"Praktek Spesialis Keperawatan merupakan sebuah proses penting dari pendidikan profesi dalam rangka mengaplikasikan peran perawat spesialis yang berdampak pada upaya peningkatan kualitas layanan keperawatan. Praktek ini dilakukan dan dikembangkan berdasarkan ilmu pengetahuan dan teknologi melalui berbagai penelitian. Teori Virginia Henderson dan Teori Sosial Kognitif Bandura di RSUP FatmawatiJakarta. Tiga kompetensi yang harus dicapai dalam praktek residensi ini adalah memberikan asuhan keperawatan menggunakan pendekatan teori keperawatan, menerapkan implementasi keperawatan yang berbasis bukti ilmiah (evidence based nursing practice), dan melakukan proyek inovasi keperawatan yang berguna bagi lahanyang menjadi tempat praktek residensi. Sebagai pemberi asuhan keperawatan, mahasiswa residensi telah memberikan asuhan keperawatan menggunakan pendekatan teori Virginia Henderson pada 1 kasus kelolaan utama dan 30 kasus resume pasien HIV/AIDS dengan berbagai infeksi oportunitis. Sebagai peneliti, mahasiswa residensi telah mengaplikasi penerapan skrining Mual dan Muntah pasien HIV/AIDS menggunakan Index of Nausea, Vomiting and Retcing dan hasil dari evidence based nursing ini menunjukkan bahwa kuisioner INVR akurat dan reliable untuk menilai status mual dan muntah pada pasien HIV. Sedangkan sebagai innovator, telah dilakukan inovasi optimalisasi teknologi digital dalam meningkatkan kepatuhan minum obat ARV pada pasien dengan HIV / AIDS yang mana media teknologi digital berupa laman web yang dengan berbagai topik edukasi dengan web yang dapat di akses menggunakan barcode. Hasil penerapan proyek inovasi ini menunjukkan manfaat yang sangat signifikan bagi peningkatan pengetahuan dan kemudahan pasien dalam mengakases informasi penting tentang penyakit HIV sehingga dapat meningkatkan kepatuhan dalam minum obat ARV.

As researchers, residency students have applied the application of Nausea and Vomiting screening for HIV/AIDS patients using the Index of Nausea, Vomiting and Retcing and the results of this evidence-based nursing show that the INVR questionnaire is accurate and reliable to assess the status of nausea and vomiting in HIV patients. Meanwhile, as an innovator, digital technology optimization innovations have been carried out in improving adherence to taking ARV drugs in patients with HIV / AIDS where digital technology media is in the form of web pages with various educational topics with the web that can be accessed using barcodes. The results of the implementation of this innovation project show very significant benefits for increasing knowledge and ease of patients in accessing important information about HIV disease so as to improve adherence in taking ARV drugs."

"Praktek Keperawatan residensi yang di laksanakan di Rumah Sakit Fatmawati adalah suatu proses dalam Pendidika yaitu Profesi Keperawatan yang mana diharapkan dapat mengaplikasikan sebagaimana mestinya. Peran perawat spesialis sangat berdampak pada peningkatan kualitas layanan keperawatan yang berdasarkan Evidennce base nursing. Perawat Spesialais harapkan juga dapat menjadi seorang Clinical Care Manager (CCM), yaitu sebagai perawat konsultan dalam pemberian asuhan keperawatan untuk kubutuhan dasar manusia dimana teori ini menunjukan peran sebagai perawat pemberi asuhan keperawatan dalam membantu dan menolong pasien baik dalam keadaan sehat maupun dalam keadaan sakit, dimana induvidu dapat berusaha melakukan beberapa aktifitas untuk mendukung penyembuhan dan menjaga kesehatan sehingga menyiapkan kematian dengan damai. Tiga kompetensi yang harus di capai dalam praktik residensi yaitu memberikan Asuhan Keperawatan menerapkan Implementasi keperawatan yang berbasis bukti Ilmiah (Evidennce base nursing), dan melakukan proyek inovasi keperawatan yang dapat di gunakan di tempat lahan praktek residensi.Mahasiswa residensi telah melakukan penerapan mindfulness pada pasien HIV khusunya perempuan yang mengalami kualitas hidup yang menurun dan dalam kondisi seperti ini pasien mengalami kesendirian atau kesepian. Hasil dari evidence based nursing menunjukan bahwa intervensi mindfulness dapat meningkatkan kualitas hidup khusunya pada wanita yang terdiagnosa HIV. Sedangkan proyek inovasi yang telah di lakukan oleh residensi keperawatan tentang edukasi dalam meningkatkan kedisiplinan minum AR dimana media edukasi yang akan di berikan kepada psien HIV ber bentuk wab page sehingg dapat di akses melalui barcode dan hasil dari penerapan proyek inovasi dapat menunjukan manfaat yang terlihat secara signifikan dalam meningkatkan pengetahuan dan dalam pencegahan infeksi oportunitik pada ODHA.

