"Daun keji beling merupakan salah satu tanaman herbal yang bermanfaat sebagai antihiperkolesterolemia, dimana dalam ekstraknya mengandung senyawa fenolik dan flavonoid. Pada penelitian ini, ekstrak didapatkan dengan metode ekstraksi UAEATPE (Ultrasound-Assisted Enzymatic Aqueos Two Phase Extraction). Proses pra-perlakuan dilakukan dengan hidrolisis enzimatik menggunakan enzim selulase. Proses ekstraksi dilakukan dengan sistem ATPS etanol-garam ammonium sulfat, dimana konsentrasi masing-masing adalah 33% w/w dan 14% w/w. Proses ekstraksi menghasilkan yield ekstrak rata-rata sebesar 59%. Nilai TPC (Total Phenolic Content) dan TFC (Total Flavonoid Content) pada crude ekstrak dianalisis menggunakan spektrofotmetri UV-Vis, dengan panjang gelombang masing-masing 765 nm dan 434 nm. Masing-masing nilai TPC pada fasa atas dan fasa bawah didapatkan sebesar 4,15 mg GAE/g sampel dan 1,08 mg GAE/g sampel. Sedangkan nilai TFC yang diperoleh adalah 1,88 mg QE/g sampel pada fasa atas; serta 0,29 mg QE/g sampel pada fasa bawah. Nilai perolehan (R) dan koefisien partisi (K) yang diperoleh yaitu sebesar 79,29% dan 2,22 untuk fenolik; serta 86,66% dan 3,78 untuk flavonoid, dimana semakin besar nilai koefisien partisi, semakin baik pemisahan yang terjadi. Hasil analisis Gas Chromatography dan Mass Spectrometry (GC-MS) dari ekstrak etanol daun keji beling yang memiliki aktivitas anti-hiperkolesterolemia adalah 1-Docosanol.

Keji beling leaves are one of the herbal plant that has benefit as an anti-hypercholesterolemia, in which the leave’s extract contain a phenolic and flavonoid compound. In this study, extract were obtained by extraction using the UAEATPE (Ultrasound-Assisted Enzymatic Aqueous Two Phase Extraction) method. Pre-treatment process was done with hydrolysis enzymatic using enzyme cellulose. Extraction process were done with ethanol-ammonium sulfate salt ATPS system, where the concentrations were 33% w/w and 14% w/w, respectively. The result from extraction process are an average yield extract of 59%. The TPC and TFC values in crude extract were analyzed using UV-Vis spectrophotometry, with a wavelength of 765 nm 434 nm, respectively. Each TPC values on the top and bottom phases obtained were 4.15 mg GAE/g sample and 1.08mg GAE/g sample. Meanwhile, the TFC values obtained were 1.88mg QE/g sample for top phase; and 0.29 mg QE/g sample for bottom phase. The recovery value and partition coefficients values are 79.29% and 2.22 for phenolic; and 86.66% and 3.78 for flavonoid, respectively. In which indicate the bigger the partition coefficients values, the better the separation occurred. The analysis result of ethanol extract of keji beling leaves with Gas Chromatography and Mass Spectrometry (GC-MS) which has anti-hypercholesterolemia activity is 1-Docosanol."

"Biji Leucaena leucocephala (Lam.) de Wit atau petai cina memiliki manfaat sebagai antioksidan yang didapatkan dari senyawa sulfhidril, namun biji ini juga memiliki efek toksik dari senyawa mimosin. Pada penelitian ini, dilakukan ekstraksi biji petai cina menggunakan konsep ekstraksi ramah lingkungan dengan Natural Deep Extraction (NADES) berbasis kolin klorida-gula sederhana dan Ultrasound Asissted Extraction (UAE) dibandingkan dengan ekstraksi menggunakan maserasi dengan pelarut etanol 30% dengan kadar senyawa sulfhidril lebih tinggi dan kadar senyawa mimosin lebih rendah pada kombinasi UAE-NADES. Optimasi kondisi ekstraksi ditentukan melalui metode analisis Response Surface Methodology (RSM). Penetapan kadar senyawa sulfhidril dilakukan menggunakan Microplate Reader dan penetapan kadar senyawa mimosin dengan Spektrofotometri UV-Vis. Optimasi kadar senyawa sulfhidril dilakukan sebanyak 17 kali dengan menggunakan tiga variabel yaitu persentase penambahan air pada NADES (65%, 70%, dan 75%), waktu ekstraksi (5 menit, 10 menit, dan 15 menit), serta rasio pelarut terhadap serbuk (3 mL/g, 5 mL/g, dan 7 mL/g).
Hasil penelitian menunjukkan bahwa kadar tertinggi senyawa sulfhidril yang didapatkan pada ekstrak UAE-NADES berbasis kolin klorida-sukrosa sebesar 0,6667 mg/g serbuk dengan kondisi penambahan air pada NADES sebanyak 75%, waktu ekstraksi 10 menit, dan rasio pelarut terhadap serbuk sebanyak 3 mL/g, sedangkan kadar yang didapatkan pada ekstrak maserasi-etanol 30% sebanyak 0,5206 mg/g serbuk. Kadar senyawa mimosin yang didapatkan pada ekstrak UAE-NADES sebanyak 4,946 mg/g serbuk, sedangkan sebanyak 12,5631 mg/g serbuk pada ekstrak maserasi-etanol 30%. Oleh karena itu, dapat disimpulkan bahwa penggunaan ekstraksi UAE-NADES berbasis kolin klorida-sukrosa lebih selektif pada penarikan senyawa sulfhidril dibandingkan dengan penggunaan ekstraksi maserasi-etanol 30%.

