"Apoteker memiliki peranan yang penting dalam industri farmasi, apotek, serta distributor obat dan alat kesehatan. Apoteker perlu memenuhi standar kompetensi sebagai persyaratan untuk menjalankan praktik profesi di tempat-tempat tersebut. Standar kompetensi apoteker Indonesia terdiri dari sepuluh hal sebagai kemampuan mendasar yang harus dimiliki oleh apoteker saat lulus dan memasuki dunia praktik profesik. Sebagai calon apoteker, mahasiswa diberikan bekal dan pengalaman melalui praktik kerja agar dapat memahami peran apoteker dan meningkatkan kompetensi yang dimilikinya. Oleh karena itu, dilakukan praktik kerja pada periode bulan Januari hingga April 2019 di beberapa tempat yaitu PT. Abbott Indonesia, Apotek Kimia Farma no.282, serta PT. Anugerah Pharmindo Lestari. Dengan adanya praktik kerja profesi ini, diharapkan calon apoteker dapat memperluas wawasan, pemahaman, dan pengalaman untuk melakukan pekerjaan kefarmasian yang sebenarnya.

Pharmacists have important role in the pharmaceutical industry, pharmacy, and drugs and medical devices distributors. Pharmacists need to meet competency standards as a requirement for carrying out professional practice in these places. Indonesian pharmacist competency standards consist of ten things as a basic ability that must be possessed by pharmacists when graduating and entering the world of professional practice. As a pharmacist, students are given the preparation and experience through internship in order to understand the role of the pharmacist and improve our competence. Therefore, internship were carried out in the period of January to April 2019 in several places, which are PT. Abbott Indonesia, Apotek Kimia Farma no. 282, and PT. Anugerah Pharmindo Lestari. This professional internship is expected can expand insights, understandings and experiences of pharmacy students to do the real pharmaceutical practices."

"Seorang apoteker memegang peranan penting di apotek, pedagang besar farmasi, dan industri farmasi. Seorang apoteker harus memenuhi standar kompetensi apoteker sebagai persyaratan untuk memasuki dunia kerja dan menjalani praktik profesi. Standar kompetensi apoteker Indonesia terdiri dari sepuluh standar kompetensi sebagai kemampuan yang diharapkan dimiliki oleh seorang apoteker. A pharmacist plays an important role in pharmacy, pharmaceutical wholesalers, and the pharmaceutical industry. A pharmacist must meet pharmacist competency standards as a requirement for entering the workforce and undergoing professional practice. Indonesian pharmacist competency standards consist of ten competency standards as a capability expected by a pharmacist. As a provision and experience for prospective pharmacists to be able to understand the role of pharmacists and improve competence, the Pharmacist Professional Work Practice (PKPA) was implemented at Apotek Kimia Farma No. 375 Depok, PT Anugerah Pharmindo Lestari and PT Sydna Farma for the February-May 2019 Period. Through PKPA , prospective pharmacists are expected to broaden their horizons, understanding, and experience to do pharmaceutical work in professional workplaces."

"Praktik kerja profesi apoteker dilakukan di PT Dankos Farma, PT Anugerah Pharmindo Lestari, dan Apotek Kimia Farma 352 Margonda pada bulan Juli-Desember tahun 2021. Laporan Praktik Kerja Profesi Apoteker ini memberikan beberapa informasi terkait dengan peran apoteker dalam mengaplikasikan Annex 1 Good Manufacturing Practices (GMP) Guide tentang Manufacture of sterile drugs (GUI-0119) dan Cara Pembuatan Obat yang Baik (CPOB) 2018 Aneks 1 tentang Pembuatan Produk Steril, mengaplikasikan Corrective and Preventive Action (CAPA) dalam upaya pencegahan dan penyelesaian masalah, merancang aplikasi kualifikasi pelanggan untuk PBF Anugerah Pharmindo Lestari, dan melakukan proses praktik kefarmasian jarak jauh atau Telefarma. Laporan disusun menggunakan metode pengkajian atau penelusuran studi literatur dan wawancara dengan pasien atau narasumber yang dilakukan pada periode Juli – Desember tahun 2021. Wawancara dengan pasien atau narasumber dibimbing secara langsung oleh Pembimbing Lapangan (Pembimbing I) dan secara tidak langsung oleh Pembimbing II. Hasil dari beberapa kegiatan ini menunjukkan bahwa apoteker baik di industri farmasi, PBF, maupun apotek memiliki peran yang sangat penting dalam meningkatkan kualitas produk farmasi, pendistribusian produk farmasi, dan pelayanan kefarmasian. Selain itu, kegiatan ini penting dilaksanakan sebagai upaya untuk meningkatkan pengalaman dan pengetahuan seorang calon apoteker.

