"Stroke iskemik disebabkan oleh oklusi akut arteri yang memasok otak teritori tertentu. Menyelamatkan penumbra adalah tujuan terapi stroke iskemik akut. Progresifitas kematian jaringan otak atau keberhasilan pengobatan bergantung waktu. Trombolisis intravena merupakan pilihan penting dalam terapi stroke iskemik akut dan telah digunakan secara luas, namun keterlambatan pra rumah sakit masih menjadi masalah utama. Penelitian ini bertujuan untuk mengenali faktor yang berhubungan dengan keterlambatan pra rumah sakit dalam pemberian terapi trombolisis intravena pada pasien stroke iskemik akut. Penelitian ini berdesain deskriptif analitik dengan pendekatan Cross Sectional yang melibatkan 110 responden. Analisis data menggunakan analisis deskriptif, uji Chi Square, dan Regresi Logistik. Hasil penelitian menunjukkan bahwa sebagian besar pasien stroke iskemik akut mengalami pre hospital delay dalam golden period terapi trombolisis intravena. Karakteristik pasien yang terlibat dalam penelitain ini menunjukkan bahwa sebagian besar pasien berusia 65 tahun, mengalami onset serangan gejala stroke pada pagi hari, dengan perkembangan gejala stroke yang menetap/membaik, keparahan stroke yang ringan-sedang, mengalami serangan stroke yang pertama kalinya, datang ke IGD dengan ambulans atau datang langsung, dan jarak dari tempat kejadian onset ke rumah sakit/IGD 15 km. Sebagian besar keluarga pasien memiliki tingkat pengetahuan tentang stroke yang baik, kemampuan mengenali gejala stroke yang kurang, dan respon terhadap tanda dan gejala stroke yang adekuat. Terdapat hubungan bermakna antara respon terhadap gejala stroke, perkembangan gejala stroke, keparahan stroke, dan jarak dengan keterlambatan dalam golden period terapi trombolisis Intravena untuk stroke iskemik akut, di mana keparahan stroke menjadi factor paling dominan. Perlunya dilakukan edukasi tentang tanda dan gejala kegawatan stroke serta respon yang tepat saat menghadapi tanda gejala stroke.

Ischemic stroke is caused by acute occlusion of an artery supplying a specific region of the brain. Saving the penumbra is the goal of acute ischemic stroke therapy. The progression of brain tissue death or the success of treatment depends on time. Intravenous thrombolysis is an important option in the treatment of acute ischemic stroke and has been widely used, but prehospital delays remain a major problem. This study aims to identify factors associated with pre-hospital delays in administering intravenous thrombolysis therapy in acute ischemic stroke patients. This research had a descriptive analytical design with a cross sectional approach involving 110 respondents. Data analysis uses descriptive analysis, Chi Square test, and Logistic Regression. The results showed that the majority of acute ischemic stroke patients experienced prehospital delays in the golden period of intravenous thrombolysis therapy. The characteristics of the patients involved in this study showed that the majority of patients were aged 65 years, experienced the onset of stroke symptoms in the morning, with the development of stroke symptoms persisting/improving, the severity of the stroke was mild-moderate, experienced the first stroke attack, came to the emergency room by ambulance or come directly, and the distance from the scene to the hospital/emergency room is 15 km. Most of the patient's families have a good level of knowledge about stroke, poor ability to recognize stroke symptoms, and adequate response to stroke signs and symptoms. There was a significant relationship between response to stroke symptoms, development of stroke symptoms, stroke severity, and distance to delay in the golden period of intravenous thrombolysis therapy for acute ischemic stroke, where stroke severity was the most dominant factor. There is a need for education about the signs and symptoms of emergency stroke and the appropriate response when facing signs of stroke."
