Tujuan dari praktik kerja profesi apoteker di Apotek Kimia Farma No. 389 Depok Jawa Barat Periode Bulan Juli Tahun 2017 adalah untuk memahami tugas, tanggung jawab dan pelayanan kefarmasian serta menambah wawasan, keterampilan dan pengalaman praktis sebagai apoteker di apotek. Selain itu, kegiatan ini juga bertujuan untuk memberikan gambaran nyata tentang permasalahan kefarmasian di apotek. Tugas khusus yang dilakukan berjudul Analisis Resep Benign Prostatic Hyperplasia di Apotek Kimia Farma No. 389 Depok. Tugas ini bertujuan untuk mengkaji resep dan obat yang dilayani di apotek Kimia Farma nomor 389. Dari kegiatan praktik ini, penulis telah mengetahui tugas, tanggung jawab, wawasan dan pengalaman praktis serta solusi dari permasalahan praktik pelayanan kefarmasian di apotek

ABSTRACT
The purpose of Internship at Kimia Farma No. 389 Pharmacy Depok West Java Period July 2017 was to understand the duties responsibilities and pharmacy care and to add insight, skills and practical experience as a pharmacist in pharmacies. In addition, this activity also aimed to provide a real picture of pharmaceutical problems in pharmacies. The special assignment was entitled Analysis of Benign Prostatic Hyperplasia Prescription at Kimia Farma No. 389 Pharmacy Depok. This assignment aimed to examine prescriptions and medicines served at Kimia Farma No. 389 Pharmacy. Tujuan dari praktik kerja profesi apoteker di PT. Medifarma Laboratories Periode Bulan Agustus-September Tahun 2017 adalah untuk memahami peranan, tugas dan tanggung jawab serta menambah wawasan, keterampilan dan pengalaman praktis sebagai apoteker di industri farmasi. Selain itu, kegiatan ini juga bertujuan untuk memahami penerapan CPOB dan gambaran nyata tentang masalah yang terjadi di industri farmasi. Tugas khusus yang dilakukan berjudul Pembuatan MS Excel Spreadsheet Analytical Report-Stability Sample. Tugas ini bertujuan untuk membuat Microsoft Excel Spreadsheet untuk Analitycal Report stability sample dan membuat protokol validasi dari Microsoft Excel Spreadsheet tersebut. Dari kegiatan praktik ini, penulis telah mengetahui peran, tugas, tanggung jawab, wawasan dan pengalaman praktis serta solusi dari permasalahan di industri farmasi. PT. Medifarma Laboratories juga telah menerapkan semua aspek CPOB dengan benar.

The purpose of the Internship at PT. Medifarma Laboratories Period August-September 2017 was to understand the role, duties and responsibilities and to add insight, skills and practical experience as pharmacists in the pharmaceutical industry. In addition, this activity also aimed to understand the application of the CPOB and a real picture of the problems occurring in the pharmaceutical industry. The special assignment was done entitled Making MS Excel Spreadsheet Analytical Report-Stability Sample. This assignment aimed to create a Microsoft Excel Spreadsheet for the Analitycal Report stability sample and to create a validation protocol of the Microsoft Excel Spreadsheet. Tujuan dari praktik kerja profesi apoteker di Direktorat Jenderal Kefarmasian dan Alat Kesehatan Kementerian Kesehatan RI Periode Bulan Oktober Tahun 2017 adalah untuk memahami peranan, tugas, dan tanggung jawab serta menambah wawasan, keterampilan dan pengalaman praktis sebagai apoteker di pemerintahan. Selain itu, kegiatan ini juga bertujuan untuk memberikan gambaran nyata tentang permasalahan kefarmasian di pemerintahan. Tugas khusus yang dilakukan berjudul Evaluasi Penggunaan Obat di Tujuh RSUD Wilayah Indonesia Tengah Tahun 2014-2017. Tugas ini bertujuan untuk mengetahui persentase kesesuaian penggunaan obat dengan Formularium Nasional di tujuh RSUD serta mengetahui kelas terapi dari obat-obatan yang tidak sesuai Fornas yang digunakan pada RSUD tersebut. Dari kegiatan praktik ini, penulis telah mengetahui peran, tugas, tanggung jawab, wawasan dan pengalaman praktis serta solusi dari permasalahan di Ditjen Kefarmasian dan Alat Kesehatan.

ABSTRACT
The purpose of Internship at Direktorat Jenderal Kefarmasian dan Alat Kesehatan Kementerian Kesehatan RI Period October 2017 was to understand the roles, duties, and responsibilities and to add insight, skills and practical experience as a pharmacist in government. The special assignment was entitled Evaluation of Drug Use in Seven Regional Hospitals of Central Indonesia of 2014-2017. This assignment aimed to determine the percentage of conformity of drug use with the national formulary in seven hospitals as well as to know the class of therapy of drugs that are not appropriate national formulary used in those hospitals.
