"Penelitian tentang pola makan dan perilaku sehat ibu hamil telah dilaksanakan di RSU Bhakti Yudha Kalif Depok, Jawa Barat. Penelitian ini bersifat cross sectional survey terhadap ibu hamil yang sedang memeriksakan kehamilannya di RSU Bhakti Yudha dalam kurun waktu satu bulan. Diantara responden (89 ibu hamil) yang diwawancarai, 95% berumur antara 20-35 tahun. Sebagian besar ibu hamil berpendidikan tamat SMU dan Akademi/Perguruan Tinggi (60% dan 35%). Pada umumnya ibu hamil tidak bekerja (52.8%). Hampir setengah ibu adalah primigravida (hamil untuk yang pertama kali), dan oleh karena itu jarang dari responden yang menggunakan alat kontrasepsi sebelum kehamilan yang sekarang ini.
Dari pola makan ibu hamil cenderung sama dengan waktu tidak hamil, yaitu 3 kali sehari, tetapi untuk porsi tiap kali makan cenderung lebih banyak, terutama pada responden dengan kehamilan trimester Ill, Dan pada umumnya konsumsi makanan 4 sehat 5 sempurna dalam 1 minggu terakhir sudah dilakukan oleh sebagian besar ibu. Makanan pokok yang dikonsumsi tersebut mencakup nasi, sayuran, lauk pauk (daging, ayam, ikan, telur, tahu, lempe), buah-buahan serta susu.
Mengenai pemeriksaan kehamilan pada kehamilan yang sekarang ini, semua responden telah melakukan pemeriksaan kehamilan sesuai dengan ketentuan pedoman ANC Departemen Kesehatan RI. Sebagian besar ibu tabu 'tablet tambah darah', dari yang tahu tersebut hampir 90% minum 'tablet tambah darah' tersebut. Umumnya mereka minum sejak tahu dirinya hamil atau pada kehamilan trimester I.
Tidur siang pada kehamilan yang sekarang ini banyak dilakukan oleh ibu yang tidak bekerja. Lama tidur siang berkisar antara 1-2 jam. Alasan bagi yang tidur siang adalah karena bekerja, walaupun ibu yang bekerja masih ada yang menyempatkan diri untuk tidur siang. Masalah lain adalah sulit tidur atau tidak merasa ngantuk pada siang hari. Pekerjaan rumah tangga yang dilakukan yang dianggap berat pada masa kehamilan adalah mencuci, mengepel, menyeterika dan memasak. Ibu yang menganggap pekerjaan tersebut berat adalah ibu yang tidak bekerja.
Gejala/penyakit seperti tekanan darah tinggi, toksoplasma, diabetes mellitus. HIV/AIDS, penyakit kelamin dan beberapa gejala/penyakit lainnya yang mentiru ibu membahayakan kehamilan disebut oleh sekitar 50% ibu. Tindakan yang dilakukan yang menurut ibu memudahkan dalam persalinan adalah jalan-jalan, mengepel, banyak gerak, senam hamil, merangkak, minum minyak, makan telur madu, minum air kelapa dan sebagainya. Alasan yang paling banyak disebut untuk pemeriksaan kehamilan di RSU Bhakti Yudha adalah karena jarak RSU tersebut dekat dengan rumahnya. Kemudian diikuti fasilitas lengkap, dokter ramah atau teliti, dirujuk, merasa yakin melahirkan di RSU Bhakti Yudha."

