"Latar belakang: Empat puluh sampai enam puluh satu persen dari pelayanan di rumah sakit adalah pelayanan keperawatan yang diberikan pada rawat jalan dan rawat inap. Mutu asuhan keperawatan yang diberikan melalui pendekatan proses keperawatan merupakan barometer baik buruknya pelayanan kesehatan di rumah sakit. Proses keperawatan yang diterapkan sebagai pendekatan penyelesaian masalah di RSJHK Jakarta belum baik sekitar 50,5% (pengamatan Tim audit kep, 2000), yang menyebabkan masalah keperawatan klien diselesaikan tidak secara tuntas. Penelitian ini bertujuan memperoleh gambaran hubungan karakteristik perawat, karakteristik pekerjaan dan karakteristik situasi pekerjaan dengan pelaksanaan proses keperawatan di RSJHK Jakarta.
Disain dan metodologi. Penelitian ini termasuk jenis penelitian deskriptif analitik dengan desain cross sectional. Data yang digunakan adalah data primer dan responden. Sampel 134 perawat pelaksana. Pengolahan data dengan analisis univariat, analisis bivariat dengan Uji Chi Square Tingkat kemaknaan 95%.
Hasil. Didapatkan: umur rata-rata perawat 31 tahun, 79,1% perempuan, lama kerja rata-rata 8 tahun, 56% telah kawin, 91,8% dengan latar belakang pendidikan DIII Keperawatan dan seluruhnya telah mendapat pendidikan tambahan, 70,9% perawat dengan tingkat tanggung jawab perawat pelaksana, .88,8 % ada bimbingan teknis dari perawat senior dan 69,4% ada supervisi kepala ruangan dalam pelaksanaan proses keperawatan. Proses keperawatan belum termasuk kategori baik dengan cut off point 80% disebabkan oleh: belum mengikuti pelatihan pelaksanaan proses keperawatan, kurang pemahaman, kurang tenaga, beban kerja berat, proses keperawatan menyita waktu, tugas lain selain tugas pokok, tidak ada bimbingan, arahan, evaluasi dan umpan balik dari kepala ruangan. Analisis bivariat didapatkan 9 dari 10 variabel yang diteliti tidak berhubungan secara bermakna dengan pelaksanaan proses keperawatan, namun mempuayai hubungan yang positif dengan pelaksanaan proses keperawatan kecuali pendidikan formal dan tingkat tanggung jawab. Hal ini diasurnsikan karena tuntutan tugas terhadap sumberdaya keperawatan dengan tingkat pendidikan lebih tinggi tidak ada perbedaan dengan perawat dari latar belakang pendidikan yang lebih rendah, sehingga tidak ada tantangan untuk bekerja lebih baik. Demikian juga dengan tingkat tanggung jawab antara perawat pelaksana dan ketua tim, didapatkan tidak ada perbedaan tuntutan tanggung jawab dalam pelaksanaan proses keperawatan. Supervisi kepala ruangan berhubungan bermakna secara statistik dengan pelaksanaan proses keperawatan dengan a = 0,04.
Kesimpulan: Dari penelitian ini dapat disimpulkan bahwa pelaksanaan proses keperawatan di ruang rawat inap RSJHK Jakarta belum termasuk kategori baik (<80%). Pengkajian, Perencanaan dan Evaluasi berkisar antara 73% - 77% sedangkan diagnosa dan implementasi masing-masing 80,3% dan 89,4%. Persentase rata-rata proses keperawatan 78,2%, lebih tinggi dari hasil pengamatan tim audit keperawatan terhadap pelaksanaan proses keperawatan (50,5%) pada tahun 2000. Ditemukan supervisi kepala ruangan berhubungan bermakna secara statistik dengan pelaksanaan proses keperawatan pada a < 0,05.
Saran: Mengacu pada kesimpulan, peneliti menyarankan agar bidang keperawatan sebaiknya mengevaluasi kinerja kepala ruangan dalam pelaksanaan supervisi kegiatan pelaksanaan proses keperawatan yang dilakukan perawat pelaksana di ruang rawat inap RSJHK Jakarta. Agar kepala ruangan meningkatkan kegiatan supervisi berupa birnbingan, arahan dan evaluasi terhadap kegiatan pelaksanaan proses keperawatan yang dilakukan oleh perawat pelaksana.

