"Latar Belakang: Adenokarsinoma duktal pankreas dan adenokarsinoma ampula vateri, tanpa melihat gambaran episentrum tumor, sulit dibedakan secara histopatologi. Gejala klinis tidak spesifik sehingga kasus yang ditemukan seringkali tidak memenuhi kriteria resectable. Gambaran radiologi juga tidak spesifik, padahal terapi dan prognosis keduanya berbeda. Adenokarsinoma duktal pankreas memiliki angka kesintasan rendah dibanding adenokarsinoma ampula vateri. Penentuan asal tumor, berasal dari duktal pankreas atau ampula vateri, sangat penting. SMAD4 diduga dapat menjadi salah satu panel diagnostik imunohistokimia. Penelitian ini dilakukan dengan melihat perbandingan ekspresi SMAD4 di adenokarsinoma ampula vateri dan adenokarsinoma duktal pankreas.
Tujuan: Mengetahui perbandingan ekspresi SMAD4 pada adenokarsinoma duktal pankreas dan adenokarsinoma ampula vateri.
Metode: Penelitian analitik observasional, desain potong lintang pada sediaan reseksi adenokarsinoma duktal pankreas dan adenokarsinoma ampula vateri, periode Januari 2013 hingga September 2021. Pengambilan sampel dilakukan secara total sampling. Adenokasinoma ampula vateri dengan subtipe pankreatobiliar dieksklusi. Pemeriksaan imunohistokimia menggunakan antibodi primer SMAD4. Data imunohistokimia dianalisis untuk melihat adakah perbedaan ekspresi SMAD4 pada adenokarsinoma di ampula vateri dan adenokarsinoma duktal pankreas.
Hasil: Loss of SMAD4 didapatkan pada 12 kasus (60 %) adenokarsinoma duktal pankreas dan 8 kasus (44,4 %) adenokarsinoma ampula vateri. Tidak didapatkan hubungan loss of SMAD4 pada adenokarsinoma duktal pankreas dan adenokarsinoma ampula vateri (p=0,338).
Kesimpulan: Tidak terdapat hubungan bermakna loss of SMAD4 pada adenokarsinoma duktal pankreas dan adenokarsinoma ampula vateri. Namun terdapat trend loss of SMAD4 lebih tinggi pada adenokarsinoma duktal pankreas dibanding adenokarsinoma ampula vateri subtipe intestinal dan mixed type dominansi intestinal.

ackground: Differentiating pancreatic ductal adenocarcinoma and ampullary adenocarcinoma without knowing the epicenter of the tumor is difficult. The clinical symptoms are non-specific. The cases found usually do not meet the operable criteria. Radiological examination is also non-specific, although the treatment and prognosis are different. Pancreatic ductal adenocarcinoma has lower survival rate than ampullary adenocarcinoma. It is very important to determine the origin of the tumor from pancreatic ductal or ampulla of Vater. SMAD4 is expected to be one of immunohistochemical diagnostic panel for the pancreatic ductal adenocarcinoma. This study compares the SMAD4 expression in pancreatic ductal adenocarcinoma and ampullary adenocarcinoma.
Objective: Knowing the comparison of SMAD4 expression in pancreatic ductal adenocarcinoma and ampullary adenocarcinoma.
Methods: Observational analytical study with cross sectional design, total sampling was performed on the resection specimens of pancreatic ductal adenocarcinoma and ampullary adenocarcinoma, period January 2013 to December 2021. Ampullary adenocarcinoma with pancreatobilliary subtype was excluded. Immunohistochemical examination using SMAD4 primary antibody. Immunohistochemical data will be analyzed to see SMAD4 expression difference between pancreatic ductal adenocarcinoma and ampullary adenocarcinoma.
Results: Loss of SMAD4 was found in 12 cases (66,7 %) of pancreatic ductal adenocarcinoma and 6 cases (44,4 %) of ampullary adenocarcinoma. There was no significant relationship between loss of SMAD4 in pancreatic ductal adenocarcinoma and ampullary adenocarcinoma (p=0,338).
Conclusions: There was no significant relationship between loss of SMAD4 in pancreatic ductal adenocarcinoma and adenocarcinoma of the ampulla of vater. However, there was a trend of higher SMAD4 loss in pancreatic ductal adenocarcinoma than ampullary vater adenocarcinoma of intestinal subtype and mixed type with intestinal dominance.

