Abstrak :
Apoteker mempunyai peran penting dalam pencegahan masalah yang berkaitan dengan obat. Pemantauan Terapi Obat (PTO) merupakan suatu proses yang mencakup kegiatan untuk memastikan terapi obat yang aman, efektif dan rasional bagi pasien. Kanker kolorektal merupakan pertumbuhan sel tidak normal yang terjadi pada usus besar atau rektum, awal mula terjadinya kanker yaitu tumbuhnya polip secara perlahan pada lapisan kolon atau rektum. Pasien kanker stadium lanjut akan sangat rentan terjadi DRPs yang disebabkan oleh penggunaan berbagai jenis obat, termasuk terapi antineoplastik dan terapi penyakit penyerta yang lain. Penelitian ini bertujuan untuk mengidentifikasi data terapi untuk menetapkan ketepatan indikasi dan dosis terapi, serta mengidentifikasi masalah interaksi obat terkait penggunaan obat pasien. Pada kasus pasien X, Drug Related Problems pada kanker kolon yang terjadi yaitu potensi interaksi obat, dimana obat 5- fluorouracil berinteraksi dengan leucovorin. Hal ini merupakan akibat dari 5- fluorouracil menginduksi antikanker dengan menghambat pembentukan sintase timidilat dan mengurangi folat, sehingga leucovorin dianjurkan diberikan 1 jam terlebih dahulu sebelum 5-fluorouracil, serta monitoring ketat efek toksisitas yang ditimbulkan dari 5-fluorouracil seperti neutropenia, trombositopenia dan diare parah. ......Pharmacists have an important role in preventing drug-related problems. Advanced cancer patients will be very susceptible to DRPs caused by the use of various types of drugs, including antineoplastic therapy and other co-morbidities. This study aims to identify therapeutic data to determine the accuracy of indications and therapeutic doses, as well as identify drug interaction problems related to patient drug use. In the case of patient X, the Drug Related Problems in colon cancer that occur are the potential for drug interactions, where the 5-fluorouracil drug interacts with leucovorin. This is a result of 5-fluorouracil inducing anticancer by inhibiting the formation of thymidylate synthase and reducing folate, so it is recommended that leucovorin be given 1 hour before 5-fluorouracil, as well as close monitoring of the toxic effects of 5-fluorouracil such as neutropenia, thrombocytopenia and severe diarrhea .
