Praktek kerja profesi di Rumah Sakit Anak dan Bunda Harapan Kita periode bulan Juli ? Agustus tahun 2015 bertujuan untuk memahami peranan, tugas dan tanggung jawab Apoteker di Rumah Sakit sesuai dengan standar pelayanan farmasi, memiliki wawasan, pengetahuan, keterampilan dan pengalaman praktis untuk melakukan praktek kefarmasian di Rumah Sakit serta memiliki gambaran nyata tentang permasalahan praktek kefarmasian serta mempelajari strategi dan kegiatan-kegiatan yang dapat dilakukan dalam rangka perkembangan praktek kefarmasian di Rumah Sakit. Praktek kerja profesi dilakukan selama dua bulan dengan tugas khusus yaitu analisis obat kadaluwarsa Rumah Sakit Anak dan Bunda Harapan Kita bulan Januari-Juli 2015. ABSTRACT The aims of apothecary profession internship program at Rumah Sakit Anak dan Bunda Harapan Kita on July ? August 2015 are to understand the duties and responsibilities clinical pharmacists in accordance with the provisions and ethics in particular the pharmaceutical and healthcare service in general, have insight, knowledge, skill and experience the practical for doing pharmaceutical care in hospitals and having an example about pharmaceutical care and learning strategy and activities can be implemented in pharmaceutical care to the hospital.The apothecary profession internship program conducted during two months in a special assignment that is the analysis of expired drugs Rumah Sakit Anak dan Bunda Harapan Kita in January-July 2015. ;The aims of apothecary profession internship program at Rumah Sakit Anak dan Bunda Harapan Kita on July ? Praktek kerja profesi di Apotek Kimia Farma No. 352 Depok periode bulan September tahun 2015 bertujuan untuk memahami tugas dan tanggung jawab apoteker dalam pengelolaan apotek, serta melakukan praktek pelayanan kefarmasian sesuai dengan ketentuan perundang-undangan dan etika yang berlaku, memiliki wawasan, pengetahuan, keterampilan dan pengalaman praktis untuk melakukan praktek kefarmasian di Apotek serta memiliki gambaran nyata tentang permasalahan praktek kefarmasian serta mempelajari strategi dan kegiatan-kegiatan yang dapat dilakukan dalam rangka pengembangan praktek kefarmasian. Praktek kerja profesi di Apotek Kimia Farma No. 352 Depok dilakukan selama empat minggu dengan tugas khusus yaitu analisis resep terapi peptic ulcer. ABSTRACT The aims of apothecary profession internship program at Apotek Kimia Farma No. 352 Depok on September 2015 are to understand the task and responsibility of pharmacists in the management of pharmacy , as well as doing pharmaceutical care in accordance with the provisions and ethics which is valid for , having insight , knowledge , skill and practical experience for doing pharmaceutical care in pharmacy as well as having an example the problems the pharmaceutical care strategy as well as studies and activities that can be conducted in the context of the development of the practice of kefarmasian .The practice of the professional work in Apotek Kimia Farma No. 352 Depok was done in four weeks by a special assignment that is analysis of prescription peptic ulcer therapy.

Praktek kerja profesi di Puskesmas Kecamatan Tambora Jakarta Barat periode Bulan Oktober tahun 2015 bertujuan untuk memahami peranan, tugas dan tanggung jawab Apoteker dalam praktek pelayanan kefarmasian di Puskesmas sesuai dengan ketentuan perundang-undangan dan etika farmasi yang berlaku dan dalam bidang kesehatan masyarakat, memahami dan memiliki pengetahuan, keterampilan, sikap perilaku serta wawasan dan pengalaman nyata untuk melakukan praktek profesi dan pekerjaan kefarmasian di Puskesmas, melihat dan mempelajari strategi dan pengembangan praktek profesi Apoteker di Puskesmas, memiliki gambaran nyata tentang permasalahan praktek dan pekerjaan kefarmasian di Puskesmas serta mampu berkomunikasi dan berinteraksi dengan tenaga kesehatan lain yang bertugas di Puskesmas. Praktek kerja profesi di Puskesmas Kecamatan Tambora Jakarta Barat dilakukan selama tiga minggu dengan tugas khusus yaitu analisis satu resep penyakit cacar air terkait kelengkapan resep dan kerasionalan resep pasien rawat jalan di apotek Puskesmas Kecamatan Tambora.ABSTRACT The aims of apothecary profession internship program at Tambora District Community Health Centers on October 2015 are to understand the role, duties and responsibilities in pharmaceutical care at Community Health Centers in accordance with the provisions and ethics pharmacy that applies in Community Health Centers, perceive and have knowledge, skill , behavior and attitude insight and experience to do the practice of a profession and pharmaceutical care in Community Health Centers, look and study the development of strategies and practice of the profession of pharmacists at Community Health Centers, having an example of the problems and work of pharmaceutical care in Community Health Centers and able to communicate and interact with other health workers working there. The practice of the professional work in Tambora District Community Health Centers was done in three weeks by a special assignment that is the analysis of one varicella prescription related to prescription completeness and rationality in outpatient in Apotek Kecamatan Tambora.

ABSTRAK
Penatalaksanaan Fistel Enterokutan masih sangat beragam dan sulit dengan tingkat kekambuhan dan kematian pasca pembedahan yang masih tinggi. Tujuan dari penatalaksanaan pasien dengan fistel enterokutan adalah koreksi defisit metabolik dan nutrisi, penutupan fistel dan mengembalikan kesinambungan saluran cerna. Tujuan dari penelitian ini adalah untuk melakukan evaluasi penatalaksanaan bedah pada fistel enterokutan yang dirawat di RSUPN Cipto Mangunkusumo selama tahun 2014-2015. Penelitian ini dirancang secara retrospektif analitik dengan mengambil data rekam medik penderita di RSUPN Cipto Mangunkusumo pada periode 2014-2015. Ditemukan 27 kasus fistel enterokutan, dimana 21 kasus yang di evaluasi, rentang umur 27-65 tahun, terbanyak pada kelompok 40-60 tahun (52,38%), letak fistel terbanyak di ileum (57,14%), high output (71,43%), gizi buruk (52,38%), dilakukan tindakan operatif (85,71%), lama rawat <20 hari (66,67%), rekurensi fistel (19,05%) dan angka kematian (14,29%).ABSTRACT
Management of enterocutaneous fistula still varies and frustrating with high recurrence and mortality rate. The goal of management for patient with enterocutaneous fistula are correct metabolic and nutritional deficits, close the fistula and reestablish continuity of the gastrointestinal tract. The purpose of this study was to evaluate the surgical management of the enterocutaneous fistula treated at Cipto Mangunkusumo Hospital during 2014-2015. This study designed analytic retrospectively by taking the patient medical record data at Cipto Mangunkusumo Hospital ini the period 2014-2015.
