"Diabetes melitus tipe 2 merupakan salah satu masalah kesehatan global yang memiliki persentase kasus yang semakin meningkat setiap tahunnya. Diabetes merupakan ancaman serius terhadap masalah kesehatan yang sebagian besar disebabkan oleh gaya hidup seperti pola makan tidak sehat dan kurang aktivitas fisik. Aktivitas fisik dilakukan dengan teratur dapat memberikan manfaat kesehatan bagi tubuh. Aktivitas fisik merupakan salah satu intervensi pilihan untuk mencegah dan mengontrol diabetes. Penulisan Karya Tulis Ilmiah ini bertujuan memberikan gambaran mengenai asuhan keperawatan keluarga dengan diabetes melalui intervensi keperawatan edukasi pengetahuan diabetes, peningkatan aktivitas fisik dengan senam kaki, dan diet 3J (jumlah, jenis, jadwal) pada keluarga di Kota Depok. Metode penelitian yang digunakan adalah case study. Intervensi keperawatan unggulan yang dilakukan selama 3 minggu adalah latihan fisik senam kaki. Aktivitas fisik senam kaki dilakukan setiap hari dengan waktu 30 menit. Hasil menunjukkan terjadi penurunan dari pertama kali pertemuan dibandingkan dengan akhir setelah diberikan intervensi senam kaki selama 3 minggu dengan diet 3J dari kadar gula darah 343 mg/dL menjadi 121 mg/dL. Dapat disimpulkan bahwa latihan fisik senam kaki dapat menjadi alternatif intervensi untuk menurunkan kadar gula darah pada penderita diabetes.

Type 2 diabetes mellitus is a global health problem which has an increasing percentage of cases every year. Diabetes is a serious threat to health problems which are mostly caused by lifestyle such as unhealthy eating patterns and lack of physical activity. Regular physical activity can provide health benefits for the body. Physical activity is one of the interventions of choice to prevent and control diabetes. The aim of writing this scientific paper is to provide an overview of nursing care for families with diabetes through nursing interventions, education on diabetes knowledge, increasing physical activity with foot exercises, and the 3J diet (amount, type, schedule) for families in Depok City. The research method used is a case study. The leading nursing intervention carried out for 3 weeks was physical foot exercise. The physical activity of foot exercise is carried out every day for 30 minutes. The results showed that there was a decrease from the first meeting compared to the end after being given foot exercise intervention for 3 weeks with the 3J diet from blood sugar levels from 343 mg/dL to 121 mg/dL. It can be concluded that physical exercise with leg exercises can be an alternative intervention to reduce blood sugar levels in diabetes sufferers."

"Diabetes melitus merupakan salah satu dari penyakit kronis penyebab kematian tertinggi di dunia. Kondisi hiperglikemia akan menyebabkan berbagai komplikasi baik akut maupun kronis mencakup komplikasi mikrovaskuler dan makrovaskuler. Salah satu diantaranya yang paling berbahaya yaitu munculnya peripheral arterial  disease (PAD) yang menyebabkan terjadinya foot ulcers. Penulisan karya tulis ilmiah ini bertujuan untuk menganalisis asuhan keperawatan pada pasien diabetes melitus serta efektivitas pemeriksaan ankle brachial index (ABI) dan senam kaki diabetes untuk mengatasi ketidakefektifan perfusi jaringan perifer. Pemeriksaan ABI dan senam kaki diabetes merupakan intervensi yang dapat digunakan untuk meningkatkan sirkulasi perifer dengan mengidentifikasi terjadinya gangguan pada sirkulasi perifer,  meningkatkan aliran kolateralisasi darah pada kaki dan meningkatkan fungsi insulin. Intervensi pemeriksaan ABI dan senam kaki diabetes dilakukan pada pasien diabetes melitus selama 7 hari perawatan. Hasilnya menunjukkan bahwa penerapan pemeriksaan ABI dan senam kaki diabetes dapat meningkatkan sirkulasi perifer dan mencegah timbulnya luka kaki. Melihat keefektifan pemeriksaan ABI dan senam kaki diabetes untuk meningkatkan sirkulasi perifer maka diharapkan intervensi ini dapat digunakan sebagai perawatan rutin pada pasien diabetes melitus.


