"Tujuan dan latar belakang : High grade glioma mecakup hanya 2% dari seluruh kanker, namun memiliki morbiditas dan mortalitas yang tinggi walaupun dengan menggunakan pendekatan terapi multimodal menggunakan kombinasi modalitas operasi, radiasi, kemoterapi dan targetd therapy. Penelitian ini bertujuan untuk mengetahui korelasi kadar MGMT, sebuah protein repair, yang diperiksa menggunakan teknik ELISA dengan respon tumor terhadap radiasi pada High Grade Glioma sehingga diharapkan dapat menambah pemahaman mengenai sifat biomolekuler dari High grade Glioma.
Metode : Studi ini merupakan sebuah studi restrospektif yang melibatkan 14 pasien yang telah didiagnosa sebagai High Grade Glioma berdasarkan histopatologi dan telah mendapatkan radiasi postoperasi dengan dan/atau tanpa chemosensitizer temozolomide di Departemen Radioterapi RSUPN Cipto Mangunkusumo dari tahun 2004-2015. MGMT diperiksa dengan teknik ELISA dari jaringan tumor yang sudah diparafinisasi. Respon tumor dihitung berdasarkan perubahan volume tumor pada imaging CT/MRI pre dan pasca radiasi.
Hasil: Rerata kada MGMT adalah 184 (160-206) pg/mL. Rerata penyusutan tumor adalah 10,64% (-75.64-80.20%). Tidak didapatkan korelasi antara kadar MGMT dengan respon tumor, dengan r= 0.065 (p=0.825). Pada kelompok yang hanya mendapat radiasi didapatkan r= 0.199 (p=0.607) dan pada kelompok yang mendapat kemoradiasi dengan TMZ didapatkan korelasi negatif dengan r= -0,447 (p=0.45).
Kesimpulan : Tidak ada korelasi antara kadar MGMT dengan respon radiasi. Baik pada kelompok yang mendapatkan radiasi saja ataupun pada kelompok yang mendapatkan kemoradiasi dengan TMZ.

Purpose and background : High Grade Glioma comprises just 2% of all cancer, but it disproportionally has the 6th lowest survival of all cancer found. Despite combined multimodality approach that has been used by clinician which can be the combination of two or more modalities of such : surgery, radiation, chemotherapy and targeted therapy, the mortality and morbidity of HGG remains high. This study aims to know the correlation between MGMT protein expression, a repair protein well known in glioma, with the radiation response, in order to gain more knowledge of the bio molecular behavior of HGG.
Material and Methods : This study is a retrospective study that involves 14 patients which were diagnosed as HGG based on histopathological findings and received postoperative radiation with or without concurrent Temozolomide (TMZ) at the Radiotherapy Department of Cipto Mangunkusumo Hospital from 2004-2015. Tumor MGMT concentration was quantified by Enzyme-Linked Immunosorbent Assay from Formalin-Fixed Paraffin-Embedded (FFPE) tissue. Tumor response was evaluated by comparing pre and post radiation tumor volume by CT and MRI.
Result: MGMT concentration was 184 (160-206) pg/mL. Mean tumor volume shrinkage was 10,64% (-75.64-80.20%). There were no correlation between MGMT concentration and tumor response (r= 0.065, p=0.825). The sample was split according to use of TMZ. In the group that had radiation only, the correlation between MGMT concentration and tumor response was not significant (r= 0.199, p=0.607). In the chemoradiation group there was a moderate negative correlation, but was not significant (r= -0,447, p=0.45).
Conclusion: MGMT protein expression was not correlated with the tumor radiation response. There was a negative moderate correlation between MGMT concentrasion and tumor response in patients who underwent chemoradiation with TMZ, but this correlation was not statistically significant.