Leucaena leucocephala (Lam.) de Wit or River Tamarind seeds have benefits as antioxidants obtained from sulfhydryl compounds, but these seeds also have a toxic effect from mimosine compounds. In this study, river tamarind seeds were extracted using an environmentally friendly extraction concept with Natural Deep Extraction (NADES) based on choline chloride-simple sugar and Ultrasound-Assisted Extraction (UAE) compared with extraction using maceration with 30% ethanol solvent with higher sulfhydryl compounds content and lower mimosine levels in the UAE-NADES combination. The optimization of the extraction conditions was determined through the Response Surface Methodology (RSM) analysis method. The assay of sulfhydryl compounds was carried out using a Microplate Reader and the assay of mimosine compounds by UV-Vis Spectrophotometry. The optimization of sulfhydryl compounds was carried out 17 times using three variables, namely the percentage of addition of water to NADES (65%, 70%, and 75%), extraction time (5 minutes, 10 minutes, and 15 minutes), and the ratio of solvent to powder ( 3 mL/g, 5 mL/g, and 7 mL/g).
The results showed that the highest content of sulfhydryl compounds obtained in UAE-NADES extract based on choline chloride-sucrose was 0.6667 mg/g powder with the condition of adding water to NADES as much as 75%, extraction time of 10 minutes, and the ratio of solvent to powder as much as 3 mL/g, while the concentration obtained in the maceration-ethanol 30% extract was 0.5206 mg/g powder. The levels of mimosine compounds obtained in the UAE-NADES extract were 4.946 mg/g powder, while as much as 12.5631 mg/g powder in the maceration-ethanol 30% extract. Therefore, it can be concluded that the use of UAE-NADES extraction based on choline chloride-sucrose is more selective in the extraction of sulfhydryl compounds compared to the use of maceration-ethanol 30% extraction.

"Latar Belakang: Plantar fasciitis merupakan penyebab tersering dari nyeri tumit inferior. Nyeri dapat menjadi kronik walaupun sudah mendapatkan terapi konservatif yang optimal. Perbandingan efektivitas antara ESWT dan terapi Ultrasound masih belum diketahui dengan jelas.
Metode: Penelitian ini merupakan studi eksperimental. Sebanyak 24 subjek plantar fasciitis dengan rentang usia 26-43 tahun yang telah terdiagnosis melalui USG mengikuti penelitian ini. Subjek dikelompokkan secara konsekutif ke dalam grup ESWT GE, n=13 dan grup Ultrasound UG, n=11 dengan durasi intervensi tiga minggu. Keluaran primer yang dinilai adalah nyeri palpasi dan nyeri pagi hari pada area medial tuberositas calcaneus. Keluaran sekundernya adalah waktu tempuh uji jalan 15 meter.
Hasil: Masing-masing grup menunjukkan perbaikan nyeri yang signifikan secara statistik tanpa adanya perbedaan bermakna antargrup. Waktu tempuh uji jalan 15 meter mengalami perbaikan yang tidak signifikan secara statistik pada kedua grup dan tidak terdapat perbedaan signifikan antargrup.
Kesimpulan: ESWT sama efektifnya dengan terapi Ultrasound dalam memberikan perbaikan nyeri pada plantar fasciitis.