Diabetes mellitus is one of the highest chronic disease causes of death in the world. The condition of hyperglycemia will cause various complications either acute and chronic including microvascular and macrovascular complications. One of the most dangerous is the emergence of peripheral arterial disease (PAD) which causes foot ulcer. The scientific paper aims to analyze nursing care in patient with diabetes mellitus and the effectiveness of measuring the ankle brachial index (ABI) and diabetic foot exercises to overcome the ineffectiveness of peripheral tissue perfusion. The examination of ABI and diabetic foot exercises are interventions that can be used to improve peripheral circulation by identifying disturbances in peripheral circulation, increasing the flow of blood collateralisation in the foot and improving insulin function. Intervention of ABI examination and diabetic foot exercises were carried out in patient with diabetes mellitus for 7 days treatment. The results show that the application of ABI examination and diabetic foot exercises can improve peripheral circulation and prevent foot injuries. Reviewing the effectiveness of ABI examination and diabetic foot exercises to improve peripheral circulation, then it is hoped that this interventions can be used as a routine treatment in patients with diabetes mellitus."

"Diabetes Melitus merupakan penyakit tidak menular serta penyakit kronis yang terjadi orang dewasa yang membutuhkan pengawasan medis berkelanjutan dan pendidikan perawatan diri. Diabetes melitus mampu mempengaruhi vaskularisasi sehingga menyebabkan terjadinya perubahan pada makrovaskular umumnya mengenai otak, jantung dan pembuluh darah dan mikrovaskular dapat terjadi pada mata dan ginjal serta terjadi perubahan pada sistem saraf atau neuropati seperti europati motorik, sensorik, ataupun neuropati otonom. Tujuan penulisan karya ilmiah ini adalah untuk menganalisis penerapan intervensi senam kaki diabetes dalam memberikan asuhan keperawatan pada pasien diabetes melitus. Senam kaki diabetes juga dapat meningkatkan perfusi, mencegah perburukan neuropati pada pasien diabetes mellitu. Intervensi dilakukan selama lima hari perawatan dan dimulai intervensi pada hari ketiga perawatan. Hasilnya menunjukkan bahwa senam kaki diabetes dapat meningkatkan sirkulasi perifer yang ditandai perubahan pulse oximetry, CRT dan nilai ABI. Selain itu terjadi penurunan keluhan yang dirasakan oleh pasien. Melihat keefektifan senam kaki diabetic pada pasien diabetes melitus maka diharapkan intervensi ini dapat digunakan sebagai perawatan rutin pada pasien diabetes melitus
Diabetes Mellitus is a non-communicable and chronic disease that occurs in adults and requires ongoing medical supervision and self-care education. Diabetes mellitus can affect vascularization, causing changes in the macrovasculature generally affecting the brain, heart, and blood vessels and microvasculature can occur in the eyes and kidneys as well as changes in the nervous system or neuropathy such as motor, sensory neuropathy or autonomic neuropathy. The aim of writing this scientific work is to analyze the application of diabetic foot exercise interventions in providing nursing care to diabetes mellitus patients. Diabetic foot exercises can also increase perfusion, preventing worsening neuropathy in diabetes mellitus patients. The intervention was carried out for five days of treatment and began on the third day. The results show that diabetic foot exercises can improve peripheral circulation as indicated by changes in pulse oximetry, CRT, and ABI values. Apart from that, there was a decrease in complaints felt by patients. Seeing the effectiveness of diabetic foot exercises in diabetes mellitus patients, it is hoped that this intervention can be used as routine care for diabetes mellitus patients"