"The urban population of Indonesia has doubled in the last three decades, and it is estimated that by the year of 2010 almost 40-45% of the population will be living in urban areas. Jakarta as the capital city is the most rapid growing city among others in Indonesia. This, for the most part, was influenced by rapid growing of industrial factories, enlargement of the administration areas of the city, as well as high population growth rate. As the consequence, many aspects of human life including housing, food stock and food production, health and environmental condition, educational situation, economical improvement, social and cultural life, political stability, and national resistance have been changed. However, the changes could be positive or negative.
Urbanization was associated with important changes in food consumption pattern. The urban diet may be more varied and may include higher levels of animal proteins and fats while being lower in calories (Pagaspas 1994, Santoso 1995). Fresh foods such as vegetables, meat, poultry, milk and other dairy products were consumed more by urban than by rural people. Another change that had occurred in urban food habit was an increase in the amount of food eaten outside the household (FAO 1984). These foods tend to be eaten by the consumer at the roadside and were relatively cheap. Ease of preparation, time saving and money, taste, identification with an urban lifestyle, and inaccessibility to cooking facilities in crowded urban slums were among the reasons for the shifts in food consumption patterns. The value of women's time was an important factor affecting household expenditure on food. A greater variety of foods were also available in urban areas, leading to more diversified diets. Information from a slum in Brazil, indicated the lower income class tended to meet a smaller amount of its energy and nutrient requirements than did the higher income class (Von Braun et al 1993).
Study carried out by SEAMEO - UI participants from 1985-1989 in five slum areas in Jakarta showed that the prevalence of nutritional deficiency in terms of moderate and severe PEM was still very high (31.9%) in East Jakarta (Von Braun et al 1993). Besides, study carried out by Susilowati (1997) revealed that mothers suffered from undenutrition (27%) and ovenutrition (30%). Mothers had higher calorie and protein intake as compared to underfive children (Pagaspas 1994, Santoso 1995). Compared to Jakarta's prevalence of PEM among underfive children (25.8%), the figures of East Jakarta was higher. Nationally, prevalence of PEM among underfive children was 28.6% (Indonesian Health Profile 1997).
As one main determinant of malnutrition, food consumption was greatly influenced by purchasing power. On average, calories correlates with income in urban area. Marginal intake of calories decreases with increasing income levels. As prices or incomes change, there was a greater tendency among urban consumers to diversify their diet and substitute more readily (Von Braun et at 1993). Study about undernutrition in low income households in West Jakarta found that at least more than 30% of food expenditure was spent on street foods.
Undernutrition among underfive children mainly due to eating snacks and street food. It was caused by social pressure from neighbor, clever promotion strategy of street vendor, especially on its convenience (located at strategic places, going house to house), tasty, snacking (child refused to eat main meals but will eat snacks) had made eating out a general habit and difficult to be modified. In another word, dietary inadequacy was not primarily due to low purchasing power but rather to habit of buying snacks outside home (Pujilestari et al 1995).
Moreover, the habitual diet of family consists of rice, soybean products (tofu, tempe), fresh fish and dark green leafy vegetables. Noodles often replaced a rice meal. Their food composition was quite simple and less varied. A rough estimate of energy intake for consumption unit (CU) was made from the amount of calories major sources bought, i.e. rice, noodles, cooking oil. The average daily energy intake from these sources covered 63% of RDA (Pujilestari et al 1995).
The study about food habits among mothers and their children 2-5 aged in urban areas in Indonesia is rare. The study was usually implemented in rural area at provincial level. It was related to the application of nutritional anthropology with agriculture science, for example study on "Social and Cultural Influences on Food Habits and Food Consumption Patterns of Staple Food in the Family with Preschool Children". The study was undertaken by Ministry of Health in collaboration with The National Development Planning Board in 1986. The study revealed that the family in areas with other staple food than rice, eating rice is not considered as to provide strength for working in the field or in the forest."

"Untuk mengantisipasi masalah gizi kurang utamanya pada keluarga miskin, pemerintah telah mengeluarkan gerakan Kadarzi (Keluarga Sadar Gizi). Hingga saat ini, hampir tidak diketahui perilaku Kadarzi ibu balita di DKI Jakarta karena masih sedikitnya data hasil penelitian yang mengukur hal tersebut. Oleh karena itu, diperlukan suatu penelitian dasar yang bertujuan untuk mengeksplorasi bagaimana gambaran pengetahuan dan praktek kelima indikator Kadarzi ibu balita di Kelurahan Penjaringan, Jakarta Utara. Metode kualitatif dengan teknik Diskusi Kelompok Terarah (DKT) dan wawancara mendalam telah dilakukan pada ibu balita, tokoh agama/masyarakat, dan kader posyandu. Hasil studi menyatakan bahwa pengetahuan ibu balita pada 4 indikator Kadarzi cukup baik, kecuali konsumsi aneka ragam makanan. Praktek 3 indikator Kadarzi juga sudah baik, kecuali pemberian ASI eksklusif dan konsumsi aneka ragam
makanan. Tokoh masyarakat dan kader posyandu belum mengenal indikator Kadarzi dengan baik. Mereka hanya sebatas mendengar dan tidak familiar dengan istilah Kadarzi. Kadarzi identik dengan makanan 4 sehat 5 sempurna bagi balita dan ibu hamil untuk meningkatkan status gizi dan kesehatan masyarakat. Disimpulkan bahwa ibu balita belum berperilaku Kadarzi karena belum mengaplikasikan lima indikator Kadarzi. Disarankan agar Sudinkes Wilayah
Jakarta Utara melakukan sosialisasi Kadarzi lebih intensif lagi bagi masyarakat melalui media cetak dan elektronik di posyandu dan puskesmas. "