Background: Plantar fasciitis is the most common cause of inferior heel pain. It may become chronic pain despite optimal conservative treatment. Comparison of effectiveness between ESWT an Ultrasound still unclear.
Methods: Experimental study on plantar fasciitis patient that diagnosed using Ultrasonography with range of age 26 63 years divided consecutively into ESWT group EG ESWT and stretching fascia plantaris gastrocnemius and Ultrasound group UG US and stretching fascia plantaris gastrocnemius for 3 weeks intervention. The primary outcomes were pain on palpation and morning pain on medial of calcaneal tuberosity. The secondary outcome was duration in performing 15 meters walk test.
Results: 24 participants EG n 13, UG n 11 enrolled in this study. Mean age of EG 46.5 years and CG 43 years. Each group showed statistically significant improvement of pain reduction with no significant different between group. The duration of 15 meters walk test improved nonsignificant in each group dan did not show significant difference between group.
Conclusions: ESWT was no more effective than Ultrasound in reducing pain in patient with plantar fasciitis.

"ABSTRAK
Latar belakang: Indonesia memiliki angka kejadian Cerebral Small Vessel Disease CSVD yang tinggi yaitu 45 dari kesuluruhan stroke iskemik dengan hipertensi sebagai faktor risiko utama. CSVD menyebabkan disabilitas fungsi kognitif dan sampai saat ini diagnosisnya tergantung pada pemeriksaan Magnetic Resonance Imaging MRI . CSVD merupakan gangguan mikrosirkulasi intrakranial dimana struktur ini memiliki homogenitas mikrovaskular dengan retina. Oleh karena itu nilai kecepatan aliran arteri sentralis retina ASR dapat menggambarkan aliran mikrovaskular intrakranial pada penderita CSVD.Metode: Penelitian ini merupakan studi asosiatif potong lintang yang bersifat analitik. Evalusasi dilakukan terhadap 39 subyek penelitian yang merupakan pasien hipertensi yang masuk dalam kriteri inklusi. Setiap pasien dilakukan pemeriksaan MRI kepala, fungsi kognitif melalui pemeriksaan MoCA-INA, trail-making test A B dan groove pegboard dan kecepatan aliran ASR melalui pemeriksaan Orbital Doppler Ultrasound.Hasil: CSVD didapatkan pada 79.5 subyek penelitian. PFV ASR pada subyek penelitian 17,93 7,1-42,58 cm/s cenderung lebih tinggi jika dibandingkan dengan nilai normal 10.3 2.1 6.4-17.2 cm/s . Pada subyek dengan CSVD sebanyak 74,2 mengalami gangguan fungi kognitif dan peningkatan PFV ASR. Tidak terdapat hubungan bermakna antara PFV dengan ada tidaknya CSVD. Selain itu juga tidak ditemukan hubungan bermakna antara PFV dengan ada tidaknya gangguan fungsi kognitif. Terdapat kecenderungan peningkatan PSV pada CSVD dan gangguan fungsi kognitif.Kesimpulan: Perubahan kecepatan aliran ASR dapat memberikan gambaran CSVD dan gangguan fungsi kognitif pada penderita hipertensi.

Background Indonesia has a high number of incidents of Cerebral Small Vessel Disease CSVD , i.e. 45 of the total ischemic stroke with hypertension as the main risk factor. CSVD led to disability in cognitive functions and up until now the diagnosis is based on the Magnetic Resonance Imaging MRI . CSVD is an intracranial microcirculation disturbance in which such structure has a microvascular homogeneity with the retina. Thus, the flow velocity of the Central Retinal Artery CRA may be feasible to reflect the intracranial microvascular flow to CSVD patients.Method This research was an analytic associative cross sectional study. This evaluation was done to 39 research subjects which were hypertension patients within inclusion criteria. Each patient undergone a head MRI, Cognitive functions through MoCA INA test, Trail making test A B and Groove Pegboard and CRA flow velocities evaluation through an Orbital Doppler Ultrasound.Result CSVD was found on 79,5 of research subjects. PFV of CRA on research subjects 17,93 7,1 42,58 cm s tend to be higher than normal levels 10.3 2.1 6.4 17.2 cm s . In subjects with CSVD as much as 74.2 had impaired cognitive function and increased PFV of CRA. There was no significant correlation between PFV of CRA and the existence of CSVD. There was also no significant correlation between PFV of CRA and the presence of cognitive function impairment. There was a tendency of increased PFV of CRA on CSVD and cognitive function impairment.Conclusion Changes in PFV of CRA may reflect CSVD and cognitive function disturbance on hypertension patients."