The professional practice of pharmacists was carried out at PT Dankos Farma, PT Anugerah Pharmindo Lestari, and Apotek Kimia Farma 352 Margonda from July-December 2021. This Pharmacist Professional Practice Report provides some information related to the role of pharmacists in applying Annex 1 Good Manufacturing Practices (GMP) Guide on Manufacturing of sterile drugs (GUI-0119) and Good Manufacturing Practices (GMP) 2018 Annex 1 on Manufacturing of Sterile Products, applying Corrective and Preventive Action (CAPA) in efforts to prevent and solving problems, designing customer qualification applications for PBF Anugerah Pharmindo Lestari, and carry out the process of remote pharmacy practice or Telefarma. The report was prepared using the study method or literature study search and interviews with patients or interviewees from July to December 2021. Interviews with patients or interviewees were guided directly by the Field Supervisor (Supervisor I) and indirectly by Supervisor II. The results of these activities indicate that pharmacists in the pharmaceutical industry, PBF, and pharmacies have a vital role in improving the quality of pharmaceutical products, distribution of pharmaceutical products, and pharmaceutical services. In addition, this activity is essential to increase the experience and knowledge of a prospective pharmacist."

"CPOB atau Cara Pembuatan Obat yang Baik bertujuan untuk menjamin obat dibuat secara konsisten, memenuhi persyaratan yang ditetapkan dan sesuai dengan tujuan penggunaannya. Dalam CPOB tercantum persyaratan untuk ruang karantina. PT Hexpharm Jaya Laboratories memiliki ruang Work In Process (WIP) sebagai ruang karantina produk yang merupakan area penting dalam proses. Area ini digunakan sebagai tempat untuk karantina produk antara dan/atau tablet yang siap untuk dilakukan proses primary packaging. Kegiatan dan produk yang ada di ruang WIP harus sesuai dengan pedoman Cara Pembuatan Obat yang Baik (CPOB), produk yang berada dalam ruang WIP harus jelas identitasnya, ruangan harus terjaga kebersihannya, wadah yang digunakan harus sesuai ketentuan, dan akses untuk memasuki ruang WIP harus terbatas hanya untuk orang yang memiliki wewenang, seperti supervisor dan PIC pada ruang produksi. Penumpukan dalam ruang WIP juga dapat menyebabkan mix up batch yang dapat terjadi apabila operator salah mengambil material atau saat operator menggeser wadah, label identitas dari produk yang menempel pada wadah terjatuh mengakibatkan kemungkinan label tertukar. Oleh karena itu, dalam tugas khusus ini penulis diminta untuk memanajemen ruang WIP agar lebih terstruktur dan sesuai dengan pedoman Cara Pembuatan Obat yang Baik dengan cara pembuatan mapping dan lay out pada ruang WIP. Hasil yang didapat, hampir semua aspek pada ruang WIP PT. Hexpharm Jaya Laboratorium sudah sesuai dengan pedoman CPOB, hanya aspek kerapihan saja yang masih perlu ditingkatkan. Aspek kerapihan dapat diatasi dengan memanajemen ruang WIP dengan pembuatan mapping, hasilnya cukup efektif untuk membuat ruang WIP lebih terstruktur sehingga dapat menghindari risiko mix up batch dan memudahkan operator dalam mencari MSC atau MSS yang akan diproses