Results from evidence-based nursing show that mindfulness interventions can improve the quality of life, especially for women diagnosed with HIV.Meanwhile, the innovation project that has been carried out by the nursing residency is about education in improving AR drinking discipline where the educational media that will be given to HIV patients is in the form of a web page so that it can be accessed via barcode and the results of implementing the innovation project can show significant visible benefits. in increasing knowledge and in preventing opportunistic infections in PLWHA."

Sedangkan sebagai innovator, telah dilakukan inovasi edukasi pengetahuan terkait pencegahan infeksi oportunistik yang mana media edukasi juga dibetuk dapat barcode dan hasil penerapan proyek inovasi ini menunjukkan manfaat yang sangat signifikan bagi peningkatan pengetahuan dalam melakukan pencegahan infeksi oportunistik pada ODHA.

Meanwhile, as an innovator, innovative knowledge education has been carried out related to the prevention of opportunistic infections in which educational media has also been formed to be barcoded and the results of implementing this innovation project show very significant benefits for increasing knowledge in preventing opportunistic infections in PLHIV."

Sedangkan sebagai peran inovator, telah dilakukan proyek “VOSIL SPARTAN” berupa pemberian edukasi pencegahan infeksi oportunistik pada ODHIV dengan hasil peningkatan pengetahuan dan sikap ODHIV guna pencegahan munculnya infeksi oportunistik.

"Praktek Spesialis Keperawatan Medikal Bedah merupakan proses penting dari pendidikan profesi dalam rangka mengaplikasikan ilmu dan peran perawat spesialis dalam memberikan asuhan keperawatan yang profesional kepada pasien. Dalam menjalankan praktek Praktek Spesialis Keperawatan Medikal Bedah, residen juga menerapkan  Evidence Based Nursing. Evidence Based Nursing yang dilakukan adalah dengan menerapkan intervensi Jacobson Progressive Muscle Relaxation, serta  menjalankan peran sebagai inovator di lahan praktek. Residen telah melakukan asuhan keperawatan pada pasien dengan HIV/AIDS, dengan 1 kasus kelolaan utama yang menggunakan pendekatan Teori Roy dan 30 kasus resume kelolaan menggunakan pendekatan  Teori Bandura untuk ruang rawat jalan pada kasus HIV. Dari hasil kelolaan kasus utama, pasien HIV yang dirawat di ruang rawat memiliki diagnosa keperawatan utama gangguan perfusi jaringan serebral sehingga intervensi utama keperawatan fokus pada manajemen tekanan intrakranial. Dari kasus resume rawat jalan, masalah keperawatan yang paling banyak ditemukan adalah ketidakpatuhan akan pengobatan, sehingga Intervensi keperawatan yang dilakukan adalah dengan melakukan proyek inovasi Counseling Minum Obat ARV yang berisikan konseling secara terstruktur pengobatan ART dalam upaya meningkatkan self confidence perawat serta meningkatkan kepatuhan pasien.

