"Paralisis diafragma pasca operasi penyakit jantung bawaan dapat meningkatkan mortalitas dan morbitas pasien. Penegakkan diagnosa kelainan ini menjadi kunci untuk pengambilan keputusan tindak lanjut seperti plikasi diafragma. Fluoroskopi sebagai baku emas memiliki keterbatasan untuk dilakukan pada pasien pasca operasi penyakit jantung bawaan. Dari 2.287 operasi penyakit jantung yang dilakukan di RSJPDHK terdapat 41 pasien yang memenuhi kriteria inklusi dan eksklusi. Median usia pasien 10 (1-119) bulan dan 43,9% berjenis kelamin laki-laki. Terdapat perbedaan bermakna pada jenis operasi yang dijalani. Dari hasil perhitungan didapatkan sensitivitas dan spesifisitas USG diafragma dibandingkan fluoroskopi pada subjek penelitian sebesar 100%(95%CI 82,35%-100%) untuk sensitivitas, dan 95.5%(95% CI 77,16%-99,88%) untuk spesifisitas. Lebih lanjut dilakukan perhitungan nilai prediksi positif dengan hasil 95%(95%CI 73,68%-99,27%) dan nilai prediksi negatif 100% (95% CI 83,89%-100%). Ultrasonografi memiliki sensitivitas dan spesifisitas yang baik dibandingkan fluoroskopi sebagai metode diagnostik pada populasi dengan kecurigaan paralisis diafragma pasca operasi penyakit jantung bawaan dengan sensitivitas 100% dan spesifisitas 95.5%.

Diaphragmatic paralysis after congenital heart disease surgery can increase patient mortality and morbidity. Establishing a diagnosis of this disorder is key for making follow-up decisions such as diaphragm plication. Fluoroscopy as the gold standard has limitations for performing post-surgical patients with congenital heart disease.This study is a comparative diagnostic study that evaluates the ability of diaphragmatic ultrasound to diagnose diaphragmatic paralysis in patients with suspected diaphragmatic paralysis after surgery for congenital heart disease at RSJPDHK from June 2022 to May 2024. Each diaphragmatic ultrasound result was recorded and compared with the findings on fluoroscopy examination. Of the 2,287 heart surgery performed at RSJPDHK, there were 41 patients who met the inclusion and exclusion criteria. The median patient age was 10 (1-119) months and 43.9% were male. There are significant differences in the type of surgery undertaken. From the calculation results, it was found that the sensitivity and specificity of diaphragm ultrasound compared to fluoroscopy in research subjects was 100% (95% CI 82.35% - 100%) for sensitivity, and 95.5% (95% CI 77.16% - 99.88%) for specificity. Furthermore, the positive predictive value was calculated with results of 95% (95% CI 73.68%-99.27%) and negative predictive value of 100% (95% CI 83.89%-100%). Ultrasonography has good sensitivity and specificity compared to fluoroscopy as a diagnostic method in the population with suspected diaphragmatic paralysis after surgery for congenital heart disease with a sensitivity of 100% and a specificity of 95.5%."

"Evaluasi kualitas citra radiografi digital merupakan aspek krusial dalam menjamin keandalan sistem pencitraan medis. Penelitian ini bertujuan untuk mengembangkan Aplikasi QC Radiografi yang mampu melakukan evaluasi kualitas citra secara otomatis menggunakan citra fantom KUCING 2.0 dengan menghitung dua parameter utama, yaitu Signal Difference to Noise Ratio (SDNR) sebagai indikator kontras dan Modulation Transfer Function (MTF) sebagai indikator ketajaman spasial. Fitur aplikasi mencakup rotasi manual, pemilihan ROI interaktif, serta ekstraksi numerik SDNR dan MTF dari elemen uji kontras dan slanted-edge fantom. Validasi hasil perhitungan SDNR dari enam citra menunjukkan selisih relatif yang sangat kecil terhadap ImageJ, yaitu dalam rentang 0,560% hingga 2,852%. Sementara itu, hasil perhitungan MTF juga menunjukkan kesesuaian tinggi, dengan selisih relatif MTF10, MTF20, dan MTF50 terhadap ImageJ masing-masing sebesar 1,337%, 1,848%, 23,291%. Aplikasi ini menunjukkan kestabilan antar pengulangan dan keakuratan dalam menampilkan kurva MTF yang valid secara visual dan numerik. Analisis tambahan terhadap variasi parameter teknik paparan (kVp dan mAs) menunjukkan bahwa SDNR meningkat secara signifikan seiring kenaikan kVp dan mAs, sedangkan nilai MTF tidak mengalami perubahan berarti. Hal ini mengindikasikan bahwa aplikasi mampu menjaga kestabilan performa meskipun terdapat variasi kondisi eksposi. Dengan demikian, aplikasi QC Radiografi yang dikembangkan terbukti dapat digunakan sebagai alat bantu evaluasi kualitas citra yang efisien, akurat, dan dapat digunakan secara luas dalam skema kendali mutu radiografi digital.