CPOB or Good Manufacturing Practices of Drugs aims to ensure that drugs are made consistently, meet the stipulated requirements and are in accordance with their intended use. In the GMP, the requirements for quarantine rooms are listed. PT Hexpharm Jaya Laboratories has a Work In Process (WIP) room as a product quarantine room which is an important area in the process. This area is used as a place for quarantine of intermediate products and/or tablets that are ready for the primary packaging process. The activities and products in the WIP room must comply with the guidelines for Good Manufacturing Practices (GMP), the product in the WIP room must have a clear identity, the room must be kept clean, the containers used must comply with the provisions, and access to enter the WIP room must be limited to authorized persons, such as supervisors and PICs in the production room. Stacking in the WIP chamber can also cause a mix up batch which can occur if the operator picks up the wrong material or when the operator shifts the container, the identity label of the product attached to the container falls, resulting in the possibility of labels being confused. Therefore, in this special task, the author is asked to manage the WIP room so that it is more structured and in accordance with the guidelines for the Good Manufacturing Practice of Medicine by mapping and laying out the WIP room. The results obtained, almost all aspects of the PT. Hexpharm Jaya Laboratory has complied with GMP guidelines, only the tidiness aspect still needs to be improved. The neatness aspect can be overcome by managing the WIP space by making a mapping, the results are quite effective in making the WIP room more structured so as to avoid the risk of batch mix ups and make it easier for operators to find MSC or MSS to be processed."

"Praktik Kerja Profesi Apoteker bertujuan untuk memperoleh pengetahuan, pemahaman dan pengalaman untuk melakukan praktis kefarmasian serta memahami tugas dan fungsi Apoteker dalam berpraktik. Tugas khusus praktik kerja di Rumah Sakit Universitas Indonesia yang diberikan berjudul Evaluasi Penggunaan Obat dengan Metode ABC di Depo Farmasi Rawat Jalan di Rumah Sakit Universitas Indonesia Periode Januari – September 2021. Tujuannya untuk mengetahui obat-obatan yang tergolong fast moving, slow moving dan dead stock. Obat yang tergolong slow moving perlu dieleminasi agar tidak menjadi dead stock. Tugas khusus praktik kerja di PT Anugerah Pharmindo lestari adalah Digitalisasi Proses Quality Assurance: Pengembangan Sistem eQMS Menggunakan Aplikasi Smartphone bertujuan untuk mengatasi masalah yang dirasakan BRQA agar mempermudah pekerjaan, mempersingkat waktu dan meminimalisir terjadinya kesalahan. Tugas khusus praktik kerja di Apotek Kimia Farma adalah Evaluasi Kerasionalan Peresepan Obat Pada Pasien Diabetes Melitus di Apotek Kimia Farma 382 Depok. Tujuannya untuk mengetahui apakah obat yang diberikan sudah rasional atau belum agar dilakukan pengkajian ulang agar tidak terjadi kesalahan berulang.

Apothecary internship has the purpose of gaining knowledge, understanding and experience to do pharmacy practice as well as understanding the duties and function Apothecary in practice. Internship at Rumah Sakit Universitas Indonesia is given an assignment entilled Evaluation of Drug Use with ABC Method in Outpatient Pharmacy at Rumah Sakit Universitas Indonesia Period January - September 2021. The purpose is to know which drugs are classified as fast moving, slow moving and dead stock. Drugs are classified as slow moving need to be eliminated so it doesn’t become dead stock. Special assignment at PT Anugerah Pharmindo lestari is Digitizing the Quality Assurance Process: eQMS System Development Using Smartphone Application aims to overcome the problems felt by BRQA in order to simplify the work, shorten the time and minimize the occurrence of errors. Special assignment at Apotek Kimia Farma is Evaluation of Rational Prescribing in Diabetes Mellitus Patients at Apotek Kimia Farma 382 Depok. The purpose is to find out whether the drug given is rational or not so that a re-examination is carried out so that there are no repeated errors."