"ALDH1A1 merupakan penanda sel induk kanker yang memiliki peran pada diferensiasi dan metastasis dari sel glioma manusia. Riset ini bertujuan untuk menganalisis ekspresi relatif dari ALDH1A1 di sel glioma manusia dengan derajat keganasan yang berbeda. Sampel diperoleh dari 32 pasien di Rumah Sakit Cipto Mangunkusumo yang terdiri dari 19 jaringan glioma derajat rendah, 11 jaringan glioma derajat tinggi, dan 2 jaringan otak normal. Isolasi RNA pada glioma dengan derajat keganasan yang berbeda dilakukan sebelum mengukur kuantifikasi relatif ALDH1A1 menggunakan Real-Time Quantitative Reverse Transcription PCR (qRT-PCR). Riset ini menunjukkan terdapat kecenderungan ekspresi berlebih yang lebih tinggi dari ALDH1A1 pada glioma derajat tinggi dibandingkan dengan glioma derajat rendah. Namun hasil ini tidak signifikan secara statistic. Ekspresi ALDH1A1 bervariasi pada glioma dengan derajat keganasan yang berbeda, tetapi
cenderung lebih tinggi pada glioma derajat tinggi. Oleh karena itu, ALDH1A1 berpotensi untuk menjadi penanda klasifikasi malignansi pada glioma. Akan tetapi, riset selanjutnya diperlukan untuk mendukung bukti ini.

ALDH1A1 is a cancer stem cell marker which plays role in differentiation and metastasis of human glioma cells. This research aims to analyze the relative expression of ALDH1A1 in different grades of malignancy of human glioma cells. Initially, the samples were collected from 32 patients in Cipto Mangunkusumo hospital which consisted of 19 low grade glioma tissues, 11 high grade glioma
tissues, and 2 normal brain tissues. Isolation of RNA was performed prior to measurement of relative quantification of ALDH1A1 by using Real-Time Quantitative Reverse Transcription PCR (qRT-PCR) in different malignancies of glioma. The research revealed a tendency of higher overexpression of ALDH1A1 in high grade human glioma compared to those in low grade. However, this result was statistically insignificant. Expression of ALDH1A1 varied in different malignancies of glioma, but tend to be higher in high grade glioma. Therefore,
ALDH1A1 may become potential marker for malignancy classification. However, further research should be conducted to support this evidence.

"[ABSTRAK
Glioma adalah tumor otak primer yang sampai saat ini sering timbul resistensi
terapi. Sel punca glioma diduga berperan penting dalam resistensi dan rekurensi
sel tumor. Sel punca glioma memiliki penanda permukaan CD133 dan mampu
berpluripotensi dengan mengekspresikan Oct4. Kondisi hipoksia tumor juga
berperan dalam self renewal sel punca glioma. Tujuan dari penelitian ini adalah
untuk mengetahui hubungan keberadaan sel punca glioma dengan keganasan,
pluripotensi dan kondisi hipoksia. Cross sectional digunakan sebagai desain
penelitian dengan jumlah sampel sebanyak 35 jaringan, terdiri atas 15 glioma
derajat keganasan tinggi dan 20 glioma derajat keganasan rendah. Pengukuran
ekspresi relatif mRNA CD133, Oct4 dan HIF-1α menggunakan metode qRTPCR.
Protein HIF-1α dilihat ekspresinya melalui teknik imunohistokimia.
Ekspresi relatif mRNA CD133 dan Oct4 lebih tinggi bermakna (p < 0.05, Mann-
Whitney) pada glioma derajat keganasan tinggi dibanding glioma derajat
keganasan rendah. Protein HIF-1α lebih tinggi bermakna (p < 0,01, Mann-
Whitney) pada glioma derajat keganasan tinggi dibanding glioma derajat
keganasan rendah. Terdapat hubungan ekspresi sel punca glioma CD133 dengan
pluripotensi serta kondisi hipoksia (r = 0,518, r = 0,339; Spearman?s rho) serta
pluripotensi dengan kondisi hipoksia pada derajat keganasan tinggi (r = 0,749;
Spearman?s rho). Ekspresi relatif mRNA CD133, Oct4 dan HIF-1α meningkat
seiring dengan peningkatan derajat keganasan. Terdapat hubungan yang bermakna
antara keberadaan penanda sel punca glioma CD133 dengan pluripotensi dan
kondisi hipoksia pada glioma derajat keganasan tinggi.

