"ABSTRAK
Praktek Kerja Profesi (PKP) di apotek bertempat di Apotek Kimia Farma No. 295. Kegiatan ini
berlangsung selama empat minggu dari tanggal 1 sampai dengan tanggal 30 Oktober 2015. PKP
di apotek bertujuan agar mahasiswa apoteker mengerti peranan Apoteker Pengelola Apotek
(APA), memiliki wawasan tentang pelaksanaan pekerjaan kefarmasian, dan memiliki gambaran
nyata akan permasalahan pekerjaan kefarmasian yang terjadi di apotek. Berdasarkan kegiatan
PKP yang dilakukan, APA di Apotek Kimia Farma No. 295 secara umum telah melaksanakan
tugas dan tanggung jawab dalam pengelolaan apotek terkait kegiatan kefarmasian meliputi
kegiatan manajerial dan pelayanan farmasi klinik sesuai dengan peraturan perundang-undangan
yang berlaku. Mahasiswa apoteker telah berhasil memperoleh wawasan mengenai kegiatan ?
kegiatan yang dilakukan di apotek. Kegiatan manajerial yang dilakukan di apotek meliputi
pengadaan, penerimaan, penyimpanan, penjualan dan pelayanan resep, pemusnahan, dan
pelaporan. Adapun kegiatan pelayanan kefarmasian klinik kefarmasian dilakukan di apotek
adalah pengkajian resep, dispensing dan pelayanan informasi obat. Masalah yang terjadi di
Apotek Kimia Farma No. 2 adalah ketiadaan Apoteker Pendamping yang dapat menggantikan
APA ketika sedang tidak ada di tempat dan seringnya terjadi kekosongan stok perbekalan
farmasi. ABSTRACT Profession Internship Program located at Kimia Farma Pharmacy No. 295. This activity lasted
for four weeks from October 1st to October 30th , 2015. Profession Internship Program in
pharmacy aims to make students understand the role of Pharmacist in Charge at Kimia Farma
Pharmacy No 295, have insight into the implementation of pharmacy work, and have a real
picture of the issues pharmacy jobs occurred at the pharmacy. Based on the activities,
Pharmacist in Charge at Kimia Farma Pharmacy No. 295 in general have been carrying out the
duties and responsibilities in the management of activities related managerial and clinical
pharmacy services. Student pharmacists have managed to gain insights about the activities -
activities carried out in the pharmacy. Managerial activities performed in pharmacies include
procurement, receipt, storage, sale and service of recipes, extermination, and reporting. The
activities of clinical pharmacy services performed are the assessment of prescription, dispensing
and drug information services. Problems that occur in Kimia Farma No. 295 is the absence Co
Pharmacist in Charge who can replace the Pharmacist in charge while he/she is not in place and
empty drug stock frequently occurred.

"ABSTRAK
Pelayanan kefarmasian di Rumah Sakit merupakan bagian yang tidak terpisahkan dari sistem pelayanan kesehatan di Rumah Sakit yang berorientasi pada pelayanan pasien, penyediaan sediaan farmasi, alat
kesehatan dan bahan medis habis pakai yang bermutu dan terjangkau termasuk pelayanan farmasi klinik. Praktek Kerja Profesi (PKP) di rumah sakit dilaksanakan di Rumah Sakit Anak dan Bunda Harapan Kita. PKP di rumah sakit bertujuan agar mahasiswa apoteker mengerti peranan apoteker di rumah sakit, memiliki wawasan tentang pelaksanaan pekerjaan kefarmasian di rumah sakit, dan memiliki gambaran nyata tentang permasalahan pekerjaan kefarmasian di rumah sakit. Kegiatan ini berlangsung selama enam minggu dari tanggal 6 Juli sampai dengan tanggal 31 Agustus 2015 dengan tugas khusus yaitu evaluasi interaksi obat dan kesesuaian dosis pada resep depo farmasi unit gawat darurat bulan Juli 2015. Masalah yang terjadi di Rumah Sakit Anak dan Bunda Harapan Kita
adalah belum optimalnya pelaksanaan pelayanan farmasi klinik mulai dari jumlah tenaga apoteker dan sarana pra sarana yang mendukung.ABSTRACT Hospital pharmacy services are an integral part of the health care system in the Hospital which focused on patient care, the provision of pharmaceutical, medical devices and medical consumable material quality and affordable including clinical pharmacy services. Profession Internship Program at Harapan Kita Mother and Child Hospital aims to make students understand the role of pharmacists in the hospital, having an knowledge
about the execution of work at the hospital pharmacy, and have a real picture of the problems of employment at the hospital pharmacy. This activity lasted for six weeks from July 6 until August 31, 2015 with a special task is the evaluation of drug interactions and dose conformity to the prescription in emergency unit on July 2015. Problems that occur at Harapan Kita Mother and Child Hospital are implementation of clinical pharmacy services didn?t fully work startsg from the number of pharmacists
and facility which has been provided."

"ABSTRAK
Puskesmas merupakan sarana pelayanan kesehatan yang menyelenggarakan upaya kesehatan masyarakat dan perseorangan yang lebih mengutamakan upaya
preventif dan promotif. Praktek Kerja Profesi (PKP) di puskesmas dan instansi pemerintah bertempat di Suku Dinas Kesehatan Kota Administrasi Jakarta Timur dan Puskesmas Kecamatan Makasar. Kegiatan ini berlangsung selama tiga minggu
dari tanggal 1 sampai dengan tanggal 21 September 2015. PKP di puskesmas dan instansi pemerintah bertujuan agar mahasiswa apoteker mengerti peranan apoteker di puskesmas dan instansi pemerintah, memiliki wawasan tentang
pelaksanaan pekerjaan kefarmasian di puskesmas dan instansi pemerintah, dan memiliki gambaran nyata akan permasalahan pekerjaan kefarmasian di puskesmas
dan instansi pemerintah dalam kehidupan nyata. Berdasarkan pengamatan, Suku Dinas Kesehatan Kota Administrasi Jakarta Timur dan Puskesmas Kecamatan
Duren Sawit telah melaksanakan tugas pokok dan fungsi masing ? masing dengan baik. Masalah yang terjadi di Puskesmas Kecamatan Duren Sawit adalah belum
terpenuhinya persyaratan gudang yang baik dan pelayanan farmasi klinik yang belum berjalan secara optimal.