"Rumah Sakit dituntut untuk memberikan pelayanan yang bermutu sesuaidengan standar yang ditetapkan dan dapat menjangkau seluruh lapisan masyarakat.Standar pelayanan minimal di rumah sakit pada pelayanan farmasi salah satunyaadalah penulisan resep sesuai formularium harus 100% dengan tujuan yaitutergambarnya efisiensi pelayanan obat kepada pasien. Formularium Rumah Sakitbermanfaat dalam kendali mutu dan kendali biaya obat yang akan memudahkanpemilihan obat yang rasional, mengurangi biaya pengobatan, dan mengoptimalkanpelayanan kepada pasien. Pemantauan dan evaluasi Formularium Rumah Sakitdilakukan terhadap kepatuhan penggunaan Fornas dan kepatuhan penggunaanFormularium Rumah Sakit.Penelitian ini dilakukan di RSUI pada tanggal 6 September 2021 – 31Oktober 2021 dan merupakan penelitian deksriptif menggunakan metodepengambilan sampel secara retrospektif dengan mengamati dan mengevaluasikesesuaian peresepan obat pasien depo farmasi rawat jalan RSUI periode Januari –September 2021 dengan Formularium Rumah Sakit RSUI berdasarkan kriteriainklusi yaitu resep pasien rawat RSUI dengan tanggal resep pada periode Januari-September 2021 dan kriteria eksklusi yaitu resep alat kesehatan dan bahan medis habis pakai, juga order location yang bukan berasal dari poliklinik rawat jalan RSUI. Selain itu juga dilakukan evaluasi kesesuaian obat lost sales di depo farmasi rawat jalan RSUI periode Januari – September 2021 dengan Formularium Rumah Sakit RSUI dengan kriteria inklusi seluruh resep pasien dari depo rawat jalan yang terdapat salah satu jenis item obat atau semua item yang obatnya disalin resep (copy resep) keseluruhan atas permintaan pasien atau tidak ditanggung oleh guarantor atau stok obat habis dan kriteria ekslusi yaitu Resep pasien dari depo rawat jalan yang obatnya tidak disalin resep dan disalin sebagian saja. Data diolah dan dihitung menggunakan Microsoft Excel 2013 kemudian data yang dihasilkan ditampilkan dalam bentuk persentase. Hasil penelitian di dapat bahwa peresepan obat pada depo farmasi rawat jalan RSUI periode Januari – September 2021 secara keseluruhan didapatkan 59,52% yang sesuai dengan Formularium Rumah Sakit dan yang tidak sesuai yaitu 40,47%. Dengan peresepan obat pada depo farmasi rawat jalan non COVID-19 RSUI periode Januari – September 2021 yang sesuai dengan Formularium Rumah Sakit yaitu 61,08% dan yang tidak sesuai yaitu 38,92%. Sedangkan peresepan obat pada depo farmasi rawat jalan khusus COVID-19 RSUI periode Januari – Praktik Kerja di RSUI, PT. Anugerah PharmindoLestari, dan Apotek Kimia Farma Periode Bulan September – Desember 2021ix September 2021 yang sesuai dengan Formularium Rumah Sakit yaitu 59,38% danyang tidak sesuai yaitu 40,63%. Untuk obat lost sales yang sesuai denganFormularium Rumah Sakit yaitu 61,12% dan yang tidak sesuai yaitu 38,88%.Kata Kunci: Rumah Sakit, Formularium Rumah Sakit, Lost Sales, Depo Farmasi Rawat Jalan.

Hospitals are required to provide quality services in accordance with

established standards and can reach all levels of society. Minimum service

standards in hospitals specifically in pharmaceutical services, one of which is

writing prescriptions according to the formulary must be 100% with the aim of

depicting the efficiency of drug services to patients. Hospital Formularies are useful

in quality control and drug cost control which will facilitate rational drug selection,

reduce treatment costs, and optimize services to patients. Monitoring and evaluation

of the Hospital Formulary is carried out on compliance with the use of National

Drug Formulary and compliance with the use of Hospital Formulary.

This study was conducted at RSUI on September 6, 2021 – October 31, 2021

and is a descriptive study using a retrospective sampling method by observing and

evaluating the suitability of drug prescriptions for outpatient pharmacy depots at

RSUI for the period January – September 2021 with the RSUI Hospital Formulary

based on inclusion criteria are prescriptions for RSUI inpatients with prescription

dates in the January – September 2021 period and exclusion criteria are

prescriptions for medical devices and consumable medical materials, as well as

order locations that are not from the RSUI outpatient polyclinic. In addition, an

evaluation of the suitability of lost sales drugs at the outpatient pharmacy depot of

RSUI for the period January – September 2021 with the RSUI Hospital Formulary

was also carried out with the inclusion criteria of all patient prescriptions from the

outpatient depot which contained one type of drug item or all items whose

medicines were copied by prescription (copy of prescription) in its entirety at the

request of the patient or not covered by the guarantor or the drug stock runs out and

the exclusion criteria are patient prescriptions from outpatient depots whose

medicines are not copied and only partially copied. The data is processed and

calculated using Microsoft Excel 2013 then the resulting data is displayed in

percentage form.