"[ABSTRAK
Pendahuluan : Glioma adalah jenis tumor yang paling umum dari neoplasma intraserebral
primer. Tumor ganas primer sistem saraf pusat (SSP) mencapai sekitar 2% dari semua kanker
dan high grade glioma adalah jenis yang paling banyak ditemukan. High grade glioma
menyebabkan tingkat morbiditas dan mortalitas yang tinggi. Saat ini belum ada data yang
menggambarkan profil pasien glioma yang menjalani radioterapi di Indonesia.
Metode penelitian : Penelitian ini merupakan penelitian retrospektif deskriptif analitik
terhadap 121 pasien glioma yang mendapat radiasi di departemen Radioterapi RSUPN Dr.
Cipto mangunkusumo dari Januari 2009 sampai Januari 2014. Data diperoleh dari catatan
medisdan hasil penelusuran melalui telepon terhadap pasien atau keluarganya. Respon tumor
dianalisa terhadap 22 pasien yang mempunyai CT scan atau MRI pre dan post radiasi dengan
menggunakan kriteria RECIST.
Hasil : Sebagian besar pasien adalah laki-laki (53,7%), dengan usia rata-rata 45 tahun.
Histopatologi yang paling banyak ditemukan adalah astrositoma. Prosedur bedah yang paling
banyak ditemukan pada penelitian ini adalah craniotomi removal tumor (70%). Teknik 3D
CRT paling banyak digunakan yaitu pada 77,7% pasien. Nimotuzumab sebagai antibodi
monoklonal digunakan pada 9% pasien. Respon parsial ditemukan 59,1%. Analisis kesintasan
hidup tiga tahun dari seratus sebelas pasien yang memenuhi kriteria didapatkan angka
kesintasan yaitu 46,15%. Analisis kaplan meyer menunjukkan overall treatment time
merupakan faktor prognostik untuk kesintasan hidup (p = 0,016).
Kesimpulan : Teknik operasi terbanyak pada pasien glioma yang menjalani radiasi di
departemen radioterapi RSUPN DR. Cipto Mangunkusumo adalah kraniotomi removal tumor
(70,9%). Teknik 3D CRT adalah teknik radiasi yang paling banyak digunakan. Respon
parsial ditemukan 59,1%. Kesintasan hidup tiga tahun pasien glioma yaitu 46,2% dan overall
treatment time merupakan faktor prognostik yang bermakna untuk kesintasan hidup

grade glioma being the most frekuent type found (70,9%) causes a high morbidity and
mortality rate. There is currently no data describing the profile management of patients
undergoing radiotherapy glioma in Indonesia.
Methods : This study Retrospective analytic descriptive study of 121 glioma patients from
januari 2009 until december 2014. The data was obtained from medical records and indivual
contact via telephone. Tumor response was evaluated in 22 patients with pre and post
irradiation imaging (CT or MRI) using 3D volumetric data and assessed via RECIST
criteria.
Results : Most of our patients were male (53,7%), with median age 45 years old.
Astrocytoma was the most histopathological type found. 70.9% of Patients who received
radiotherapy in Cipto Mangunkusumo hospital were post craniotomi tumor removal. 3D
Conformal technique was used in 81.0% of patients. Seventy nine point three had a two gray
dose perfraction. Provision of chemotherapy in patients undergoing radiation still only at
7.4% of patients. Nimotuzumab as a monoclonal antibody used on 9% patient. The median
overall treatment time was 45 days and delay treatment time 38 days. Fifty nine point one
percent of partial respon was found. Local recurrences were found throughout the follow-up
of 6.6%. Analysis kaplan meyer showed that overall treatment time was a prognostic factor
for overal survival rate (p=0,016).
Conclusions : : Almost seventy one percent of glioma patients who received radiotherapy
had craniotomy removal tumor. 3D Conformal techniques is the most widely used. Fifty nine
point one percent of partial respon founded. Three years overall survival was 46,2% and
overall treatment time was found as a factor that significantly affects overall survival

