"Laporan tugas khusus ini merekap kegiatan Praktik Kerja Profesi Apoteker yang penulis lakukan di Apotek Roxy Poltangan. Karya ini dibuat dengan tujuan untuk menambah wawasan penulis mengenai kegiatan pengkajian resep dan pelayanan konseling yang merupakan tanggung jawab dari seorang apoteker di apotek. Kegiatan pembelajaran dan praktik kerja dilakukan dengan observasi, pelaksanaan kegiatan dengan pengawasan, dan pelaksanaan kegiatan mandiri. Penulis berkesempatan untuk mengkaji dan melayani resep harian serta melakukan pencarian resep tertentu dan kemudian memberikan pelayanan konseling untuk pasien Apotek Roxy Poltangan yang bersangkutan.

This special assignment report summarizes the Pharmacist Professional Internship Program that the author carried out at Apotek Roxy Poltangan. This work was created with the objective of broadening author’s insight regarding prescription review and counseling services, which are the responsibilities of a pharmacist in a pharmacy. The internship program was accomplished through observation, implementation of activities with supervision, and independent or direct activities of pharmaceutical services. The author had the opportunity to review and serve daily prescriptions as well as compiling certain prescriptions and then providing counseling services for the corresponding Apotek Roxy Poltangan patients."

"Terapi Sulih Hormon (HRT) estrogen dan progesteron digunakan untuk mengatasi gejala menopause dan untuk pencegahan osteoporosis. Terapi ini bermanfaat signifikan jika digunakan pada wanita dengan gejala dalam 10 tahun pertama setelah menopause atau pada usia di bawah 60 tahun. Penggunaannya dalam dosis tinggi dan jangka panjang dapat menimbulkan risiko kanker endometrium dan payudara. Apoteker dalam farmasi klinik bertanggung jawab memastikan resep hormon diatur dengan tepat untuk mencegah kesalahan pengobatan dengan melakukan pengkajian resep. Pengkajian resep dilakukan pada resep hormon dan kontrasepsi pasien yang masuk di Apotek Roxy Sawangan pada periode April 2023 dengan menelaah aspek administratif, aspek farmasetik dan aspek klinis. Kajian aspek administratif  pada resep 1: tidak ada SIP, paraf dokter dan berat badan pasien; dan pada resep 2 : tidak ada alamat dan berat badan pasien. Analisis farmasetik pada resep 1 menunjukkan kekeliruan penulisan cara pemberian untuk terapi cyclo progynova dan tidak ditemukan penyimpangan pada resep 2. Pada pengkajian aspek klinis resep 1 ditemukan ketidaksesuaian aturan pakai pada resep 1 dan cara pemakaian belum lengkap pada resep 2. Pengkajian lebih lanjut terhadap resep-resep hormon untuk indikasi lainnya perlu dilakukan untuk pencegahan masalah terkait obat dan kesalah pengobatan yang lebih baik.

Hormone Replacement Therapy (HRT) estrogen and progesterone is used to treat menopausal symptoms and for the prevention of osteoporosis. This therapy is of significant benefit if used in women with symptoms in the first 10 years after menopause or under the age of 60 years. Its use in high doses and long term can increase the risk of endometrial and breast cancer. Pharmacists in clinical pharmacy are responsible for ensuring hormone prescriptions are managed appropriately to prevent medication errors by reviewing prescriptions. Prescription reviews were carried out on hormone and contraceptive prescriptions for patients admitted to the Roxy Sawangan Pharmacy in the April 2023 period by examining administrative aspects, pharmaceutical aspects and clinical aspects. Study of administrative aspects of prescription 1: there is no SIP, doctor's initials and patient's weight; and in prescription 2: no patient's address and weight. Pharmaceutical analysis on prescription 1 showed an error in writing the administration method for cyclo progynova therapy and no irregularities were found in prescription 2. In assessing the clinical aspects of prescription 1, it was found that the instructions for use in prescription 1 were inconsistent and the instructions for use were incomplete in prescription 2. Further assessment of the prescriptions Prescribing hormones for other indications is necessary for better prevention of drug-related problems and medication errors."