"Lingkungan masyarakat kota yang bebas kebakaran dapat tercipta melalui upaya pencegahan dan pemadaman kebakaran dini. Penelitian ini bertujuan mengembangkan
model mitigasi kesiapsiagaan kebakaran berbasis masyarakat. Metoda kualitatif wawancara mendalam dilakukan terhadap 30 informan terpilih
di Jabodetabek meliputi aparat kecamatan, kelurahan, Lembaga Swadaya Masyarakat, dan Suku Dinas Pemadam Kebakaran. Mayoritas informan mengenal
organisasi Barisan Sukarelawan Kebakaran, tetapi dalam pelaksanaan ditemukan beberapa kendala antara lain meliputi tidak ada honorarium, status
pekerjaan, dan anggota penduduk musiman. Hal tersebut berakibat banyak anggota yang berhenti dan sulit merekrut anggota baru. Bentuk upaya pencegahan
kebakaran yang dilakukan masyarakat meliputi simulasi, penyuluhan, dan pelatihan bahaya kebakaran, serta menjadi anggota Barisan Sukarelawan
Kebakaran. Tiap wilayah mempunyai bentuk yang bervariasi sesuai dengan aturan yang berlaku di masyarakat karena masih bersifat regional serta belum
dilegitimasikan dalam Undang-Undang Nasional. Alternatif model berbasis masyarakat yang terbaik adalah memberdayakan Barisan Sukarelawan Kebakaran
mengingat program tersebut telah berjalan dan dikenal masyarakat. Diharapkan model ini lebih mudah diterima masyarakat karena menjadi bagian program
pembangunan di beberapa wilayah. Dukungan masyarakat yang kuat terhadap Barisan Sukarelawan Kebakaran menentukan kelancaran pelaksanaan di lapangan."

"Studi tentang kebiasaan makan ibu dan anak balita dilakukan terhadap 60 responden di dua keluarahan terpilih (Kelurahan Penggilingan mewakili wilayah berlatar belakang sosio ekonom rendahdan Keluarhan Rambuatan mewakili wilayah dengan sosio ekonomi tinggi) di wilayah Jakarta Timur. Metode kualitatif (Focus Group Discussion dan indept interview) digunakan dalam pengumpulan data untuk mengeksplorasi faktor-faktor yang mempengaruhi kebiasaan makan pada kedua kelompok di atas lalu dibandingkan pada kelompok sosek tinggi dan rendah. Hasil studi menunjukkan bahwa seperlima ibu-ibu yang termasuk kelompok gizi lebih (overweight) dari kedua kelompok sosek tinggi dan rendah memiliki gambaran sama yaitu sebagai berikut: mereka memiliki aktivitas fisik sedentary (mereka bekerja sebagai ibu rumah tangga dalam lingkup rumah tinggal yang kecil, dan menghabiskan 2-3 jam per hari untuk menonton televisi); mereka juga mengkonsumsi snack (ngemil) seharian sambil santai atau istirakat atau momong anak; dan mereka sering menghabiskan sisa makanan anak atau suami yang tidak habis dimakan. Sementara seperlima jumlah anak balita dengan kondisi gizi kurang (undrwight) baik kelompok sosio ekonomi tinggi dan rendah menunjukkan situasi dan kondisi yangsama yaitu mereka menderita penyakit infeksi ISPA, TBS dan bronkhitis; mereka senang bermain; dan mereka banyak jajan sehingga mempengaruhi nafsu makan mereka. Anak balita dari keluarga sosek tinggi cenderung mengkonsumsi snack dengan kalori tinggi seperti hamburger ayam goreng kentang goren dsbnya. Sementara anak balita dari kelompok ekonomi rendah cenderung membeli makanan kecil dengan kalori rendah seperti kerupik jellu dan permen. Kedua hasil di atas menyebabkan terjadinya perbedaaan status gizi kelompok ibu dan anak balita. Ibu memiliki status gizi kurang dan gizi lebih (double burden), sementara anak balita mereka menunjukkanstatus gizi kurang dengan prevalensi masing masing sekitar 20%.

