"Laporan Praktik Kerja Profesi Apoteker ini membahas terkait efektivitas mesin dan pekerja dengan menggunakan pendekatan analisa OEE (Overall Equipment Effectiveness). Penelitian ini melibatkan tiga indikator didalam penentuannya antara lain kualitas, kinerja, dan ketersediaan. Dalam melakukan Penelitian ini penulis melakukan pengamatan langsung selama proses filling pada mesin Alumunium Tube Filling Kentex di PT Guardian Pharmatama, dilakukan pendokumentasian yang kemudian data kuantitatifnya aka diolah menggunakan perhitungan analisis OEE. Penelitian ini menghasilkan nilai OEE melebihi 100% dengan masalah utama yang terjadi 5.46% terkait produk yang bermasalah. Dari hasil penelitian ini disarankan agar perlunya ketepatan waktu engineering terkait perbaikan mesin atau alat kedepannya sehingga mengurangi kerusakan saat mesin sedang beroperasi, sebaiknya operator diberikan arahan untuk menjadikan TKL sebagai bagian tanggungjawab mereka sama halnya dengan pengisian batch file untuk melihat kesesuaian proses aktual dengan man hour yang telah ditetapkan serta keadaan mesin yang digunakan.

This Pharmacist Professional Work Practices Report discusses the effectiveness of machines and workers using the OEE (Overall Equipment Effectiveness) analysis approach. This research involves three indicators in its determination, including quality, performance, and availability. In conducting this research, the author made direct observations during the filling process on the Kentex Aluminum Tube Filling machine at PT Guardian Pharmatama, documentation was carried out and then the quantitative data was processed using OEE analysis calculations. This research resulted in an OEE value exceeding 100% with the main problems occurring 5.46% related to problematic products. From the results of this research, it is suggested that there is a need for timely engineering related to machine or tool repairs in the future to reduce damage when the machine is in operation. Operators should be given direction to make TKL part of their responsibility, as well as filling in batch files to see the suitability of the actual process with the specified man hours has been determined and the condition of the machine used."

"Laporan Praktik Kerja Profesi Apoteker ini membahas terkait pemetaan suhu container chiller di PT Enseval Putera Megatrading Bekasi. Penelitian ini bertujuan untuk mengetahui letak suhu tertinggi dan suhu terendah serta titik kritis suhu pada container chiller yang dicek dengan menaruh temperature data logger di berbagai titik dan melihat apakah container chiller yang tersedia memenuhi persyaratan dan standar untuk penyimpanan produk rantai dingin atau tidak. Penelitian ini dilakukan secara observasional selama 3 hari merekam data suhu dengan temperature data logger tiap 10 menit, kemudian menarik data dari thermometer data logger mengunakan aplikasi Testo Comfort Software Basic 5.0. Dari penelitian ini dihasilkan Container chiller 1 dan 2 PT Enseval Putera Megatrading cabang Bekasi memenuhi persyaratan suhu yakni diantara 2-8o C dan titik kritisnya berada pada posisi terpanas dan terdingin di container chiller 1 terletak pada titik 3 dan 19, sedangkan pada container chiller 2 titik terpanas dan terdingin terletak pada titik 10 dan 5. Berdasarkan hasil penelitian tersebut sebaiknya limit suhu diatur pada suhu atas 7o C dan bawah 3oC untuk penanggulangan lebih cepat jika suhu berada diluar standar dan sebaiknya dalam proses penyimpanan dan pengiriman sediaan-sediaan cold chain product perlu dipantau dengan baik, terutama pada vaksin-vaksin yang memiliki indikator VVM yang mempermudah pengecekkan kestabilan obat.

This Pharmacist Professional Work Practices Report discusses temperature mapping of chiller containers at PT Enseval Putera Megatrading Bekasi. This research aims to determine the location of the highest and lowest temperatures as well as critical temperature points in chiller containers which are checked by placing temperature data loggers at various points and seeing whether the available chiller containers meet the requirements and standards for storing cold chain products or not. This research was carried out observationally for 3 days, recording temperature data with a temperature data logger every 10 minutes, and then pulling data from the thermometer data logger using the Testo Comfort Software Basic 5.0 application. From this research it was produced that Container chiller 1 and 2 of PT Enseval Putera Megatrading Bekasi branch met the temperature requirements, namely between 2-8o C and the critical points were at the hottest and coldest positions. Container chiller 1 was at points 3 and 19, while container chiller 2 was at point The hottest and coldest are located at points 10 and 5. Based on the results of this research, the temperature limit should be set at an upper temperature of 7o C and a lower temperature of 3o C for quicker response if the temperature is outside the standard and it is best to monitor the process of storing and sending cold chain products. Well, especially for vaccines that have a VVM indicator which makes it easier to check drug stability."

"Laporan Praktik Kerja Profesi Apoteker ini membahas terkait kesesuaian resep pasien BPJS Kronis Poli Jantung dan Pembuluh di Apotek Kimia Farma 0389 jika ditinjau dari segi administratif, farmasetik, dan klinis yang dianalisis melalui hasil DRPs (Drug Related Problems) yang terjadi. Penelitian ini dilakukan secara observasional dengan metode deskriptif. Dari hasil penelitian ini disarankan agar kegiatan pemberian informasi obat dan konseling oleh apoteker lebih ditingkatkan terutama pada pasien polifarmasi yang memiliki risiko interaksi antara obat yang digunakan dan perlu adanya peningkatan pengetahuan bagi pihak rumah sakit atau penulis resep melalui sosialisasi peraturan obat yang dipersyaratkan oleh BPJS sehingga pasien bisa mengklaim obat sesuai dengan apa yang doketr resepkan untuk menghindari indikasi yang tidak diobatai karena ketidaktersediaan obat dan fakktor tidak sesuai persyaratan BPJS.