Analysis of Factors Relate To the Implementation of Nursing Process in Inpatient Room of the Rumah Sakit Jantung Harapan Kita JakartaIntroduction: Forty to sixty one percent of hospital services are nursing services which is provided in to inpatients and outpatients. The quality of nursing care which is delivered by nursing process as an approach is a barometer of health services in the hospital. The nursing process as an approach to solve the nursing problem in the RSJHK Jakarta has done up to about 50.5% (Observation of the Nursing Audit Team, 2000) that may lead to incomplete solution of the client problems. The purpose of this study was to get the correlations of nursing, work and work-environment characteristics and the implementation of nursing process in the RSJHK Jakarta.
Design and Methodology: This study was the descriptive-analytic research with cross-sectional design. The data used were primary data. Sample involved 134 nursing staffs. The univariat and bivariat analysis were applied for this study. The level of significance was 95%.
Results: The average of age of the nurses was: 31 years, 79.1% of them were females, the average of work experience was 8 years, 56% were married, 91.8% educational background of the nurse were DIII of Nursing and all of them got extra education, 70.9% of them were nurses staff, 88.8% got technical guidance from senior nurses and 69.4% got supervision from Head Nurse in the implementation of nursing process. The nursing process does not at good category with cut off point 80% because-of: did not get nursing process training, lack of understanding about the nursing process, lack of nursing staff heavy work load, time-consuming, have extra duty other then nursing duty, no counseling, guidance, evaluation and feed-back from the Head Nurse. From Bivariat analysis 9 of 10 variables were not had a significant correlation with the implementation of nursing process, but have a positive correlation with implementation of nursing process except for formal education and level of responsibility. It was assumed that because of no difference between the duties of nursing staff with high and low level educational background, so there were not challenges for working better of them. Moreover, there were not differences between the responsibilities of nursing staff with team leader in concern of implementation of nursing process. Supervision of Head nurse has significantly correlation statistically with the implementation of nursing process which was; a = 0.04.
Conclusions: From this study there was a conclusion that the implementation of nursing process at Inpatients Room of RSJHK Jakarta was not in good category (<80%). Assessment, Planning and Evaluation range between 73% - 77%, diagnosis was 80.3% and evaluation was 84.9%_ Average percentage of the nursing process was 78.2%, it was higher than the Nursing Audit Team observation in the year 2000 that is just 50.5%. It was also found that supervision of the Head Nurse has a significant correlation statistically with the implementation of nursing process, which was a< 0.05.
Suggestions: Base on the conclusions, the researcher suggest that the Nursing Department should evaluate the Head Nurse performance of the head nurse in giving supervision to the nursing staffs in the implementation of nursing process at Inpatients Room of RSJHK Jakarta. There for, the Head Nurse should improve the activity in relation to the supervision of nursing staff in implementation of nursing process such as guidance, counseling, and evaluation."

"Kinerja perawat sangat penting untuk dikaji karena jumlah tenaga keperawatan pada umumnya merupakan jumlah tenaga terbesar di setiap rumah sakit. Menurut model teori perilaku dan kinerja oleh Gibson (1987 dalam Ilyas, 1999) bahwa ada tiga variabel yang mempengaruhi kinerja seseorang antara lain variabel individu diantaranya faktor demografis dan variabel organisasi. RSAB Harapan Kita Jakarta sebagai rumah sakit rujukan nasional khusus kasus anak dan ibu hamil/bersalin perlu mengetahui bagaimana kinerja perawat pelaksana di ruang rawat inap dan hubungannya dengan karakteristik demografis dan karakteristik organisasi.
Penelitian ini menggunakan desain cross sectional dengan sampel 156 responden. Variabel independen yang diteliti adalah karakteristik demografis perawat meliputi umur, jenis kelamin, status perkawinan, pendidikan dan masa kerja. Karakterisatik organisasi mencakup kepemimpinan, struktur organisasi, imbalan dan desain pekerjaan. Variabel dependen yaitu kinerja perawat pelaksana di ruang rawat inap.
Hasil penelitian ini menunjukkan bahwa semua variabel dari karakteristik demografis tidak ada hubungan dengan kinerja perawat, sedangkan ada dua variabel dari karakteristik organisasi yaitu kepemimpinan kepala ruangan (p = 0,023) dan struktur organisasi (p = 0,0001) berarti ada hubungan signifikan dengan kinerja perawat pelaksana di ruang rawat inap berdasarkan analisis bivariat dengan uji kai-kuadrat. Pada analisis multivariat dari dua variabel tersebut menunjukkan bahwa struktur organisasi merupakan variabel yang mempunyai hubungan paling bermakna dengan kinerja perawat pelaksana di ruang rawat inap dengan nilai p = 0,0021 dan odds rasio sebesar 3,15 dengan menggunakan uji regresi logistik.
