Abstrak :
Pelaporan PWS KIA secara rutin telah dilakukan oleh Puskesmas di Kabupaten Pandeglang, walaupun masih ada yang belum tepat waktu. Laporan tersebut menggambarkan cakupan program KIA di wilayah kerja Puskesmas. Selain berasal dari PWS tersebut laporan KIA juga diperoleh dari LB3 Puskesmas yang juga dilaporkan setiap bulan. Ternyata pemanfaatan laporan-laporan tersebut terlihat masih belum optimal padahal laporan tersebut dapat menjadi bahan untuk mengevaluasi kinerja program KIA di Puskesmas. Dengan PWS KIA dapat terlihat daerah yang cakupannya rendah sehingga dapat segera dilakukan tindak lanjut terhadap keadaan tersebut. Program KIA perlu didukung oleh ketersediaan tenaga, sarana dan peralatan. ApabiIa daya dukung tersebut kuat maka cakupan program KIA akan baik. Untuk itu perlu dikembangkan sebuah sistem informasi evaluasi. Dengan adanya evaluasi program dapat dilihat keterkaitan antara kinerja program dan daya dukungnya. Metode pengumpulan data yang digunakan adalah observasi dan wawancara. Selain itu dilakukan pula plotting lokasi Puskesmas dan Pustu. Sistem informasi evaluasi program KIA dikembangkan dengan rancangan formulir input dan tampilan output yang berisi cakupan program KIA dan rasio sarana dan tenaga yang dibutuhkan sebagai bahan evaluasi. Selain itu dalam program aplikasinya akan dimunculkan pula peta lokasi Puskesmas dan peta tematik cakupan program KIA di Kabupaten Pandeglang.

Routinely, the reporting of PWS KIA had been conducted by Health Centers in the District of Pandeglang although some of them had not submitted the report punctually. That report depicted the coverage of KIA Program in the Health Center working area. Beside from PWS report, KIA report was also got from LB3 of Health Center in which reported every month. The use of such report was not optimally done to evaluate the performance of KIA Program in Health Center. From PWS KIA could he assessed the area where had the coverage of KIA was low. So, the intervention could be conducted immediately on that area. KIA Program should be maintained by the availability of human resources, facilities, and equipments. The coverage of KIA Program would be excellent if those factors supported each other. For that reason, it was necessary to develop an evaluation information system so that the relationship between the performance of program and supporting factors could be reviewed. The method of data collecting used observation and interview. Besides, the study also conducted the location plotting of Health Centers and Supporting Health Centers. Evaluation Information System for KIA Program was developed by input form design and output appearance that contained the coverage of KIA Program and the ratio between required facility and human resource to assess program evaluation. In addition, from application program could be appeared the location map and thematic map of the coverage of KIA Program in the District of Pandeglang.

