"Bahan baku (raw material) awalnya disimpan di gudang akan ditimbang terlebih dahulu pada dispensing room sesuai dengan jumlah yang ada didistribusikan ke produksi. Ada kalanya bahan baku yang telah ditimbang tidak langsung diproduksi, kejadian ini dapat terjadi bila terdapat suatu hambatan atau masalah terutama jika masalah tersebut memerlukan waktu lama untuk memperbaikinya. Sehingga diperlukan studi raw material holding time, sehingga dapat diketahui berapa waktu maksimum suatu bahan baku yang telah didispensing dapat disimpan dan tetap memenuhi spesifikasi yang ditetapkan. PT Darya-Varia Laboratoria saat ini ingin melakukan studi dalam rangka pemastian terhadap holding time bahan baku. Oleh karena itu, dalam pengerjaan tugas khusus ini dilakukan pembuatan protokol studi raw material holding time Thymol NF. Desain studi raw material holding time Thymol NF ini diawali dengan menimbang raw material sesuai dengan berat satu batch produksi rutinnya. Bahan baku yang ditimbang (dispensing) disimpan pada staging room selama 30 hari dengan suhu 20 - 27℃ dan RH 70% yang diperiksa secara rutin setiap tiga kali dalam sehari. Wadah yang digunakan untuk menyimpan bahan baku Thymol NF pada studi holding time adalah kantong plastik berukuran 10 cm x 20 cm. Pengambilan sampel Thymol NF pada studi raw material holding time ini dilakukan pada hari ke-0, hari ke-14, dan hari ke-30 selama periode penelitian. Jumlah sampel yang akan diambil adalah sebanyak 300 gram untuk tiap kali sampling. Weighed raw materials (dispensing) are stored in the staging room for 30 days at a temperature of 20 - 27 ℃ and RH 70% which is checked regularly every three times a day. Dalam rangka memastikan proses distribusi Cold Chain Product (CCP) yang baik dan benar, maka perlu dilakukan validasi pengiriman Cold Chain Product (CCP). Pada PT Era Caring Indonesia, dilakukan validasi pengiriman CCP sesuai dengan Pedoman CDOB menggunakan kotak berbahan styrofoam yang didalamnya diletakkan 5 buah water pack untuk menjaga suhu, dan temperature data logger untuk mencatat suhu selama pengiriman. Tujuan dari penulisan tugas khusus ini adalah untuk mengetahui kemampuan 5 water pack untuk mempertahankan suhu 2-8℃ pada proses pengiriman Cold Chain Product (CCP) selama waktu tertentu. Berdasarkan data hasil validasi pengiriman dengan menggunakan styrofoam box yang diisi dengan 5 water pack, menunjukkan suhu rereta yang tercatat selama 8 jam yaitu 4,82℃. Dengan suhu terendah tercatat pada 3,8℃ dan suhu tertinggi pada 5,9℃. Dari data ini menunjukkan bahwa hasil validasi pengiriman produk rantai dingin menggunakan styrofoam box yang diisi dengan 5 water pack memenuhi syarat CDOB yaitu suhu produk dapat dipertahankan pada rentang suhu 2 - 8℃ selama waktu pengiriman terjauh PT. Era Caring Indonesia adalah 2-3 jam. Dengan terjaganya suhu produk rantai dingin selama pengiriman pada 2 - 8℃ dapat dipastikan bahwa kualitas produk rantai dingin tetap terjaga.......PBF must have a CDOB CCP certificate to ensure that the quality of CCP drugs during the distribution process is in accordance with the requirements. "Drug Related Problem (DRPs) adalah kesalahan pengobatan yang mempengaruhi pada outcome klinis dari pasien. Jika tidak dikelola dengan baik, mengakibatkan dampak yang lebih luas dan serius bahkan sampai terjadi kejadian sentinel. Di Puskesmas Kecamatan Kalideres terdapat poli PTM yang melayani pasien dengan penyakit kronis seperti hipertensi, diabetes melitus tipe 2, hiperglikemia dan sebagainya. Pasien poli PTM ini biasa mendapat resep yang berisikan obat polifarmasi yang berisiko terjadi Drug Related Problems (DRPs). Oleh karena itu pada penulisan tugas khusus ini dilakukan pengidentifikasian jenis DRP yang terjadi pada pasien poli PTM di Puskesmas Kecamatan Kalideres pada periode April 2023. Metode yang digunakan adalah pengklasifikasi DRP berdasarkan Hepler-Strand dan kemudian direkomendasikan penyelesaian permasalahan berdasarkan literatur. DRPs menurut Hepler-Strand diklasifikasikan sebagai berikut: 1) Pemilihan obat yang tidak tepat; 2) Dosis Subterapeutik; 3) Kegagalan menerima obat; 4) Overdosis; 5) Reaksi obat yang merugikan; 6) Interaksi obat ; 7) Penggunaan obat tanpa indikasi dan; 8) Indikasi yang tidak diobati. Berdasarkan hasil validasi yang dilakukan dapat disimpulkan bahwa dari 5 resep pasien Poli PTM di Puskesmas Kecamatan Kalideres yang diindentifikasi DRPs, terdapat 1 resep yang tidak terdapat DRP dan 4 resep lainnya terdapat DRP. DRP yang didapat adalah