Abstrak :
Trauma Brain Injury (TBI) merupakan kondisi cedera pada otak yang terjadi ketika terjadi serangan fisik secara tiba-tiba dari kondisi eksternal. Kerusakan dapat bersifat focal (terbatas pada satu area otak) atau diffuse (terjadi di lebih dari satu area otak). Perubahan fisiologis pada pasien geriatri seperti penurunan fungsi ginjal, fungsi hati dan komposisi tubuh menyebabkan adanya perbedaan farmakokinetika dan farmakodinamika pada pasien geriatri. Dengan dilakukannya PTO diharapkan dapat mengoptimalkan efek terapi obat dan mencegah atau meminimalkan efek merugikan akibat penggunaan obat. Apoteker berperan penting dalam pelaksanaan PTO untuk mencegah terjadinya masalah terkait obat. Tujuan dalam penyusunan laporan tugas khusus ini adalah melakukan pemantauan terapi obat dan mengidentifikasi masalah terkait penggunaan obat pada pasien Ny. YS di lantai 6 Selatan Gedung Anggrek di Rumah Sakit Umum Pusat Fatmawati. Berdasarkan hasil kegiatan Pemantauan Terapi Obat pada pada pasien Ny. YS dengan Trauma Brain Injury di Gedung Teratai Lantai 6 Selatan RSUP Fatmawati dapat disimpulkan bahwa pengobatan yang diterima oleh pasien Ny. YS telah tepat indikasi dan tepat dosis. Penggunaan antibiotik seftriakson telah tepat sesuai dengan PPK KSM Bedah Saraf RSUP Fatmawati dengan diagnosis Trauma Brain Injury. Namun durasi penggunaannya terlalu lama yaitu 22 hari. ......Trauma Brain Injury (TBI) is a condition of injury to the brain that occurs when there is a sudden physical attack from external conditions. Physiological changes in geriatric patients such as decreased kidney function, liver function and body composition lead to differences in pharmacokinetics and pharmacodynamics in geriatric patients. Drug Therapy Monitoring can optimize the effects of drug therapy and prevent or minimize adverse effects due to drug use. Apothecary play an important role in implementing Drug Therapy Monitoring to prevent drug-related problems. The purpose of preparing this special assignment report is to monitor drug therapy and identify problems related to drug use in Mrs. YS on the 6th floor South of the Orchid Building at Fatmawati General Hospital. Based on the results of monitoring drug therapy activities in patients Ny. YS with Trauma Brain Injury at Teratai Building South Floor 6 Fatmawati Hospital, it can be concluded that the treatment received by patient Ny. YS has the right indication and the right dose. The use of ceftriaxone antibiotics was appropriate according to PPK KSM Neurosurgery at Fatmawati Hospital with a diagnosis of Trauma Brain Injury. But the duration of use is too long (22 days).

Jaminan Kesehatan Nasional (JKN) merupakan program dari pemerintah yang memberikan perlindungan kesehatan kepada peserta yang telah membayar iuran atau iurannya dibayar oleh Pemerintah. JKN bertujuan agar peserta dapat memperoleh manfaat pemeliharaan kesehatan dan perlindungan dalam memenuhi kebutuhan dasar kesehatan. Pengelolaan persediaan obat di Apotek perlu dilakukan untuk mendukung pelayanan kepada pasien. Apoteker berperan dalam manajemen sediaan farmasi sehingga harus memiliki kemampuan manajerial untuk mengelolah data kebutuhan obat yang dituangkan dalam Rencana Kebutuhan Obat sesuai pagu anggaran. Apotek Kimia Farma 267 Bintaro merupakan salah satu Faskes yang bekerjasama dengan BPJS Kesehatan untuk pelayanan obat Pelayanan Rujuk Balik (PRB). Perkembangan teknologi informasi menyebabkan pengadaan sediaan farmasi menjadi lebih mudah dan cepat. Pengadaan obat dapat menggunakan E-Purchasing melalui Katalog Elektronik pada website Lembaga Kebijakan Pengadaan Barang dan Jasa Pemerintah (LKPP). Pengadaan obat BPJS melalui Katalog Elektronik dapat mempermudah dalam kegiatan melaksanakan kebutuhan obat sehingga pasien dapat menerima obat sesuai dan tepat waktu. Penyusunan laporan khusus ini bertujuan untuk mengevaluasi sistem pengadaan obat BPJS melalui Katalog Elektronik di Apotek Kimia Farma 267 Bintaro. Pengadaan obat BPJS di Apotek Kimia Farma 267 Bintaro menggunakan metode Manual (Offline) yaitu membuat paket pembelian obat berdasarkan Daftar Pengadaan Obat Berdasarkan Katalog Elektronik dengan Formulir 2 dikarenakan kendala operasional. Procurement of BPJS drugs through Electronic Catalogs can facilitate the activities of carrying out drug needs so that patients can receive drugs according to and on time. The preparation of this special report aims to evaluate the BPJS drug procurement system through the Electronic Catalog at the Kimia Farma 267 Bintaro Pharmacy. This special report aims to provide information regarding the roles and duties of pharmacists, especially the marketing sub-division in preparation for product re-launching at Abbott EPD Indonesia, namely product XX. Standar pelayanan kefarmasian adalah tolak ukur yang dipergunakan sebagai pedoman bagi tenaga kefarmasian dalam menyelenggarakan pelayanan kefarmasian. Standar pelayanan kefarmasian di puskesmas meliputi manajemen farmasi dan pelayanan farmasi klinik. Apoteker di puskesmas mempunyai tugas dan tanggung jawab untuk menjamin terlaksananya pengelolaan sediaan farmasi dan Bahan Medis Habis Pakai (BMHP) dan memberikan pelayanan farmasi klinik kepada pasien dengan efektif dan efisien, tepat sasaran, dan memprioritaskan pasien. Pelayanan resep merupakan salah satu kegiatan pelayanan farmasi klinik berupa rangkaian kegiatan mulai dari peracikan obat, penyerahan obat hingga pemberian informasi obat kepada pasien. Cara Distribusi Obat yang Baik (CDOB) mengatur mengenai ketentuan khusus untuk Narkotika, Psikotropika dan Prekursor Farmasi supaya memenuhi ketentuan perundang-undangan. Ketentuan tersebut bertujuan untuk mencegah kejadian penyalahgunaan, penyimpangan dan kehilangan Narkotika, Psikotropika dan Prekursor Farmasi dari jalur distribusi resmi. Pedagang Besar Farmasi (PBF) berperan untuk memastikan kualifikasi pemasok yaitu memiliki izin produksi narkotika dari Menteri Kesehatan. Apoteker berperan sebagai penganggung jawab fasilitas distribusi Narkotika, Psikotropika dan Prekursor Farmasi (NPP) seperti melakukan pembaharuan spesimen tanda tangan dan stempel untuk memastikan keabsahan tersebut. KFTD Tangerang merupakan salah satu Perusahaan Besar Farmasi yang telah memiliki izin penyaluran narkotika ke fasilitas lain seperti instalasi farmasi pemerintah, apotek, klinik dan rumah sakit sesuai ketentuan perundang-undangan dan CDOB.
