ABSTRAK Transmisi epiderniologi dan demografi, serta pergeseran struktur ekonomi menyebabkan meningkatnya tuntutan akan pelayanan kesehatan Perkembangan ekonomi masyarakat Jepara meningkat tetapi ternyata sejak tahun 1994 sampai dengan tahun 1996 terjadi penurunan jumlah pasien di R.S.U.D RA.Kartini khususnya yang berasal dari konsumen perusahaan swasta, B.U.M.N. dan Asuransi. R.S.U.D RA Kartini adalah rumah sakit klas C milik Pemerintah Daerah Tingkat II Jepara. Masalah pokok yang menjadi dasar penelitian adalah belum diketahuinya gambaran tentang faktor-faktor yang menjadi permintaan konsumen perusahaan terhadap pelayanan kesehatan di R.S.U.D RA Kartini dalam rangka rnenyusun strategi bauran pemasaran (product, price, place, promotion). Jenis penelitian yang dilakukan adalah penelitian operasional dengan analisis strategik. Penelitian dilakukan terhadap 12 perusahaan yang terdiri dari perusahaan swasta, BUMN/BUMD dan asuransi dan terletak di Kabupaten Jepara, Kudus dan Pati, 6 perusahaan telah mengadakan kerjasama dan 6 perusahaan belum mengadakan kerja lama dengan RSUD R.A Kartini. Pengolahan dan analisa data dilakukan secara deskriptif dengan membandingkan antara kedua kelompok tersebut. Perusahaan yang telah mengadakan kerjasama didapatkan bahwa prosedur dan keputusan untuk dapat berobat telah dibuat suatu aturan yang jelas. Sedangkan kebutuhan berupa produk inti ( core product ) banyak tertuju kepada kualitas dokter, perawat, tenaga non medis. Kebutuhan fasilitas yang dapat dirasakan dan dilihat ( tangible product ) berupa keramahan petugas, kenyamanan, kebersihan ruangan dan lingkungan. Suatu kebutuhan yang membuat pasien lebih yakin untuk kembali kerumah sakit (augment product) berupa perawatan pasien setelah pulang dari rumah sakit. Sedangkan yang belum mengadakan kerjasama: prosedur dan keputusan untuk berobat belum diatur secara jelas dan bila terjadi masalah kemudian diselesaikan secara kekeluargaan. Dan semua perusahaan rata-rata menghendaki sarana transport siap setiap saat dan proses menunggu dan mendapat pelayanan tidak terlalu lama. Informasi tentang rumah sakit disamping dari masyarakat, peranan tenaga kesehatan/dokter juga cukup besar. Sedangkan faktor-faktor yang ikut berperan untuk mengambil keputusan berobat dan bekerjasama dengan rumah sakit adalah: staf perusahaan, individu , dokter perusahaan yang ditunjuk/dokter keluarga sedangkan faktor power/people ikut juga menentukan keberhasilan proses ini. Sebagai langkah selanjutnya perlu pendidikan dan latihan untuk tenaga medis/nonmedis. Ditunjuk staf rumah sakit untuk dapat mengadakan pendekatan formal dan informal kepada perusahaan dalam rangka kerjasama.
ABSTRACT The transmission of epidemiology and demography and the shift in the economic structure result in the increase for medical care demands. Between 1994 and 1996 the development of economic status of Jepara society increased but the number of patients in R.S.U.D R.A. Kartini decreased, especially those the consumer of private firms, BUMN (State owned corporations), and insurance firms. R.S.U.D. RA. Kartini is a General Hospital of the Second Autonomous Region of Jepara, a C class with 144 beds. The main issue underlying this research is that there is not yet a clear perspective of factors that is related to the demand of hospital benefits mainly from the third firm-consumers in relation with the marketing mix (product, price, place, promotion). The type of research to be undertaken is the operational research through strategic analysis. This research is carried out against 12 private firms, BUMNBUMD (State owned corporations), insurance firms, located in the Regencies of Jepara, Kudus and Pati, of which 6 of them have made a cooperation with RSUD RA Kartini, while the rest have not. Data processing and analyze is done in a descriptive manner by making a comparison between those two groups. The companies which have made a cooperation were found to have settled a procedure for applying medical treatment where the payment system was also clear. While the need in the form of core product substantially was aimed at the quality of physicians, nurses, and non medical personnel?s. The need of patient for tangible product facilities was met by the enmities of the officials, comfort, room and environment sanitation. The need of the patient for an augment product was the medical care after leaving the hospital. For the corporations which have not had cooperation, the procedures and decision to have medical care have not yet been clearly regulated, and when a problem arises it is solved in a family atmosphere .On the average all companies insist that transportation facilities should stand-by and waiting process/getting medical services should not take too long. Information about the hospital besides from the public the role of the medical team/the physicians is also of great importance . While other factors also playing a big role in deciding whether to purchase a medical treatment and cooperate with the hospital are, the company's staff, individuals, the appointed company's physicians /the family doctor, while the power/people's factor also determines the success of this process. As the next step, an education and training facility is needed for medical/non medical personnel in or outside the hospital .A staff should be appointed to make an informal/formal approach towards companies/third party within this frame work. Penyakit batuk pilek adalah merupakan penyakit infeksi saluran pernapasan yang ternyata sebagai penyebab kematian utama bayi dan balita di Indonesia. Prevalensi tertinggi ditemukan di Propinsi Jawa Barat khususnya dipedesaan, yaitu tercatat 36% kematian bayi dan balita akibat penyakit Infeksi Saluran pernapasan pada tahun 1992 (Profil Kesehatan Jawa Barat 1993). Salah satu upaya untuk mengatasi penyakit ini sebelum keadaannya bertambah parah adalah memberi pengobatan sedini mungkin dirumah sesuai anjuran dan harapan pemerintah (Pedoman Pemberantasan