"[Pendahuluan: Proses karsinogenesis adenokarsinoma prostat terjadi akibat disregulasi kadar zinc dalam sel. Molekul zinc intrasel berperan dalam metabolisme aerob mitokondria dan induksi apoptosis. Penyerapan zinc diatur oleh protein ZIP1, berperan meningkatkan kandungan zinc sitoplasmik intrasel dengan membawa zinc dari cairan ekstrasel. Kadar zinc yang tinggi dan ekspresi protein ZIP1 banyak ditemukan pada epitel prostat normal, sedangkan pada kanker prostat ditemukan sedikit atau tidak ada ekspresi protein ZIP1. Penurunan ekspresi ZIP1 diduga dapat menghambat apoptosis, serta memacu perkembangan adenokarsinoma prostat. Penelitian ini bertujuan menganalisis korelasi ekspresi protein ZIP1 dan Caspase- 3 pada jaringan adenokarsinoma prostat berdasarkan Gleason score yang berbeda. Metode: Desain studi analitik retrospektif dengan desain potong lintang. Sampel penelitian ini adalah 31 sediaan blok parafin adenokarsinoma prostat yang memenuhi kriteria inklusi. Sediaan dipulas menggunakan teknik imunohistokimia untuk mengetahui ekspresi protein ZIP1 dan caspase-3. Ekspresi protein pada pulasan slide dihitung menggunakan program imageJ. Gleason score sebagai data sekunder yang didapatkan dari laporan kasus. Korelasi ekspresi kedua protein berdasarkan Gleason score dianalisis dengan uji korelasi Pearson menggunakan SPSS 11.5. Hasil: Rerata positivitas ekspresi ZIP1 pada adenokarsinoma prostate adalah 35% dan rerata positivitas caspase-3 adalah 33%. Terdapat korelasi positif bermakna antara ekspresi ZIP1 dan caspase-3 (r = 0.379 , p = 0,018). Terdapat korelasi positif antara ekspresi ZIP1 dan caspase-3 pada kelompok intermediate grade (r = 0.73, p = 0.01) dan korelasi lemah tidak bermakna pada kelompok high grade (r = 0.04, p = 0.48). Kesimpulan: Terdapat korelasi positif antara ekspresi ZIP1 dan ekspresi caspase- 3 pada adenokarsinoma prostat.

, Introduction: Carcinogenesis of adenocarcinoma of the prostate occurs due to dysregulation of zinc level within the cells. Intracellular zinc molecules contributes to mitochondrial aerobic metabolism. Its influx is regulated by a transporter protein ZIP1, whose non-presence is predicted to inhibit apoptosis, thus leads to the development of prostate adenocarcinoma. This study was aimed to analyze the correlation of ZIP1 and Caspase-3 expression in prostate adenocarcinoma with respect to its grading as represented by Gleason Score. Methods: This was a cross-sectional, retrospective analytical study on 31 formalyn-fixed, paraffin-embedded tissue that meet inclusion criteria. The specimen was stained using immunohistochemical technique for ZIP1 and Caspase-3. Protein expression of each case were counted using ImageJ analysis. Gleason score were acquired as secondary data from the cases’ reports. The correlation of their expression with respect of Gleason score were analyzed with Pearson’s correlation using SPSS 11.5.
Results: Mean expression level of ZIP1 and Caspase-3 in prostate adenocarcinoma were 35% and 33%, respectively. There was a significantly positive correlation between ZIP1 and Caspase-3 expression (r=0.379; p=0.018). However, their correlation was stronger in intermediate-grade group (r=0.73; p=0.01) and the correlation was much weaker in high-grade group (r=0.04; p=0.48).
Conclusion: There was a positive correlation between ZIP1 and caspase-3 expression in adenocarcinoma prostate.]

