"[ABSTRAK
Masalah gizi kurang balita merupakan masalah aktual di wilayah Puskesmas Sentolo 1, Yogyakarta. Tujuan penelitian ini untuk mengetahui faktor-faktor yang mempengaruhi status gizi balita. Penelitian ini menggunakan cross sectional, sampel balita 12-59 bulan sebanyak 155 orang. Data diperoleh melalui kuesioner, status gizi diukur dengan indeks Berat Badan/Umur. Hasil menunjukkan terdapat hubungan bermakna antara usia balita, riwayat pemberian ASI, asupan makanan, persepsi ibu, pola pengasuhan dengan status gizi balita. Faktor dominan yang mempengaruhi status gizi adalah asupan makanan. Diperlukan peran perawat komunitas dalam edukasi, pemberdayaan, dan kerjasama untuk meningkatkan status gizi balita.

ABSTRACT
Undernutrition in under five years old children is still a problem in Sentolo 1 Public Health Center, Yogyakarta. The purpose of this study was to determine the factors related nutritional status in children under five years. Cross sectional study was conducted to 155 children under five years old. Nutritional status was assessed using anthropometric measurement. There were significantly relationships between child?s age, exclusife breastfeeding, child?s dietary intake, caregivers? practice and mother?s perception and child?s nutritional status. Child?s dietary intake was the most factor that significantly correlated. "ABSTRAK
Cedera berkontribusi sebanyak 40% kematian pada anak usia sekolah. Cedera menyebabkan
peningkatan angka kesakitan, pengeluaran biaya kesehatan dan angka absensi siswa.
Pencegahan cedera di komunitas sangat dibutuhkan sebelum menimbulkan dampak negatif
pada anak, salah satunya dengan pendekatan Knowledge, Skill, Motivation, Opportunity and
Access, Suportive Environment (Kosmos). Penulisan ini bertujuan untuk memberikan
gambaran pelaksanaan Kosmos dalam manajemen pelayanan dan asuhan keperawatan
komunitas pada anak usia sekolah dengan risiko cedera melalui integrasi Teori Manajemen,
Triad epidemiological, Comprehensive School Health Model, dan Family Centered Nursing.
Metode yang digunakan adalah kuasi eksperimen dengan sampel adalah anak usia sekolah
dasar yang berjumlah 234 orang, diambil dengan teknik total sampling. Intervensi dilakukan
selama 6 bulan. Hasilnya adalah terjadi peningkatan motivasi kader dan tim pelaksana UKS
dalam pencegahan cedera. Terjadinya peningkatan yang signifikan pada pengetahuan, sikap
dan keterampilan kader kesehatan sekolah, siswa, dan keluarga dalam pencegahan cedera,
serta terjadinya peningkatan kemandirian keluarga setelah diberikan intervensi kosmos.
Intervensi kosmos merupakan salah satu pendekatan yang berbasis komunitas yang terbukti
efektif dalam meningkatkan perilaku pencegahan cedera. Diharapkan intervensi kosmos
dapat dijadikan sebagai salah satu pendekatan intervensi keperawatan dalam penyelesaian
masalah kesehatan pada anak usia sekolah dengan risiko cedera.

ABSTRACT
Injuries caused 40% of deaths in school age children. Injuries not only caused increased
morbidity and health care cost, but also decreased student’s school attendance. Injury
prevention is needed before it can cause negative impacts on children. One of the prevention
approach is Knowledge, Skill, Motivation, Opportunity and Access, Suportive Environment
(Kosmos). The aim of this paper was to provide an overview the implementation of Kosmos in
nursing care and service management on school-age children with a risk of injury through
the integration of Management theory, Triad epidemiological theory, Comprehensive School
Health Model, and Family Centered Nursing Theory. The method used a quasy experimental
study, with samples are 234 school age children, taken through total sampling technique.
This intervention conducted for 6 months. The result shows that there was an increases of
Kader and Tim Pelaksana UKS’ motivation in injury prevention. There were a significant
increases in knowledge, attitudes and skills of kader, students, and families in the injury
prevention, as well as the increased independence of the family after given cosmos
intervention. Cosmos intervention can be used as a nursing intervention approach to solving
health problems in children of school age with risk for injury."