"Diabetes Melitus (DM) merupakan masalah kesehatan kronik yang dapat mempengaruhi kualitas hidup serta menyebabkan kematian. Lansia sebagai populasi yang mengalami proses penuaan, rentan menderita penyakit DM. Rendahnya pengendalian terhadap faktor risiko DM meningkatkan keparahan hiperglikemia dan risiko komplikasi kronis berupa neuropati diabetes. Latihan senam kaki sebagai salah satu intervensi praktik keperawatan berbasis bukti dapat mengurangi keparahan gejala neuropati dan meningkatkan sensitivitas kaki pada penderita neuropati dibetes. Penulisan ini bertujuan untuk memaparkan asuhan keperawatan lansia dengan DM dan gejala neuropati menggunakan latihan senam kaki untuk mencegah risiko kerusakan integritas jaringan pada Nenek I di PSTW Budi Mulya 1 Cipayung. Nenek I usia 66 tahun memiliki penyakit DM yang terdeteksi beberapa tahun lalu dengan kadar GDP saat ini 400mg/dl. Gejala neuropati yang dirasakan adalah telapak kaki terasa kebas, kesemutan, terasa tebal saat berjalan, serta terjadi penurunan sensitivitas kaki. Setelah dilakukan intervensi sebanyak 10 kali selama 3 minggu, terjadi penurunan gejala neuropati dan peningkatan sensitivitas kaki. Latihan senam kaki diharapkan dapat menjadi program rutin dan berkelanjutan yang diterapkan di PSTW Budi Mulya 1 Cipayung untuk mencegah terjadinya risiko kerusakan jaringan pada lansia dengan DM dan gejala neuropati.

Diabetes mellitus (DM) is a chronic health problem that can affect the quality of life and cause death. The elderly as a population experiencing the aging process, are susceptible to diabetes. The low control of DM risk factors will increase the severity of hyperglycemia and the risk of chronic complications—such as diabetic neuropathy. Foot gymnastics exercise as an evidence-based nursing practice intervention can reduce the severity of neuropathic symptoms and increase foot sensitivity in diabetic neuropathy sufferers. This writing aims to describe nursing care for the elderly with DM and symptoms of neuropathy using foot gymnastics  exercises to prevent the risk of tissue integrity damage to Nenek I at PSTW Budi Mulya 1 Cipayung. Nenek I aged 66 years, had DM which was detected a few years ago with a current GDP level of 400mg/dl. Symptoms of neuropathy that are felt at this time are numbness of the feet, tingling, feeling thick when walking, and decreased foot sensitivity. After the intervention 10 times for 3 weeks, there was a decrease in neuropathy symptoms and an increase in foot sensitivity. Foot gymnastics exercise is expected to become a routine and sustainable program implemented at PSTW Budi Mulya 1 Cipayung to prevent the risk of tissue damage in the elderly with DM and symptoms of neuropathy.

"Hipertensi merupakan salah satu masalah kesehatan terbanyak dialami masyarakat seperti di pedesaan. Hipertensi dapat terjadi di berbagai kelompok usia, terutama dewasa dan lansia. Pesatnya peningkatan teknologi ditemui dapat memengaruhi perubahan gaya hidup menjadi tidak sehat, seperti rendahnya tingkat aktivitas maupun pola makan yang tidak sehat. Masalah kesehatan yang terjadi pada pasien hipertensi perlu diatasi sebagai tugas bersama, antara individu, keluarga dan tenaga kesehatan. Karya ilmiah ini bertujuan untuk menggambarkan dukungan keluarga dalam mengelola tekanan darah melalui latihan fisik. Hasil evaluasi menunjukkan adanya penurunan tekanan darah setelah dilakukan latihan fisik selama dua minggu dengan dukungan keluarga. Latihan fisik yang diberikan yaitu senam selama 15 menit dan jalan selama 30 menit rutin setiap hari. Karya ilmiah ini merekomendasikan latihan fisik sebagai salah satu intervensi untuk mengelola tekanan darah pada pasien hipertensi dan pentingnya dukungan keluarga dalam perawatan hipertensi.

Hypertension is one of the most common health problems experienced by people such as in rural areas. Hypertension can occur in various age groups, especially adults and the elderly. The rapid increase in technology found can affect changes in lifestyle to become unhealthy, such as low levels of activity and unhealthy eating patterns. Health problems that occur in hypertensive patients need to be addressed as a joint task, between individuals, families and health workers. This scientific work aims to describe family support in managing blood pressure through physical exercise. Evaluation results show a decrease in blood pressure after physical exercise for two weeks with family support. Physical exercise provided is 15 minutes of exercise and 30 minutes of walking every day. This scientific work recommends physical exercise as an intervention to manage blood pressure in hypertensive patients and the importance of family support in the treatment of hypertension."