A food habits study is reported from 60 respondents in the urban areas. A qualitative approach was undertaken to explore factors influencing food habits and to compare it within low and high socioeconomic status (SES) by using Focus Group Discussion (FGD) and indepth interview. One fifth of the overweight mothers (low and high SES) showed a similar condition as follow: they had low to moderate physical activity (they worked in a relatively small house and they spent 2-3 hours per day for watching television), they consumed snacks continuously for a whole day, and they ate left-over food of their children or husbands. One fifth of the underweight children 2-5 years old from both SES showed similar situation: they suffered from infectious disease (ARI, TBC, and bronchitis); they played a lot; and they lost appetite eating main meals after they consumed snacks. The high SES under-fives were more likely to buy snacks which were high in calorie such as hamburger, fried chicken, fried potatoes. While the low SES under-fives bought snacks which were considered as ?empty calorie? i.e. jelly, and candy. Both condition above caused the difference of nutritional status among mothers and their children. Mothers had double burden (underweight and overweight), while their children 2-5 years age suffered from under-nutrition (20%)."

"The high prevalence of anemia and vitamin A deficiency among underfive children is related to their food intake and health status. The situation appeared worst after multiple crisis hit Indonesia in 1997/1998. To obtain factors affecting the nutritional status of children aged 1-5 years, a baseline study was conducted in DKI Jakarta, Bogor, and Tangerang in collaboration with UN-WFP. The sample size was 1337 underfive children consisting of 666 boys and 666 girls.
The study showed that malnutrition among underfive children was still high which indicated the high prevalence of underweight and stunting, especially in Bogor District and Tangerang District. The micronutrient intake of vitamin A, iron, and zinc among underfive children were certainly below the Indonesian RDA. The level of poverty and the education of woman as well as husbands were two conditions that were obvious from all areas, mainly Tangerang District.

"Tinggi badan adalah salah satu indikator klinik utama dalam menentukan Indeks Massa Tubuh (IMT) dalam menentukan status gizi individu/populasi. Namun, pengukuran tinggi badan manusia usia lanjut (manula) cukup sulit dilakukan dan reliabilitasnya diragukan. Persamaan estimasi tinggi badan dari pengukuran tinggi lutut untuk memprediksi tinggi badan manula yaitu persamaan Chumlea telah dikembangkan beberapa tahun lalu, tetapi belum ada studi yang dilakukan di Indonesia untuk mengembangkan suatu persamaan bagi pengukuran tinggi badan populasi usia lanjut menurut bermacam-macam kelompok etnis. Oleh karena itu, suatu cross sectional studi untuk mengembangkan persamaan tinggi badan manula berdasarkan pengukuran dua parameter yaitu tinggi lutut dan panjang depa (knee height dan arm span) telah dilakukan pada bulan Desember 2005 lalu. Total 217 manula (usia 60 - 92 tahun) dari 3 kelompok etnik yaitu: Jawa (56,7%), Cina (31,3%), dan lain-lain (12,0%) berpartisipasi dalam studi ini. Pengukuran antropometri termasuk berat badan, tinggi badan, panjang depa, dan tinggi lutut dilakukan oleh ahli gizi terlatih. Kesalahan inter dan intra observer dilakukan untuk pengukuran antropometri tinggi lutut dan panjang depa manula. Temuan utama studi adalah rata-rata usia manula asal Cina adalah tertinggi di antara suku lainnya; kebanyakan manula mengalami gizi kurang (43%); distribusi rata-rata tinggi lutut dan panjang depa hampir sama di tiap kelompok etnis; ada perbedaan signifikan antara tinggi lutut dengan tinggi badan sebenarnya pada wanita lanjut usia (lansia), dan korelasi tertinggi ditunjukkan oleh parameter tinggi lutut pada wanita lansia dan panjang depa pada pria lansia. Persamaan Chumlea menunjukkan kecenderungan under-estimate pada pria lansia dan over-estimate pada tinggi badan wanita lansia. Kesimpulannya, tinggi badan tegak/sebenarnya merupakan teknik ideal untuk estimasi tinggi badan lansia. Tetapi, pada kasus di mana pengukuran itu sendiri tidak memungkinkan atau tidak reliable, maka tinggi badan dapat diestimasi dari indikator proksi tinggi badan. Pada studi ini, panjang depa menggambarkan korelasi tertinggi dengan tinggi badan sebenarnya pada pria lansia , dan tinggi lutut pada wanita lansia.

