"ABSTRAK
Trauma merupakan penyebab kematian tertinggi pada populasi manusia berusia kurang dari 40 tahun. Adapun cedera kepala merupakan penyumbang kematian yang cukup besar, yaitu mencakup 30% di mana apabila disertai dengan kelainan koagulasi/koagulopati maka mortalitas pada cedera kepala dapat lebih tinggi lagi. Koagulopati dapat menyebabkan lesi perdarahan baru atau penambahan lesi perdarahan yang sudah ada (pada kasus hipokoagulopati) ataupun iskemia/thrombosis intravaskular (pada kasus hiperkoagulopati). Studi mengenai koagulopati pada cedera kepala telah banyak dilakukan tapi sampai saat ini belum dapat dengan jelas dipaparkan.
Evaluasi status koagulasi secara konvensional dapat dilakukan dengan melakukan pemeriksaan trombosit, PT. dan APTT. Namun saat ini, terdapat pemeriksaan yang menilai koagulasi secara menyeluruh dengan melihat viskoelastisitas bekuan darah, yaitu thromboelastografi (TEG).
Penelitian ini merupakan studi pilot pertama yang bersifat prospektif untuk mengevaluasi adanya gangguan koagulasi pada pasien cedera kepala sedang di RSUPN Dr. Cipto Mangunkusumo menggunakan TEG dan pemeriksaan hemostasis konvensional dan hubungannya dengan luaran klinis pasien berupa lama rawat inap dan mortalitas.
Dua puluh pasien dengan cedera kepala sedang yang telah memenuhi kriteria inklusi dan eksklusi dilakukan pengambilan darah untuk pemeriksaan hemostasis konvensional rutin dan dari sampel darah tersebut, diambil sebanyak 0,3 mL untuk pemeriksaan dengan TEG. Dari hasil pemeriksaan, didapatkan bahwa terdapat 60% pasien dengan status koagulasi yang tidak normal dari pemeriksaan TEG, sedangkan dari pemeriksaan hemostasis konvensional hanya didapatkan 5% pasien yang sama dengan status koagulasi yang tidak normal. Selain itu dari hasil TEG, diketahui bahwa mayoritas koagulopati yang terjadi adalah hiperkoagulopati. Lalu dari sisi luaran yang dinilai dari lama rawat inap (LOS) dan mortalitas, didapatkan tidak adanya mortalitas dalam studi ini. Lalu didapatkan perbedaan antara pasien dengan status TEG yang tidak normal dengan yang normal (7 hari berbanding dengan 5,5 hari). Namun setelah dilakukan uji statistik dari masing-masing variabel yang ada, tidak didapatkan kemaknaan secara statistik. Dalam prosesnya, didapatkan data sekunder berupa hasil CT scan kepala yang setelah dilakukan analisa dengan komponen TEG, ternyata didapatkan parameter R time dan alpha angle memiliki kemaknaan statistik dengan temuan CT scan (p<0,001 dan p=0,028).
Dari penelitian ini dapat ditarik kesimpulan bahwa hiperkoagulopati merupakan kelainan status koagulasi tersering yang didapatkan pada cedera kepala sedang. Adapun terdapat perbedaan hasil pemeriksaan status koagulasi dengan TEG dan pemeriksaan konvensional serta perbedaan LOS antara pasien dengan status TEG yang normal dan tidak normal, meksipun tidak didapatkan adanya kemaknaan secara statistik. Hal ini dapat dianalisa dengan lebih baik nantinya dengan menambahkan jumlah sampel yang lebih banyak. Selain itu, dapat dipertimbangkan penggunaan data CT scan sebagai salah satu variabel penelitian yang ikut diteliti.

ABSTRACT
Trauma is the leading cause of death in population with age less than 40 year old. Traumatic brain injury (TBI) contribute to 30% in overall mortality that is caused by traumatic event. Traumatic brain injury that is accompanied by coagulopathy is known to have higher mortality rate. Coagulopathy in TBI could cause rebleeding or new bleeding lesion in hypocoagulopathy cases or could cause ischemia or intravascular thrombosis in hypercoagulopathy cases. Many studies have been done regarding coagulopathy in TBI but up until now it is still now clear enough. Platelet count, PT. and APTT test are usually used in the evaluation of coagulation status, but currently, it can be done with viscoelastisity test using thromboelastography (TEG).
This is the first prospective pilot study that try to evaluate coagulation status in moderate TBI and it's outcome (length of stay and mortality) in Dr. Cipto Mangunkusumo Hospital using TEG and conventional hemostasis test using platelet count, PT. and APTT.
Twenty patients with moderate TBI that is enrolled in this study has passed the inclusion and exclusion criteria. We collect 0,3 mL of their blood sample that is being used initially for conventional hemostasis test. We found that 60% patient with abnormal coagulation status from TEG, but from the same patient using conventional test, we only got 5% with abnormal result. Majority of abnormal coagulation status are hypercoagulopathy. From the outcome that is measured by LOS and mortality, we found zero mortality and that there is difference in LOS between patient with normal and abnormal TEG test (7 days compare with 5,5 days). From the statistical analysis we got the result that are not statistically significant. We also got secondary data, which is CT scan of the patient and after we tried to do the statistical analysis we found that there are 2 parameters of TEG (R time and alpha angle) that is highly significant with abnormality on CT scan (p<0,001 dan p=0,028).
From this study, we can conclude that hypercoagulopathy is the most common coagulopathy that can be found in moderate TBI. We also found that there is a difference result between TEG test and conventional hemostasis test. From the outcome's perspective, we also found a difference between LOS of the patient that has abnormal TEG and normal TEG results. Higher sample in future study might be helpful in the analysis of the statistical test's result in this study and CT scan could also be a better additional variable to be studied."

