Praktik Kerja Profesi Apoteker di Rumah Sakit Pusat Pertamina RSPP bertujuan agar mahasiswa calon Apoteker dapat memahami peran dan tanggung jawab Apoteker di Instalasi Farmasi Rumah Sakit, memperoleh wawasan dan pengalaman mengenai praktik kefarmasian di Rumah Sakit, serta mengetahui gambaran nyata permasalahan terkait praktik kefarmasian di Rumah Sakit. Tugas khusus yang diberikan berjudul Penelusuran Literatur Data Farmakokinetik, Stabilitas dan Risiko Medication Error pada Obat-obat High Alert di Rumah sakit Pusat Pertamina. Tugas khusus ini bertujuan untuk mendapatkan data farmakokinetik yang terdiri atas onset, waktu puncak dan durasi; data stabilitas; serta risiko medication error masing-masing obat high alert yang ada di RSPP melalui studi literatur.

ABSTRACT
Internship Program at Pertamina Central Hospital for pharmacist students aims to make students understand roles and responsibilities of pharmacist at hospital rsquo s pharmacy installation, to expand horizon and gain experience about pharmaceutical practice and to know the real condition of pharmaceutical practice rsquo s problems at the hospital. The specific assignment rsquo s title that was given is Literature Investigation on Pharmacokinetic Datas, Stability and Risk of Medication Error of High Alert Medication at Pertamina Central Hospital. The aim of this assignment is to get the pharmacokinetic datas such as onset, peak time and duration stability data and risk of medication error from each of high alert medications at Pertamina Central Hospital by investigating the literature.

Puskesmas merupakan fasilitas pelayanan kesehatan yang menyelenggarakan upaya kesehatan perseorangan dan masyarakat dengan mengutamakan upaya promotif dan preventif. Praktik kefarmasian di Puskesmas merupakan tanggung jawab seorang Apoteker. Praktik Kerja Profesi Apoteker di Puskesmas bertujuan agar calon Apoteker mengetahui peran dan tanggung jawab Apoteker di Puskesmas, memperoleh wawasan dan pengalaman terkait Praktik kefarmasian di Puskesmas serta melihat gambaran nyata permasalahan terkait pelayanan kefarmasian di Puskesmas. Tugas khusus yang diberikan berjudul Penyuluhan ldquo;Bijak Menggunakan Antibiotik rdquo; dengan Media Booklet di Puskesmas Kecamatan Setiabudi Jakarta Selatan. Tujuan dilakukannya tugas khusus ini adalah memberi pendidikan kesehatan dengan tema Bijak Menggunakan Antibiotik kepada pasien dan membuat booklet sebagai media penyuluhan yang menarik, informatif dan edukatif.

Community Health Center is a health care facility that provides health service for individuals and community with prioritizing in promotive and preventive efforts. Pharmaceutical practice at the Community Health Center is Pharmacist rsquo s responsibility. Internship at Community Health Center aims to make pharmacist students know the roles and responsibilities of pharmacist at Community Health Center, to gain the knowledges and experiences about pharmaceutical practice at Community Health Center and observe the real condition of pharmaceutical practice rsquo s problems at Community Health Center. The spesific assignment rsquo s title that was given is Health Education about ldquo Be Wise in Using Antibiotics rdquo with Booklet as the Media of Education at Setiabudi Community Health Center, South Jakarta. The aim of this assignment is to give health education about being wise in using antibiotics for patients and to make the booklet as an informative, educative and attractive media for the people.

Berdasarkan Peraturan Menteri Kesehatan Nomor 73 Tahun 2016, Apotek adalah tempat dilaksanakannya praktik kefarmasian oleh Apoteker. Apoteker harus memiliki kompetensi dalam melakukan manajemen dan pelayanan farmasi klinik di Apotek. Praktik Kerja Profesi Apoteker dilakukan mengingat pentingnya peran Apoteker di Apotek. Tujuan dari Praktik Kerja Profesi Apoteker ini adalah untuk memahami peran dan tanggung jawab Apoteker di Apotek, memperolah pengetahuan dan pengalaman terkait praktik kefarmasian di Apotek, serta memperolah gambaran nyata permasalahan praktik kefarmasian di Apotek. Tugas khusus yang diberikan berjudul Analisis Rancangan Klinik Pratama Kerjasama BPJS dengan Tiga Dokter Umum dan Waktu Pelayanan 24 Jam. Tujuan penulisan tugas khusus ini adalah membuat rancangan klinik pratama kerjasama BPJS yang dapat diimplementasikan.