"Glioma is one of the most common central nervous system tumors that show variant responses towards radiotherapies. Most of the cases especially the high grade Glioblastoma Multiforme have very poor prognosis. Pluripotency of cMyc genes might be another factor for the high glial cell differentiation in glioma thus it may become an alternative therapeutic target. mRNA obtained from 20 glioma samples with different degree of malignancy are converted to cDNA and then amplified. Relative quantification of cMyc mRNA expression is measured by calculating the cycle threshold values of Real Time RT PCR and normalized towards 18s rRNA to predict the relationship between the expression of cMyc and the degree of malignancy. The cMyc expression is increased in accordance with the tumor grade. The cMyc expressions in high grade glioma are 17424.23 folds higher when calibrated to the normal cell, whereas the genes in lower grade tumors are expressed with the rate of 6167.35. Despite the statistically insignificant values the genes express, this research has strengthened molecular diagnosis, specifically pluripotency, to be the factor that gives a greater prognostic relevance than the histopathologic diagnosis. As a conclusion, there is a clinical tendency where the c Myc expression is higher than in high degree glioma compared to low degree malignancy, however it is not statistically significant.

Glioma adalah salah satu tumor sistem saraf pusat yang sering terjadi dan memiliki respon yang variatif terhadap radioterapi. Glioblastoma Multiforme cenderung memiliki prognosis buruk terhadap pengobatan. Pluripotensi mRNA cMyc dapat menjadi salah satu faktor tingginya diferensiasi sel glial pada gliom sehingga dapat menjadi target terapi alternatif. mRNA yang diperoleh dari 20 sampel glioma dengan derajat keganasan berbeda ditransformasi menjadi cDNA dan diamplifikasi menggunakan Accupower Two-Step RT-PCR with SYBR Green. Kuantifikasi relatif mRNA cMyc ditentukan dengan menghitung nilai cycle threshold pada RT PCR ang dinormalisasi dengan rRNA 18S untuk melihat hubungan antara ekspresi cMyc dan derajat keganasan glioma. Ekspresi cMyc ternyata lebih tinggi seiring dengan meningkatnya tingkat keganasan. Ekspresi cMyc pada glioma klasifikasi WHO derajat tinggi senilai 17424.23 kali lebih tinggi dibandingkan dengan ekspresi pada sel otak normal, sedangkan glioma derajat rendah menurut klasifikasi WHO mengalami ekspresi gen cMyc senilai 6167.35. Meskipun nilai yang diperoleh tidak signifikan secara statistik, penelitian ini telah menunjukkan bahwa diagnosis molekuler, terutama pluripotensi, dapat menjadi faktor penentu prognosis glioma selain ditentukan dengan derajat keganasan melalui pemeriksaan histopatologis. Terdapat kecenderungan secara klinis dimana ekspresi relatif mRNA cMyc lebih tinggi pada glioma derajat tinggi dibandingkan dengan glioma derajat rendah, namun nilainya tidak signifikan secara statistik."