Kurangnya pencegahan faktor risiko diabetes menyebabkan keparahan dan durasi hiperglikemia yang lama, berisiko terjadi komplikasi kronis seperti neuropati diabetik. Latihan senam tangan dan kaki merupakan tindakan keperawatan berbasis bukti yang dapat mengurangi gejala neuropati, serta meningkatkan sensasi dan kemampuan ADL. Penulisan ini bertujuan untuk menjabarkan asuhan keperawatan pada lansia dengan diabetes tipe 2 dengan gejala neuropati melalui latihan senam tangan dan kaki untuk mengurangi keluhan nyeri kronis dan menurunkan risiko kerusakan integritas jaringan pada Nenek L di PSTW Budi Mulia 4 Ciracas. Nenek L berusia 64 tahun mengalami diabetes tipe 2 dengan kadar GDS saat ini 335 mg/dL. Gejala yang dirasakan berupa kesemutan, nyeri (seperti tertusuk), terasa tebal saat berjalan dan beristirahat, serta penurunan sensasi pada tangan dan kaki. Setelah dilakukan intervensi sebanyak 24 kali selama 8 hari, terjadi peningkatan sensasi pada kaki dan tangan, serta penurunan gejala neuropati. Latihan senam tangan dan kaki diharapkan dapat menjadi program harian yang dilaksanakan di PSTW Budi Mulia 4 Ciracas pada lansia dengan diabetes tipe 2 dengan gejala neuropati untuk mengurangi keluhan nyeri kronis dan mencegah risiko kerusakan integritas jaringan.

This paper aims to describe nursing care for the elderly with type 2 diabetic with symptoms of neuropathy through exercise of the hands and feet to reduce complaints of chronic pain and reduce the risk of damage to tissue integrity in Nenek L at PSTW Budi Mulia 4 Ciracas. Nenek L, 64 years old, has type 2 diabetes with current GDS level of 335 mg/dL. Symptoms are tingling, pain (like being stabbed), feeling thick when walking and resting, and decreased sensation in the hands and feet. After 24 interventions for 8 days, can increase in sensation in the feet and hands, and reduce in neuropathy symptoms. Hand and foot exercises expected to become a daily program carried out at PSTW Budi Mulia 4 Ciracas in elderly with type 2 diabetes with neuropathy symptoms to reduce chronic pain complaints and prevent the risk of damage to tissue integrity."

"Tuberkulosis (TBC) merupakan penyakit menular yang disebabkan oleh bakteri Mycobacterium Tuberkulosis yang menular melalui droplet nuclei. Pada tahun 2020, diperkirakan sekitar 10 juta orang mengidap TBC di seluruh dunia dan hal ini menjadi penyebab kematian tertinggi ke-13 serta menjadi penyebab kematian dari penyakit menular nomor dua setelah COVID-19. Salah satu faktor risiko TBC adalah Diabetes Melitus (DM). Penderita DM mengalami penurunan sistem kekebalan tubuh sehingga memiliki 2-3 kali risiko lebih tinggi terkena TBC bila dibandingkan dengan seseorang tanpa DM. Penerapan intervensi unggulan pada asuhan keperawatan keluarga ini dilakukan untuk menganalisis keefektifan terapi farmakologis melalui kepatuhan minum obat yang dikombinasikan dengan terapi non-farmakologis berupa aktivitas fisik senam aerobik low impact terhadap penurunan kadar gula darah pada penderita DM. Metode praktik dilakukan dengan intervensi kepatuhan minum obat serta melakukan senam aerobik low impact selama 20 menit dengan pembagian waktu 5 menit pemanasan, 10 menit gerakan inti, dan 5 menit pendinginan. Setelah dilakukan pemberian intervensi, terjadi penurunan kadar gula darah sebanyak 333 mg/dL dengan rata-rata penurunan per harinya sebanyak 25,612 mg/dL. Penerapan kepatuhan minum obat dan senam aerobik low impact direkomendasikan untuk dapat diterapkan setiap harinya secara mandiri di rumah agar dapat mengontrol kadar glukosa darah sehingga dapat menurunkan risiko terjadinya TBC.

