Pasien hemodialisis rawat jalan beresiko mengalami masalah terkait obat.. Apoteker berperan dalam mengidentifikasi dan mencegah terjadinya masalah terkait obat. Penelitian ini bertujuan untuk menilai pengaruh peran serta apoteker terhadap penurunan jumlah dan jenis masalah terkait obat pada pasien hemodialisis rawat jalan di RSU UKI. Penelitian ini dilakukan secara prospektif pada bulan Maret-Mei 2013 dengan menggunakan rancangan pre-post design. Evaluasi dilakukan terhadap 805 obat yang diberikan pada 86 orang pasien. Jumlah masalah terkait obat yang diid entifikasi adalah 337 masalah (41.86% dari jumlah terapi obat yang diresepkan). Jenis masalah terkait obat yang diidentifikasi terdiri dari 18,69% tidak ada efek terapi obat atau terapi gagal; 52,23% efek terapi tidak optimal; 2,37% terdapat indikasi yang tidak diterapi dan 26.71% pasien menderita reaksi obat yang tidak diinginkan bukan alergi. Lima puluh sembilan rekomendasi yang diberikan kepada dokter dan 278 rekomendasi kepada pasien. Penelitian ini dapat menurunkan masalah terkait obat jenis tidak ada efek terapi obat atau terapi gagal (18,69% menjadi 0%), terapi tidak optimal (52,23% menjadi 21,36%), terdapat indikasi yang tidak diterapi (2,37% menjadi 2,08%), dan pasien mendapat reaksi obat yang tidak diinginkan bukan alergi (26,71% menjadi 9,20%). Faktor yang signifikan mempengaruhi terjadinya masalah terkait obat adalah frekuensi hemodialisis, jumlah penyakit penyerta dan jumlah terapi obat. Pasien yang menjalani hemodialisis dengan frekuensi 3X seminggu lebih mungkin mengalami penurunan masalah terkait obat dibandingkan yang 2X seminggu (OR 26.33, 95% CI 2.710-255.884).

ABSTRACT
Outpatient hemodialysis patients prone to risk of drug-related problems. Pharmacists play an important role in identifying and preventing drug-related problems. This study aimed to asses the effect of the role of the pharmacist in reducing the number and types of drug-related problem in ambulatory hemodialysis patients in UKI hospital. A pre-post prospective research was conducted from March to May 2013. A total of 805 drugs given to 86 outpatient of hemodialysis patients. There were 337 drug-related problems (41.86% of total prescribed drug therapy) comprised of 18.69% no effect of drug treatment/ therapy failure , 52.23% in effect of drug treatment not optimal, 2.37% untreated indication and 26.71% adverse drug event (non-allergic). Fifty-nine recommendations were given to the doctors and 278 recommendations to the patients as well. Drug-related problems were reduced no effect of drug treatment/ therapy failure from 18.69% to 0%; effect of drug treatment not optimal from 52.23% to 21.36%; untreated indication from 2.37% to 2.08%; and adverse drug event (non-allergic) from 26.71% to 9.20%. Significant factors influencing the occurrence of drug-related problem are the frequency of hemodilysis, commorbidities and amount of drugs therapy. Patients who had hemodialysis 3 times a week with frequencies more likely to experience decline in drug-related problems than twice a week (OR 26.33, 95% CI 2.710-255.884).

Praktik Kerja Profesi (PKP) di rumah sakit dilaksanakan di Rumah Sakit Anak dan Bunda Harapan Kita. Kegiatan ini berlangsung selama 6 minggu dari tanggal 7 September sampai dengan 16 Oktober 2015. PKP di rumah sakit bertujuan agar mahasiswa apoteker mengerti peranan apoteker dalam pengelolaan sediaan farmasi, alat kesehatan, dan bahan medis habis pakai, dan pelayanan farmasi klinik, serta bekerjasama dengan profesi kesehatan lain. Berdasarkan kegiatan PKP yang dilakukan, dapat disimpulkan bahwa Apoteker di Rumah Sakit Anak dan Bunda Harapan Kita memiliki peran, tugas, dan tanggung jawab dalam pengelolaan sediaan farmasi, alat kesehatan, dan bahan medis habis pakai serta pelayanan farmasi klinik. Mahasiswa apoteker telah berhasil memperoleh wawasan mengenai kegiatan rutin yang dilakukan di rumah sakit. Kegiatan pengelolaan meliputi pemilihan, perencanaan kebutuhan, pengadaan, penerimaan, penyimpanan, pendistribusian, pemusnahan, pengendalian dan administrasi, serta kegiatan farmasi klinis meliputi pengkajian dan pelayanan resep, pelayanan informasi obat, visite, dan dispensing sediaan steril (penanganan obat sitostatik). Kerjasama dengan tenaga kesehatan lain bertujuan untuk menjamin keselamatan pasien demi terciptanya pelayanan kefarmasian yang bermutu. Profession Internship at pharmacy was intended to make apothecary student understand the role of pharmacists, have insight into the implementation of pharmaceutical practice, learning strategies and the development of professional practice of pharmacists, have real description of the issues of pharmaceutical practice in health centers, as well as to communicate and interact with other health professionals. Based on the activities, it can be concluded that the pharmacist at Women's and Children's Hospital Harapan Kita have a role, duties and responsibilities in the management of pharmaceutical, medical devices, and medical materials and consumables pharmaceutical care clinic. Apothecary student have obtained insight into routine activities in pharmacy. Pharmaceutical technical activities include selecting, requirement planning, procurement, reception, storage, extermination, control and administration, and activities include assessment of clinical pharmacy services and prescription drug information service, visite, and dispensing sterile preparations (handling cytostatic drugs).