"Multidetector-row computed tomography (MDCT) has become a fundamental imaging technique for the study of many anatomical districts in different clinical situations, as it provides a fast, reliable, and accurate simultaneous evaluation of different organs, including parenchyma, hollow viscera, vessels, and bony structures. It has also equipped the radiologist with the ability to explore areas that in the last decade were largely ignored by CT, especially the coronary arteries and the colon, and to limit invasive diagnostic tests (e.g., catheter angiography) to those cases in which interventional procedures are required.
The examination quality and the consequent diagnostic accuracy of MDCT are the results of an optimized study technique, which nonetheless needs to be adapted to the particular clinical situation of the patient, while bearing in mind the radiation exposure. Another fundamental parameter in the optimization of MDCT is the protocol for the intravenous injection of iodinated contrast material; in these studies, multiple variables, some modifiable by the operator and others patient-dependent, must be considered based on the aims of maximizing arterial and venous enhancements whilst minimizing the dose of iodine injected, saving money, and increasing patient safety.
Through the presentation of cases addressing different anatomical regions and various clinical indications, including emergency and neuro-imaging, this volume seeks to provide the general radiologist and trainee specialist with a guide to the main study protocols to be implemented in order to optimize examination quality and, consequently, facilitate the diagnostic process."

"Extra-pulmonary tuberculosis covers all forms of tuberculosis in which the disease process occurs outside the lungs. Tuberculosis is one of the common infectious causes of splenic enlargement, but tuberculous splenic abscess is a rare presentation.
We report the finding of tuberculous multiple splenic abscesses in a young male patient aged 33 years with prolonged fever. The main clinical feature was fever for 6 months prior to admission. Abdominal ultrasound revealed the spleen was normal in size but showed multiple abscesses. CT scan abdomen showed splenomegaly with multiple solid nodules. Patient underwent laparatomy and splenectomy, the spleen indeed showed multiple abscesses. Histopatology result showed Langhan's multi nucleated giant cells, caseous necrosis. The patients was diagnosed to have tuberculous multiple abscesses of the spleen. The patient was started on daily short-course antituberculosix drugs with isoniazid, rifampicin, pyrazinamide and ethambutol. He responded well to oral antituberculosis treatment.

"ABSTRAK
The physiotherapy statistics from the Saraburi Hospital in Thailand show that patients with chronic lower back pain from muscle tearing have a higher tendency toward chronic pain, requiring continuous treatment. Most physiotherapists use radial shockwave therapy (RSWT) or radial pulse therapy to treat patients with chronic muscle tears. The aim of this study was to determine the immediate efficacy of RSWT on patients with lower back pain caused by chronic muscle strain. The purpose of this work is to assess the effectiveness of RSWT in the treatment of chronic back muscle pain. The type of this research is quasi-experimental research study. The location is this study is Physical Therapy Clinic at the Saraburi Hospital in Thailand. The study participants had pain intensities of greater than 5 out of 10. The experimental group (30 patients) received RSWT to the lower back for 10-15 minutes once a week for 6 weeks. The control group (30 patients) received ultrasound (US) treatments to the lower back using the semi-static technique with gel, but without turning the machine on, for 15 minutes once a week for 6 weeks. Both groups were taught to avoid bending and carrying heavy loads, as well as the correct way to lift heavy objects. None of the patients received medications, injections, massages, or acupuncture. The pain levels in both groups at work and at rest were measured using a visual analog scale. The treatment results were evaluated and compared at the 1st, 4th, and 6th treatments. The patient satisfaction was assessed at the 6 treatments as follows: 1 = most, 2 = high, 3 = moderate, 4 = low, and 5 = minimum. The effects on daily life and career performance were measured before the study and at the end as follows: 1 = most, 2 = high, 3 = moderate, 4 = low, 5 = minimum, and 6 = no effect. A statistical analysis was performed on the demographic data as well as on the pain level comparisons. The results of the RSWT and US treatments were measured while the patients were resting and working at 1, 4, and 6 months. Twelve and 18 of the patients were cured, respectively. In the US groups, 6 patients were statistically significantly better at the , , and treatments (but no cures). The post-treatment satisfaction was not significantly different between the RSWT and US groups. Consequently, the use of RSWT to treat chronic muscle strain can reduce pain immediately, and it can cure pain completely in some cases. Therefore, it should be used to treat more patients with chronic muscle strain. It should also be considered for use in chronic pain cases in which the patient does not respond to other conservative treatments."
