"Bedah jantung terbuka merupakan salah satu tindakan untuk memperbaiki kondisi kelainan anatomis pada pasien dengan berbagai kelainan jantung yang pasti menggunakan mesin CPB (Cardiopulmonary bypass). Penggunaan mesin ini menimbulkan hemodilusi yang berakibat pada penurunan kadar hemoglobin darah pascabedah. Pada pasien pediatrik, efek hemodilusi yang terjadi lebih besar dibandingkan dengan pasien dewasa. Penelitian sebelumnya berhasil mengidentifikasi bahwa sebagian besar pasien mengalami komplikasi pascabedah, dengan mortalitas sebesar 13,6%. Hemoglobin pascabedah dihipotesiskan menjadi faktor yang kuat dalam mortalitas pasien. Penelitian kohort retrospektif ini menghimpun data hemoglobin pascabedah dari rekam medik elektronik pasien yang menjalani bedah jantung terbuka di Pelayanan Jantung Terpadu Rumah Sakit Dr. Cipto Mangunkusumo pada Januari 2021 hingga Desember 2022 dilanjutkan dengan uji komparatif pada kelompok dengan dan tanpa mortalitas. Total 317 pasien diikutkan dalam penelitian ini. Mortalitas secara umum sebesar 11,7%. Hemodilusi teramati pada populasi pasien. Tidak ditemukan perbedaan signifikan dari hemoglobin pascabedah pada kelompok pasien dengan dan tanpa mortality (p=0,249). Didapatkan pula besar penurunan Hb prabedah ke pascabedah berbeda secara signifikan pada kedua kelompok tersebut (p<0,05). Hemoglobin pascabedah tidak berhubungan dengan mortalitas pasien pediatrik.
Open heart surgery is a procedure aimed to correct anatomical abnormalities in patients with various heart conditions, invariably employing the use of a Cardiopulmonary Bypass machine. The use of this machine induces hemodilution, resulting in a decrease in postoperative blood hemoglobin levels. In pediatric patients, the hemodilution effect is more pronounced compared to adult patients. Previous study showed that a significant proportion of patients experience postoperative complications, with a mortality rate of 13.6%. Postoperative hemoglobin is hypothesized to be an important factor in patient mortality. In this study, a retrospective cohort observational study was conducted to compare postoperative hemoglobin levels in patients with and without mortality. The data were obtained from the medical records of patients undergoing open heart surgery at the Integrated Cardiac Services at Dr. Cipto Mangunkusumo Hospital from January 2021 to December 2022. Total of 317 patients included in this study and overall patient mortality was 11,7%. There was no significant difference in postoperative hemoglobin in the groups of patients with and without mortality (p=0,249). This study also found that the decrease in Hb from pre-surgery to post-surgery was significantly different between the two groups (p<0,05). Postoperative hemoglobin is not associated with mortality in pediatric patients."