"Antihistamin merupakan golongan obat yang sering digunakan untuk mengobati alergi. Penggunaan antihistamin pada pasien pediatrik dan geriatri perlu mendapat perhatian khusus karena kelompok usia ini memiliki karakteristik fisiologis yang berbeda dengan orang dewasa. Tujuan dari tugas khusus ini adalah mengkaji resep antihistamin untuk pediatri dan geriatri yang diterima di apotek Roxy Hasyim Ashari. Metode penelitian ini menggunakan metode deskriptif retrospektif dengan teknik pengumpulan data melalui analisis resep obat. Data yang diperoleh dianalisis dengan menggunakan statistik deskriptif. Berdasarkan hasil penelitian, pola penggunaan antihistamin pada pasien pediatrik dan geriatri di Apotek Roxy Hasyim Ashari menunjukkan beberapa temuan, antara lain: Jenis antihistamin yang paling sering digunakan pada pasien pediatrik adalah loratadine dan cetirizine; Jenis antihistamin yang paling sering digunakan pada pasien geriatri adalah diphenhydramine dan chlorpheniramine; Dosis antihistamin yang digunakan pada pasien pediatrik umumnya lebih rendah daripada pasien geriatri dan lama penggunaan antihistamin pada pasien pediatrik umumnya lebih singkat daripada pasien geriatri. Kesimpulan Berdasarkan hasil di Apotek Roxy Hasyim Ashari yang telah dilakukan oleh dapat disimpulkan bahwa penggunaan antihistamin pada pasien geriatric dan pediatri sudah sesuai dengan dosis penyesuaian untuk pasien pediatri dan geriatri.

Antihistamines are a class of drugs that are often used to treat allergies. The use of antihistamines in pediatric and geriatric patients needs special attention because this age group has different physiological characteristics from adults. The aim of this special assignment is to examine antihistamine prescriptions for pediatrics and geriatrics received at the Roxy Hasyim Ashari pharmacy. This research method uses a retrospective descriptive method with data collection techniques through drug prescription analysis. The data obtained was analyzed using descriptive statistics. Based on the research results, the pattern of antihistamine use in pediatric and geriatric patients at the Roxy Hasyim Ashari Pharmacy shows several findings, including: The types of antihistamines most frequently used in pediatric patients are loratadine and cetirizine; The types of antihistamines most frequently used in geriatric patients are diphenhydramine and chlorpheniramine; The dose of antihistamines used in pediatric patients is generally lower than in geriatric patients and the duration of antihistamine use in pediatric patients is generally shorter than in geriatric patients. Conclusion Based on the results at the Roxy Hasyim Ashari Pharmacy that have been carried out, it can be concluded that the use of antihistamines in geriatric and pediatric patients is in accordance with dose adjustments for pediatric and geriatric patients."

"Tenaga Kefarmasian melaksanakan Pekerjaan Kefarmasian di industri farmasi obat, apotek, rumah sakit, ataupun pada pemerintahan seperti di Badan Pengawasan Obat dan Makanan. Penelitian ini memiliki beberapa tujuan, yaitu untuk mengetahui proses pengujian sampel QC serta mengetahui kebutuhan biaya analisa sampel di laboratorium Departemen Quality Control pada PT. Soho Industri Pharmasi selama periode Juli – Agustus 2021; diharapkan dapat menambah pengetahuan dan pengalaman para calon Apoteker sehingga dapat memahami pengawasan Obat dan Makanan yang dilakukan oleh BPOM, serta berkontribusi sebagai seorang apoteker di lingkungan masyarakat; untuk mengetahui kesesuain resep pasien baik kajian administratif, farmasetik, dan klinis, serta untuk melaksanakann praktik kefarmasian berupa konseling obat terhadap pasien di Apotek Roxy Poltangan, Jakarta Selatan periode 01 – 30 November 2021. Pengambilan data diperoleh dari dokumen prosedur tetap setiap produk finished goods dan raw material and packaging; diperoleh dari seluruh UPT di Indonesia yang diakses melalui SIPT (Sistem Informasi Pengawasan Terpadu); resep diperoleh dari pengunjung yang melakukan penebusan resep di Apotek Roxy Poltangan. Hasil penelitian diperoleh yakni kegiatan kefarmasian di PT. Soho Industri Pharmasi khususnya pada quality control telah sesuai dengan penerapan CPOB dan GMP untuk memastikan hasil produk yang konsisten sesuai persyaratan kualitas, keamanan, dan kemanjuran; Badan POM bertugas untuk melakukan pengawasan Obat dan Makanan sesuai dengan ketentuan peraturan perundang-undangan, dengan berkoordinasi dengan Menteri Kesehatan dan bertanggung jawab kepada Presiden; Apotek Roxy Poltangan melakukan pekerjaan kefarmasian yang telah sesuai dengan peraturan perundang-undangan yang berlaku, melalui pelaksanaan penyiapan dan pelayanan resep yang diawali dengan skrining resep secara administratif, farmasetik dan klinis, kemudian dilanjutkan dengan dispensing obat.