Tuberculosis (TB) is an infectious disease caused by the Mycobacterium tuberculosis transmitted through droplet nuclei. In 2020, it was estimated that around 10 million people suffered from TBC. TBC is the 13th leading cause of death and the second leading cause of death from infectious diseases after COVID-19. One of the risk factors for TB is Diabetes Mellitus (DM). People with DM have a decreased immune sistem so they have a 2-3 times higher risk of developing TB when compared to someone without DM. The superior intervention of family nursing care is carried out to analyze the effectiveness of pharmacological therapy through adherence to medication combined with non-pharmacological therapy in the form of low-impact aerobic exercise to reduce blood sugar levels in DM patients. The practical method was carried out by intervening with medication adherence and doing low-impact aerobic exercise for 20 minutes with 5 minutes of warm-up time, 10 minutes of core movement, and 5 minutes of cooling down. After the intervention, there was a decrease in blood sugar levels by 333 mg/dL with an average daily decrease of 25,612 mg/dL. The application of drugs and low-impact aerobic exercise is recommended to be applied every day independently at home to control blood glucose levels and reduce the occurrence of tuberculosis."

"Hipertensi merupakan salah satu penyakit kardiovaskuler dan dapat dikenal sebagai the serial killer yang sering terjadi pada masyarakat dan menyerang berbagai kalangan usia, salah satunya dewasa. Gaya hidup tidak sehat seperti kurangnya melakukan latihan fisik, pola makan tidak sehat dengan banyak konsumsi makanan asin, dan stress, menjadi penyebab terjadinya masalah hipertensi. Masalah kesehatan hipertensi perlu diatasi bersama, antara individu, keluarga, dan tenaga kesehatan. Latihan fisik senam hipertensi menjadi salah satu alternatif penanganan non farmakologi untuk menurunkan tekanan darah. Karya ilmiah ini bertujuan untuk mengetahui efektifitas penerapan latihan fisik senam hipertensi untuk menurunkan tekanan darah pada Bapak S. Hasil implementasi dilakukan senam hipertensi selama 3-4 kali dalam seminggu dengan durasi 20 menit selama dua minggu. Hasil evaluasi menunjukkan adanya penurunan tekanan darah sistolik 175 mmHg menjadi 129 mmHg dengan rata-rata penutunan 5,75 mmHg dan tekanan darah diastolik dari 95 mmHg menjadi 75 mmHg dengan rata-rata penurunan 2,5 mmHg. Intervensi latihan fisik senam hipertensi dapat direkomendasikan menjadi salah satu intervensi keluarga dengan hipertensi untuk menurunkan tekanan darah dan meningkatkan kesehatan.

Hypertension is a cardiovascular disease and can be know as the serial killer that often occurs in the community and attacks various age groups, especially adult. Unhealthy lifestyles such as lack of physical exercise, unhealthy eating patterns with a lot of salty food consumption, and stress are the causes of hypertension problems. Hypertension health problems need to be addressed, between individu, family, and health workers. Physical exercise: hypertension gymnastics is an alternative non-pharmacological treatment to reduce blood pressure. This scientific work aims to knowing the effectiveness of implementing physical exercise for hypertension gymnastics to reduce Mr. S blood pressure. The results of this implementation carried our hypertension gymnastics for 3-4 times a week with a duration of 20-30 minutes for two weeks. The evaluation results showed a decrease in systolic blood pressure from 175 mmHg to 129 mmHg with avarage decrease 5,75 mmHg and diastolik blood pressure from 95 mmHg to 75 mmHg with avarage decrease 2,5 mmHg. Interventions of hypertension gymnastics can be recommended as a family intervention with hypertension to reduce blood pressure and improve health."