Penelitian ini bertujuan untuk menganalisis pengaruh role stressor dan budaya
organisasi terhadap komitmen perubahan afektif perawat RSIA Budhi Jaya, RS
Medistra dan RS PMI Bogor dengan metode regresi berganda terhadap 119
perawat. Role stressor diukur dengan instrument yang dikembangkan oleh Rizzo
et al. (1970) untuk bagian role ambiguity dan role conflict. Instrumen yang
digunakan untuk mengukur role overload dikembangkan oleh Beehr, Walsh dan
Taber (1976). Budaya organisasi diukur dengan instrument yang dikembangkan
oleh Daniel R. Denison dan Aneil K.Mishra (1995) yang terdiri dari sub variabel
involvement, consistency, adaptability, dan mission. Komitmen perubahan afektif
diukur oleh instrument yang dikembangkan oleh Herscovitch dan Meyer.
Penelitian ini membuktikan bahwa role conflict memiliki pengaruh signifikan dan
langsung terhadap komitmen perubahan afektif perawat RSIA Budhi Jaya, RS
Medistra dan RS PMI Bogor sementara consistency memiliki pengaruh tidak
langsung terhadap perawat melalui mediasi role conflict.

hospital , Medistra hospital and PMI Bogor hospital using multiple regression
toward 119 nurse in those three hospital. Role Stressor was measured with
instrument that developed by Rizzo et al. (1970) for role ambiguity and role
conflict, meanwhile for role overlaod used instrument that developed by Beehr,
Walsh and Taber (1976). Organizational culture was measured with an
instrument developed by Daniel R. Denison and Aneil K.Mishra which consist as
sub-variables such as involvement, consistency, adaptability and mission.
Affective commitment to change was measured with instrument developed by
Herscovitch and Meyer (2002). The result has shown role conflit has significant
influence toward nurse?s affective commitment to change consistency also has
indirect influence toward nurse?s affective commitment to change with role

"Pandemi COVID-19 merupakan peristiwa tidak terduga dan belum pernah terjadi sebelumnya sehingga setiap individu harus dapat beradaptasi terhadap situasi ini termasuk santri di pesantren. Penelitian ini bertujuan untuk mengetahui stresor biologis, psikologis, dan sosial, masalah kesehatan jiwa: stres, ansietas, dan depresi, serta hubungan keduanya. Desain penelitian menggunakan deskriptif korelatif dengan pendekatan cross-sectional. Pengumpulan data dilakukan secara offline mulai tanggal 11 hingga 16 Maret 2021, dengan 222 responden santri tingkat Madrasah Aliyah di Pondok Pesantren. Responden dipilih secara purposive sampling. Penelitian ini menggunakan kuesioner stresor, Perceived Stress Scale (PSS-10), Self Reporting Questionnaire (SRQ-20), dan Beck Depression Inventory (BDI-II). Data dianalisis menggunakan uji Korelasi Spearman (CI 95%). Terdapat hubungan yang signifikan antara stresor psikologis dengan stres, ansietas dan depresi pada santri. Penelitian ini menyimpulkan bahwa santri juga membutuhkan dukungan kesehatan jiwa dan psikososial selama pandemi COVID-19

The COVID-19 pandemic is an unexpected and unprecedented event, so everyone must be able to adapt to this situation, including students at Islamic boarding schools (Pesantren). This study aimed to identify biological, psychological, and social stressors, mental health problems: stress, anxiety, and depression, and the correlation between them. The study was used a descriptive correlative with a cross-sectional approach. The data was collected offline from March 11th to 16th, 2021, with 222 respondents at the Madrasah Aliyah-level students in Islamic Boarding Schools. Respondents were selected by purposive sampling. This study used the Stressor Questionnaire, Perceived Stress Scale (PSS-10), Self Reporting Questionnaire (SRQ-20), and Beck Depression Inventory (BDI-II). Data were analyzed using the Spearman Correlation analysis (95% CI). There was a significant correlation between psychological stressors with stress, anxiety, and depression among students. This research concluded that students of Islamic Boarding Schools also need mental health and psychosocial support during the COVID-19 pandemic. "