The Equation of Prediction Stature Based on Age and Ethnic in Six Institutionalized Elderly at DKI Jakarta and Tangerang, Year 2005. Height is an important clinical indicator to derive body mass index (BMI) predicting the nutritional status. However, height measurement in the elderly may impose some difficulties and the reliability is doubtful. Equations estimating height from knee height parameter to predict stature in elderly i.e. Chumlea have been developed, but no one study has developed an equation for Indonesian population according to variety of ethnics. Therefore, a cross sectional study was conducted to develop equations using two types of anthropometric measurements (knee height and arm span) for estimating stature in Indonesian elderly. A total of 217 elderly (aged 60 to 92 years old) from three major ethnic groups Javanese (56.7%), Chinese (31.3%), and others (12.0%) participated in this study. Anthropometric measurement included body weight, height, arm span, and knee height were carried out by trained nutritionist. Inter and intra observer errors was calculated for each anthropometric measurement of arm span and knee height of elderly. Main findings of this study were the mean of age of Chinese was the highest among other ethnics; the most elderly suffered from underweight (43%); the distribution of mean knee height and arm span was almost similar in each ethnic group; there was a significant difference between knee height with stature in elderly women, and the highest correlation indicated by knee height in elderly women and arm span in elderly men. Chumlea equation showed tend to be under-estimate in stature of elderly men and over-estimate in stature of elderly women. In conclusion, standing height is an ideal technique for estimating the stature of elderly. However, in cases where its measurement is not possible or reliable, height can be estimated from proxy indicators of stature. "Penuaan (aging) dikaitkan dengan sejumlah besar perubahan fungsi imunitas tubuh, terutama penurunan Cell Mediated Immunity (CMI) atau imunitas yang diperantarai sel. Kemampuan imunitas kelompok lanjut usia menurun sesuai peningkatan usia termasuk kecepatan respons imun melawan infeksi penyakit. Hal itu berarti bahwa kelompok lansia beresiko tinggi terserang penyakit seperti infeksi, kanker, jantung koroner, kelainan autoimmun atau penyakit kronik lainnya. Seluruh penyakit ini mudah terjadi pada lansia karena produksi imunoglobulin menurun. Akibatnya vaksinasi yang diberikan pada kelompok orang tua seringkali tidak efektif melawan penyakit. Orang-orang tua yang umumnya menderita kekurangan gizi makro dan mikro akan memiliki respons sistem dan fungsi imun yang rendah. Oleh karena itu, kasus malnutrisi pada lansia seharusnya memiliki perhatian khusus secara dini, termasuk pemberian vaksinasi untuk pencegahan penyakit. Penyakit infeksi yang dialami oleh lansia dapat dicegah atau diturunkan melalui upaya-upaya perbaikan gizi karena sistem imun akan meningkat. Jika fungsi imun lansia dapat ditingkatkan, maka kualitas hidup individu meningkat dan biaya pelayanan kesehatan dapat ditekan.

Low Immunity Response in the Elderly. Aging is related to a number of changes in the immunity function, mainly the reducing of Cell Mediated Immunity (CMI). The immunocompetence of elderly worsen with age including the rate of immune respons against infection. It means that older people have a high risk of getting diseases such as infection, cancer, cardiovascular, autoimmune disorder, or other chronic diseases. All of these diseases occured in elderly due to the immunoglobulin production decrease. Thus, vaccination given to elderly often might not be effective against diseases. Older people who commonly suffer from a decrease of macro and micronutrients will have a low function and response of the immune system. Therefore, malnutrition cases in elderly should have early specific attention including consideration in given vaccination for preventing diseases. Infectious diseases mostly suffered by older people can be prevented or reduced through improving nutrition efforts because the immune system will be improved. If the immune function of the elderly can be improved, the individual quality of life increases and the health cost can be suppressed."