Mempertimbangkan hasil penelitian bahwa variabel struktur organisasi yang mempunyai hubungan paling signifikan dengan kinerja perawat, maka upaya peningkatan kinerja perawat pelaksana di ruang rawat inap tidak terlepas dari sosialisasi struktur organisasi yang ada disamping pengaruh berbagai faktor lainnya baik karakteristik demografis maupun karakteristik organisasi.

An Analysis on Executing Nurse's Work and Its Correlation with Demographic Characteristic and Organizational Characteristic at Inpatient Unit of Harapan Kita Maternal Hospital RSAB JakartaTo study executing nurse's work is very important because they commonly constitute the biggest personnel in number in every hospital. According to Gibson's behavior and work theory (1987 in Ilyas, 1999), there are three variables that influence someone's work. Among of them is individual variable that consists of demography and organizational variable. Harapan Kita Maternal Hospital RSAB Jakarta as national reference maternal hospital should understand executing nurse's work at inpatient unit and its relation with demography and organizational characteristic.
This research used cross sectional design with 156 samples of respondents. The researched independent variable was demography characteristics of nurses covering age, sex, marital status, education and tenure. Organizational characteristic involved leadership, structure of organization, rewards and work design. And the dependent variable was executing nurses' work at inpatient unit of the hospital.
The findings indicated that there was no relation between the entire variables of demography characteristics with nurses' work. Meanwhile there were two variables of organizational characteristic, that were unit manager's leadership (p = 0.023) and structure of organization (p 0.0001), indicated a significant relation with executing nurses' work at inpatient unit based on bivariat analysis with chi-square test. The multivariat analysis of the two variables implied that the structure of organization was the variable that had the most significant relation with their work at inpatient unit with p = 0.0021 and odds ratio as much 3,15 with logistic regression test.
Considering the findings that structure of organization had the most significant relation with nurses' work, so the improvement of their work at inpatient unit could not be separated with socialization of organization structure and some influences of other factors either demography characteristic or organizational one."

"Beban kerja adalah upaya merinci komponen dan target volume pekerjaan dalam satuan waktu dan satuan hasil tertentu. Kinerja adalah penampilan hasil karya personel baik secara kuantitas maupun kualitas dalam suatu organisasi. Kinerja dapat merupakan penampilan individu maupun kelompok kerja personel. Penampilan hasil karya tidak terbatas kepada personal yang memangku jabatan fungsional maupun struktural tetapi juga pada seluruh jajaran personal didalam organisasi. Keduanya ini perlu diseimbangkan dalam pelayanan asuhan keperawatan.
Kinerja perawat pelaksana merupakan masalah selama mereka belum menyadari tentang beban kerja yang meliputi jumlah pasien, tingkat ketergantungan pasien, jenis kegiatan yang akan dilakukan, rata-rata waktu tindakan keperawatan, sistem penugasan dan fasilitas yang merupakan tanggung jawabnya untuk dilaksanakan dengan baik.
Tujuan dari penelitian ini adalah untuk mendapatkan informasi hubungan beban kerja dengan kinerja perawat pelaksana di ruang rawat inap medika bedah RSU Labuang Baji Makassar. Penelitian merupakan penelitian deskriptif analitikal dengan desain cross sectional. Sampel berjumlah 104 orang perawat yaitu total populasi. Tempat pelaksanaan penelitian yaitu di ruang rawat inap penyakit bedah, ruang penyakit dalam, dan ruang gabungan penyakit bedah dan dalam RSU Labuang baji Makassar.