Practice of Medical Surgical Nursing Specialist is an important process of professional education in order to apply the knowledge and role of specialist nurses in providing professional nursing care to patients. In carrying out the practice of Medical Surgical Nursing Specialist, the resident also applies Evidence Based Nursing. Evidence Based Nursing is carried out by applying the Jacobson Progressive Muscle Relaxation intervention, and playing the role of an innovator in the practical field. Resident has provided nursing care to patients with HIV / AIDS, with 1 main case managed using the Roy Theory approach and 30 cases managed resumes using the Bandura Theory approach for outpatient rooms in HIV cases. From the results of the main case management, HIV patients who were treated in the ward had a major nursing diagnosis of cerebral tissue perfusion disorders so that the main nursing interventions focused on the management of intracranial pressure. From outpatient resume cases, the most common nursing problem found was non-adherence to treatment, so that the nursing intervention was carried out by conducting the innovative ARV Medication Counseling project which contained structured counseling on ART treatment in an effort to increase nurses' self-confidence and increase patient compliance.

Sedangkan sebagai innovator, telah dilakukan konseling terstruktur ”CEMARA” pada perawat dan ODHA putus ARV dan hasil penerapan proyek inovasi ini menunjukkan manfaat yang sangat signifikan bagi peningkatan self conffidence perawat dalam melakukan konseling kepatuhan dan meningkatnya kepatuhan minum ARV pasien ODHA dengan riwayat putus obat.

As a researcher, residency students have applied scientific evidence-based nursing actions "Brief Risk Reduction Intervention" in men who have sex with men (MSM) with HIV/AIDS, where the results show an increase in knowledge about sexual health of them, and a decrease in the frequency of unprotected anal sex practice that has an impact on decreasing the risk of HIV transmission among MSM with HIV/AIDS. While as an innovator, "CEMARA" structured counseling for nurses and PLWHA dropped out and the results of the application of this innovation project showed a very significant benefit for the increase in nurse self confidence in conducting adherence counseling and increasing adherence to ARV consumption of PLWHA patients with a history of drug withdrawal."

"Pelaksanaan praktek residensi keperawatan medical bedah peminatan imunologi ditujukan supaya perawat residen mampu memberikan asuhan keperawatan spesialis dalam menerapkan evidence-based nursing (EBN) yang berperan sebagai inovator dan edukator di pelayanan keperawatan. Peran perawat residen sebagai pemberi asuhan keperawatan dilakukan dengan menerapkan teori adaptasi Roy pada pasien dengan HIV dan AIDS dengan TB Paru dan 30 pasien HIV dan AIDS dengan infeksi oportunistik lainnya. Masalah keperawatan yang paling banyak didapatkan yaitu risiko ketidakseimbangan elektrolit, intervensi keperawatan yang dilakukan adalah pemantauan elektrolit dengan tujuan keseimbangan elektrolit meningkat. Penerapan EBN pemberian probiotik yogurt pada pasien dengan HIV, AIDS dan keganasan yang menunjukkan kandidiasis oral berupa plak putih pada lidah. Probiotik yogurt diberikan 2 kali sehari selama 5 hari dengan penilaian evaluasi skor tongue coating Miyazaki, hasil menunjukkan 45,6% terjadi penurunan skor dari grade 3 menjadi grade 0. Proyek inovasi pemberian edukasi dengan materi infeksi oportunistik yang menunjukkan pengetahuan dan sikap ODHA meningkat setelah pemberian edukasi.

The role of the resident nurse as a care giver is carried out by applying Roy's adaptation theory to patients with HIV and AIDS with pulmonary TB and 30 HIV and AIDS patients with other opportunistic infections. The most commonly encountered problem is the risk of going to electrolyte court, the engagement intervention for the risk of electrolyte stress is electrolyte monitoring with the aim of increasing electrolyte balance. Application of EBN giving probiotic yogurt to patients with HIV, AIDS and malignancy showing oral candidiasis in the form of white plaque on the tongue. Probiotic yogurt was given 2 times a day for 5 days with an evaluation of the Miyazaki tongue coating score, the results showed a 45.6% decrease in score from grade 3 to grade 0. An innovation project providing education with material on opportunistic infections which shows that the knowledge and attitudes of PLWHA have increased after providing education."