Penyakit ISPA,Depkes RI 1993). Penelitian ini bertujuan untuk mengkaji informasi tentang hubungan tindakan ibu mengobati sendiri anak balitanya yang menderita batuk pilek dengan faktor-faktor yang diperkirakan mempengaruhi tindakan ibu tersebut. Penelitian ini adalah merupakan penelitian deskriptif analitik dengan pendekatan analisa silang, dengan menggunakan data sekunder yang diperoleh dari hasil kuesioner modul dan kor Susenas Biro Pusat Statistik tahun 1992. Hipotesa yang diajukan adalah apakah ada hubungan tindakan ibu mengobati sendiri anak balitanya dengan faktor pendidikan ibu, pekerjaan ibu, penghasilan keluarga, jarak rumah ke fasilitas kesehatan terdekat, lama sakit balita dan disertainya demam dipedesaan Jawa Barat. Berdasarkan hasil analisis bivariat didapatkan dari enam variabel babas maka koreIasi yang paling bermakna adalah variabel pekerjaan, lama sakit dan demam (p<0,01), disusul dengan variabel pendidikan dan jarak rumah ke fasilitas kesehatan terdekat (p<0,05). Variabel belanja rumah tangga sebagai proyeksi penghasilan keluarga adalah satu-satunya variabel yang tidak mempunyai hubungan yang bermakna terhadap tindakan pengobatan sendiri (p>0,05). Sedangkan hasil analisis multivariat didapatkan hanya variabel pekerjaan, lama sakit balita dan demam yang mempunyai pengaruh terhadap tindakan ibu mengobati sendiri dirumah (p<0,05). Kesimpulan analisis diatas menyatakan bahwa faktor pekerjaan, pendidikan dan jarak rumah kefasilitas kesehatan terdekat sebagai faktor predisposisi dan juga faktor lama sakit dan keadaan demam balita sebagai faktor kebutuhan adalah sangat berperan terhadap tindakan pengobatan sendiri bagi para ibu dirumah. Untuk itu disarankan perlu adanya peningkatan pengetahuan, kemampuan dan keterampilan para ibu tentang tata cara pengobatan sedini mungkin dirumah yang lebih efektif dan lebih efisien, melalui upaya-upaya penyuluhan dari petugas-petugas kesehatan setempat.
Cough-common cold diseases are made up of respiratory tract infection which are inducing the main factor of infant and under five years old children death in Indonesia. The highest prevalence is found at West Java province, especially at rural area, that is, listed for 36% infant and under five years old children mortality caused by ISPA (ARI) on 1992 (East Java Health Profiles, 1993). One of the effort to overcome this kind of disease before getting worse is commit self treatment at their respective houses in line with government?s hope and suggestion (ARI Fight Management, Depkes RI 1993). The purpose of this research is to analyze information regarding the mother self treatment together with factors which are deemed to have impact on it. This is descriptive analytic research with cross sectional approach by means of secondary data obtained from the result of module and questionnaire from Nationality Social Economy Survey (Susenas) by Statistic Centre Bureau (BPS) 1992. The proposed hypothesis is whether there are Relationship between the mother self treatment and the mother's education, job, the distance of house to the nearest health service facility, family's income (expenditure), the length of suffering from the diseases and or fever by infant and under five years old children at rural area West Java. There are 6 variables based upon the bivariate analysis result, the most significant correlation is job, the length of suffering from and or fever variables (p<0,01), and then education and distance (p<0,05). Family income (expenditure) variable is one of self treatment that not significant (p>0,05). Meanwhile, based on multivariate analysis result, it is found only job, length of suffering from the diseases and fever variable that have impact on self treatment (p<0,05). The conclusion of the said analysis stated that predispose factors (job, education, distance) and necessity factors (length of suffering from the diseases and fever) play important role over the mother self treatment at home. It is suggested that there should have knowledge, ability, skill improvement of the mother as to the ''self treatment manners be more effective and efficient through extension from local health providers.

Pelayanan rawat Inap merupakan pelayanan medis yang utama dirumah sakit dan merupakan tempat untuk interaksi antara pasien dan rumah sakit yang berlangsung dalam waktu yang lama. Pelayanan rawat Inap berhubungan sangat sensitif karena menyangkut perasaan pasien, mutu pelayanan dan citra rumah sakit. Semua itu sangat membutuhkan perhatian pihak manajemen rumah sakit. Kepuasan merupakan perasaan senang atau kecewa seseorang yang muncul setelah membandingkan antara persepsi terhadap kinerja atau hasil suatu produk dan harapannya. Tujuan penelitian ini adalah untuk mengetahui pengaruh antara kualitas pelayanan dengan kepuasan pasien rawat inap di RSU Bunda Margonda,Depok. Hasil penelitian menyimpulkan ada pengaruh antara kualitas pelayanan dengan kepuasan pasien rawat inap di RSU Bunda Margonda dikategorikan tinggi karena memiliki nilai hubungan sebesar 0,810.
Inpatient treatment is the major medical services in a hospital in which the interaction between the patient and hospital take place in a long period of time. Inpatient services is a sensitive subject to to the reason that it is interrelated directly with patient?s emotion and reception, service quality and image of the hospital. Thus, the management executives of a hospital should pay their fullest attention to it. Satisfaction is the feeling of happiness or disappointment that accurs ofter te inpatients of hospital evaluating the hospital ?s performance by comparing it to their expectation. It is the same case when a custumer assessing the quality of a product by comparing it with his or her expectation. The aim of this research is to identity the effect between the service quality and the satisfaction of hospitalized patients of RSU Bunda Margonda, which could be classified as high because the relationship value is as big as 0.810.