Pharmaceutical fields carry out Pharmaceutical Work in the pharmaceutical industry, pharmacies, hospitals, or in government such as the Food and Drug Administration. This study has several objectives, to determine the process of testing QC and the cost of analysis in the laboratory of the Quality Control Department at PT. Soho Industri Pharmasi during the period July – August 2021; increase the knowledge and experience of prospective Pharmacists so that they can understand the supervision of Drugs and Food carried out by BPOM, as well as contribute as a pharmacist in the community; determine the suitability of patient prescriptions for administrative, pharmaceutical, and clinical studies, as well as to carry out pharmaceutical practices in the form of drug counselling to patients at the Roxy Poltangan Pharmacy, South Jakarta for the period 01 – 30 November 2021. Data were obtained from the standard procedure documents for raw materials and packaging finished goods products; obtained from all UPTs in Indonesia which are accessed through SIPT (Integrated Monitoring Information System); prescriptions are obtained from visitors who redeem prescriptions at the Roxy Poltangan Pharmacy. The results obtained are pharmaceutical activities at PT. Soho Industri Pharmasi, especially in quality control, has complied with the implementation of GMP and GMP to ensure consistent product results according to quality, safety, and efficacy requirements; BPOM Agency is in charge of supervising Drugs and Food by the provisions of the legislation, in coordination with the Minister of Health and is responsible to the President; Roxy Poltangan Pharmacy carries out pharmaceutical work that is by applicable laws and regulations, through the preparation and service of prescriptions, beginning with administrative, pharmaceutical and clinical prescription screening, followed by the drug dispensing."

"Kegiatan pengkajian resep meliputi kajian administrasi, kesesuaian farmasetik, dan pertimbangan klinis. Jika ditemukan adanya ketidaksesuaian dari hasil pengkajian apoteker harus menghubungi dokter penulis resep. Pada setiap tahap alur pelayanan Resep dilakukan upaya pencegahan terjadinya kesalahan pemberian obat (medication error). Penyusunan tugas khusus Praktik Kerja Profesi Apoteker di Apotek Roxy Sawangan bertujuan untuk mengidentifikasi kelengkapan resep obat terapi gangguan pencernaan berdasarkan aspek administratif, aspek farmasetik dan aspek klinis yang terdapat pada Apotek Roxy Sawangan periode April 2023.Tugas khusus ini disusun dengan metode deskriptif dan melakukan kegiatan pengkajian resep pada dua resep pasien di Apotek Roxy Sawangan. Obat yang terdapat pada kedua resep tersebut memiliki indikasi yang beririsan yaitu sebagai terapi gangguan pencernaan. Berdasarkan pengkajian resep yang dilakukan terhadap kedua resep dapat disimpulkan pada resep 1 aspek administratif dan farmasetik sudah lengkap tetapi pada aspek klinis menunjukkan ketidaktepatan dosis pemberian sirup sukralfat. Selanjutnya pada resep 2, aspek administratif kurang lengkap karena tidak ada alamat , paraf dokter dan berat badan pasien pada aspek administratif dan farmasetik sudah lengkap dan sesuai.

The preparation of a special assignment for Pharmacist Professional Work Practice at Roxy Sawangan Pharmacy aims to identify the completeness of the prescription of indigestion therapy drugs based on administrative aspects, pharmaceutical aspects and clinical aspects found at Roxy Sawangan Pharmacy for the period April 2023.This special assignment was prepared using a descriptive method and carried out prescription review activities on two patient prescriptions at Roxy Sawangan Pharmacy. The drugs contained in the two prescriptions have indications that overlap, namely as therapy for indigestion. Based on the prescription review carried out on the two prescriptions, it can be concluded that in recipe 1 the administrative and pharmaceutical aspects are complete but the clinical aspects show the inaccuracy of the dose of sucralfate syrup. Furthermore, in prescription 2, the administrative aspect is incomplete because there is no address, the doctor's initials and the patient's weight in the administrative and pharmaceutical aspects are complete and appropriate."