Pendahuluan: Penurunan kadar Zink (Zn) pada prostat berkorelasi dengan peningkatan skor Gleason adenokarsinoma prostat dan menyebabkan rendahnya Caspase-3 sebagai eksekutor apoptosis. Tingkat ekspresi transporter Zn pada sel tumor prostat berhubungan dengan tingkat keganasannya. ZnT-1 merupakan transporter Zn ke luar sel prostat, sedangkan ZIP-1 merupakan transporter Zn ke dalam sel prostat. Ekspresi ZIP-1 turun pada adenokarsinoma prostat. Korelasi ZnT-1, ZIP-1 dan Caspase-3 diduga berpengaruh dalam karsinogenesis prostat, sehingga berpotensi untuk menjadi faktor prognosis adenokarsinoma prostat. Hasil: Ekspresi ZnT-1 berkorelasi dengan skor Gleason adenokarsinoma prostat. Ekspresi ZIP-1 berkorelasi dengan aktivasi Caspase-3 pada adenokarsinoma prostat dan adenokarsinoma prostat skor Gleason ≤ 7. Kesimpulan: Transporter Zn berpotensi untuk menjadi faktor prognosis adenokarsinoma prostat.

Sembilan puluh lima persen kanker kolorektal merupakan adenokarsinoma yang
berasal dari lesi prekursor adenoma. Dilaporkan 15%-20% kanker terkait dengan
infeksi virus. Virus yang diduga berhubungan dengan kanker kolorektal adalah
human papilloma virus (HPV) dan tipe tersering adalah 16 dan 18. Hubungan
antara HPV dan kanker kolorektal masih menjadi perdebatan. Penelitian ini
bertujuan untuk mengetahui perbedaan prevalensi infeksi HPV pada adenoma dan
adenokarsinoma kolorektal di Departemen Patologi Anatomik FKUI/RSCM
Jakarta dengan menggunakan teknik polymerase chain reaction (PCR). Bahan
dan Metode: Pemeriksaaan DNA HPV pada 33 kasus adenoma dan 33 kasus
adenokarsinoma kolorektal dengan teknik nested PCR MY/GP dan elektroforesis.
Pada kasus dengan hasil HPV positif, dilanjutkan PCR menggunakan primer
spesifik HPV 16 dan HPV 18. Subjek penelitian berasal dari Departemen Patologi
Anatomik FKUI/RSCM. Hasil: Satu dari 33 kasus (3,0%) adenoma dan 3 dari 33
kasus (9,1%) adenokarsinoma positif infeksi HPV. Satu kasus adenoma positif
HPV bukan merupakan tipe 16 dan 18. Satu kasus adenokarsinoma dengan
positif, HPV merupakan tipe 16, 2 kasus merupakan gabungan tipe 16 dan 18.
Kesimpulan: Prevalensi infeksi HPV pada adenokarsinoma lebih tinggi
dibandingkan adenoma kolorektal. Tipe HPV pada kasus adenokarsinoma
kolorektal merupakan tipe 16 dan 18.

