Apoteker sebagai tenaga profesi di rumah sakit memiliki peran yang sangat penting dalam melaksanakan pelayanan kesehatan di bidang kefarmasian. Untuk dapat memberikan pelayanan kefarmasian yang baik di rumah sakit, seorang Apoteker harus memiliki kemampuan profesional dan pengetahuan yang memadai serta berorientasi kepada kepentingan pasien. Kemampuan profesional apoteker tidak terbatas pada sisi teknis kefarmasian saja, tapi juga dalam fungsi manajemen kefarmasian di rumah sakit. Oleh sebab itu, Fakultas Farmasi Universitas Indonesia bekerjasama dengan Rumah Sakit Kanker Dharmais untuk menyelenggarakan program Praktek Kerja Profesi Apoteker pada bulan Juli-Agustus tahun 2016. "TO diperlukan untuk memastikan keamanan, efektifitas dan rasionalis penggunaan terapi obat pasien sehingga meminimalisir reaksi obat yang tidak dikehedaki apabila terdapat masalah terkait obat pada pasien. Pada laporan ini dilakukan pemantauan terapi obat terhadap pasien pediatrik dengan diagnosis utama pneumonia yang menerima polifarmasi di RSUD Tarakan. Laporan ini dibuat berdasarkan pengumpulan data dan observasi langsung terhadap pasien yang merujuk pada Peraturan Menteri Kesehatan Nomor 72 tahun 2016. Hasil PTO menunjukan bahwa terdapat dua masalah terkait obat pada pengobatan pasien yaitu dosis parasetamol dan octreotida yang rendah, serta interaksi obat antara salbutamol dengan octreotida. Hasil ini menunjukan perlunya peningkatan dosis pada salbutamol dan ocreotida dan pemantauan interaksi obat.

Pediatric patient in intensive care unit with polypharmacy requires therapy drug monitoring (TDM). TDM is needed to ensure safety, effectiveness and rationale of drug therapy hence minimize adverse drug reactions if there are drug-related problems with patients. This report monitors drug therapy for pediatric patients with pneumonia who receive polypharmacy at RSUD Tarakan. This report was based on data collection and observation of patient referring to Minister of Health Regulation Number 72 of 2016. This report shows that there are two drug-related problems in patient treatment, i.e low doses of paracetamol, octreotide and drug interactions between salbutamol and octreotide. Therefore, it requires for adjustment of the dose and monitoring of the drug interactions."

"Interaksi obat terjadi apabila efek dari suatu obat berubah dengan adanya obat lain, obat herbal, makanan, minuman, atau zat kimia lainnya. Apabila pasien mengonsumsi dua atau lebih obat secara bersamaan mempunyai potensi untuk terjadinya interaksi obat, baik menghasilkan respon peningkatan atau penurunan konsentrasi obat di dalam darah. Semakin banyak obat yang dikonsumsi oleh pasien, maka semakin besar kemungkinan terjadinya reaksi yang merugikan. Oleh karena itu, penelitian ini bertujuan untuk mengetahui interaksi obat pada peresepan pasien rawat jalan (depo farmasi 24 jam) RSAB Harapan Kita selama 14 hari periode Bulan April 2022.Penelitian ini merupakan penelitian observasional deskriptif dengan pengumpulan data secara retrospektif. Data penelitian diperoleh dari resep obat pada tanggal 1 April - 18 April 2022 yang diterima oleh depo farmasi 24 jam RSAB Harapan Kita baik pasien golongan BPJS maupun pasien non-BPJS. Interaksi obat-obat per lembar resep dianalisis menggunakan software Lexicomp. Berdasarkan hasil penelitian yang telah dilakukan dapat disimpulkan bahwa interaksi obat ditemukan pada 50% lembar resep BPJS dan 36% lembar resep non-BPJS pada pasien rawat jalan di depo farmasi 24 jam RSAB Harapan Kita. Sebagian besar kejadian interaksi antar obat yang terjadi baik pada resep BPJS maupun Non-BPJS tergolong tidak berisiko tinggi (risk rating C).