"Berdasarkan PMK no. 73 tahun 2016 tentang Standar Pelayanan Kefarmasian di Apotek, Apotek merupakan sarana bagi para apoteker untuk melakukan pelayanan kefarmasian. Salah satu kegiatan pelayanan kefarmasian yang dilakukan yaitu melakukan pengkajian atau skrining resep. Pemilihan resep yang akan di skrining dilakukan dengan melihat produk anti inflamasi fast moving pada penjualan bulan Juni 2023. Pengkajian ini dilakukan dengan melakukan pencarian daftar obat pada departemen anti inflamasi berdasarkan jumlah penjualan pada bulan Juni 2023 di aplikasi apotek dan memilih tiga resep yang terdapat produk fast moving sebagai representatif. Pengkajian ini bertujuan untuk mengidentifikasi obat yang terjual lebih dan melakukan analisa resep yang didalamnya tercantum obat anti inflamasi fast moving. Terdapat 82 obat anti inflamasi yang tersedia dengan asam mefenamat sebagai obat dengan jumlah penjualan terbanyak. Dari pengkajian resep, masih ditemukan masalah terkait obat sehingga dibutuhkan ketelitian lebih dalam melakukan pengkajian resep.

Based on Ministerial Regulation No. 73 of 2016 regarding Pharmaceutical Service Standards in Pharmacies, pharmacies serve as facilities for pharmacists to provide pharmaceutical services. One of the pharmaceutical services performed is prescription review or screening. The selection of prescriptions for screening is based on the fast-moving anti-inflammatory products sold in June 2023. This review involves searching the list of medications in the anti-inflammatory department based on sales data from June 2023 in the pharmacy application and selecting three prescriptions containing fast-moving products as representatives. The objective of this review is to identify drugs with higher sales and analyze prescriptions that include fast-moving anti-inflammatory drugs. There are 82 available anti-inflammatory drugs with mefenamic acid being the highest-selling drug. From the prescription review, issues related to drugs are still found, thus requiring greater precision in conducting prescription reviews."

"Penulisan resep memainkan peran penting dalam memastikan komunikasi yang optimal antara dokter, apoteker, dan perawat untuk meningkatkan keselamatan pasien. Kesalahan dalam penulisan resep sering terjadi dan dapat menyebabkan kesalahan pengobatan yang dapat berakibat fatal bagi pasien. Skrining resep, khususnya untuk polifarmasi, sangat penting untuk mencegah masalah terkait obat dan kesalahan pengobatan. Masalah dan Tujuan Penelitian: Masalah penelitian berfokus pada pentingnya skrining resep, khususnya untuk polifarmasi, untuk mencegah kesalahan pengobatan dan meningkatkan outcome pasien. Tujuan dari penelitian ini adalah untuk menyelidiki dampak skrining resep dalam mengurangi kesalahan pengobatan dan meningkatkan keselamatan pasien di lingkungan apotek. Metodologi: Desain penelitian melibatkan pelaksanaan skrining resep, termasuk skrining administratif, farmasi, dan klinis, untuk memastikan legalitas obat. resep dan meminimalkan kesalahan pengobatan. Partisipan penelitian terdiri dari apoteker yang bekerja di Apotek Roxy Beji, dan konteksnya adalah pada periode bulan Juli saat program Praktek Kerja Profesi Apoteker (PKPA). Temuan menunjukkan bahwa skrining resep, khususnya untuk polifarmasi, sangat penting dalam mencegah masalah terkait obat dan kesalahan pengobatan. Proses skrining membantu menghindari salah tafsir, kegagalan komunikasi, dan kesalahan pengobatan di antara para profesional layanan kesehatan, yang pada akhirnya meningkatkan keselamatan pasien dan kualitas hidup. Implikasi: Studi ini menyoroti pentingnya skrining resep dalam mengurangi kesalahan pengobatan dan meningkatkan keselamatan pasien. Penerapan protokol skrining yang ketat di apotek dapat membantu mencegah masalah terkait obat dan meningkatkan hasil pengobatan bagi pasien.