Latar Belakang: Beberapa studi menyebutkan bahwa penilaian kelainan yang terjadi pada prostat adalah dengan mengukur jumlah Androgen Receptor AR pada setiap kasus dan dipakai sebagai evaluasi terhadap keberhasilan terapi hormonal baik pada hiperplasia prostat/benign prostat hyperplasia BPH maupun adenokarsinoma prostat /adenocarcinoma prostate CaP . AR memerankan peran dalam proses pertumbuhan, differensiasi dan memelihara kondisi jaringan prostat tetap normal. Pada penelitian lain menyebutkan bahwa ekspresi AR positif berhubungan erat dengan gambaran derajat dan differensiasi tumor serta nilai skor Gleason. Tujuan penelitian ini untuk mengetahui perbedaan ekspresi AR pada hiperplasia prostat dan adenokarsinoma prostat.Bahan dan Metode: Penelitian ini menggunakan metode potong lintang. Sampel penelitian terdiri atas 20 kasus hiperplasia prostat dan 25 kasus adenokarsinoma prostat tipe asinar. Dilakukan pulasan imunohistokimia AR dan penilaian intensitas pulasan pada inti stromal dan inti epitel dengan menggunakan histoscore H-score . Hasil: Terdapat perbedaan bermakna kadar PSA antara hiperplasia prostat dan adenokarsinoma prostat p=0,004 . Ekspresi AR pada inti sel stromal pada hiperplasia prostat lebih tinggi dibandingkan pada adenokarsinoma prostat, dan mempunyai hasil statistik yang bermakna p=0,000 . Sedangkan ekspresi AR pada inti sel epitel menunjukkan hasil yang tidak bermakna pada kedua kelompok kasus p=0,152 . Intesitas ekspresi AR pada adenokarsinoma prostat dengan skor Gleason rendah le;7 lebih kuat dibandingkan adenokarsinoma prostat dengan skor Gleason tinggi >7 .Kesimpulan: Ekspresi AR pada inti sel stromal pada hiperplasia prostat lebih tinggi dibandingkan pada adenokarsinoma prostat. Peningkatan skor Gleason cenderung diikuti dengan penurunan intensitas ekspresi AR. Ekspresi AR pada hiperplasia prostat dan adenokarsinoma prostat dapat digunakan dalam prognosis dan prediksi serta evaluasi keberhasilan terapi hormonal.

ABSTRACT
"Background Previous studies suggested that Androgen Receptor AR expression could be used to evaluate the successful rate among patient with Benign prostate hyperplasia BPH or Adenocarcinoma acinar of the prostate CaP treated with hormonal therapy. AR plays role in prostate growth and differentiation, and maintains the normal state of the tissue. While in pathologic state, AR expression correlate with tumor grade and differentiation, and Gleason score GS . This study aimed to compare the expression of AR between BPH with CaP.Method This research use a cross sectional study conducted twenty cases of BPH and twenty five cases of CaP. Each tissue were stained using antibody against AR, and histologically reviewed. The captured photomicrographs were further analyze using histoscore H score .Result There was statistically signifinat levels PSA between BPH and CaP p 0,004 . The nucleus of stromal AR expression in BPH group compare to CaP group was statistically significant difference diagnose vs nucleus of stromal AR expression p 0,000 . Meanwhile, no significant difference is found between nucleus of epithelial AR expression between two group p 0,152 . The intensity expression AR in CaP with low Gleason score GS le 7 is higher than CaP with high Gleason score GS 7 Conclusion BPH expresess more nucleus of stromal AR than CaP. The higher Gleason score tends followed by declining intensity expression of AR. Thus suggest AR rsquo s role can use in prognosis, prediction and evaluation of hormonal theraphy of BPH or prostate malignancy."

"CA 19-9 merefleksikan derajat keparahan adenokarsinoma kaput pankreas ditunjukkan oleh beberapa studi berhasil menemukan korelasi peningkatan CA 19-9 dengan resektabilitas adenokarsinoma kaput pankreas. Penelitian ini bertujuan melakukan evaluasi hubungan dan nilai diagnostik CA 19-9 dalam memprediksi resektabilitas adenokarsinoma kaput pankreas. Penelitian dilakukan secara potong lintang mengambil data dari rekam medis Rumah Sakit dr. Cipto Mangunkusumo tahun 2016–2019. Pasien terdiagnosis adenokarsinoma kaput pankreas secara histopatologis atau pencitraan abdomen, berusia ≤65 tahun, dan memiliki catatan pemeriksaan kadar CA 19-9 diikutsertakan dalam penelitian ini. Selain kadar CA 19-9, peneliti juga menilai factor terkait operabilitas. Tercatat 54 subjek dengan rerata usia 53,78±11,13 tahun. Ditemukan adanya korelasi positif (0,850) dan signifikan antara tingginya kadar CA 19-9 dengan resektabilitas tumor kaput pankreas. Untuk operabilitas, ditemukan perbedaan bermakna kadar CA 19-9, albumin, dan skor Karnofsky pada kelompok pasien tumor kaput pankreas resectable dan unresectable. Titik potong kadar CA 19-9 tercatat sebesar 140,65 U/mL, dengan sensitivitas sebesar 82,76% (64,23%–94,15%), spesifisitas sebesar 72,00% (50,61%–87,93%), dan AUC sebesar 0,784. CA 19-9 berhubungan secara signifikan dengan resektabilitas tumor kaput pankreas. CA 19-9 memiliki nilai diagnostik yang baik dalam mempredisksi resektabilitas tumor ini.