Selama proses residensi menerapkan intervensi berdasarkan evidence based nursing, mampu menjadi seorang Clinical Care Manager (CCM) sebagai konsultan staf keperawatan, dan memberikan terapi keperawatan yang profesional. Dalam memberikan asuhan keperawatan pada kasus kelolaan pasien dengan Bronkiektasis dan 30 kasus resume menggunakan teori Henderson 14 kebutuhan dasar manusia yang menunjukkan peran perawat sebagai pemberi asuhan keperawatan dalam membantu individu baik dalam keadaan sehat maupun sakit, melalui usahanya melakukan berbagai aktivitas guna mendukung kesehatan dan penyembuhan individu atau proses meninggal dengan damai. Masalah keperawatan yang banyak muncul yaitu tentang pemenuhan kebutuhan bernapas normal. Penerapan EBN cold application dan latihan napas dan batuk untuk menurunkan tingkat nyeri pada pasien terpasang chest tube. Pelaksanaan proyek inovasi pengembangkan video edukasi monitoring dan perawatan di rumah pasien terpasang WSD sebagai media self-management bagi perawat ruang rawat dan pasien yang terpasang WSD. Hasil analisis praktik residensi keperawatan didapatkan bahwa asuhan keperawatan menggunakan pendekatan teori Henderson bertujuan untuk membantu sesegera mungkin kemandirian pasien. Penerapan cold application dan latihan napas dan batuk dapat menurunkan skor nyeri pasien terpasang chest tube. Proyek inovasi video edukasi monitoring dan perawatan di rumah pasien terpasang WSD sebagai media self-management meningkatkan pengetahuan perawat dan kemandirian pasien.

Medical Surgical Nursing Residency Practice is a series of activities for megisters and Nursing Specialists carried out in Health care facilities. The practice of medical surgical nursing residency focuses on the application of science, research and nursing technology in meeting the basic needs of adult patients with cardiovascular disorders in a comprehensive manner. Cardiovascular medical surgical nursing clinical practice is carried out at Heart and Vascular Hospital Harapan Kita (RSJPHK) Jakarta, by providing nursing care using the Virginia Henderson theoretical approach in one managed case with a medical diagnosis of UAP TIMI 2/7 GRACE 139 CRUSADE 57, AV Block degree 1, LBBB ect NSTEMI With History of CABG x2 (SVG-OM, SVG-LAD) and mechanical AVRAV block grade I and 30 cases resumed. Implementation of Evidence Based Nursing (EBN) using a single session 20-minute mindful breathing intervention to reduce complaints of dyspnea in cute Decompensation Heart Failure (ADHF) patients. The residency also made innovations, namely the application of pressure injury assessment applications in perioperative patients using the Munro scale as an instrument for preventing pressure injury in patients planning cardiac surgery."

"Praktik klinik residensi keperawatan medikal bedah pada pasien osteomielitis bertujuan untuk mengembangkan dan mengimplementasikan peran perawat spesialis sebagai pemberi asuhan keperawatan profesional, edukator, peneliti, dan pembaharu inovator. Peran pemberi asuhan keperawatan dilakukan dengan menggunakan pendekatan Teori Self Care Orem pada pasien Infected union of left tibia post debridement and bone graft application from iliac wing dan 30 pasien lainnya dengan berbagai gangguan sistem muskuloskeletal. Peran sebagai edukator dan peneliti dilakukan oleh perawat spesialis terhadap pasien dan juga rekan sejawat di tatanan pelayanan rumah sakit. Penerapan EBN (Evidence Based Nursing) yaitu dengan mengaplikasikan intervensi diaphragmatic breathing exercise pada pasien pasca operasi Total Knee Replacement untuk mengurangi nyeri dan kecemasan. Proyek inovasi juga dilakukan sebagai perwujudan peran sebagai pembaharu atau inovator yaitu melalui pelaksanaan berupa Pengkajian Risiko DVT skala Autar dalam rangka meningkatkan kualitas asuhan keperawatan profesional di rumah sakit.

The clinical practice of medical-surgical nursing residency in osteomyelitis patients aims to develop and implement the role of specialist nurses as professional nursing care providers, educators, researchers, and reformers innovators.