This Pharmacist Professional Work Practices Report discusses the suitability of prescriptions for BPJS Chronic Heart and Vessel Poly patients at Kimia Farma 0389 Pharmacy when viewed from an administrative, pharmaceutical, and clinical perspective which is analyzed through the results of the DRPs (Drug Related Problems) that occur. This research was conducted observationally using descriptive methods. From the results of this research, it is recommended that the activities of providing drug information and counseling by pharmacists be improved, especially for poly pharmacy patients who have a risk of interactions between the drugs used and there needs to be increased knowledge for hospitals or prescribers through socialization of drug regulations required by BPJS so that patients can claim medicine according to what the doctor prescribed to avoid indications that are not treated due to unavailability of medicine and factors that do not meet BPJS requirements."

"Laporan Praktik Kerja Profesi Apoteker ini membahas terkait pembuatan leaflet atasi TB RO dan kenali obat FDC pada TB dewasa dan anak sebagai sarana pendukung pelayanan farmasi klinis di Rumah Sakit Universitas Indonesia. Pembuatan leaflet menggunakan aplikasi Canva dengan isi leaflet yang mengacu pada pedoman nasional tatalaksana tuberculosis tahun 2020, petunjuk teknis penatalaksanaan tuberkulosis resisten obat di Indonesia tahun 2020, dan buku saku pasien TB resisten obat tahun 2020. Dari hasil penelitian ini disarankan agar sebaiknya media leaflet bisa dijadikan tambahan informasi dan secara langsung diberikan kepada pasien saat melakukan edukasi maupun konseling sehingga diharapkan pasien lebih mudah mengerti dan memahami konsep pengobatannya dan peneliti selanjutnya sebaiknya dilakukan evaluasi sebelum dan setelah diberikan leaflet apakah ada kemajuan dari segi pengetahuan dan kepatuhan selama pengobatan TB.

This Pharmacist Professional Work Practices Report discusses the creation of leaflets for dealing with RO TB and recognizing FDC drugs for adult and pediatric TB as a means of supporting clinical pharmacy services at the University of Indonesia Hospital. Making leaflets using the Canva application with leaflet contents referring to the 2020 national guidelines for managing tuberculosis, technical instructions for managing drug-resistant tuberculosis in Indonesia in 2020, and the pocketbook for drug-resistant TB patients in 2020. From the results of this research, it is recommended that leaflets should be used as media. additional information and directly given to patients during education and counseling so that it is hoped that patients will more easily understand and comprehend the concept of treatment and future researchers should evaluate before and after being given the leaflet whether there is progress in terms of knowledge and compliance during TB treatment."

"Laporan Praktik Kerja Profesi Apoteker ini membahas terkait medication error yang terjadi pada tahap peresepan di Puskesmas Kecamatan Pasar Rebo. Dalam penelitian ini peneliti menilai dari berbagai aspek antara lain dari segi kelengkapan administrasi, farmasetis, dan klinis. Kemudian dikelompokkan juga temuan yang terjadi tergolong Kejadian Nyaris Cedera(KNC), Kejadian Tidak Diharapkan (KTD), atau Kejadian Tidak Cedera (KTC) dan juga dilakukan grading risk untuk melihat tergolong minor, moderat, atau ekstrim temuan yang terjadi. Setelah dilakukan penentuan kategori kemudian dianalisa akar masalah untuk mengetahui penyebab masalah tersebut dari segi orang, mesin, bahan, lingkungan, dan juga prosedur. Penelitian ini dilakukan secara observasional dengan metode deskriptif dengan teknik random sampling dalam menentukan sampel resep yang diambil. Terdapat 6 (5.00%) medication error yang terjadi ketidaktepatan indikasi, ketidaktepatan dosis, ketidaktepatan obat, adanya alergi, adanya interaksi obat, dan adanya kontraindikasi. Dari hasil penelitian ini disarankan agar sebaiknya pendokumentasian tabel checklist pengkajian resep tetap terlaksana aktif, pelaporan insiden keselamatan pasien tetap selalu berjalan dengan baik dan ditindaklanjuti untuk memberikan pelayanan yang optimal terhadap pasien.

This Pharmacist Professional Work Practices Report discusses medication errors that occur at the prescribing stage at the Pasar Rebo District Health Center. In this study, researchers assessed various aspects, including administrative, pharmaceutical, and clinical completeness. Then the findings that occur are grouped into Near-Injury Events (KNC), Unexpected Events (KTD), or Non-Injury Events (KTC), and risk grading is also carried out to see whether the findings that occur are minor, moderate, or extreme. After determining the category, the root of the problem is then analyzed to find out the cause of the problem in terms of people, machines, materials, environment, and also procedures. This research was carried out observationally using descriptive methods with random sampling techniques to determine the recipe samples taken. There were 6 (5.00%) medication errors involving inaccurate indications, inaccurate dosages, inaccurate medications, allergies, drug interactions, and contraindications. From the results of this research, it is recommended that documentation of the prescription review checklist table should continue to be carried out actively, patient safety incident reporting should always run well and be followed up to provide optimal service to patients."
