahan yang ditemukan dari hasil pengkajian resep di Apotek Kimia Farma Kejayaan bulan Mei 2023. Metode pelaksanaan yang dilakukan dalam laporan ini adalah studi literatur. Resep yang diperoleh adalah dari resep bulan Mei tahun 2023 di Apotek Kimia Farma 202 Kejayaan. Resep dipilih karena kompleksitas yang ada di dalam resep dan dapat dijadikan bahan ajar tentang pengkajian resep. Pengkajian salah satu resep di Apotek Kimia Farma 202 Kejayaan dilihat dari aspek administratif, yaitu melihat kelengkapan data pasien, dokter, serta obat yang diberikan pada resep. Aspek farmasetik dilihat dengan menyesuaikan nama obat serta kekuatan obat dengan literature. Aspek klinis dilakukan untuk melihat indikasi, dosis, efek samping, kontra indikasi, dan potensi interaksi obat yang ada pada resep. Permasalahan yang sering ditemukan pada penulisan resep yaitu terkait aspek administrasi masih ditemukan beberapa poin yang jarang dicantumkan dalam penulisan resep. Ada pula permasalahan terkait potensi interaksi obat yang harus diwaspadai yang dapat terjadi kepada pasien.

The professional pharmacist work practice at Apotek Kimia Farma 202 Kejayaan aims to review a prescription from a diabetes mellitus patient at Apotek Kimia Farma Kejayaan in May 2023 in terms of administrative, pharmaceutical, and clinical aspects. Additionally, it seeks to understand the issues found from the prescription review at Apotek Kimia Farma Kejayaan in May 2023. The implementation method used in this report is literature study. The prescription reviewed is from May 2023 at Apotek Kimia Farma 202 Kejayaan. This prescription was chosen due to its complexity and can be used as a teaching material for prescription review. The review of the prescription at Apotek Kimia Farma Kejayaan is conducted from three aspects: Administrative aspect involves checking the completeness of the patient data, doctor information, and the medications prescribed. Pharmaceutical aspect involves verifying the drug names and their strengths against the literature. Clinical aspect involves assessing the indications, dosage, side effects, contraindications, and potential drug interactions in the prescription. Common issues found in prescription writing include some administrative details that are often omitted. Additionally, there are potential drug interaction problems that need to be closely monitored to prevent adverse effects on the patient."

"Resep merupakan salah satu bagian dari pelayanan kefarmasian. Resep yang baik harus memiliki cukup informasi untuk memungkinkan ahli farmasi mengerti obat apa yang harus diberikan kepada pasien. Penulisan resep yang tidak jelas dapat memicu kesalahan interpretasi yang dapat berefek pada terjadinya kesalahan medikasi (medication error) sehingga berdampak fatal bagi pasien. Pengkajian resep dilakukan untuk meminimalisir kemungkinan timbulnya permasalahan terkait obat. Pengerjaan tugas khusus ini menggunakan metode kualitatif melalui studi literatur sebagai acuan dan metode observasi dengan melakukan pengkajian atau skrining terhadap resep yang ada di Apotek Kimia Farma Salemba. Resep yang diambil bersifat acak yaitu resep pembelian tunai, resep asuransi kesehatan seperti BPJS, Mandiri Inhealth, Pertalife dan resep Klinik Tunggal. Aspek yang dikaji berupa aspek administrasi, farmasetik dan klinik. Kesalahan peresepan terbanyak pada aspek administrasi yaitu tidak dicantumkan keterangan berat badan pasien. Aspek farmasetik yaitu keterangan aturan pakai dan cara penggunaan obat yang belum terdapat pada semua item obat. Sedangkan pada aspek klinis, kesalahan peresepan terbanyak yaitu tidak adanya keterangan lama pemakaian obat.

Prescription is one part of pharmaceutical services. A good prescription should have enough information to enable the pharmacist to understand appropriate medication given to the patient. Unclear prescription can lead to misinterpretation, which result in medication errors that are fatal to the patient. Prescription-screening is done to minimize the possibility of drug-related problems. This assignment uses a qualitative method through literature study as a reference and observation method by assessing or screening existing prescriptions at Kimia Farma Salemba Pharmacy. Prescriptions were taken randomly, cash purchase prescriptions, health insurance prescriptions such as BPJS, Mandiri Inhealth, Pertalife and Tunggal Clinic prescriptions. The aspects studied were administrative, pharmaceutical and clinical aspects. The most common prescribing error in the administrative aspect is that the patient's weight information is not included. The pharmaceutical aspect is the use of medication that are not yet contained in all drug items. While in the clinical aspect, the most prescribing error was the absence of information on the duration of drug use."

"Kualifikasi merupakan kegiatan pembuktian bahwa peralatan, sistem atau fasilitas yang digunakan dalam suatu proses atau sistem akan selalu bekerja sesuai dengan kriteria yang diinginkan dan konsisten. Preparation Vessel OLSA PCBS 200 merupakan alat yang digunakan untuk proses pencampuran bahan obat dengan mixer, dilengkapi dengan sistem pembersihan dan pencucian di tempat (CIP) dan dengan sambungan pipa uap yang digunakan untuk melakukan sterilisasi di tempat (SIP). Sistem SIP Preparation Vessel OLSA PCBS 200 diharapkan mampu bekerja sesuai parameter yang ditentukan dan memberikan hasil sesuai dengan yang diharapkan, yang dibuktikan melalui kualifikasi kinerja. Tujuan dari penulisan ini adalah mampu memahami parameter kualifikasi kinerja dan melakukan penyusunan laporan hasil kualifikasi kinerja Preparation Vessel OLSA PCBS 200 pada fasilitas produksi injeksi hormon PT Harsen Laboratories. Kualifikasi kinerja Preparation Vessel OLSA PCBS 200 yang berada di fasilitas produksi injeksi hormon PT Harsen Laboratories dilaksanakan dengan parameter Pengujian Heat Distribution Empty Chamber yang dilakukan dengan menggunakan sensor suhu thermocouple dan Kaye validator, serta Uji Indikator Biologi yang dilakukan dengan menggunakan Indikator Biologi berisi Bacillus stearothermophillus. Laporan kualifikasi kinerja memuat hasil dan kesimpulan dari seluruh pengujian yang dilaksanakan, sehingga dapat diketahui bahwa kinerja sistem SIP pada Preparation Vessel OLSA PCBS 200 memenuhi persyaratan sterilisasi yang telah ditetapkan.

The performance qualification of Preparation Vessel OLSA PCBS 200 at PT Harsen Laboratories hormone injection facility was carried out with the Heat Distribution Empty Chamber test which were carried out using a thermocouple temperature sensor and Kaye validator, as well as a Biological Indicator Test which was carried out using a Biological Indicator containing Bacillus stearothermophillus. The performance qualification report contains the results and results of all the tests carried out, so it can be seen that the performance of the SIP system of Preparation Vessel OLSA PCBS 200 meets the predetermined sterilization requirements."