Salah satu penyebab masih rendahnya keberhasilan program P2M berkaitan langsung dengan kinerja proyek dalam pelaksanaan kegiatan yang telah ditetapkan. Rendahnya kinerja proyek dapat dilihat dari rencana pelaksanaan kegiatan yang telah disusun dalam POA dibandingkan dengan realisasi SPM. Penyusunan laporan bulanan proyek tidak efektif, efisien, akurat dan tepat waktu dan untuk meningkatkan pencapaian kinerja proyek Peningkatan Upaya Kesehatan Program Pemberantasan Penyakit Menular Ditjen PPM-PL Jakarta.diperlukan pengembangan sistem informasi. Metode penelitian dalam penyusunan sistem ini dilakukan melalui pendekatan siklus hidup sistem atau SDLC. Dengan tahapan yang pelaksanaan : studi pendahuluan, perencanaan sistem, analisa sistem, perancangaan sistem, penerapan sistem dan evaluasi/uji coba sistem. Data dan informasi dikumpulkan melalui wawancara mendalam serta operasional reseach observasi terhadap komponen sistem informasi yaitu prosedur dan alur kerja, SDM, sarana dan prasarana dan basis data. Dari hasil penelitian awal menunjukkan adanya masalah pada komponen setiap sistem informasi terutama pada perangkat lunak basis datanya. Data laporan di ketik manual dan dalam program aplikasi lotus 123 sehingga dalam pembuatan laporan bulanan dan triwulan di entry ulang mengakibatkan kebenaran data kurang terjamin dan memerlukan banyak memori untuk penyimpanan file mengakibat untuk evaluasi pelaksanaan Proyek per tolok ukur sulit dilaksanakan. Disamping itu dari penelitian awal dapat diketahui peluang pengembangan sistem, baik dari segi kebijakan pimpinan, dan sumber daya manusia maupun pembiayaan. Hasil penelitian awal sistem menunjukan adanya peluang untuk pengembangan sistem informasi monitoring evaluasi proyek yang memberikan informasi penyerapan anggaran dan fisik sebagai dasar perencanaan anggaran tahun berikutnya dan sebagai bertanggung jawaban penggunaan anggaran secara cepat, tepat dan akurat. Perancangan sistem menggunakan alat pengembangan berupa Data Flow Diagram, algoritma pemograman, Entity Relationship Diagram, kamus data, rancangan input dan dan rancangan output. Ada berbagai keunggulan pada sistem informasi yang baru dibandingkan dengan yang lama dilihat dari proses dan output yang dihasilkan. Kesimpulan pada tesis adalah : Pertama untuk memonitor dan mengevalnasi menilai kemajuan pelaksanaan proyek yang hasilnya dapat digunakan sebagai informasi manajemen, sebagai bahan bimbingan teknis dan perencanaan. Kedua Sistim informasi dirancang untuk mendukung manajer dalam fungsinya sebagai pengambil keputusan. Ketiga tersusunnya basis data proyek secara efektif dan efisien di lingkungan Ditjen PPM & PL sebagai baseline model untuk evaluasi proyek. Ke empat sistem informasi mempunyai peluang pengembangan : berupa dukungan manajemen dan pengelola, tersedianya dana, SDM dan sarana dan prasarana. Ke lima prototype yang dibuat dapat diterapkan di Sub. Bagian Evaluasi dan perlaporan, Bagian Program dan Informasi Ditjen PPM-PL dan indikator kinerja yang dihasilkan evaluasi anggaran per tolok ukur.

One reason for the low effective of the communicable diseases program directly has linked with the low capacity of the Project in the line with the stated program planning. The low effective of the project could be see at the planning of action (POA) which is compare to SPM realization . Un effectiveness, un efficient, low accurate and not punctual with the preparation of the monthly project report. Then, to increase the project capacity needs to develop the information system which bolsters up the implementation of the PUK at the Communicable Diseases Control Program Directorate General of Communicable Disease and Environmental Health Jakarta. (DG CDC and EH). The method for this study was using System Development Life Cycle or SDLC which has some steps for the implementation such as : preliminary study, system planning, system analysis, design system, implementation system, and try out for the system. Information and data was gathered by doing indepth interview and observation through component of the information system by following the procedure and scheme work, human resource, infrastructure and bases of the data. Data from the preliminary study showed that for every component in information system has some problem especially for the soft ware of the data base. Monthly report and quarterly report are typed by manually and Lotus 123. To make a report, reentry data was commonplace and has implication with validity and need ample memory for filing. The above situation then evoke the difficulty for doing a project evaluation for every tolok ukur. Beside, from the preliminary study was known that there are some leeway for system development both from the managerial policy and resources for human and financing aspects. Again, from the preliminary study showed that the leeway to develop projcet monitoring and evaluation system which will give absorption and physically data as a base for the next year program planning and could be responsible to give the information for the budget utilization in fast, effective and accurate. To design the system in this study was using some items like data flow diagram, program algorithm, entity relationship diagram, data dictionary, input design and out design. There are some predominance from the system in this study comparing with the old one from the process and output. From this study a conclusion could be drawn as: first, the system in the study will be good to monitor and to evaluate the advancement for the project implementation where the result from the data monitoring could be good for the managerial information, technical supervision and planning tool. Second, the system was developed to provide the decision maker with the such information to make a decision. Third, as a baseline data for the Directorate General of CDC and EH for the project evaluation. Fourth, information system in this study has some leeway for the improvement in future regarding the management support and stake holder, adequate financial aspect, human resource and infrastructure. Fifth, the prototype which was developed in this study could be implement at Sub Division of Program Evaluation at Division of Planning and Information - Directorate General CDC and EH. Further, the indicator which was using to evaluate the budgeting for every tolok ukur.