Prescription writing plays a crucial role in ensuring optimal communication between doctors, pharmacists, and nurses to enhance patient safety. Errors in prescription writing are common and can lead to medication errors, which can be fatal for patients. Screening prescriptions, especially for polypharmacy, is essential to prevent drug-related problems and medication errors.Research Problem and Aim: The research problem focuses on the importance of screening prescriptions, particularly for polypharmacy, to prevent medication errors and improve patient outcomes. The aim of the study is to investigate the impact of prescription screening on reducing medication errors and enhancing patient safety in a pharmacy setting.Methodology: The research design involves conducting prescription screening, including administrative, pharmaceutical, and clinical screening, to ensure the legality of prescriptions and minimize treatment errors. The study participants consist of pharmacists working at Apotek Roxy Beji, and the context is within the period of July during a Professional Work Practice of a Pharmacist (PKPA) program. The findings indicate that prescription screening, especially for polypharmacy, is essential in preventing drug-related problems and medication errors. The screening process helps in avoiding misinterpretations, communication failures, and medication errors between healthcare professionals, ultimately improving patient safety and quality of life. The study highlights the significance of prescription screening in reducing medication errors and enhancing patient safety. Implementing rigorous screening protocols in pharmacies can help prevent drug-related problems and improve treatment outcomes for patients."

"Apotek sebagai tempat melakukan praktek kefarmasian yang dilakukan oleh tenaga kefarmaian. Pelayanan kefarmasian yang dilakuakan di apotek salah satunya adalah pelayanan farmasi klinik yang meliputi pengkajian resep. Salah satu kajian resep yang dilakukan adalah resep untuk obat penyakit lambung. Menurut data dari Departemen Kesehatan RI angka presentase dari kejadian gastritis di Indonesia adalah 40,8% salah satu faktornya adalah tingkat kesadaran masyarakat tentang kesahatan lambung masih sangat rendah. Namun, penyakit lambung tidak dapat dianggap ringan sehingga perlu dilakukan Pengkajian Resep dan Pembuatan Media Edukasi Terkait Penyakit Lambung Untuk Pasien di Apotek Roxy Condet. Kegiatan pengkajian resep dilakukan dengan skrining resep dan membandingkan hasil skrining resep dengan dengan ketentuan obat pada lieteratur serta membuat media edukasi terkait penyakit lambung. Resep yang dilayani sebagian besar telah sesuai kajian administrasi dan farmasetik namun masih terdapat beberapa kajian administrasi, farmasetik, dan pertimbangan klinis yang belum lengkap sehingga konfirmasi kepada dokter pada resep yang kurang lengkap secara administrasi dan farmasetik dianggap perlu untuk meningkatkan jaminan terhadap kesehatan pasien. Pembuatan informasi (leaflet) mengenai penyakit dan obat-obatan serta terapi penunjang dapat dilakukan secara rutin untuk meningkatkan edukasi pasien terhadap kesehatan.

A pharmacy is where pharmaceutical staff carry out pharmaceutical practice. One of the pharmaceutical services provided in pharmacies is clinical pharmacy services, which include reviewing prescriptions. One of the prescription studies carried out was a prescription for medication for gastric diseases. According to data from the Indonesian Ministry of Health, the percentage of gastritis incidents in Indonesia is 40.8%, one of the factors being that the level of public awareness about stomach health is still very low. However, gastric disease cannot be considered mild, so it is necessary to review recipes and create educational media related to gastric disease for patients at the Roxy Condet Pharmacy. Prescription review activities are carried out by screening prescriptions, comparing the results of the prescription screening with the drug provisions in the literature, and creating educational media related to gastric diseases. Most of the prescriptions served are by administrative and pharmaceutical studies. However, several administrative, pharmaceutical, and clinical considerations still need to be completed, so confirmation of incomplete prescriptions to the doctor is necessary to increase patient health guarantees. Information (leaflets) regarding diseases and medicines and supporting therapy can be created regularly to increase patient education regarding health."