This study would like to evaluate the relationship and diagnostic value of CA 19-9 in predicting the resectability of pancreatic head carcinoma. The cross-sectional study took data from the medical records at dr Cipto Mangunkusumo Hospital in 2015–2019. Patients diagnosed with pancreatic head carcinoma based on histopathologic or abdominal imaging, aged ≤75 years, and who had a recorded CA level of 19-9 were enrolled in the study. The investigators also assessed parameters of operability. Of 54 patients with similar characteristics were enrolled, with a mean age of 53.78 ± 11.13 years. It was found that there was a positive (0.850) and significant correlation between high levels of CA 19-9 and unresectable pancreatic head carcinoma. We found significant differences in levels of CA 19-9, albumin, and Karnofsky score in the resectable and unresectable groups of pancreatic head carcinoma. The cut-off point for CA 19-9 levels was 140.65 U / mL, with a sensitivity of 82.76% (64.23%-94.15%), specificity of 72.00% (50.61%-87.93 %), and AUC of 0.784. CA 19-9 was significantly associated with the pancreatic head carcinoma resectability. CA 19-9 has a good diagnostic value in predicting the resectability of these tumors."

"ABSTRAK
Adenocarcinoma paru merupakan salah satu jenis kanker paru yang sering ditemukan pada pasien. Pada pasien yang mengalami berupa efusi pleura dapat memperburuk prognosis dan mempersulit terapi pasien. Deteksi mutasi genetik penyebab timbulnya sel kanker dapat membantu menentukan terapi yang tepat bagi pasien dengan prognosis yang lebih baik. Metodi genotyping saat ini telah banyak dikembangkan dalam menentukan biormarker sel kanker, salah satunya adalah teknik RFLP Restriction Fragment Length Polymorphism . Penelitian ini bertujuan untuk mendeteksi adanya salah satu mutasi gen yaitu KRAS kodon 12 dan kodon 13 dengan metode RFLP yang relatif lebih sederhana dibanding metode lainnya. Partisipan pada penelitian potong lintang cross section adalah pasien adenocarcinoma paru stage 4 dengan komplikasi efusi pleura. Rentang usia partisipan adalah 28-71 tahun. Sampel yang digunakan pada studi ini berjumlah 28 dan berasal dari efusi pleura yang disimpan pada kertas saring. Hasilnya tidak ditemukan adanya mutasi KRAS kodon 12 dan kodon 13 pada seluruh sampel 0/28 .

ABSTRACT
Pulmonary adenocarcinoma is one type of lung cancer that is often found in patients. In patients experiencing pleural effusion may aggravate prognosis and complicate patient therapy. Detection of genetic mutations causing cancer cells can help determine the right therapy for patients with a better prognosis. A current genotyping methodology has been widely developed in determining cancer cell biomarkers, one of which is the RFLP Restriction Fragment Length Polymorphism technique . This study aims to detect the presence of one of the gene mutations KRAS codon 12 and codon 13 with RFLP method is relatively more simple than other methods. Participants in cross sectional study were stage 4 adenocarcinoma patients with complications of pleural effusion. The age range of participants is 28 71 years. The sample used in this study amounted to 28 and came from pleural effusions stored on filter paper. The result was no mutation of KRAS codon 12 and codon 13 on all samples 0 28 ."

