Rumah sakit sebagai institusi pelayanan umum, membutuhkan sistem informasi yang tepat dan akurat serta cukup memadai untuk meningkatkan pelayanan kepada pasien. Pelayanan rumah sakit tersebut termasuk pelayanan laboratorium. Masalah dalam sistem informasi laboratorium di RSUD Pringsewu adalah input yang manual, proses belum otomatisasi dan output belum tepat waktu dan tidak akurat. Tujuan studi untuk mengembangkan model sistem informasi di instalasi laboratorium, sehingga tersedia data dan informasi untuk mendukung evaluasi pelayanan bagi pihak manajemen. Metode menggunakan model incremental and iterative yaitu menggabungkan elemen-elemen dalam model urutan System Development Life Cycle (SDLC) dengan filosofi iterative. Tahapannya yaitu analisis, desain, pengkodean, dan ujicoba. Analisis dilakukan berdasarkan hasil wawancara mendalam menggunakan kuesioner terhadap beberapa informan, telaah dokumen dan observasi pada instalasi laboratorium. Prototype dirancang menggunakan bahasa pemrograman PHP yang bersifat open source. Sistem informasi diharapkan menghasilkan informasi yang cepat, tepat dan akurat yang dapat digunakan pihak manajemen dalam pengambilan keputusan untuk melakukan evaluasi pelayanan.

ABSTRACT
Hospital as a public service institution, requires precise and accurate information systems, as well as sufficient to improve services to patients. The hospital services including laboratory services. Problems in laboratory information systems in Pringsewu hospitals are manual input, unautomation process and output have not been on time and accurate. Studies aim to develop model of information systems in laboratory installation, so available data and information to support service evaluation for the management. The method uses incremental and iterative models which combine the elements in the model order of the System Development Life Cycle (SDLC) with the iterative philosophy. Stages: analysis, design, coding, and testing. Analysis is performed based on the results of in-depth interviews using questionnaires to some informants, document review and observations on a laboratory installation. Prototype is designed using PHP programming language that is open source. Information system is expected to produce information quickly, precisely and accurately which can be used in management decision-making to evaluate the service.

Studi ini bertujuan untuk menyusun perencanaan aplikasi web dinamis yang sesuai dan memenuhi kebutuhan informasi dan komunikasi di RSMM Bogor tahun 2013. Jenis penelitian ini adalah penelitian kualitatif dengan metode riset operasional (operational research) dengan menggunakan metodologi prototyping yang mengikuti tahap siklus hidup pengembangan sistem. Informan untuk pengisian kuesioner, wawancara mendalam dan diskusi kelompok terpadu (focus group discussion) ini adalah informan eksternal dari pasien, dan informan internal dari petugas kesehatan (dokter dan perawat) dan petugas lain. Hasil yang ditemukan dalam penelitian ini adalah seluruh komponen input, proses, output, menggambarkan bahwa semua kebutuhan penyusunan prototype web dinamis sudah terpenuhi sehingga proses penyusunan prototype web dinamis dapat di implementasikan RSMM Bogor.

ABSTRACT
This study aimed to develop a dynamic web application planning appropriate and meet the needs of information and communication in RSMM Bogor in 2013. This research is a qualitative research method of operational research by using the following methodology prototyping phase of system development life cycle. Informant to the questionnaires, in-depth interviews and focus group discussions is the external informants of patients and internal informants from health workers (doctors and nurses) and other officers. Results found in this study are all components of the input process output illustrates that all dynamic web prototype development needs are met so that the process of developing dynamic web prototype can be implemented RSMM Bogor.

Pokok permasalahan yang terdapat pada bagian rawat jalan pada RSIA SINTA, Bandar Lampung saat ini adalah belum adanya sistem administrasi dikarenakan rumah sakit yang akan mulai berdiri. Untuk itu diperlukan sistem informasi manajemen yang memadai untuk menunjang proses sehari-harinya. Sehingga tujuan dari penelitian ini adalah untuk merancang suatu usulan pengembangan sistem informasi yang terintegrasi secara baik dengan bagian-bagian yang terkait. Pemecahan terhadap masalah diawali dengan penelitian kualitatif berupa pengumpulan data, analisis data, dilanjutkan dengan pembuatan rancangan awal sistem usulan, pembuatan kamus data, normalisasi data yang akhirnya dilanjutkan dengan perancangan struktur database. Data-data untuk keperluan pengembangan sistem informasi ini dikumpulkan melalui teknik wawancara dan observasi langsung, terutama mengenai sistem dan aliran informasi. Dari hasil pengumpulan data ini, kemudian dibuat rancangan sistem informasi manual berdasarkan hasil interview, lalu diuraikan kelemahan-kelemahan yang ada serta usulan-usulan perbaikan yang harus dilakukan. Berdasarkan pengumpulan dan analisa data, kemudian dirancang sistem informasi usulan untuk bagian rawat jalan serta hubungannya dengan bagian lainnya dalam RSIA SINTA, yang pada dasarnya merupakan pengembangan dari rancangan sistem informasi manual berdasarkan pengalaman responden pegawai RSIA SINTA.

ABSTRACT
SINTA Maternal and Child Hospital is a start-up hospital. There is an absence in administration system for the outpatient department to support day-to-day process. The purpose of this research is to design an information system improvement that would be well integrated with other departments. Fragmentation of the problem begins with the form of qualitative research data collection, data analysis, followed by making the initial plan proposed system, data dictionary, data normalization which eventually extended with database structure planning. The data for this research was collected through in-depth interviews and direct observations, especially in systems and information flow. From the results of this data collection, manual information system is made based on interviews, then outlined some weaknesses and suggestions for improvement. Based on data collection and analysis, a proposal of information system was designed for the outpatient department and its relationship with the rest department of SINTA Maternal and Child Hospital, which is basically an improvement of the manual information system based on the experience of respondent officers at SINTA Maternal and Child Hospital