"Osteoartritis (OA) adalah kelainan umum dan progresif yang menyerang aktivitas sendi diarthrodial yang ditandai dengan kerusakan progresif dan hilangnya tulang rawan artikular, terjadinya pembentukan osteofit, nyeri, keterbatasan gerak, deformitas, dan disabilitas. Pemilihan terapi untuk pengobatan OA harus dipertimbangkan dengan tepat agar tidak menimbulkan resiko interaksi atau efek samping obat yang tidak diinginkan. Hal tersebut dikarenakan jika penatalaksanaan terapi tidak dilakukan dengan tepat (keliru) dapat menyebabkan sendi tidak dapat digunakan dan menimbulkan rasa nyeri yang berkepanjangaan pada penderita. Oleh karena itu, Apotek Roxy Jakasampurna melakukan pengkajian resep penyakit osteoartritis (OA) baik secara administratif, farmasetik dan klinis dan studi literatur terkait penatalaksanaan terapi penyakit osteoartritis (OA). Berdasarkan hasil pengkajian resep dan studi literatur mengenai penyakit Osteoartritis (OA) dapat disimpulkan bahwa kegiatan pengkajian dan pelayanan resep di Apotek Roxy Jakasampurna sudah terdokumentasi dengan baik dari segi administratif, farmasetik dan klinis. Hanya saja secara administratif tidak ditemukan kelengkapan data terkait identitas pasien seperti berat badan dan alamat pasien. Berdasarkan studi literatur terkait tatalaksana penyakit osteoartritis (OA), obat osteoartritis (OA) yang diresepkan juga sudah sesuai berdasarkan simtomatik dan patogenesis keluhan pasien dengan memberikan suplemen glukosamin yang dapat membantu menghambat degradasi tulang rawan dan eperisone hidroklorida yang bekerja sebagai relaksan otot.

Osteoarthritis (OA) is a common and progressive disorder that attacks diarthrodial joint activity which is characterized by progressive damage and loss of articular cartilage, osteophyte formation, pain, limited movement, deformity and disability. The choice of therapy for the treatment of OA must be considered appropriately so as not to pose a risk of interactions or unwanted drug side effects. This is because if the therapeutic management is not carried out correctly (wrongly) it can cause the joint to become unusable and cause prolonged pain in the sufferer. Therefore, Roxy Jakasampurna Pharmacy conducted a review of prescriptions for osteoarthritis (OA) both administratively, pharmaceutically and clinically and studied literature related to the therapeutic management of osteoarthritis (OA). Based on the results of the review of prescriptions and literature studies regarding Osteoarthritis (OA), it can be concluded that the study activities and prescription services at the Roxy Jakasampurna Pharmacy have been well documented from an administrative, pharmaceutical and clinical perspective. It's just that administratively, complete data regarding patient identity, such as weight and patient address, was not found. Based on literature studies related to the management of osteoarthritis (OA), the prescribed osteoarthritis (OA) medication is also appropriate based on the symptomatic and pathogenesis of the patient's complaints by providing glucosamine supplements which can help inhibit cartilage degradation and eperisone hydrochloride which works as a muscle relaxant."

"Seorang apoteker memiliki peran krusial dalam menjaga keselamatan pasien dengan memastikan pengelolaan maupun penggunaan obat yang aman. Adapun,  kesalahan pengobatan yang dapat membahayakan keselamatan pasien dapat terjadi  di berbagai tahap pelayanan kefarmasian, baik pada proses penulisan resep,  pembacaan resep, peracikan resep, maupun saat penggunaan obat. Sehingga, untuk menghindari terjadinya kesalahan pengobatan,  maka peran seorang apoteker sangat penting dalam melakukan pengkajian resep yang merupakan bagian dari pelayanan kefarmasian. Kegiatan pengkajian ini setidaknya mencakup tiga aspek yakni aspek  administrasi,  farmasetik serta klinis. Pada tugas khusus Praktek Kerja Profesi Apoteker (PKPA) di apotek Atrika, dilakukan pengkajian dua resep obat jantung. Adapun, dari telaah dan pengkajian resep yang telah dilakukan, diketahui bahwa dua resep obat jantung tersebut tergolong resep yang baik dari segi administrasi, farmasetis serta klinis.

A pharmacist contributes to patient safety by ensuring the safe management and use of medications. Potential medication errors that pose risks to patient safety may occur at various stages of pharmaceutical care, including prescribing, transcribing, dispensing, and during medication administration. Therefore, the role of a pharmacist is essential in conducting prescription reviews to prevent medication errors. As part of Praktek Kerja Profesi Apoteker (PKPA) at Atrika Pharmacy, two prescriptions for cardiovascular drug were reviewed. Based on the analysis and review of the prescriptions, it can be concluded that both prescriptions are considered good in terms of administrative, pharmaceutical, and clinical aspects."