"Latar Belakang : Mutasi pada gen Epidermal Growth Factor Receptor (EGFR) berhubungan dengan karsinogenesis Adenokarsinoma paru terutama pada usia muda yang tidak terpajan cukup lama oleh rokok sebagai karsinogen. Penelitian ini untuk mengetahui profil mutasi gen EGFR dan angka tahan hidup pasien adenokarsinoma paru usia muda.
Metode: Penelitian observasional kohort retrospektif dan prospektif pada Adenokarsinoma paru usia muda yakni usia <45 tahun dibandingkan dengan usia yang lebih tua. Data diambil dari catatan medis rumah sakit umum pusat Persahabatan Jakarta 2012-2013 dan dilakukan observasi progresivitas dan kematian selama 2 tahun pasca tegak diagnosis.
Hasil: Total pasien Adenokarsinoma paru adalah 218 orang terdiri dari 65 orang usia muda dan 153 orang usia tua. Karakteristik Adenokasrsinoma paru usia muda adalah perempuan (58,3%), bukan perokok (66,7%), stage lanjut (98,5%), status tampilan WHO ≤2, metastasis ke luar rongga toraks (43,1%). Proporsi mutasi EGFR positif pada usia muda lebih tinggi dibandingkan usia tua (70,8%vs51%; p=0,007). Mutasi gen EGFR usia muda terdiri dari 36,9% delesi ekson 19; 30,8% mutasi ekson 21 L858R; 3,1% mutasi ekson 21 L861Q dan 29,2% wild type. Masa tengah tahan hidup Adenokarsinoma paru usia muda dengan EGFR positif yang diberikan EGFR tyrosine kinase inhibitor adalah 652 hari (590-713 hari, IK 95%) dengan angka tahan hidup 1 tahun adalah 87,5% dan masa bebas penyakit adalah 345 hari (IK 95%, 323-366 hari). Delesi ekson 19 memberikan masa bebas penyakit yang lebih baik dibandingkan dengan mutasi ekson 21 (RR 2,361; IK 95% 1,126-4,952; p=0,023). Masa tengah tahan hidup Adenokarsinoma paru usia muda dengan mutasi EGFR wild type yang mendapat kemo/kemoradioterapi adalah 515 hari (IK 95%, 487-542) dengan angka tahan hidup 1 tahun adalah 70,7% dan masa bebas penyakit adalah 202 hari (IK 95%, 137-266 hari).
Kesimpulan: Profil mutasi gen EGFR pada Adenokarsinoma paru usia muda sangat penting dalam pemilihan terapi lini pertama sehingga dapat meningkatkan angka tahan hidup.

Introduction: Epidermal Growth Factor Receptor (EGFR) mutation is associated with Lung Adebocarcinoma carcinogenesis particularly in young patients which don?t have long exposure to smoke as carcinogen. This study investigate Profile of Epidermal Growth Factor Receptor Mutation and Survival in Young Patients with lung Adenocarcinoma.
Method: Retrospective and prospective observational cohort study in lung Adenocarcinoma <45 years old compare with olders. Data are taken from medical record Persahabatan hospital Jakarta 2012-2013 and we observed for progressivity and mortality in 2 years since diagnosis.
Results: A total 218 lung Adenocarcinoma consists of 65 young patients and 153 olders. Young lung Adenocarcinomas are female (58,3%), nonsmokers (66,7%), advanced stage (98,5%), performance status WHO ≤2, extrathoracic metastatics (43,1%). EGFR mutation in youngers are higher than olders (70,8%vs51%; p=0,007). Mutation in young patients consists are 36,9% exon 19 deletion; 30,8% exon 21 L858R mutation; 3,1% exon 21 L861Q mutation and 29,2% wild type. Overall survival (OS) young patients with EGFR mutation positive treated with EGFR tyrosine kinase inhibitor are 652 days (95% CI, 590-713 days), 1 year survival is 87,5% and progression free survival (PFS) are 345 days (95% CI, 323-366 days). Exon 19 deletion give higher PFS than exon 21 (RR 2,361; IK 95% 1,126-4,952; p=0,023). Overall survival young patients with EGFR wild type treated with conventional chemotherapy are 515 days (487-542 days, 95%), 1 year survival is 70,7% and their PFS are 202 days (137-266 days, 95% CI).
Conclusion: EGFR mutation profile in young lung Adenocarcinoma is important to choose first line therapy so that can increase their survival.

