Praktek Kerja Profesi Apoteker di Puskesmas Kecamatan Tamansari Jakarta Barat Periode Bulan Januari Tahun 2017 dilakukan selama dua minggu. Praktek Kerja Profesi Apoteker ini bertujuan agar calon apoteker mampu memahami peranan, tugas dan tanggung jawab sesuai dengan standar pelayanan farmasi; memiliki wawasan, pengetahuan, keterampilan, dan pengalaman praktis untuk melakukan praktek kefarmasian; dan memiliki gambaran tentang permasalahan praktik kefarmasian serta mempelajari strategi dan kegiatan-kegiatan yang dapat dilakukan dalam rangka pengembangan praktek kefarmasian di puskesmas. Sedangkan tugas khusus yang dilakukan selama Praktek Kerja Profesi Apoteker berjudul ldquo;Analisis Potensi Interaksi Obat pada Peresepan Obat antihipertensi di Puskesmas Kecamatan Tamansari Periode Januari 2017 rdquo; bertujuan untuk memperoleh gambaran potensi interaksi obat pada resep obat yang mengandung obat antihipertensi di Puskesmas Kecamatan Tamansari.

ABSTRAK
Pharmacists Professional Practice at Tamansari Community Health Center West Jakarta January Month Period January 2017 carried out for three weeks. The goal of Pharmacists Professional Practice was the pharmacist able to understand the role, duties and responsibilities in accordance with the standards of pharmacy services had the insight, knowledge, skills, and practical experience to carry out the practice of pharmacy and had an idea about the problems of pharmacy practice and learn the strategies and activities that can be done within the framework of the development of pharmacy practice at the Community Health Center. While the specific task in the Pharmacists Professional Practice titled Analysis of Potential Drug Interactions of Antihypertentive Drugs Prescribing at Tamansari Community Health Center West Jakarta January Month Period January 2017 aims to obtain potential drug interactions in prescription that containing antihypertensive drug in Tamansari Community Health Center West Jakarta January Month Period January 2017.

Praktek Kerja Profesi Apoteker di Apotek Kimia Farma No. 352 dilakukan selama satu bulan yaitu Bulan Februari 2017. Praktek Kerja Profesi Apoteker ini bertujuan agar calon apoteker mampu memahami peranan, tugas dan tanggung jawab apoteker di apotek sesuai dengan standar pelayanan farmasi; memiliki wawasan, pengetahuan, keterampilan, dan pengalaman praktis untuk melakukan praktek kefarmasian di apotek; dan memiliki gambaran tentang permasalahan praktik kefarmasian di apotek serta mempelajari strategi dan kegiatan yang dapat dilakukan dalam rangka pengembangan praktek kefarmasian di apotek. Sedangkan tugas khusus yang dilakukan selama Praktek Kerja Profesi Apoteker berjudul ldquo;Pemberian Informasi Obat pada Pasien Diabetes Mellitus Melalui Pelayanan Kefarmasian di Rumah rdquo; bertujuan untuk melakukan memonitoring terhadap terapi pasien diabetes mellitus demi tercapainya keberhasilan terapi dan meningkatkan kualitas hidup pasien.

The goal of Pharmacists Professional Practice was the pharmacist able to understand the role, duties and responsibilities of a pharmacist in a pharmacy in accordance with the standards of pharmacy services had the insight, knowledge, skills, and practical experience to carry out the practice of pharmacy in pharmacy and had an overview of the practice of pharmacy in pharmacy issues and learn the strategies and activities that can be done in order to develop the practice of pharmacy in pharmacy. While the specific tasks in the Pharmacists Professional Practice titled Drug Information Service in Diabetes Mellitus patient with Home Pharmacy Care aims to monitoring diabetes mellitus patient to achieve the goals therapy and increasing the quality of life.

Praktek Kerja Profesi Apoteker ini bertujuan agar calon apoteker mampu memahami peranan, tugas, dan tanggung jawab apoteker sesuai dengan standar pelayanan farmasi; memiliki wawasan, pengetahuan, keterampilan, dan pengalaman praktis untuk melakukan praktek kefarmasian di rumah sakit; dan memiliki gambaran tentang permasalahan praktik kefarmasian di rumah sakit serta mempelajari strategi dan kegiatan-kegiatan yang dapat dilakukan dalam rangka pengembangan praktek kefarmasian di rumah sakit. Sedangkan tugas khusus yang dilakukan selama Praktek Kerja Profesi Apoteker berjudul ldquo;Gambaran Drug Related Problem pada Pasien High Care Unit Neurologi Instalasi Rawat Inap Teratai Periode April 2017 rdquo; bertujuan untuk memperoleh persentase terjadinya Gambaran Drug Related Problem pada Pasien High Care Unit Neurologi Instalasi Rawat Inap Teratai Periode April 2017.

While the specific tasks in the Pharmacists Professional Practice titled Drug Related Problem in High Care Unit inpatients Teratai Month Period April 2017 aims to obtain the percentage of Drug Related Problem Problem in High Care Unit inpatients Teratai Month Period April 2017.