"[ABSTRAK
Diabetes melitus merupakan sekelompok gangguan metabolisme tubuh, ditandai
dengan hiperglikemik kronis yang dapat mengakibatkan komplikasi akut dan
kronis. Salah satu peran perawat sebagai pemberi asuhan adalah memberikan
asuhan keperawatan yang berkualitas melalui pendekatan Teori Self Care Orem.
Penggunaan pendekatan teori ini diharapkan dapat membantu perawat dalam
menanggulangi keterbatasan yang dimiliki pasien dan melibatkan pasien secara
aktif dalam proses perawatannya melalui peilaku perawatan mandiri. Tujuan
penulisan karya ilmiah akhir ini adalah menganalisis hasil proses belajar residensi
keperawatan medikal bedah melalui penerapan asuhan keperawatan, penerapan
evidence based nursing practice, dan inovasi keperawatan. Data yang diperoleh
selama praktik klinik menunjukkan sebagian besar pasien diabetes melitus
mengalami komplikasi ulkus kaki diabetik yang disebabkan oleh perilaku
perawatan kaki mandiri belum optimal. Hasil penerapan evidence based nursing
practice berupa kegiatan edukasi perawatan kaki mandiri dapat meningkatkan
pengetahuan dan membentuk perilaku perawatan kaki sebagai tindakan
pencegahan dini. Hasil kegiatan inovasi keperawatan melalui pengkajian
kesehatan mandiri adalah pasien dapat melakukan pengkajian mandiri terhadap
kebutuhan edukasi dan mendapatkan edukasi sesuai kebutuhan mereka.
Kesimpulan dari karya ilmiah akhir ini adalah perlunya peningkatan kualitas
asuhan keperawatan pada pasien diabetes melitus melalui pendekatan teori keperawatan untuk meningkatkan kemandirian pasien dalam pengelolaan penyakitnya.ABSTRACT Diabetes mellitus is a metabolic disorder that characterized by chronic
hyperglycemia which could lead to acute and chronic complications. One of nurse
roles in managing diabetes mellitus is deliver nursing care using Orem?s Theory
of Self Care as guide of nursing practice. With this theoretical approach, nursing
is expected to be able to overcome the limitations and inability of the patients and
actively involve patients in the treatment process through self-care behaviors. The
aims of this paper is to analyze the implementation of nursing care, application of
evidence based nursing practice, and innovation program which integrated to
clinical practice. Based on patient collective data, that was found that the majority
of diabetes mellitus patients had been hospitalized with diabetic foot ulcers
complications that caused by non-optimal foot self-care behavior. The results of
evidence based nursing practice showed that the diabetic foot care education
program can improve the knowledge and build the foot self-care behavior as early
prevention action. The results of innovation program through self-health
assessment program are patients could assess their needs of educations and get
health education according to assessment results. In conclusion, it?s needed to

evidence based nursing practice showed that the diabetic foot care education
program can improve the knowledge and build the foot self-care behavior as early
prevention action. The results of innovation program through self-health
assessment program are patients could assess their needs of educations and get
health education according to assessment results. In conclusion, it?s needed to

"ABSTRAK
Diabetes melitus merupakan sekelompok gangguan metabolisme tubuh, ditandai dengan hiperglikemik kronis yang dapat mengakibatkan komplikasi akut dan kronis. Salah satu peran perawat sebagai pemberi asuhan adalah memberikan asuhan keperawatan yang berkualitas melalui pendekatan Teori Self Care Orem. Penggunaan pendekatan teori ini diharapkan dapat membantu perawat dalam menanggulangi keterbatasan yang dimiliki pasien dan melibatkan pasien secara aktif dalam proses perawatannya melalui peilaku perawatan mandiri. Tujuan penulisan karya ilmiah akhir ini adalah menganalisis hasil proses belajar residensi keperawatan medikal bedah melalui penerapan asuhan keperawatan, penerapan evidence based nursing practice, dan inovasi keperawatan. Data yang diperoleh selama praktik klinik menunjukkan sebagian besar pasien diabetes melitus mengalami komplikasi ulkus kaki diabetik yang disebabkan oleh perilaku perawatan kaki mandiri belum optimal. Hasil penerapan evidence based nursing practice berupa kegiatan edukasi perawatan kaki mandiri dapat meningkatkan pengetahuan dan membentuk perilaku perawatan kaki sebagai tindakan pencegahan dini. Hasil kegiatan inovasi keperawatan melalui pengkajian kesehatan mandiri adalah pasien dapat melakukan pengkajian mandiri terhadap kebutuhan edukasi dan mendapatkan edukasi sesuai kebutuhan mereka. Kesimpulan dari karya ilmiah akhir ini adalah perlunya peningkatan kualitas asuhan keperawatan pada pasien diabetes melitus melalui pendekatan teori keperawatan untuk meningkatkan kemandirian pasien dalam pengelolaan penyakitnya.