"Prevalensi kanker kolorektal semakin meningkat di Indonesia. Salah satu penanganan kanker kolorektal, khususnya pada pasien kanker rektum adalah dengan prosedur pembedahan Abdominoperineal Resection APR . Hasil dari tindakan ini stoma permanen. Pasien dengan tindakan APR mengalami banyak perubahan yang dapat mempengaruhi kualitas hidup pasien. Studi kasus ini bertujuan untuk menganalisis keefektifan implementasi discharge planning dengan materi hidup dengan stoma pada pasien Ca Recti post tindakan Abdominoperineal Resection. Analisis kasus dilakukan dengan pendekatan Keperawatan Kesehatan Masyarakat Perkotaan. Studi kasus ini merekomendasikan discharge planning yang komprehensif mengenai hidup dengan stoma pada pasien kanker kolorektal yang terpasang stoma.

The prevalence of colorectal cancer increases in Indonesia. One of the treatment for colorectal cancer, especially for rectal cancer is by Abdominoperineal Resection APR surgery. The result of this surgery is permanent stoma. Patients with APR surgery experience many changes that can affect their quality of life. This case study aims to analyze the effectiveness of the discharge planning implementation with ldquo;Living with Stoma rdquo; topic in Ca Recti post Abdominoperineal Resection patient. The case analysis is done with the Urban Community Health Nursing approach. This case study recommends a comprehensive discharge planning of living with stoma in colorectal cancer patients with stoma. "

"Keganasan pankreas merupakan keganasan dengan angka kematian yang tinggi, dengan Adenokarsinoma Duktal Pankreas/Pancreatic Ductal Adenocarcinoma (PDAC) mencakup 85-90% kasus. PDAC memiliki perjalanan penyakit yang sangat agresif, dan seringkali baru terdiagnosis pada stadium lanjut. Penegakan diagnosis pasti PDAC seringkali hanya dapat dilakukan melalui sediaan terbatas baik berupa biopsi maupun endoscopic ultrasound-guided fine-needle aspiration/EUS-FNA. Salah satu tantangannya adalah membedakan PDAC dari jaringan pankreas non-neoplastik/reaktif. Penelitian ini akan membahas mengenai peran von Hippel-Lindau gene product/pVHL dalam membedakan PDAC dengan jaringan pankreas non-neoplastik, serta hubungannya dengan profil klinikopatologiradira PDAC. Penelitian ini merupakan penelitian observasional analitik dengan desain cross-sectional pada kasus PDAC dan jaringan pankreas non-neoplastik yang dilakukan di RSCM pada sampel yang diperoleh pada bulan Januari 2012 hingga September 2023. Sampel penelitian dibagi menjadi 2 kelompok besar, yaitu kelompok PDAC dan pankreas non-neoplastik. Pemilihan sampel dilakukan dengan menggunakan simple random sampling dari kasus-kasus yang memenuhi kriteria inklusi dan tidak termasuk dalam kriteria eksklusi. Dilakukan pulasan imunohistokimia pVHL dan perhitungan Histoscore/H-score serta penentuan cut-offnya untuk membagi ekspresi pVHL menjadi tinggi dan rendah dan hubungannya dengan PDAC dan non-neoplastik, serta profil klinikopatologi pada kelompok PDAC. Penelitian ini menunjukkan tidak terdapat perbedaan ekspresi pVHL pada kelompok PDAC dan non-neoplastik, dan staging pN memiliki hubungan bermakna dengan ekspresi pVHL pada PDAC. Ekspresi pVHL yang rendah lebih banyak ditemukan pada PDAC berdiferensiasi sedang, tidak ditemukan invasi limfovaskular maupun invasi perineural, memiliki batas sayatan yang tidak bebas, memiliki staging pT2, pN0, M0, dan kesintasan > 7 bulan. Sebaliknya, ekspresi pVHL yang tinggi juga lebih banyak ditemukan pada PDAC berdiferensiasi sedang, ditemukan invasi limfovaskular, tidak ditemukan invasi perineural, status batas sayatan yang bebas, staging pT2 dan pT3, pN1 dan pN2, M0, dengan kesintasan ≤ 7 bulan. Temuan ini berbeda dengan penelitian sebelumnya yang mendapati hilangnya ekspresi pVHL pada tumor PDAC, dan sebaliknya pada duktus pankreas non-neoplastik. Hal ini kemungkinan disebabkan oleh perbedaan klon antibodi yang digunakan pada penelitian ini dibandingkan dengan penelitian sebelumnya. Klon antibodi yang digunakan adalah VHL40, sedangkan penelitian-penelitian sebelumnya menggunakan klon FL-181 yang berikatan dengan asam amino yang berbeda dan memiliki klonalitas yang berbeda pula. Selain itu, pada PDAC dapat terjadi mutasi pada gen VHL yang menghasilkan protein VHL yang non-fungsional yang kemungkinan masih dapat terdeteksi dengan ikatan antigen-antibodi pada penelitian ini. 