Drug interactions occur when the effect of a drug changes with the presence of other drugs, herbal medicines, food, drinks, or other chemical substances. If a patient takes two or more drugs simultaneously, there is the potential for drug interactions to occur, either resulting in a response of increasing or decreasing the concentration of the drug in the blood. The more drugs consumed by the patient, the greater the possibility of adverse reactions. Therefore, this study aims to determine drug interactions in prescribing outpatients (24-hour pharmacy depot) at RSAB Harapan Kita for the 14-day period in April 2022.

This research is a descriptive observational study with retrospective data collection. Research data were obtained from drug prescriptions on April 1 - April 18 2022 which were received by the 24-hour pharmacy depot at RSAB Harapan Kita, both BPJS class patients and non-BPJS patients. Drug-drug interactions per prescription sheet were analyzed using Lexicomp software. Based on the results of the research that has been done, it can be concluded that drug interactions were found in 50% of BPJS prescription sheets and 36% of non-BPJS prescription sheets for outpatients at the 24-hour pharmacy depot at Harapan Kita Hospital. Most of the interactions between drugs that occurred in both BPJS and Non-BPJS prescriptions were classified as not having high risk (risk rating C)."

"Efek samping obat merupakan salah satu masalah terkait obat (drug related problems) terbesar yang sering ditemui pada pasien dan kemungkinan terjadinya lebih tinggi pada resep-resep polifarmasi yang penggunaan obatnya dalam jumlah banyak. Meski begitu, admisi rumah sakit karena efek samping obat masih dapat dicegah. Oleh karena itu, efek samping sebaiknya dicatat, didefinisikan, dan dikaji untuk meminimalkan risiko kemunculan ulang dan mengoptimalkan hasil pengobatan, terutama pada pasien-pasien lanjut usia dengan resep polifarmasi. Pemantauan efek samping obat ini harus dilakukan secara berkesinambungan untuk mengevaluasi konsistensi profil keamanannya, dan membutuhkan banyak kontribusi dari tenaga kesehatan, terutama apoteker, bahkan pasien. Tugas khusus ini dilakukan untuk melihat kemungkinan adanya efek samping dan/atau interaksi pada resep-resep polifarmasi periode Desember 2022 yang masuk ke Apotek Roxy Depok dengan skrining sampel resep polifarmasi di Apotek Roxy Depok periode Desember 2022 dan studi literatur mengenai efek samping dan interaksi obat pada resep tersebut. Untuk tugas akhir ini, diambil 2 sampel resep polifarmasi untuk dilihat kemungkinan efek samping dan/atau interaksi obatnya. Dari kedua sampel resep polifarmasi tersebut, terdapat kemungkinan efek samping maupun interaksi obat, tetapi masih dapat dicegah dan diatasi dengan berbagai cara.

Drug side effects are one of the biggest drug-related problems that are often encountered in patients. The possibility of drug side effects occurrence is also higher in polypharmacy prescriptions that involve large amounts of medication. Even so, hospital admissions due to drug side effects can still be prevented. Therefore, side effects should be recorded, defined, and assessed to minimize the risk of recurrence and optimize treatment outcomes, especially in elderly patients prescribed with polypharmacy. Monitoring the drug's side effects should be carried out continuously to evaluate the consistency of its safety profile, and requires a lot of contribution from health workers, especially pharmacists, and even patients. This project was carried out to see the possibility of drug side effects and/or interactions in polypharmacy prescriptions received by Apotek Roxy Depok throughout December 2022. This project was done by screening samples of polypharmacy prescriptions at the Roxy Depok Pharmacy for the December 2022 period and studying the literature regarding side effects and drug interactions in the prescriptions. 2 samples of polypharmacy prescriptions were taken and from those two samples, there is the possibility of side effects and drugs interactions, but they can still be prevented and overcome in various ways."