terjadinya interaksi obat dan penggunaan obat tanpa indikasi.......Drug Related Problems (DRPs) are medication errors that affect the clinical outcomes of patients. If not managed properly, it can have a wider and more serious impact and even lead to sentinel events. At the Kalideres Sub-district Health Center, there is a NCD clinic that serves patients with chronic diseases such as hypertension, type 2 diabetes mellitus, hyperglycemia and so on. These NCD poly patients usually receive prescriptions containing polypharmacy drugs that are at risk of Drug Related Problems (DRPs). Therefore, in this special assignment writing, the types of DRPs that occurred in Poly PTM patients at the Kalideres District Health Center in the April 2023 period were identified. The method used is DRP classification based on Hepler-Strand and then recommended problem solving based on the literature. DRPs according to Hepler-Strand are classified as follows: 1) Inappropriate drug selection; 2) Subtherapeutic dose; 3) Failure to receive the drug; 4) Overdose; 5) Adverse drug reactions; 6) Drug interactions; 7) Use of drugs without indications and; 8) Untreated indications. Based on the results of the validation carried out, it can be concluded that of the 5 prescriptions of PTM Poly patients at the Kalideres Subdistrict Health Center that DRPs were identified, there was 1 prescription that did not have DRPs and 4 other prescriptions had DRPs. DRPs obtained are the occurrence of drug interactions and the use of drugs without indications."

"Kegiatan pengkajian resep meliputi kajian administrasi, kesesuaian farmasetik, dan pertimbangan klinis. Jika ditemukan adanya ketidaksesuaian dari hasil pengkajian apoteker harus menghubungi dokter penulis resep. Pada setiap tahap alur pelayanan Resep dilakukan upaya pencegahan terjadinya kesalahan pemberian obat (medication error). Penyusunan tugas khusus Praktik Kerja Profesi Apoteker di Apotek Roxy Sawangan bertujuan untuk mengidentifikasi kelengkapan resep obat terapi gangguan pencernaan berdasarkan aspek administratif, aspek farmasetik dan aspek klinis yang terdapat pada Apotek Roxy Sawangan periode April 2023.Tugas khusus ini disusun dengan metode deskriptif dan melakukan kegiatan pengkajian resep pada dua resep pasien di Apotek Roxy Sawangan. Obat yang terdapat pada kedua resep tersebut memiliki indikasi yang beririsan yaitu sebagai terapi gangguan pencernaan. Berdasarkan pengkajian resep yang dilakukan terhadap kedua resep dapat disimpulkan pada resep 1 aspek administratif dan farmasetik sudah lengkap tetapi pada aspek klinis menunjukkan ketidaktepatan dosis pemberian sirup sukralfat. The drugs contained in the two prescriptions have indications that overlap, namely as therapy for indigestion. Based on the prescription review carried out on the two prescriptions, it can be concluded that in recipe 1 the administrative and pharmaceutical aspects are complete but the clinical aspects show the inaccuracy of the dose of sucralfate syrup. "Apotek online BPJS merupakan layanan terbaru dari BPJS untuk membantu masyarakat peserta BPJS dalam mendapatkan obat-obatan secara mudah. Unit Farmasi Rawat Jalan RSUI dalam melakukan klaim obat kronis pasien BPJS dengan pemberian selama 7 hari menjadi satu paket pembayaran INA CBG dan untuk 23 hari dibayarkan dengan tarif apotek online BPJS. Proses pengadaan obat Formularium Nasional yang dilakukan oleh RSUI tidak selalu mendapatkan harga beli yang lebih murah atau sama dengan tarif apotek online BPJS. Untuk itu perlu dilakukan perbandingan harga pembelian obat Formularium Nasional RSUI dengan harga obat Apotek online BPJS pasien rawat jalan periode Mei- Juni 2023 sehingga dengan data ini RSUI dapat meminimalisir kerugian dari harga beli obat yang terlalu mahal dibandingan dengan tarif apotek online BPJS. Tugas khusus ini menggunakan metode deskriptif observasional dengan metode pengumpulan data retrospektif menggunakan data sekunder yang di dapat dari sistem informasi rumah sakit dan juga dari aplikasi apotek online. Berdasarkan hasil analisa data perbandingan harga beli RSUI dengan harga apotek online yang dilakukan, didapatkan obat kategori lebih mahal mendapat persentase tertinggi yaitu sebesar 77%, kemudian obat kategori sama 15 % dan lebih murah 8%. Ini dapat menggambarkan bahwa mayoritas obat Formularium Nasional di RSUI memiliki harga beli yang mahal dibandingkan dengan tarif klaim dari apotek online BPJS. Hal ini dapat mengakibatkan kerugian kepada pihak rumah sakit.