"Gen NANOG berperan dalam pembentukan faktor transkripsi yang memiliki DNA binding domain. Protein yang dibentuk oleh gen ini memiliki kemampuan untuk menimbulkan dan mempertahankan sifat pluripotent dari sebuah sel. Dikarenakan sifat sel tumor yang pluripotent, banyak studi telah dilakukan untuk menilai peran NANOG dalam keganansan tumor. Namun, data mengenai peran NANOG pada keganansan gliom belum cukup untuk mengklarifikasi efek NANOG pada perkembangan glioma. Glioma merupakan tumor otak yang paling dijumpai dalam praktik klinis. Tantangan dalam penanganan glioma terletak pada lokasi tumor yang susah dan beresiko untuk dijangkau. Penanganan glioma, beresiko tinggi untuk mengakibatkan kerusakan pada jaringan otak yang dapat berakibat pada kehilangan fungsi tubuh dan bahkan berakibat pada kematian. Dalam kesempatan ini, studi ini dilakukan untuk meninjau peran NANOG dalam keganasan glioma untuk menunjang penanganan dini dan mengurangi mortalitas dan morbiditas penderita. Studi dilaksanankan melalui kuantifikasi gen dengan metode real-time RT-PCR atas specimen glioma yang diperoleh melalui operasi pengangkatan tumor di Departemen Bedah Saraf FKUI-RSCM. Hasil yang diperoleh menunjukan adanya kecenderungan ekspresi NANOG untuk lebih tinggi di glioma tingkat tinggi dibandingkan glioma tingkat rendah walau tidak signifikan menurut statisitik. Diperlukan studi yang lebih besar untuk menunjang peran NANOG sebagain penanda keganasan pada kasus glioma.
NANOG gene codes for a transcription factor with a DNA binding domain that has been found to contribute in maintenance and induction of cell pluripotency. Due to this characteristic, extensive studies have been done to evaluate its function as biomarker of tumor malignancy. However, the role of NANOG in glioma malignancy is not yet elucidated. Glioma, a leading tumor of the brain remains a challenging medical condition due to the location of the tumor. Treatment is complicated due to the high chance of compromising the brain structure which could lead to detrimental effects in body functions. The study conducted is to determine the role of NANOG in glioma malignancy by performing NANOG gene quantification using real-time RT-PCR in low-grade and high-grade glioma samples that was obtained from resection surgery in the Neurosurgery Department of FKUI-RSCM. Statistical analysis, showed that there was not significant difference in NANOG expression between the low-grade and high-grade glioma. Despite the absence of significance, there is a trend for higher expression of NANOG in high-grade glioma compared to low-grade glioma. The result, supports the proposition of NANOG as glioma malignancy biomarker. Further studies need to be conducted with greater sample to bolster the NANOG role in glioma malignancy."

"ABSTRAK
Karsinoma sel basal (KSB) merupakan salah satu tipe kanker kulit yang memiliki prevalensi yang tinggi di berbagai belahan dunia. Adapun disregulasi jalur sonic hedgehog agaknya memiliki peran krusial dalam patogenesis KSB, baik pada KSB herediter maupun sporadik. Meskipun sejauh ini banyak kasus KSB dapat ditangani melalui pendekatan terapi bedah maupun radioterapi, beberapa kasus agaknya bersifat resisten dengan pendekatan-pendekatan di atas. Berdasarkan investigasi kami sebelunya, ditemukan bahwa sebagian kasus, terutama subtipe yang tergolong agresif, memiki kecenderungan terjadinya kasus rekurensi yang lebih tinggi. Sesuai dengan panduan NCCN bagi KSB, kasus-kasus yang demikian agaknya memerlukan pendekatan lainnya; di sinilah peran dari preparat inhibitor jalur sonic hedgehog menjadi bermakna. Untuk menyelidiki lebih lanjut sejauh mana peran jalur sonic hedgehog dalam perkembangan kasus KSB di Indonesia, serta perannya dalam perkembangan pola pertumbuhan agresif yang ditemukan pada beberapa subtipe KSB, kami mengobservasi ekspresi faktor transkripsi GLI1 ? yang berperan sebagai marker dari jalur ini - pada sejumlah subtipe histologi KSB.
Metode penelitian menggunakan blok parafin sebagai sampel, yang diperoleh dari arsip Instalasi Patologi Anatomi Rumah Sakit Kanker Dharmais (RSKD), Indonesia, yang berasal dari jaringan KSB pasien yang berobat di RSKD sepanjang periode 2006-2010. Ekspresi GLI1 dievaluasi melalui teknik imunohistokimia. Dari hasil yang diperoleh, tidak diteukan korelasi yang kuat dan bermakna antara ekspresi GLI1 dengan agresifitas pola pertumbuhan subtipe histologi KSB.