Data yang diperoleh dianalisis dengan analisis univariat, bivariat dengan uji statistik chi square dan tingkat kemaknaan a = 0,05. Tampilan distribusi frekuensi dan persentase dari tiap variabel dan hasilnya menunjukkan antara lain beban kerja tinggi, sedangkan kinerja perawat pelaksana menunjukkan hasil yang seimbang antara yang baik dan buruk. Hubungan beban kerja dengan kinerja perawat menggambarkan bahwa semua variabel beban kerja ada hubungan bermakna dengan kinerja perawat pelaksana. Selanjutnya analisis multivariat, variabel beban kerja yang paling berhubungan dengan kinerja adalah tingkat ketergantungan pasien, hal ini disebabkan karena variabel ini termasuk salah satu yang erat kaitannya dalam produktifitas seseorang dalam melaksanakan asuhan keperawatan. Sedangkan variabel confounding menggambarkan bahwa umur, tingkat pendidikan, dan masa kerja ada hubungan bermakna dengan kinerja perawat pelaksana. Untuk jenis kelamin dan status perkawinan tidak ada hubungan bermakna dengan kinerja perawat. Variabel confounding yang paling berhubungan dengan kinerja perawat adalah masa kerja, oleh karena masa kerja yang lama seseorang dalam suatu tempat / bagian semakin tinggi produktifitasnya, karena semakin berpengalaman dan memiliki keterampilan tinggi dalam menyelesaikan tugas.
Berdasarkan hasil penelitian, disarankan kepada penentu kebijakan di RSU Labuang Baji Makassar untuk lebih meningkatkan kinerja perawat yaitu meninjau kembali keseimbangan antara beban kerja, jumlah pasien, dan jumlah tenaga dengan mengacu pada SK Menkes NO.262 tahun 1979, atau mendekati, penambahan fasilitas baik jumlah dan jenisnya. Untuk kepala bidang perawatan, supaya menerapkan metoda penugasan tim dengan membuat model praktek keperawatan professional di ruang rawat inap medikal bedah. Untuk kepala ruangan, supaya menfasilitasi perawat dalam melaksanakan asuhan keperawatan dan membagi habis tugas sesuai jumlah tenaga. Dan untuk perawat pelaksana agar melaksanakan tugas sesuai beban yang diberikan. Untuk penelitian selanjutnya menggunakan metoda deskriptif analitikal dengan pendekatan observasional, dengan jumlah sampel yang lebih besar untuk seluruh ruang rawat inap.

Relation between Workload and Work of Patience-Faced Nurse at the Inpatient Unit of Medical-Surgical Operation of Labuang Baji Public Hospital MakassarWorkload constitutes an effort to break down component and target of work volume into certain squads of time and out puts. Work is performance of personnel's out come in form of either quality or quantity in an organization. Work could form performance of individual or group of working personnel. Work performance is not merely limited to personnel who have functional or structural position but also to entire staffs of an organization. The both two groups should be balanced in carrying out nursing care service.
The work of patient-faced nurse could be a problem if they are not aware of workloads that cover a number of patients, patient?s independence level, and sort of work they should handle, the average of nursing action time, and assignment system, as well as facilities. They should carry out these responsibilities well.
This research is to gain information on relation between workload and work of patient-faced nurse at the inpatient unit of medical-surgical nursing of Labuang Baji Public Hospital Makassar. This is an analytical-descriptive research designed with cross sectional. The number of total population was 104 nurses. The location of this research was at inpatient unit of surgical disease, unit of inner disease, and joining unit of surgical disease and inner disease of Labuang Baji Public Hospital Makassar.
The obtained data was processed with univariat and bivariat analysis through statistic chi-square test and significant level a = 0.05. The feature of frequency distribution and presentation of each variable as well as its results indicated that the workload was high meanwhile the work of patient-faced nurse showed the balance out come between good and bad. The relation between workload and the nurses' work described that there was a significant correlation between the whole variables of workload and the work of the patient-faced nurses. And multivariate analysis pointed out that the most related variable with their work was the patient's independence because this variable was included one of the very close connections to one's productivity in running nursing care. In the same time, variable confounding described that there was a significant relation between age, education level, and year of work in one group and work of patient-faced nurse in the other group. And there was no significant correlation between gender, marital status in one side and nurse work in the other side. The most connected variable with nurse work was year of work due to the rationale that the longer year of work one possesses in a post or section, the higher productivity he/she owns. And moreover she/he must have got deeper experience and greater skill in executing a duty.
Based on the research, it is recommended that the policy maker of Labuang Baji Public Hospital Makassar should improve nurse work more by reviewing the balance of workload, number of patient and personnel. And it should refer to minister of health's decree No. 262, 1979, or in another word, approximately should add both quantity and kind of facilities. The head of caring department should implement team assignment method by creating professional nursing practice in the inpatient unit of the medical-surgical operation. For the head of unit, he/she should facilitate nurses in handling nursing care and share up the jobs in line with the number of the nurses. And the patient-faced nurses should execute the assignment in connection with their workload given. The following up research conductor is advised to utilize analytical descriptive method combined with observational approach, and greater in number of samples of the whole inpatient unit."
