Asiklovir, 9-[(2-Hidroksietoksi)-metil]guanin, adalah derivat guanosin asiklik yang merupakan penghambat selektif terhadap replikasi virus herpes dengan aktivitas antiviral yang poten secara klinis terhadap herpes simpleks dan virus Varicella zoster. Metode kromatografi cair kinerja tinggi yang sederhana dan sensitif telah dikembangkan dan dioptimasi serta divalidasi untuk menganalisis asiklovir dalam plasma manusia in vitro. Penelitian ini bertujuan untuk menentukan jenis dan konsentrasi garam yang digunakan sebagai reagen salting out sehingga metode yang digunakan lebih sensitif dan valid untuk penetapan kadar asiklovir. Sampel plasma (500 μl) diekstraksi dengan 3 ml diklormetan-isopropanol (1:1, v/v) dan penambahan 500 μl larutan amonium sulfat 4 M. Pemisahan komponen dalam ekstrak plasma dianalisis menggunakan kolom Kromasil® fase terbalik 100-5C18 (250 x 4,6 mm, 5μm) dan dideteksi pada panjang gelombang 253 nm. Analisis dilakukan dengan fase gerak yang terdiri dari metanol-larutan natrium dihidrogen fosfat 0,02 M yang mengandung natrium dodesil sulfat 5 mM pH 3,04 dengan variasi elusi gradien perbandingan komposisi fase gerak yaitu 30:70 (v/v) dan 25:75 (v/v) dengan kecepatan alir 1,0 ml/menit. Zidovudin digunakan sebagai baku dalam. Diperoleh koefisien korelasi kurva kalibrasi (r = 0,99546) pada rentang konsentrasi 20,32-1016 ng/ml. Nilai LLOQ yang diperoleh adalah 20,32 ng/ml. Nilai koefisien variasi dan % diff pada tiga konsentrasi asiklovir lebih rendah dari 15%. Uji perolehan kembali asiklovir diperoleh antara 80-120 %. Hasil validasi metode memenuhi kriteria yang ditetapkan.
Acyclovir, 9?[(2-Hidroxysietoxy)-methyl]guanine, is an acyclic guanosine derivative that show selective inhibition of herpes virus replication by clinically potent antiviral activity against herpes simplex and Varicella zoster virus. A simple and sensitive high performance liquid chromatography method has been developed, optimized, and validated for analysis of acyclovir in human plasma in vitro. This study aims to determine the type and concentration of salt used for salting out reagent so that the method used is more sensitive and valid for the assay of acyclovir. Plasma sample (500 μl) was extracted with 3 ml dichlormethane-isopropyl alcohol (1:1, v/v) and the addition of 500 μl ammonium sulphate solution 4M as salting out reagent. Separation of components in the plasma extracts were analyzed using reversed-phase on Kromasil® 100-5C18 (250 x 4,6 mm) column and detected at wavelength of 253 nm. The analysis was done by using mobile phase consisting of methanol-sodium dihydrogen phosphate 0,02 M solution containing sodium dodecyl sulphate 5 mM pH 3,04 with gradient elution variation composition of mobile phase that was 30:70 (v/v) and 25:75 (v/v) at flow rate 1,0 ml/min. Zidovudine used as internal standard. The coefficient of correlation value obtained by calibration curve in the concentration range of 20,32?1016 ng/ml. The lower limit of quantitation (LLOQ) was found to be 20,32 ng/ml. Value of the coefficient of variation and % diff at three concentrations of acyclovir is lower than 15%. The acyclovir recovery percentage was between 80-120 %. The result of validation method fulfilled for the given criteria.

Recurrent intraoral herpes infection (RIH) is one of the recurrent herpes simplex virus type-1 infections (HSV-1). It presents as single or clustered painful ulcers occur mainly on the keratinized mucosa, with the size of one to five millimeter. The Onset of RIH is considered to be associated with various endogenous and exogenous triggers. Therefore, it is important to recognize each individual triggers to prevent and to minimize and the infection for recurring. This report will describe three cases of RIH associated with emotional and physical stress. All patients have experienced continous recurrence of the disease for almost a year. Symptoms included painful multiple minute ulcers on oral mucosa initiated with fever. Serology investigation on antibody against HSV-1 confirmed a reactive elevation in these patients. Treatment consisted of acyclovir per oral 1000 mg daily per oral, counseling on stress condition and intake of nutritional supplementation. Nevertheless, stress conditions were difficult to overcome, therefore long-term medications were recommended to prevent recurrence episodes. One patient was taken a long-term acyclovir therapy and the other two patients were taken long-term acyclovir therapy with Phyllanthusniruri (Meniran) supplementation. Follow-ups on three cases showed the benefit of taking Phyllanthusniruri (Meniran) supplementation as reduction in RIH symptoms, shorten healing time and prolonged recurrence interval.