The results of the study showed that drug prescriptions at the outpatient

pharmacy depot of RSUI for the period January – September 2021 overall obtained

59.52% which were in accordance with the Hospital Formulary and those that were

not appropriate, 40.47%. By prescribing drugs at non-COVID-19 outpatient

pharmacy depots at RSUI for the period January – September 2021, which are in

accordance with the Hospital Formulary, 61.08% and those that are not appropriate,

38.92%. Meanwhile, drug prescriptions at the outpatient pharmacy depot

specifically for COVID-19 RSUI for the period January – September 2021 which

are in accordance with the Hospital Formulary are 59.38% and those that are not

appropriate are 40.63%. For lost sales drugs that are in accordance with the Hospital

Formulary are 61.12% and those that are not suitable are 38.88%.

Praktik kerja profesi apoteker ini dilakukan di PT Dankos Farma, Apotek Roxy Hasyim Ashari dan PT Anugerah Pharmindo Lestari. Laporan ini disusun dengan tujuan memberikan informasi terkait pelaksanaan nyata pelayanan kefarmasian yang dilakukan di industri, apotek, maupun pedagang besar farmasi. Apoteker memiliki wewenang dan tanggung jawab dalam pembuatan serta pengendalian mutu sediaan farmasi di industri farmasi. Dalam penyaluran obat, apoteker bertanggung jawab dalam pelaksanaan pengamanan, pengadaan, penyimpanan dan pendistribusiannya. Sementara dalam bidang pelayanan, apoteker bertanggung jawab dalam pengelolaan dan pelayanan informasi obat. Laporan ini disusun menggunakan metode observasi langsung dan penelusuran studi literatur. Dalam laporan praktik kerja di industri farmasi, dibahas terkait studi analisis jenis afkir yang terjadi di salah satu departemen produksi PT Dankos Farma untuk menentukan tindakan perbaikan dan menurunkan hasil afkir dalam rangka pemastian mutu produk. Dalam laporan praktik kerja apotek, dibuat daftar penggunaan obat-obat off-label indikasi untuk meningkatkan pemahaman agar dapat memastikan bahwa semua obat yang diberikan kepada pasien aman dan bermanfaat. Dalam laporan praktik kerja di pedagang besar farmasi, dibahas mengenai perancangan dan pembuatan penandaan produk untuk mencegah sediaan farmasi bercampur satu dengan lainnya sehingga memastikan proses penyaluran obat terlaksana dengan baik. Oleh karena itu, laporan ini secara garis besar akan memaparkan terkait analisis permasalahan yang ada beserta solusi yang dilakukan di masing-masing tempat praktik kerja profesi apoteker.

Pharmacist Professional Internship is an essential medium for pharmacist students to apply and develop their knowledge in the implementation of pharmaceutical work. This internship was conducted at PT Dankos Farma, Roxy Hasyim Ashari Pharmacy, and PT Anugerah Pharmindo Lestari. This report aims to provide information about the actual implementation of pharmaceutical services in the industry, pharmacy, and pharmaceutical distributors. Pharmacists have the authority and responsibility to manufacture and ensure the quality control of pharmaceutical dosage forms in the pharmaceutical industry. In the distribution of drugs, pharmacists are responsible for implementing security, procurement, storage, and distribution. While in the service sector, pharmacists are responsible for the management and service of drug information.

This report was prepared using the direct observation method and literature study. The pharmaceutical industry practice report discusses the analysis study of the types of rejected products that occurred in one of the production departments of PT Dankos Farma to determine corrective actions and reduce the results of rejected products to ensure product quality. In the pharmacy practice report, a list of off-label drugs is made to increase understanding to ensure that all drugs given to patients are safe and valuable.

The pharmaceutical distributor practice report discusses the design and manufacture of product markings to prevent pharmaceutical preparations from being mixed to ensure the drug distribution process is carried out correctly. Therefore, this report will outline the analysis of existing problems and solutions in each internship place."

Indonesian pharmacist competency standards consist of ten (10) competency standards as abilities expected by pharmacists when they graduate and enter the professional workplace. As a preparation and experience of prospective pharmacists to be able to understand the role of pharmacists and improve competence, the Pharmacist Professional Work Practices are implemented at Apotek Prima Sehat , PT Anugerah Pharmaindo Lestari, and PT Sydna Farma during the period February-May 2019. During the internships, it is expected that prospective pharmacists can broaden insight, understanding, and experience to do pharmaceutical work in professional workplaces."