Digital radiographic image quality evaluation is a crucial aspect in ensuring the reliability of medical imaging systems. This study aims to develop the QC Radiography Application capable of automatically evaluating image quality using the KUCING 2.0 phantom image by calculating two key parameters: Signal Difference to Noise Ratio (SDNR) as a contrast indicator and Modulation Transfer Function (MTF) as an indicator of spatial resolution. The application features include manual image rotation, interactive ROI selection, and numerical extraction of SDNR and MTF from contrast test elements and slanted-edge regions of the phantom. Validation results for SDNR measurements from six images showed a minimal relative difference compared to ImageJ, ranging from 0.560% to 2.852%. Similarly, MTF results demonstrated high agreement, with relative difference in MTF10, MTF20, and MTF50 values of 1.337%, 1.848%, and 23.291%, respectively, relative to ImageJ. The application exhibited consistency across repetitions and accuracy in presenting MTF curves that are valid both visually and numerically. Additional analysis of exposure technique variations (kVp and mAs) indicated that SDNR increased significantly with higher kVp and mAs values, whereas MTF remained relatively unaffected. These findings suggest that the application maintains stable performance under varying exposure conditions. Therefore, the developed QC Radiography Application is proven to be an efficient, accurate, and widely applicable tool for quality assessment in digital radiographic quality control schemes. "

"ABSTRAK
Perkembangan CT scan generasi multislice yang begitu pesat membuat pemeriksaan CT angiografi coroner sering dilaksanakan, akan tetapi pemberian informasi tentang dosis yang di terima pasien masih jarang dilakukan. Sehingga perlu dilakukan estimasi dosis pasien pada pemeriksaan CT angiografi coroner untuk mengetahui nilai dosis yang diterima oleh organ-organ yang sensitive terhadap radiasi seperti esophagus, paru-paru, payudara (pada wanita) dan jantung. Estimasi dosis dilakukan menggunakan program imPACT® dengan nilai nCTDIw didapat dari hasil pengukuran mengunakan detector pencil ion chamber menggunakan phantom acrilic 32 cm. Dari hasil estimasi di dapat dosis ekivalen yang diterima jantung 110 mSv ? 140 mSv, dosis efektif esophagus (thymus) 2,9 mSv ? 5.7 mSv, dosis efektif paru-paru 10 mSv -14 mSv, dosis efektif payudara 10 mSv ? 13 mSv dan total dosis efektif berkisar antara 31 mSv ? 42 mSv. Mengingat nilai total dosis efektif yang diterima pasien cukup tinggi, maka pasien CT angiografi coroner harus mendapatkan justifikasi yang kuat.

ABSTRACT
The Fast development of CT generation makes CT angiography coroner examination more frequence to be done, but the dose information of patient is rarely to be done. So it require to make patient dose estimation on CT angiography coroner examination. In order to know the dose receive by sensitive organ set of oesophagus, lung, brest and heart. Dose estimation is done using imPACT® program, using CTDI value obtain measurement using acrylic phantom with 32 cm diameter. From dose calculation the dose equivalent by heart is between 110 mSv - 140 mSv, and effective dose for oesophagus 2.9 mSv ? 5.7 mSv, lung 10 mSv ? 14 mSv and total effective dose between 31 mSv ? 42 mSv. Because effective dose receive by patient is very high, the CT angiography coroner patient must have a very strong justification. "