ABSTRACT
Basal cell carcinoma (BCC) is one form of skin cancer that has high prevalence worldwide. It seems that the aberration of sonic hedgehog pathway plays pivotal roles in its pathogenesis, whether hereditary or sporadically. Though many cases of BCC can be treated by surgical procedures or radiotherapy, some cases seem to be resistant with the aforesaid approaches. In our previous investigation, we found that several cases, especially subtypes with aggressive growth pattern, have high tendency of recurrence. In accordance with NCCN Guidelines for BCC, such cases may need another approach; hence the use of the inhibitor of sonic hedgehog pathway (such as SMO-inhibitor), may be crucial. In order to further investigate the role of sonic hedgehog signaling pathway in the development of BCC cases in Indonesia, as well as its role in aggressive growth pattern of some BCC subtypes, we observe the expression of GLI1 transcription factor ? as the marker of the pathway ? in various histological subtypes of BCCs.
The method used paraffin blocks, as samples, collected from the archives of Department of Anatomical Pathology, Dharmais Cancer Hospital, Indonesia, which were originated from BCC patients of the hospital from the period of 2006 up to 2010. The expression of GLI1 was investigated by immunohistochemical technique. According to the result, there is no significant correlation between the expressions of GLI1 and the aggressiveness of the growth pattern of BCCs histological subtypes.

"Latar Belakang. Epidermal Growth Factor Receptor mengatur beberapa proses selular penting. Overekspresi EGFR merupakan penanda negatif prognosis pada glioma.
Metode. Tujuh belas pasien astrositoma dilakukan pemeriksaan imunohistokimia untuk mengevaluasi overekspresi EGFR. Seluruh pasien dievaluasi Karnofsky Performance Score 1 tahun pasca operasi.
Hasil. Sembilan pasien high-grade glioma, 67 memiliki overekspresi EGFR yang positif dan hanya 1 pasien yang hidup dengan KPS 70. Empat pasien low-grade glioma dengan overekspresi EGFR yang positif, memiliki KPS 0.
Kesimpulan. Low-grade glioma dengan overekspresi EGFR yang positif memiliki KPS yang rendah pada 1 tahun pasca operasi.

Background. Epidermal Growth Factor Receptor regulate several important cellular process. EGFR overexpression is one of negative prognostic marker in glioma.
Method. Seventeen patients with astrocytoma were performed immunohistochemistry to evaluate EGFR overexpression. All of this patient evaluate for Karnofsky Performance Score at 1 year after surgery.
Hasil. Nine patients with high grade glioma, 67 have positive EGFR overexpression, and only 1 patient still alive with KPS 70. Four patient in low grade glioma with positive EGFR overexpression have KPS 0.
Conclusion. Low grade glioma with positive EGFR overexpression have a poor KPS at 1 year after surgery.

"HIF-2α adalah mediator yang penting dalam reaksi hipoksia di situasi keganasan dan tingginya tingkat ekspresi HIF-2α berkorelasi dengan konsep metastasis, resistensi terapi dan penurunan kualitas prognosis dalam berbagai bentuk pertumbuhan kanker. Karena kemampuan sel glioma otak yang sangat infiltratif, glioma tidak dapat sepenuhnya dihilangkan dengan pembedahan dimana tingkat kekambuhan juga tinggi. Tujuan penelitian ini untuk mengidentifikasi ekspresi relatif dari gen HIF-2α dihubungkan dengan keganasan glioma. Spesimen yang digunakan dalam penelitian ini terdiri dari 22 sampel yang diperoleh dari Departemen Bedah Saraf Fakultas Kedokteran Universitas Indonesia- Rumah Sakit Cipto Mangunkusumo. Ekspresi relatif HIF-2α dianalisis dengan menggunakan quantitative RT-PCR. Hasil penelitian menunjukkan peningkatan ekspresi relatif HIF-2α pada glioma derajat tinggi dibandingkan dengan glioma derajat rendah, namun tidak bermakna secara statistik. Dengan demikian kemungkinan HIF-2α dapat digunakan sebagai penanda prognostik untuk pasien yang didiagnosis glioma, meskipun eksperimen tambahan perlu dilakukan untuk memperkuat fakta ini.