ABSTRACT
According to Minister of Health Decree number 73 year 2016, Pharmacy is a place of pharmaceutical facility where pharmaceutical practice is done by a pharmacist. Pharmacist should have competencies in conducting management and clinical pharmacy at a Pharmacy. Internship for Pharmacist is held in order to understand the roles of Pharmacist in a Pharmacy. The aims of the Internship are to understand the roles and responsibilities of Pharmacist, gain knowledges and experiences about pharmaceutical practices and get to know the real condition of pharmaceutical practice rsquo s problems at a pharmacy. The special assignment rsquo s title that was given is Design Analysis of First Degree Clinic in collaboration with National Medical Insurance, Three Doctors and Twenty Four Hour Service Time. The aim of this assignment is to make a design of First Degree Clinic in collaboration with National Medical Insurance that can be implemented in the future.

Obat golongan proton pump inhibitor (PPI) digunakan secara luas dalam berbagai gangguan pada saluran gastrointestinal terkait asam lambung dan sering digunakan bersama dengan obat lain sehingga meningkatkan risiko terjadinya interaksi obat. Tujuan dilakukannya penelitian ini adalah untuk menganalisis potensi interaksi obat golongan PPI pada pasien rawat jalan di RSPAD Gatot Soebroto periode Juli-Desember 2015. Penelitian ini menggunakan metode deskriptif analitik-retrospektif pada data resep dan rekam medis pasien rawat jalan periode Juli-Desember 2015 yang menerima obat golongan PPI dan satu atau lebih item obat lain yang dipilih dengan metode pursposive sampling. Analisis dilakukan terhadap 400 lembar resep yang berasal dari 192 pasien. Berdasarkan penelitian ini dapat disimpulkan bahwa terapi obat golongan PPI pada pasien rawat jalan di RSPAD Gatot Soebroto periode Juli-Desember 2015 memiliki potensi interaksi obat pada 324 lembar resep (81,00%) dengan total kasus sebanyak 475 kasus yang terdiri dari 42 kasus interaksi mayor, 138 kasus interaksi moderat dan 295 kasus interaksi minor. Terdapat 14 obat yang memiliki potensi interaksi dengan obat golongan PPI antara lain mikofenolat mofetil, klopidogrel, kilostazol, warfarin, besi, levotiroksin, propranolol, siklosporin, simvastatin, atorvastatin, sianokobalamin, sukralfat, teofilin dan antasida. ......Proton pump inhibitor (PPI) drugs are widely used for the treatment of gastrointestinal tract with gastric-acid disorder and usually used concomitant with other drugs so that increase the risk of drug interaction. This study aimed to analyse the potential of PPI drug interaction in outpatients at Gatot Soebroto Army Centre Hospital period of July-December 2015. This study use analytical descriptive-retrospective method on prescriptions and medical records of outpatients who get PPI drugs with one or more other drugs in the period of July-December 2015, which were selected by purposive sampling method. This analysis study was conducted on 400 prescriptions from 192 patients. This study concluded that PPI therapy in outpatients at RSPAD Gatot Soebroto period of July-December 2015 had potential drug interaction on 324 prescriptions (81,00%) with total of 475 cases, which are consisted of 42 cases of major interactions, 138 cases of moderate interactions, and 295 cases of minor interactions. There are 14 drugs that can potentially interact with PPI drugs, such as: mycophenolate mofetil, clopidogrel, cilostazol, warfarin, iron, levothyroxine, propranolol, cyclosporine, simvastatin, atorvastatin, cyanocobalamin, sucralfate, theophylline and antacid.