Pancreatic malignancy is a malignancy with a high mortality rate, with Pancreatic Ductal Adenocarcinoma (PDAC) accounting for 85-90% of cases. PDAC has a very aggressive disease course, and is often only diagnosed at an advanced stage. Establishing a definite diagnosis of PDAC can often only be done through limited sample from biopsy or endoscopic ultrasound-guided fine-needle aspiration/EUS-FNA. In such limited sample, differentiating PDAC from non-neoplastic/reactive pancreatic tissue can be challenging. This research will discuss the role of von Hippel-Lindau gene product/pVHL in PDAC and non-neoplastic pancreatic tissue, as well as their relationship with PDAC pathological factors. This research is an analytical observational study with a cross-sectional design on cases of PDAC and non-neoplastic pancreatic tissue conducted at RSCM on samples obtained from January 2012 to September 2023. The research samples were divided into 2 large groups, namely the PDAC and non-neoplastic pancreatic groups. Sample selection was carried out using simple random sampling from cases that met the inclusion criteria and were not included in the exclusion criteria. Immunohistochemistry of pVHL was performed along with calculation of Histoscore/H-score and determination of cut-offs to divide pVHL expression into high and low and its relationship with PDAC and non-neoplastic, as well as pathological factors in the PDAC group. This study shows that there is no difference in pVHL expression in the PDAC and non-neoplastic groups, and pN staging has a significant relationship with pVHL expression in PDAC. Low pVHL expression is more often found in moderately differentiated PDAC, no lymphovascular invasion or perineural invasion, non-free incision margins, staging pT2, pN0, M0, and survival > 7 months. In contrast, high pVHL expression was also found more frequently in moderately differentiated PDAC, lymphovascular invasion was found, no perineural invasion was found, free incision margin status, pT2 and pT3 staging, pN1 and pN2, M0, with survival ≤ 7 months. This finding is different from previous studies which found loss of pVHL expression in PDAC tumors, and vice versa. This difference in results is likely due to differences in the antibody clones used in this study compared to previous studies. The antibody clone used was VHL40, whereas previous studies used the FL-181 clone which binds to different amino acids and has different clonality. In addition, in PDAC there is a mutation in the VHL gene which may produce a non-functional VHL protein that still be detectable by antigen-antibody binding in this study."
